  • The Effect of Downwelling Longwave and Shortwave Radiation on Arctic Summer Sea Ice

    • The Arctic summer sea ice has diminished fast in recent decades. A strong year-to-year variability on top of this trend indicates that sea ice is sensitive to short-term climate fluctuations. Previous studies show that anomalous atmospheric conditions over the Arctic during spring and summer affect ice melt and the September sea-ice extent (SIE). These conditions are characterized by clouds, humidity and heat anomalies which all affect shortwave (SWD) and longwave (LWD) radiation to the surface. In general, positive LWD anomalies are associated with cloudy and humid conditions, whereas positive anomalies of SWD appear under clear-sky conditions. Here we investigate the effect of realistic anomalies of LWD and SWD on summer sea ice, by performing experiments with the Community Earth System Model. The SWD and LWD anomalies are studied separately and in combination for different seasons. It is found that positive LWD anomalies in spring and early summer have significant impact on the September SIE, whereas winter anomalies show only little effect. Positive anomalies in spring and early summer initiate an earlier melt onset, hereby triggering several feedback mechanisms that amplify melt during the succeeding months. Realistic positive SWD anomalies appear only important if they occur after the melt has started and the albedo is significantly reduced relative to winter conditions. Simulations where both positive LWD and negative SWD anomalies are implemented simultaneously, mimicking cloudy conditions, reveal that clouds during spring have a significant impact on summer sea ice while summer clouds have almost no effect.

  • Defining a Standard Set of Patient-centered Outcomes for Men with Localized Prostate Cancer

    • BACKGROUND: Value-based health care has been proposed as a unifying force to drive improved outcomes and cost containment.OBJECTIVE: To develop a standard set of multidimensional patient-centered health outcomes for tracking, comparing, and improving localized prostate cancer (PCa) treatment value.DESIGN, SETTING, AND PARTICIPANTS: We convened an international working group of patients, registry experts, urologists, and radiation oncologists to review existing data and practices.OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: The group defined a recommended standard set representing who should be tracked, what should be measured and at what time points, and what data are necessary to make meaningful comparisons. Using a modified Delphi method over a series of teleconferences, the group reached consensus for the Standard Set.RESULTS AND LIMITATIONS: We recommend that the Standard Set apply to men with newly diagnosed localized PCa treated with active surveillance, surgery, radiation, or other methods. The Standard Set includes acute toxicities occurring within 6 mo of treatment as well as patient-reported outcomes tracked regularly out to 10 yr. Patient-reported domains of urinary incontinence and irritation, bowel symptoms, sexual symptoms, and hormonal symptoms are included, and the recommended measurement tool is the Expanded Prostate Cancer Index Composite Short Form. Disease control outcomes include overall, cause-specific, metastasis-free, and biochemical relapse-free survival. Baseline clinical, pathologic, and comorbidity information is included to improve the interpretability of comparisons.CONCLUSIONS: We have defined a simple, easily implemented set of outcomes that we believe should be measured in all men with localized PCa as a crucial first step in improving the value of care.PATIENT SUMMARY: Measuring, reporting, and comparing identical outcomes across treatments and treatment centers will provide patients and providers with information to make informed treatment decisions. We defined a set of outcomes that we recommend being tracked for every man being treated for localized prostate cancer.

  • Measuring PI3K Activation : Clinicopathologic, Immunohistochemical, and RNA Expression Analysis in Prostate Cancer

    • Assessing the extent of PI3K pathway activity in cancer is vital to predicting sensitivity to PI3K-targeting drugs, but the best biomarker of PI3K pathway activity in archival tumor specimens is unclear. Here, PI3K pathway activation was assessed, in clinical tissue from 1,021 men with prostate cancers, using multiple pathway nodes that include PTEN, phosphorylated AKT (pAKT), phosphorylated ribosomal protein S6 (pS6), and stathmin. Based on these markers, a 9-point score of PI3K activation was created using the combined intensity of the 4-markers and analyzed its association with proliferation (Ki67), apoptosis (TUNEL), and androgen receptor (AR) status, as well as pathologic features and cancer-specific outcomes. In addition, the PI3K activation score was compared with mRNA expression profiling data for a large subset of men. Interestingly, those tumors with higher PI3K activation scores also had higher Gleason grade (P = 0.006), increased AR (r = 0.37; P < 0.001) and Ki67 (r = 0.24; P < 0.001), and decreased TUNEL (r = -0.12; P = 0.003). Although the PI3K activation score was not associated with an increased risk of lethal outcome, a significant interaction between lethal outcome, Gleason and high PI3K score (P = 0.03) was observed. Finally, enrichment of PI3K-specific pathways was found in the mRNA expression patterns differentiating the low and high PI3K activation scores; thus, the 4-marker IHC score of PI3K pathway activity correlates with features of PI3K activation.

  • Effects of early sensorimotor disorder on contextual learning in autism

    • Cognitive explanations of autism often involve higher order cognitive functions developing late in childhood, such as theory of mind, executive functions or central coherence. In home videos of infants later diagnosed with autism, children display early signs of developmental disorders, for example impaired sensorimotor functions, attention to social and non-social stimuli and a lack of circadian regulation. We propose that these early signs need to be understood using a framework of context learning. It is also important to understand the role for context understanding in guiding the maturation of behavior. The role for inhibition in context learning as understood within learning theory provides us with helpful tools for this analysis. Our research aim is not to identify and explain early markers for autism, but to understand the cognitive developmental pathway set into rolling by an early impairment. This will help us understand the seemingly unrelated symptoms that define the complex syndrome of autism.

    • The EXTRAP T2R feedback system (active coils, sensor coils and controller) is used to study and develop new tools for advanced control of the MHD instabilities in fusion plasmas. New feedback algorithms developed in EXTRAP T2R reversed-field pinch allow flexible and independent control of each magnetic harmonic. Methods developed in control theory and applied to EXTRAP T2R allow a closed-loop identification of the machine plant and of the resistive wall modes growth rates. The plant identification is the starting point for the development of output-tracking algorithms which enable the generation of external magnetic perturbations. These algorithms will then be used to study the effect of a resonant magnetic perturbation (RMP) on the tearing mode (TM) dynamics. It will be shown that the stationary RMP can induce oscillations in the amplitude and jumps in the phase of the rotating TM. It will be shown that the RMP strongly affects the magnetic island position.

  • Shifting to Circular Manufacturing in the Global South : Challenges and Pathways

    • As the Global South shifts towards increased manufacturing, the negative effects on climate change and environmental pollution raise serious concerns. These global effects are increasingly felt locally, as reflected in health surveys throughout the Global South. The world cannot afford to wait for a natural development process to take place in which rising incomes might curb pollution. This article examines the challenges of reforming manufacturing in the Global South towards more sustainable practices. It also focuses on the lessons of the Sustainable Manufacturing and Environmental Pollution Program (SMEP) which has funded a series of environmental improvement projects across sub-Saharan Africa and South Asia aimed at reducing pollution in the manufacturing process. The lessons learned from these projects include the need to improve the tracking of the negative effects of the environmental damages caused by manufacturing and analyze the manufacturing supply chain processes to better identify potential points of intervention; as well as the need for more external financial and technical resources to expand these projects.
