    • Rational and aim: Knowledge of which aspects of care are more important to patients is crucial for health providers. The aim of the work presented here was to explore changes over time in patients’ preferences in hospital and in out-patient settings.Method: Cross-sectional data were collected among patients at a number of Swedish hospital wards and out-patient clinics in 2003, 2004, 2005 and 2008 (N = 46,525). Data for men and women were analysed separately. The main outcome measure consisted of replies to the questionnaire “Quality from the Patient’s Perspective” (QPP) (numerous references).Results: (1) Subjective importance (preference) ratings increased throughout the time period studied, (2) in-patients gave higher ratings than out-patients and (3) women gave higher ratings than men, particularly in out-patient settings.Conclusion: The increase over time in ratings of the importance ascribed to different aspects of care may reflect a general trend towards strengthened consciousness among citizens as consumers of care and is an important index for improving the person-centeredness of clinical services.

  • Birthgiving women's feelings and perceptions of quality of intrapartal care : a nationwide Swedish cross-sectional study

    • Aim. The aim was to explore the relationships between the feelings of women who give birth and several birth-related care conditions, the womens' socio-demographic background, personality and perceptions of the quality of intrapartum care. Background. Childbirth is a highly significant emotional event for women and it is an explicit goal that each woman should have a positive childbirth experience. Design. The nationwide study is cross-sectional and was conducted at Swedish maternity units over two months in 2007. Methods. The study group consisted of 739 women (63% response rate). A dimensionality analysis of six emotion items was performed using structural equation modelling (SEM). Non-parametric tests and generalised linear mixed effects model analyses were used to study relationships and to make subgroup comparisons. Results. In the SEM analysis, a positive and a negative feeling factor were identified. Strong associations between the feeling factors and the womens' perceptions of the quality of care were found, particularly in regard to the so-called identity-oriented approach of the midwives and other health professionals. In addition, a higher intensity of negative feelings was associated with a personality pattern characterised by extraversion, emotional instability and lower degree of conscientiousness, a lower level of education and having had delivery through Caesarean section. Conclusions. The positive and negative feelings of women who give birth strongly covary with the women's perception of the quality of their intrapartum care. This pertains in particular to information issues, the midwives showing commitment, empathy and respect, being present during labour and allowing the women to actively participate as much as they want to. Relevance to clinical practice. Subgroup differences indicate that midwives should pay increased attention to childbearing women who have primary school education only, a lower emotional stability and/or deliver through Caesarean section.

The sample included 1,474 patients and 458 healthcare providers in six participating wards before and after PFC implementation, plus five additional randomly chosen wards, which only featured in the post-assessment.Findings– No pre-post differences were found regarding care perceptions or provider work climate evaluations. Statistically significant improvements were noted among provider care evaluations. Using aggregate-level ward data, multiple regression analyses showed that high adherence to PFC principles and a positive work climate contributed significantly to variance among care quality ratings.Research limitations/implications– Among healthcare providers, questions related to specific PFC aspects during evenings, nights and weekends had to be dropped owing to a low response rate.Practical implications– An important requirement for both practice and research is to tailor PFC to various health and social care contexts.Originality/value– The study is large-scale before-after PFC model review, where patient and provider data were collected using well-established measurements.

  • Womens’ opinions on intrapartal care : development of a theory-based questionnaire

    • Aim.To develop a patient questionnaire specific to intrapartal care, based on the theoretical foundation of the general instrument Quality from the Patient's Perspective (QPP).Background.Existing general patient questionnaires do not take intrapartal care aspects fully into account, and available intrapartal-specific patient questionnaires tend to have a weak theoretical foundation.Design.A cross-sectional, nationwide study carried out during a two-week period in 2007 at Swedish maternity units.Method.An intrapartal-specific QPP-questionnaire (QPP-I) was developed by combining a selection of 22 items from the short and long versions of the QPP with 10 newly constructed items. Responses were obtained from 739 women (63% response rate). The dimensionality of the QPP-I was assessed using structural equation modelling (a nested factor model). Non-parametric statistics were used for subgroup comparisons.Results.A nested model with a general factor including all 32 items and 10 subordinate factors was developed. Most scales had acceptable reliability coefficients (0 center dot 73-0 center dot 93), and a meaningful pattern of subgroup differences was obtained.Conclusions.The QPP-I is theory based and has its roots in a patient perspective. It was developed using a nationwide sample of Swedish women receiving intrapartal care, and an advanced statistical method was used. The outcome of this initial empirical trial was promising but needs to be tested in different countries and cultures.Relevance to clinical practice.By combining womens' responses on perceived reality and subjective importance ascribed to the various aspects of care measured, the QPP-I offers the care provider better guidelines on which elements to focus quality improvement work on, than would be the case if only a rank order of perceived reality ratings was available.

    • This study illuminates a deeper understanding of the processes of confidence building and emotion management among boundary spanners from organisations with a bureaucratic structure in a multinational civil-military operational context. Twenty-one informants (mainly military officers) were interviewed. They had a variety of experiences and occupational roles in civil-military collaboration contexts. A grounded theory analysis of interview data yielded a model according to which the most central aspects of civil-military collaboration are confidence building, maintaining existing confidence, avoiding distrust and rebuilding damaged confidence. The study shows that boundary spanners strategically utilise a variety of emotion management strategies in order to fulfil the demands laid upon them by their collaborating counterparts in the hostile environment and by their own organisation. Three interrelated dimensions of smoothness were identified: cultural, structural and smoothness in risky situations. By acting ‘smoothly’, an adaption to the dynamic environment can be achieved. The proposed model is compared to existing writings on emotional labour and self-presentation.

  • Omgivningens påverkan på ledarskap : En studie av jägare i utlandstjänst

    • Denna studie syftar till att få en fördjupad förståelse för hur omgivnings­förhållanden påverkar militärt ledarskap under påfrestande förhållanden på olika hierarkiska nivåer. Studien genomfördes genom intervjuer med 28 officerare från de tre svenska jägarförbanden AJB, Amf 1 och K 3. Samtliga hade erfarenhet av starkt påfrestande episoder. Intervjuerna tog sin utgångspunkt i en modell över kontextuella förhållanden som påver­kar ledarskap. Resultatet visade att modellen stämde relativt väl samt att ett antal nya påverkansfaktorer tillkom. I flertalet fall där skillnader framkom mellan chefer på lägre och högre nivåer tycktes skillnaderna spegla högre chefers större ansvar för verksamheten respektive lägre chefers mer vardagliga fälterfarenheter. Vid en fördjupad kvalitativ analys kom de ”mjuka” aspekterna ”jägarkultur” och ”grupp­kli­mat och sammansvetsning” att dominera svarsmönstret. Här rådde också samstämmighet mellan chefer på lägre och högre nivå. Detta resultat över­ensstämmer med resultaten i den ursprungliga studien där den teoretiska modellen togs fram och visar att organisations- och gruppför­hållanden av mellanmänsklig karaktär tycks ha en likvärdig inverkan på ledarskap oavsett chefsnivå. Resultatet från studien av jägare behöver prövas bland fler typer av officerare. Om resultaten står sig innebär det att ledarskapsmodellen Utvecklande ledarskap bättre kan situationsanpassas för en given chefsnivå vid framtida ledarskapsutbildning.

  • Structure versus freedom of action : Leadership during the rescue operation following the 2004 tsunami

    • The purpose of this study was to develop a theoretical understanding of leadership during a complex rescue operation following a major disaster in a foreign country. The analysis followed a grounded theory approach. Seventeen informants from three Swedish authorities were interviewed on leadership in the emergency handling of the 2004 tsunami in Southeast Asia. A theoretical conceptualisation was developed which includes three superior categories: antecedent conditions, situational constraints and core aspects of leadership. Within the last-mentioned superior category a core variable was identified: a balance between the need for structure and the need for freedom. Leaders who strive to create structure at the expense of freedom of action are less inclined to delegate and more likely to wear themselves out. Conversely, those who strive to create great freedom of action bypass many links in the organisational chain, thus 'short-circuiting' the organisation as a whole.

The results suggest that collaboration during crisis management is facilitated by two types of links—planned and spontaneous—and both can act vertically and horizontally. The results imply an elaboration of the organizational concept boundary spanners.

  • Authentic leadership and its relationship with risk perception and safety climate

    • Purpose – This study aims to examine how authentic leadership relates to risk perception in safety critical organizations (SCOs). It is hypothesized that authentic leaders influence risk perception through the mediating effect of safety climate.Design/methodology/approach – Using a survey design, the variables were assessed in a cross-sectional sample of 293 offshore oil installation workers from a single company.Findings – The authors’ findings show that follower ratings of authentic leadership are negatively related to risk perception and positively associated with ratings of safety climate. Controlling for personality characteristics and leadership responsibility among respondents, the results confirm the hypothesis in that safety climate mediates the relationship between authentic leadership and risk perception. Safety climate had the strongest relationship with risk perception when assessed as a higher order construct.Originality/value – This study is one of the first to investigate the relationship between authentic leadership and safety. The results indicate that authentic leadership and safety climate are important factors that relate to risk perception in SCOs. The authors’ findings suggest that SCOs should consider recruiting and developing authentic leaders to foster positive safety climates and risk management.

    • Previous research has shown that psychological capital (PsyCap) is associated with desired employee behavioral and performance outcomes. Extending previous research, we examine, in two studies, if the PsyCap of Norwegian air traffic controllers is related to their perceptions of safety climate. Based on bootstrapping procedures, results from Study 1 (N = 77) and Study 2 (N = 38) revealed that PsyCap was positively correlated with safety climate. In Study 1, PsyCap explained 31% of the variance in safety climate. Controlling for mediating effects of positive and negative emotions in Study 2, PsyCap explained 15.5% of the variance. These results provide first-time empirical evidence linking PsyCap to safety climate in safety critical organizations.

    • This study examined relationships between authentic leadership and safety climateamong 463 seafarers sailing on 23 merchant vessels in the international shipping industry. A multiple regression analysis was performed, where authentic leadership made a statistically significant contribution to explaining variance in safety climate, controlling for age, rank on board, and social desirable responding. Adding an interaction between authentic leadership and rank on board (officers vs. crew) to the multiple regression analysis did not add to the amount of explained variance (R 2 = .38). Implications for leadership training and safety in shipping are discussed.

    • Ett antal faktorer som Försvarsmaktens (FM) volymminskning, värnpliktens vilande och avsaknaden av en mera akut nationell hotbild, har lett till att försvarets förankring i samhället inte är lika självklar som tidigare. Detta kan bland annat ge negativa effekter på FM personalförsörjning liksom på förståelsen i samhället för FM verksamhet och i förlängningen befolkningens stöd för verksamheten. Syftet med denna studie var att hos företrädare för FM högre nivåer kartlägga uppfattningar av vad som är viktiga faktorer för förankringen i samhället. I rapporten redovisas informanternas uppfattningar om bland annat allmänhetens syn på FM, om relationen mellan FM och den politiska nivån samt politikernas respektive FM ansvar för myndighetens förankring i samhället. Rapporten lämnar förslag på åtgärder som FM kan göra för att positivt förbättra förankringen. Inom projektet har det tidigare tagits fram en forskningsöversikt och gjorts en intervjustudie med informanter från näringsliv, andra myndigheter, lokala politiker och politiska ungdomsförbund.

  • The heritability of emergent leadership : age and gender as moderating factors

    • In this study, we examined the moderating influences of gender and age with respect to testing the heritability of leadership emergence. A large data base of 12,112 twins from Sweden was used in the current study to decompose the variance of emergentleadership into an unobservable genetic component and environmental components that are either common or unshared among twin pairs. Consistent with prior leadership research on genetics, we found that a genetic factor is able to explain a significant proportion of the variation across individuals in predicting how twins perceive their emergentleadership behavior (about 44% for women and 37% for men). Furthermore, we also found that the magnitude of genetic influence on emergentleadership varied with age, but only for women with the heritability estimate being highest for the mid-age women versus lowest for the older women. Implications for advancing research on the genetic and environmental influences on leadership emergence are discussed.

    • The purpose of this study is to investigate what influences the formation of a common operating picture and situational awareness during collaborative efforts to deal with complex disasters. Particular focus is given to the relationship between the operating picture and situational awareness. The study was conducted using qualitative interviews and a grounded theory approach, and is based on interview data from three crisis events. The results show that the operating picture is influenced by information coming together from different sources and forming a snapshot of the event, a portrait which changes and is updated over time, while organisational belonging, role and occupationally specific knowledge provide a framework for and influence situational awareness. The results also show that the operating picture forms the basis for an occupationally specific and role-based situational awareness and its subsequent decision-making. Further studies on the relation between common operating picture, sense making and situational awareness are discussed.

  • Influence of IED attacks on leadership : Dealing with the invisible enemy

    • The aim of this study was to gain a deeper understanding of leadership in military operations characterized by the presence of critical incidents (IED attacks). Twenty-one Swedish soldiers and officers (ranking from soldier to colonel) who had experienced IED attacks in Afghanistan during the period 2005-2008 were interviewed, and their responses were analyzed according to grounded theory methodology. A model was developed according to which the essence of military leadership immediately following an IED attack could be summarized as recapturing control over self, the group and the task. The model also illuminates the importance of selected pre-existing conditions and the leader's appraisal/ sensemaking processes. Nine leadership acts of balance were identified, including balanced grief (self), balanced focus on emotional and functional recovery (group), and balanced handling of safety versus necessary risk-taking (task).

  • Job satisfaction among the new Swedish all-volunteer soldiers : Challenges for leaders of the armed forces

    • The Swedish Armed forces has been transformed to an all-volunteer force where the first soldiers began their employment in May 2011. The number of applicants has been satisfying but experiences from other countries indicate that it has been easier to recruit soldiers than to retain them. The aim of this study was to investigate factors affecting the job satisfaction of soldiers. Twenty-one informants who had been employed as soldiers for about a year participated in the study. They came from all three fighting services (army, air force, navy). The data was analyzed according to a grounded theory approach.      The results show that the soldiers’ job satisfaction is related to their inner motivation to join the armed forces. The inner motivation can be divided into: (a) primarily searching for a job and a steady income, (b) a great interest in the armed forces and military tasks, and (c) an aim for a certain education/position. Factors that appear to affect the job satisfaction are, for example: co-workers, salary and experienced meaningfulness. Another result from the study is that strategic decisions at the Head Quarter level that involve unexpected readjustments at the local level tend to have a negative effect on of the soldiers’ motivation and job satisfaction because  the leaders at the local units are poorly prepared to handle the changes. This will be more thoroughly discussed during the presentation.

  • Trust in a military context : What contributes to trust in superior and subordinate leaders?

    • The aim was to explore which components military personnel in peacekeeping operations perceive as contributing to trust in their superior and subordinate leaders during international or national operations or exercises. Data were collected among 591 military officers and cadets from Norway and Sweden using a questionnaire with open-ended questions. Two superior categories emerged: Individual-Related Characteristics and Communication- and Relationship-Related Characteristics. The former is made up of the higher-level categories Personal attributes and Experience and competence. The latter is derived from the higher-level categories Consideration and inspiration and Effective communication. In turn, all higher-level categories are underpinned by a number of subcategories (13 altogether). When comparing trust in superior leaders with trust in subordinate leaders, the results show that trust in superiors is characterised to a greater extent by Communication- and Relationship-related characteristics and also by Experience and professional knowledge. Trust in subordinate leaders is characterised to a greater extent by Personal attributes and Management competence.

  • Den osynliga fienden : IED-attentats påverkan på ledarskap, tillit och motivation

    • Syftet med studien var att erhålla fördjupad förståelse av hur allvarliga incidenter som inträffat under tjänstgöring i Afghanistan (huvudsakligen Improvised Explosive Device-attentat) påverkar ledarskap, tillit och motivation. Totalt intervjuades 21 personer, 16 officerare och fem civila. Alla informanter har tjänstgjort i Afghanistan och kan delas in i följande kategorier: (a) indirekta ledare (förbandschefer), (b) direkta ledare (grupp/teamledare för grupper som varit involverade i incidenter), (c) individer involverade i incidenten, (d) individer tillhörande involverad grupp men ej direkt involverade i incidenten, samt (e) individer som inte tillhörde involverad grupp men aktuellt förband. Incidenterna som studerades var framförallt IED-attentat där svenskar omkommit, skadats eller klarat sig utan skador.Resultatet visar på en kedjereaktion: individ- och omgivningsförhållanden påverkar individernas generella uppfattningar av insatsen som i sin tur påverkar uppfattningar, reaktioner och ageranden under och direkt efter incidenter. De omedelbara reaktionerna påverkar i sin tur resultatet av såväl de specifika incidenterna som insatsen generellt. Genomgående visar resultatet på svårigheter att hantera "den osynliga fienden". IED-attentat uppfattas vara svåra, om inte omöjliga, att upptäcka och påverka, vilket leder till en frustration hos både underställda och ledare. Andra aspekter som behandlas i rapporten är bland annat hur incidenter uppfattas, tillit, motivation, sorghantering och beslutsfattande. Slutligen ges praktiska råd till ledare som ska tjänstgöra i Afghanistan.

    • The vast majority of leadership studies have focused on positive aspects such as how leadership can contribute to organisational effectiveness, individual work satisfaction, etc. In an attempt to capture real leadership processes, and thus to avoid a long time one-sided focus upon the positive aspects of leadership, an evaluation form of destructive leadership behaviours, Destrudo-L, was developed. This instrument comprises five factors measuring destructive leader behaviours: (1) Arrogant, unfair (2) Threats, punishments, over-demands (3) Ego-oriented, false (4) Passive, cowardly, and (5) Uncertain, unclear, messy.

    • Purpose – The study seeks to illuminate factors that benefit, or do not benefit, the development of swift trust towards leaders in temporary military groups.Design/methodology/approach – The study group comprised 50 Norwegian cadets, 34 Norwegian military officers, 317 Swedish cadets, and 190 Swedish military officers. Data were gathered using a questionnaire which included two open-ended questions on aspects which contribute to swift trust (and lack thereof) towards leaders, as well as Likert-scale questions on temporary group characteristics, and a personality inventory.Findings – A qualitative clustering analysis of the open-ended responses yielded a hierarchical model of aspects which contribute to swift trust (or the lack thereof) with the following two superior categories: individual-related characteristics such as emotional stability and relationship-related characteristics such as encourage involvement and creativity. The latter superior category covaried most strongly with ratings of the groups' performance.Research limitations/implications – The results need to be substantiated by further research in other professional groups and cultures.Practical implications – The findings can help leaders of temporary groups become more conscious of how they may affect the group members' development of swift trust.Originality/value – The hierarchical and detailed model of aspects which contribute to swift trust in leaders of temporary groups is new.
