    • With the advent of multimodality therapy, the overall five-year survival rate from childhood cancer has improved considerably now exceeding 80% in developed European countries. This growing cohort of survivors, with many years of life ahead of them, has raised the necessity for knowledge concerning the risks of adverse long-term sequelae of the life-saving treatments in order to provide optimal screening and care and to identify and provide adequate interventions. Childhood cancer survivor cohorts in Europe. Considerable advantages exist to study late effects in individuals treated for childhood cancer in a European context, including the complementary advantages of large population-based cancer registries and the unrivalled opportunities to study lifetime risks, together with rich and detailed hospital-based cohorts which fill many of the gaps left by the large-scale population-based studies, such as sparse treatment information. Several large national cohorts have been established within Europe to study late effects in individuals treated for childhood cancer including the Nordic Adult Life after Childhood Cancer in Scandinavia study (ALiCCS), the British Childhood Cancer Survivor Study (BCCSS), the Dutch Childhood Oncology Group (DCOG) LATER study, and the Swiss Childhood Cancer Survivor Study (SCCSS). Furthermore, there are other large cohorts, which may eventually become national in scope including the French Childhood Cancer Survivor Study (FCCSS), the French Childhood Cancer Survivor Study for Leukaemia (LEA), and the Italian Study on off-therapy Childhood Cancer Survivors (OTR). In recent years significant steps have been taken to extend these national studies into a larger pan-European context through the establishment of two large consortia - PanCareSurFup and PanCareLIFE. The purpose of this paper is to present an overview of the current large, national and pan-European studies of late effects after childhood cancer. This overview will highlight the strong cooperation across Europe, in particular the EU-funded collaborative research projects PanCareSurFup and PanCareLIFE. Overall goal. The overall goal of these large cohort studies is to provide every European childhood cancer survivor with better care and better long-term health so that they reach their full potential, and to the degree possible, enjoy the same quality of life and opportunities as their peers.

  • Clinical Feasibility of Synthetic MRI in Multiple Sclerosis : A Diagnostic and Volumetric Validation Study.

    • BACKGROUND AND PURPOSE: Quantitative MR imaging techniques are gaining interest as methods of reducing acquisition times while additionally providing robust measurements. This study aimed to implement a synthetic MR imaging method on a new scanner type and to compare its diagnostic accuracy and volumetry with conventional MR imaging in patients with MS and controls.MATERIALS AND METHODS: Twenty patients with MS and 20 healthy controls were enrolled after ethics approval and written informed consent. Synthetic MR imaging was implemented on a Siemens 3T scanner. Comparable conventional and synthetic proton-density-, T1-, and T2-weighted, and FLAIR images were acquired. Diagnostic accuracy, lesion detection, and artifacts were assessed by blinded neuroradiologic evaluation, and contrast-to-noise ratios, by manual tracing. Volumetry was performed with synthetic MR imaging, FreeSurfer, FMRIB Software Library, and Statistical Parametric Mapping. Repeatability was quantified by using the coefficient of variance.RESULTS: Synthetic proton-density-, T1-, and T2-weighted images were of sufficient or good quality and were acquired in 7% less time than with conventional MR imaging. Synthetic FLAIR images were degraded by artifacts. Lesion counts and volumes were higher in synthetic MR imaging due to differences in the contrast of dirty-appearing WM but did not affect the radiologic diagnostic classification or lesion topography (P = .50-.77). Synthetic MR imaging provided segmentations with the shortest processing time (16 seconds) and the lowest repeatability error for brain volume (0.14%), intracranial volume (0.12%), brain parenchymal fraction (0.14%), and GM fraction (0.56%).CONCLUSIONS: Synthetic MR imaging can be an alternative to conventional MR imaging for generating diagnostic proton-density-, T1-, and T2-weighted images in patients with MS and controls while additionally delivering fast and robust volumetric measurements suitable for MS studies.

    • Precise microbeam U-Pb zircon dates have been determined for 17 granitoid samples which crop out along the southwestern margin of the Kaapvaal Craton. In the Marydale High two main types of granitoid are distinguishedmainly by their normative Quartz - Alkali Feldspar – Plagioclase mineral proportions. The Draghoender Granite type is generally tonalitic to trondhjemitic and some samples have very low heavy Rare Earth Elements, probably originating as melts of eclogitic protoliths. Their ages vary by ~50 Ma from 2946 ±9 Ma to 2892 ±6 Ma (2σ), reflecting a long period of magmatism possibly due to subduction. The granodioritic to monzogranitic Skalkseput Granite type intrudes the Draghoender granite in places and has a coherent age of 2901 ±14 Ma. These two granite types thus overlap in time and space. The more evolved Skalkseputs type could have been derived by melting of the Draghoender type during assembly of the Kaapvaal Craton by collision of the Kimberley and Witwatersrand Terranes. A third granite type is exemplified by the 2721 ±6 Ma monzogranitic Steenkop Granite Gneiss which occurs south of Prieskapoort, and corresponds in age to a 2718 ±8 Ma monzogranite dyke which intrudes a 2871 Ma Skalkseput monzogranite. The previous age of 2718 ±8 Ma for the Skalkseput Granite probably also dated a Steenkop-type dyke. A basal conglomerate to the Zeekoebaard Formation of the Ventersdorp Supergroup has a single 2720 ±4 Ma detrital zircon population. South of the Marydale High a narrow strip of Draghoender-type Granite extends to Franzenhof where it is exposed in a Dwyka glacial outwash surface and is dated at 2931 ±9 Ma. The southernmost exposures of granite are windows in the Dwyka group about 130 km southeast of Marydale on both sides of the Doornberg Fault. The 2905 ±7 Ma Maritzdam Granite which is basement to Ventersdorp Supergroup volcanic rocks at Kuip is monzogranitic and corresponds to the Skalkseput type. On the southwest side of the Doornberg Fault the 2907 ±4 Ma Welgevonden Granite underlies Marydale Group volcanic rocks, showing that the Kaapvaal basement extends across the fault.

    • Cognitive impairment is common in multiple sclerosis (MS) and may be subtle. The corpus callosum is essential for connectivity-demanding cognitive tasks and is significantly affected in MS, therefore it may serve as a marker for cognitive function. Objective: The objective of this paper is to longitudinally study the normalized corpus callosum area (nCCA) as a marker of cognitive function and disability in MS. Methods: Thirty-seven MS patients were followed from 1996 with follow-ups in 2004 and 2013. A healthy matched control group was recruited. The Expanded Disability Status Scale (EDSS) and Symbol Digit Modalities Test (SDMT) were assessed. The nCCA was measured on T2-weighted images. Volumetry was performed with FreeSurfer. Results: Disease duration spanned five decades (1.6–46 years). Annual corpus callosal atrophy rate decreased with disease duration. nCCA was strongly correlated with SDMT ( r = 0.793, p < 0.001) and moderately correlated with EDSS ( r = −0.545, p < 0.001) after adjusting for disease duration, age and sex. The correlations of brain parenchymal fraction, white matter fraction, gray matter fraction and normalized lesion volume were less strong. Conclusions: The nCCA correlates well with physical and cognitive disability in time perspectives close to two decades, outperforming volumetric measurements. The nCCA is fast and could be feasible for clinical implementation where it may help identify patients in need of neuropsychological evaluation.

  • The proximal hamstring avulsion clinical trial (PHACT)-a randomised controlled non-inferiority trial of operative versus non-operative treatment of proximal hamstrings avulsions : study protocol

    • Introduction The treatment of proximal hamstring avulsions is controversial. While several trials have investigated the outcome for patients treated surgically, there is today no prospective trial comparing operative treatment with non-operative treatment. This protocol describes the design for the proximal hamstring avulsion clinical trial (PHACT)-the first randomised controlled trial of operative versus non-operative treatment for proximal hamstring avulsions. Methods and analysis PHACT is a multicentre randomised controlled trial conducted across Sweden, Norway and Finland. Eligible patients (60 participants/treatment arm) with a proximal hamstring avulsion of at least two of three tendons will be randomised to either operative or non-operative treatment. Participants allocated to surgery will undergo reinsertion of the tendons with suture anchors. The rehabilitation programme will be the same for both treatment groups. When patient or surgeon equipoise for treatment alternatives cannot be reached and randomisation therefore is not possible, patients will be invited to participate in a parallel observational non-randomised cohort. The primary outcome will be the patient-reported outcome measure Perth hamstring assessment tool at 24 months. Secondary outcomes include the Lower Extremity Functional Score, physical performance and muscle strength tests, patient satisfaction and MR imaging. Data analysis will be blinded and intention-to-treat analysis will be preformed. Ethics and dissemination Ethical approval has been granted by the Ethical Committee of Uppsala University (DNR: 2017-170) and by the Norwegian ethical board (REC: 2017/1911). The study will be conducted in agreement with the Helsinki declaration. The findings will be disseminated in peer-reviewed publications.

  • Low-δ18O zircon xenocrysts in alkaline basalts; a window into the complex carbonatite-metasomatic history of the Zealandia lithospheric mantle

    • Megacrystic zircon grains from alkaline basaltic fields are rare but can provide fundamental insights into mantle metasomatic processes. Here, we report in-situ U-Pb ages, trace element concentrations and hafnium and oxygen isotopes for fourteen zircon megacrysts from two intraplate alkaline basalt locations in New Zealand. U-Pb ages indicate the zircons crystallised between 12.1 and 19.8 Ma. Zircon oxygen isotopic compositions range from low to mantle-like compositions (grain average δ18O = 3.8–5.1‰). Hafnium isotopes (εHf(t) = +3.3 to +10.4) mostly overlap with intraplate mafic rocks and clinopyroxene in metasomatized peridotitic mantle xenoliths but show no correlation with most trace element parameters or oxygen isotopes. The zircons are interpreted to have formed by the reaction between low-degree melts derived from pre-existing mantle metasomes and the depleted mantle lithosphere prior to eruption and transport to the surface. The low Hf concentration, an absence of Eu anomalies, and elevated U/Yb compared to Nb/Yb in the megacrystic zircons are interpreted to show that the source metasomes comprised subduction- and carbonatite-metasomatised lithospheric mantle. As these trace element characteristics are common for megacrystic zircon in intra-plate basaltic fields globally, they suggest the prevalence of subduction- and carbonatite-metsasomatised mantle under these intraplate volcanic regions. The unusually low δ18O was likely present prior to metasomatic enrichment and may have resulted from high-temperature hydrothermal alteration during initial mantle lithosphere formation at a mid ocean ridge or, possibly, during subduction-related processes associated with continent formation. The combination of proportionally varied contributions from carbonatite- and subduction-metasomatised lithospheric melts with asthenospheric melts may explain the variety of primitive intraplate basalt compositions, including low δ18O reported for some local intraplate lavas.
