  • Mineral fibres, fibrosis, and asbestos bodies in lung tissue from deceased asbestos cement workers

    • Lung tissue from 76 deceased asbestos cement workers (seven with mesothelioma) exposed to chrysotile asbestos and small amounts of amphiboles, has been studied by transmission electron microscopy, together with lung tissue from 96 controls. The exposed workers with mesothelioma had a significantly higher total content of asbestos fibre in the lungs than those without mesothelioma, who in turn, had higher concentrations than the controls (medians 189, 50, and 29 x 10(6) fibres/g (f/g]. Chrysotile was the major type of fibre. The differences were most pronounced for the amphibole fibres (62, 4.7, and 0.15 f/g), especially crocidolite (54, 1.8 and less than 0.001 f/g), but were evident also for tremolite (2.9, less than 0.001, and less than 0.001 f/g) and anthophyllite (1.7, less than 0.001, and less than 0.001 f/g). For amosite, there was no statistically significant difference between lungs from workers with and without mesothelioma; the lungs of workers had, however, higher concentrations than the controls. Strong correlations were found between duration of exposure and content of amphibole fibres in the lungs. Asbestos bodies, counted by light microscopy, were significantly correlated with the amphibole but not with the chrysotile contents. Fibrosis was correlated with the tremolite but not the chrysotile content in lungs from both exposed workers and controls. Overall, similar results were obtained using fibre counts and estimates of mass.

  • Retention patterns of asbestos fibres in lung tissue among asbestos cement workers

    • Retention patterns in lung tissue (determined by transmission electron microscopy and energy dispersive spectrometry) of chrysotile, tremolite, and crocidolite fibres were analysed in 69 dead asbestos cement workers and 96 referents. There was an accumulation of tremolite with time of employment. Among workers who died within three years of the end of exposure, the 13 with high tremolite concentrations had a significantly longer duration of exposure than seven in a low to intermediate category (medians 32 v 20 years; p = 0.018, one sided). Crocidolite showed similar patterns of accumulation. In workers who died more than three years after the end of exposure, there were no correlations between concentrations of amphibole fibres and time between the end of exposure and death. Chrysotile concentrations among workers who died shortly after the end of exposure were higher than among the referents (median difference in concentrations 13 million fibres (f)/g dry weight; p = 0.033, one sided). No quantitative differences in exposure (duration or intensity) could be shown between workers with high and low to intermediate concentrations. Interestingly, all seven workers who had had a high intensity at the end of exposure (> 2.5 f/ml), had low to intermediate chrysotile concentrations at death, whereas those with low exposure were evenly distributed (31 subjects in both concentration categories); hence, there was a dependence between last intensity of exposure and chrysotile concentration (p = 0.014). Among 14 workers with a high average intensity of exposure, both those (n = 5) with high tissue concentrations of chrysotile and those (n = 10) with high tissue concentrations of tremolite fibres had more pronounced fibrosis than those with low to intermediate concentrations (median fibrosis grades for chrysotile: 2 v 1, p = 0.021; for tremolite: 2 v 0.5, p = 0.012). Additionally, workers who died shortly after the end of exposure with high concentrations of chrysotile and crocidolite had smoked more than those with low intermediate concentrations (medians for chrysotile 35 v 15 pack-years, p = 0.030; for crocidolite 37 v 15 pack-years, p = 0.012). The present data indicate that chrysotile has a relatively rapid turnover in human lungs, whereas the amphiboles, tremolite and crocidolite, have a slower turnover. Further, chrysotile retention may be dependent on dose rate. Chrysotile and crocidolite deposition and retention may be increased by tobacco smoking; chrysotile and tremolite by fibrosis.

  • A comparative study of feelings, attitudes and behaviors of patients with fibromyalgia and rheumatoid arthritis.

    • The purpose of this explorative study was to administer a battery of questionnaires related to a broad number of psychosocial factors in patients with fibromyalgia syndrome (FS). By doing this, psychological factors associated with the consequences of chronic pain in patients with FS could be identified and studied in more depth. Thirty-one patients with FS were compared to 30 patients with rheumatoid arthritis (RA) with regard to feelings about self, pain/ache preoccupation, support from significant others, psychosomatic symptoms, activities of daily living, job satisfaction, and future expectations. The results of the study showed that patients with FS had significantly more negative feelings toward themselves, were more preoccupied with thinking about their pain/ache, received more practical help from significant others, experienced more limitations with regard to activities of daily living, and experienced more negative feelings about employment than patients with RA. Patients with FS were also more pessimistic about future employment than RA patients.

  • Myocardial infarction pain : systematic description and analysis

    • The aim of the study was to describe various components of pain in suspected acute myocardial infarction (MI). Ninety-four patients admitted to a Coronary Care Unit (CCU) complaining of chest pain with the preliminary diagnosis suspect MI were included in the study. Thirty-eight subjects were eventually diagnosed as having MI and 56 subjects as non-MI. A comparison of chest pain description was performed between MI and non-MI subjects. The Pain-o-meter (POM) and the Visual Analogue Scale (VAS) were used to assess pain intensity. MI patients reported more intense sensory and affective pain than non-MI patients. MI patients also reported more intense affective pain than sensory pain, whereas non-MI patients reported just the opposite. The number of affective words chosen by MI patients differentiated them more clearly from non-MI patients than any other factor in the pain description. Pain intensity was significantly correlated to the estimated size of the infarct.

    • BACKGROUND: Acute myocardial infarction (AMI) will not develop in a large proportion of patients admitted to the coronary care unit because of suspected AMI. OBJECTIVE: To evaluate whether patients with confirmed AMI use different words to describe their pain than patients in whom AMI was not confirmed. DESIGN: A retrospective descriptive design. METHODS: The Pain-O-Meter, a verbal pain scale composed of 12 sensory and 10 emotional word descriptors, was used to assess pain. SETTING: Coronary care unit in Sahlgrenska Hospital in Goteborg, Sweden. SUBJECTS: A convenience sample of 889 consecutive patients admitted to the coronary care unit because of suspected AMI. RESULTS: SENSORY COMPONENT--patients with confirmed AMI differed from those without AMI in the use of the words "pricking" (12% vs 17%; p < 0.05) and "tearing" (11% vs 6%; p < 0.05). No difference was found in the remaining 10 words. Affective components--Patients with confirmed AMI differed from those without AMI in the use of "terrifying" (29% vs 18%; p < 0.001), "intolerable" (16% vs 10%; p < 0.01), and "worrying" (48% vs 59%; p < 0.01). Women used stronger word descriptors more frequently than men. CONCLUSION: The use of specific or more word descriptors to separate patients with AMI from those without AMI was not supported by the study data. Although patients with AMI more frequently used the sensory word tearing as well as the affective words terrifying and intolerable and less frequently used the sensory word pricking and the affective word worrying than patients without AMI, the differences do not suggest a clinical profile that can be used to differentiate these clinical entities.

  • Quantification of aroma vapours sorbed in polyethene films using supercritical carbon dioxide

    • Extraction with supercritical carbon dioxide was used to quantify the amounts of seven different aroma vapours sorbed in polyethene films. The method was found to completely extract all aroma compounds from the films. The solution of aroma compounds in the polymer films decreased with increasing polymer density. Monoterpenes were always completely sorbed in the films, whereas aldehydes and ketones had a much lower affinity for the films. The sulphurcontaining compound, thiophene, was difficult to analyse due to its adsorption on metal surfaces. © 1993 SCI.

    • An extensive multiconfiguration Hartree-Fock (MCHF) calculation of the isotope shift and hyperfine structure in the resonance line of B II at 1362 angstrom has been performed. The calculated B-11-B-10 isotope shift is 13.3 mangstrom with an estimated uncertainty of 1%. The calculated interaction constants A and B give a hyperfine structure splitting of less than 0.45 mangstrom for both isotopes. The size of the isotope shift in the B II line is similar to the limit of detection in high-resolution observations with the Goddard High Resolution Spectrograph aboard the Hubble Space Telescope when side A becomes operational after the service mission.

  • TROPHIC INTERACTIONS IN TEMPERATE LAKE ECOSYSTEMS - A TEST OF FOOD-CHAIN THEORY

    • In a comparative study covering 11 temperate lake ecosystems of low to intermediate productivity, we tested the predictions of the food chain model developed by L. Oksanen et al. The pelagic habitat of the most unproductive lakes lacked secondary carnivores (piscivores), whereas secondary carnivores inhabited the more productive lakes in agreement with the assumption that food chain length is limited by the productivity of the system. The observed relationships between planktivores (primary carnivores), zooplankton (herbivores), and phytoplankton (primary producers) were in agreement with the predictions of the Oksanen et al. model for three- and four-trophic-link systems. Piscivore relationships were significant only when the biomass of piscivorous perch only was used as the independent variable (piscivorous perch amounted to greater-than-or-equal-to 97% of total piscivore biomass in five out of six four-link systems). The results suggest that gross patterns of food web dynamics in temperate lakes may be collapsed into food chain dynamics. However, as the higher trophic levels of most lakes were dominated by the same two species, an alternative hypothesis is that the simplicity of the food webs studied allowed food chain dynamics to approximate food web dynamics. The food chain model tested gave accurate predictions of trophic-level relationships within the rather narrow productivity range studied. However, previous data suggest that, due to size-structured interactions, secondary carnivores (piscivores) play a minor role in highly productive systems, a circumstance that may explain the postulated increase in mortality rate of zooplankton with increasing productivity. The observed increase in number of trophic levels with productivity does not necessarily imply a causal relationship between productivity and food chain length, as lake productivity is also correlated with structural complexity (in the form of submerged vegetation and an oxygenated hypolimnion). Habitat heterogeneity may thus influence food chain length, and in general it is likely that food chain length will depend on more than one environmental variable.
