    • This thesis consists of two separate yet connected studies which investigate how some adult second language speakers of Swedish with various backgrounds reflect on their experiences of language use. The studies are based on the same empirical data, which consists of questionnaires, diaries, essays and observations, but primarily of conversations conducted with a time interval of six years. This data is analysed from a theoretical framework based on Paul Ricœur’s ideas of interpretation, narrative analysis and identity creation, which recur as a leitmotif throughout the thesis.In the first study, data from three second language speakers are analysed by means of the concept of narrative identity, shedding light on the interplay between a static and a dynamic identity. The result is presented in the form of life narratives, which are commented on in relation to those strategies the participants express in relation to their second language use. The analysis shows a development over time moving from defiance to acceptance, from avoidance to flexibility and from planning to simplification. Adult language learning stands out as an arduous and time-consuming process with major consequences for the learner’s ego, a conclusion confirmed and clarified in the metaphor analysis of the second study in which language learning is related to hard work, a constant struggle and a long and strenuous journey. Here it also becomes evident that the participants equate language learning and language use to physical exertion and challenge as well as to individual achievement. This becomes particularly clear in the linked chains of metaphors in which language use is compared with skiing down the steepest slopes or appearing on stage.The participants in both studies are quoted by means of an ethnopoetic method which aims to bring out the creative aspects of their language use and to give second language speakers a voice – a voice which does not need to be corrected and gives the narratives a new dimension.

  • Comprehensive profiling of the vaginal microbiome in HIV positive women using massive parallel semiconductor sequencing

    • Infections by HIV increase the risk of acquiring secondary viral and bacterial infections and methods are needed to determine the spectrum of co-infections for proper treatment. We used rolling circle amplification (RCA) and Ion Proton sequencing to investigate the vaginal microbiome of 20 HIV positive women from South Africa. A total of 46 different human papillomavirus (HPV) types were found, many of which are not detected by existing genotyping assays. Moreover, the complete genomes of two novel HPV types were determined. Abundance of HPV infections was highly correlated with real-time PCR estimates, indicating that the RCA-Proton method can be used for quantification of individual pathogens. We also identified a large number of other viral, bacterial and parasitic co-infections and the spectrum of these co-infections varied widely between individuals. Our method provides rapid detection of a broad range of pathogens and the ability to reconstruct complete genomes of novel infectious agents.

  • Digital support for medication administration : Status-enhancing innovation for care workers?

    • Background: There are assumptions that innovative ICT solutions in healthcare can improve the efficiency and contribute to increased quality. Innovation through ICT is also expected to increase status of and attract men to care work.A project aimed at finding innovative ICT solutions was realized between 2009 and 2012. The project involved a healthcare center and two nursing homes. Technicians (all men) were those in the project given the assignment of finding innovative solutions together with staff. Involved in the project were also ICT companies and academic researchers. This research study was undertaken in a nursing home context. A tool for medication administration was under development and to be tested.Purpose; to describe staff’s perceptions of digital support for medication administration and understand staff’s underlying values when arguing for or against the tool.Data collection; focus-group interviews were carried out to collect data. To analyze and interpret the content of the data, a phenomenographic method was used.Findings; Participants questioned the utility and also the need of the ICT solution. Participants also expressed a risk for impaired working environment as an effect of the tool. They also thought the tool would be complicated to use and sometimes would lack in performance. Increased status as an effect of introducing ICT was a belief that was not held by the staff.Conclusions; Setting aside the fact that from the perspective of an outsider, the intention of the project was indeed to do good , staff did not perceive the ICT solution in a positive manner. Politicians and policymakers may be over emphasizing the possibilities of ICT for solving future challenges in healthcare. Focus on technological innovations and a discourse in which care workers are excluded can actually be contra-productive to the possibilities of enhancing the status of care work, which is still predominantly performed by women

  • Gender mainstreaming in EU's promotion of innovation and clusters

    • In this paper, we elaborate a gendered perspective on entrepreneurship, technology and innovation as implemented in European Union policies promoting innovation and clusters. In this endeavour, we employ Squire's (2005), Walby's (2005) and Rees' (2005) classifications of different kinds of gender mainstreaming. Our study comprises the European Regional Development Fund (ERDF), which provides resources for regions throughout the Union. During the period 2007-2013 priority is given to measures promoting entrepreneurship, development of small and medium-sized enterprises, innovation and clusters. Gender mainstreaming is perceived as a key element in the implementation of ERDF. However, evaluations carried out between 2000-2006 show that there are several obstacles for the integration of a gender perspective in ERDF, e.g. that concepts such as entrepreneurship, innovation, technology and clusters are perceived of as gender neutral and that a gender perspective is not applied when promoting innovation and clusters. Our study investigates how gender mainstreaming is approached in the Operational Programmes (OP) for the period 2007-2013 within the framework of the European Territorial Cooperation Objective supported by the ERDF. Ten of in total 13 programmes supporting transnational cooperation between different European regions are analysed in a gender perspective. The study shows that most OPs lack gender SWOT-analyses and/or an integration of a gender perspective in the overall SWOT analysis for the territorial cooperation areas. Few make any references linking gender equality to economic growth, entrepreneurship, innovation or clusters. Positive actions for women in entrepreneurship, innovation and technology are rare. Gender equality is referred to as a horizontal objective and in relation to non-discrimination, but it is not mentioned in relation to specific objectives and measures. In theory, most of the programmes are open to initiatives supporting entrepreneurship, innovation and clusters in sectors where many women are active, such as for example tourism. But underlying assumptions of innovation and clusters, as well as an extensive focus on high-tech sectors, in practice constitute obstacles for women's participation and potential benefits of the ERDF.

  • Why do smokers diagnosed with COPD not quit smoking? - a qualitative study

    • Chronic Obstructive Pulmonary Disease (COPD) is currently one of the most widespread chronic lung diseases and a growing cause of suffering and mortality worldwide. It is predicted to become the third leading cause of death in the near future. Smoking is the most important risk factor, and about 50% of smokers develop COPD. Smoking cessation is the most important way to improve prognosis. The aim of the study was to describe difficulties of smoking cessation experienced by individuals with COPD who are unable to stop smoking. MethodsTen smokers (five women) with COPD, GOLD stage II, participated in semi-structured interviews in 2010. The data were analyzed using qualitative content analysis. The participants were recruited from the Obstructive Lung Disease in Northern Sweden (OLIN) studies. ResultsThe participants lives were governed by a lifelong smoking habit that was difficult to break although they had knowledge about the harmful effects and the consequences of COPD. The participants described incidents in their lives as reasons for never finding the time to quit smoking. Demands to quit smoking from other people could lead to continued smoking or get them started again after cessation as they did not want to be patronized. They wanted to receive support from relatives and care providers but they wanted to make the decision to quit on their own. ConclusionFor successful smoking cessation, it is important to understand the difficulties smokers are experiencing that influence their efforts to quit smoking. To achieve a successful lasting smoking cessation it might be more effective to first ensure that the smoker has the right internal motivation to make the decision to quit, then assist with smoking cessation.

  • Critical care nurses' experiences of nursing mothers in ICU after complicated childbirth

    • Background: Providing nursing care for a critically ill obstetric patient or a patient who has just become a mother after a complicated birth can be a challenging experience for critical care nurses (CCNs). These patients have special needs because of the significant alterations in their physiology and anatomy together with the need to consider such specifics as breastfeeding and mother-child bonding. Aim: The aim with this study was to describe CCNs' experience of nursing the new mother and her family after a complicated childbirth. Method: The design of the study was qualitative. Data collection was carried out through focus group discussions with 13 CCNs in three focus groups during spring 2012. The data were subjected to qualitative content analysis. Findings: The analysis resulted in the formulation of four categories: the mother and her vital functions are prioritized; not being responsible for the child and the father; an environment unsuited to the new family and collaboration with staff in neonatal and maternity delivery wards. Conclusion and relevance to clinical practice: When nursing a mother after a complicated birth the CCNs give her and her vital signs high priority. The fathers of the children or partners of the mothers are expected to take on the responsibility of caring for the newborn child and of being the link with the neonatal ward. It is suggested that education about the needs of new families for nursing care would improve the situation and have clinical implications. Whether the intensive care unit is always the best place in which to provide care for mothers and new families is debatable. © 2013 British Association of Critical Care Nurses.

    • Background: To be cared for in an intensive care unit (ICU) after a complicated childbirth is often an unplanned and transforming experience, and there is lack of studies describing mothers' experiences of this phenomenon.Aim: The aim of this study was to describe the experiences of becoming a mother after a complicated delivery and a stay in an ICU.Methods: Qualitative personal interviews were conducted with eight mothers. The interview texts were subjected to qualitative thematic content analysis.Findings: The analysis resulted in one theme; wishing to be in control and together as a family, and six categories; being or not being prepared, feeling afraid, not being as ill as the others, knowing about the baby, worrying about the father and having someone to talk to. The findings highlight the need to receive continual information about what is happening, especially with the baby, and the need to be together as a family.Conclusion and relevance to clinical practice: The mothers need support and encouragement from the staff throughout their hospital stay, and sometimes afterwards. There is a need to receive information, especially about the baby, and to have one's family close by, when in an ICU despite illness severity. How the new family is met by the staff is of great importance.

    • Svenska och finska (och finlandssvenska) elevers naturvetenskapliga prestationer uppvisar markanta skillnader i internationella mätningar som PISA. Mot bakgrund av att Finland och Sverige har ett till synes likartat skolsystem är det inte helt enkelt att förstå varför de finska och finlandssvenska eleverna presterar så mycket bättre än de svenska. Vad är det som skapar sådana skillnader? Många olika förklaringar har förts fram, tex i relation till lärarutbildning. Men vilket kemilärande möjliggörs i svenska och finlandssvenska undervisningspraktiker? Kemiinnehållet i läromedel från Sverige och Finland är i stort det samma och i finlandssvenska skolor fram till 2007 användes ofta svenska läromedel. I projektet genomfördes 20-40h videobandade klassrumsobservationer relaterade till kemiundervisningen i tre skolor vardera i Svenskfinland (2007-08) och Sverige (2009). Observationerna, kombinerade med intervjuer och dokumentationer, fokuserade periodiska systemet och kemiska bindningar. De första analyserna ger en bild av att det finns skillnader i innehållets behandling och vad som karaktäriserar de konstituerade undervisningspraktikerna i de båda länderna. I finlandssvenska skolor fokuseras t.ex. faktareproduktion (memorering), formelskrivning och detaljerade provfrågor, samma läromedel används i alla skolor. I svenska skolor betonas förståelse av vardagsfenomen, diskussioner och prov som skiljer på G, VG och MVG-frågor där G-frågor utgörs av enkla faktakunskaper. Undervisningen i de finlandssvenska skolorna framstår således som mera lika varandra medan undervisningen i de svenska skolorna uppvisar större variation gällande innehållets behandling och klassrumskommunikation.

    • Introduction. Video consultation (VC) can improve access to specialist care, especially for individuals who live in rural areas that are long distances from specialist clinics. Aim. The aim of this study was to describe patients’ experiences with specialist care via VC encounters. Method. Interviews were conducted with 26 patients who had participated in a VC encounter. The data were analysed using thematic content analysis. Result. The analysis resulted in two themes. The theme “confident with the technology” was constructed from the categories “possibilities and obstacles in using VC encounters” and “advantages and disadvantages of the technology.” The theme “personal satisfaction with the VC encounters” was constructed from the categories “support from the healthcare personnel,” “perceived security,” and “satisfaction with the specialist consultation.” Conclusion. The patients who did not think that the VC was the best care still considered that the visit was adequate because they did not have to travel. An important finding was that the patients’ perceived even short distances to specialty care as expensive journeys because many patients had low incomes. Among the patients who had more than one VC, the second encounter was perceived as safer. Additionally, good communication was essential for the patient’s perception of security during the VC encounter.

    • Aim The aim of this study was to describe the views of health-care personnel about video consultation (VC) prior to implementation in primary health care in rural areas. BACKGROUND: For people living in rural areas, it is often a long distance to specialist care, and VC could be an opportunity for increased access to care. Therefore, this study was to investigate what views primary health-care personnel had on VC as a working method in the distance between primary and specialist care. The development of technology in society and the introduction of technology in health care mean that the working methods must be adapted to a new approach. It is therefore important that in the initial phase of the introduction of new working methods to capture the personnel views regarding this. METHODS: Focus group (FG) discussions with health-care personnel from five primary health-care centres in northern Sweden. The transcribed FG discussions were analysed with qualitative content analysis. Findings The analysis revealed four main categories: a patient-centred VC; the importance of evaluating costs and resources; new technology in daily work; technology gives new possibilities in future health care.

    • BACKGROUND: Accessibility to specialized health care is important for residents in rural areas. Videoconsultation can provide increased accessibility to health care. Before implementation of new technologies, it is important to study residents' views.OBJECTIVE: This study describes views of residents of rural areas on accessibility to specialist care and the use of videoconsultation as tool to increase accessibility.METHOD: A questionnaire was distributed via the primary health care centres in rural areas of northern Sweden. Data was analysed with both quantitative and qualitative methods.RESULTS: The quantitative analysis revealed three main areas: savings in time, environmental damage and cost of not having to travel were important; security of information must be coupled with increased availability of specialist care; responses depended on an individual's specific health care needs. The open-ended answers resulted into two categories: 1) experiences related to availability of specialist care and 2) views on participating in VC.CONCLUSION: Despite the fact that rural residents considered the distance as important factor in access to care, there was uncertainty regarding videoconsultation as solution. Although respondents were familiar with different types of communication technologies, they probably hesitated because they did not have information on what videoconsultation would mean to them.

    • ObjectivesThe objective of the study was to describe new fathers’ experiences with care related to complicated childbirth.MethodsA qualitative approach consisting of individual interviews using a semi-structured interview guide with open-ended questions was applied. A purposive sample of eight fathers participated. The interview text was subjected to qualitative thematic content analysis.ResultsAnalysis revealed the following three categories: 1) feeling scared and uncared for during acute situations; 2) appreciating the opportunity to participate in care and becoming a family; and 3) needing continued care. Based on these three categories, a recurring theme was identified: struggling to be recognized by care staff as a partner in the family was revealed.ConclusionAlthough fathers lack support and understanding from care staff, they strive to fulfill their roles as fathers by guarding their families and keeping them together. Caregivers involved in the childbirth process should realize that by acknowledging and encouraging fathers in these roles, they in turn support the entire family unit. Interventions developed for fathers and family care requires further development. Additional research concerning how midwives and critical care nurses (CCNs) view the presence of fathers in the emergency situations that may accompany childbirth is also needed.

    • The continuous cast of ethnic and linguistic diversity in negative terms in Swedish schools reveals systematic mismatches between intended policies and everyday classroom practices. In this article, failures in terms of the implementation of language education policy are attributed to languagepolicies not being sufficiently attuned to the sociolinguistic realities in multilingual contexts and at the same time disregarding issues of power and identity. With reference to findings in critical, ethnographic research, it is suggested that strategic essentialism at a general policy level as well as linguicism as a manifestation of prevailing deficit ideologies at an institutional and individual level of practice may contribute to an unresolved tension between the official policy and local educational practices. It is concluded that a number of challenges remain to be addressed before the vision of equal access to linguistic resources in Swedish schools can be realized.

  • Midwives' experiences of sphincter tears

    • Perineal tears are very difficult to prevent, thus one of the midwife's responsibilities during childbirth is to minimise trauma to the perineum. This study examines the experiences of eight midwives who attended births that resulted in a sphincter rupture. Midwives from two maternity clinics in northern Sweden were interviewed using semi-structured questionnaires. Data were analysed using qualitative content analysis and four themes were identified: feeling guilt, shame and failure; working through feelings; needing support from colleagues, and feeling confident with their skills. The category 'striving to be good at her job' was found to be common to all four themes. A midwife may feel guilty and unprofessional when he/she is singled out and criticised. Therefore, in order to reduce these feelings and to further develop professional skills, midwives should be offered mentoring throughout their career

    • Denna forskningsöversikt behandlar forskning om andraspråksundervisning för vuxna invandrare. Här presenteras ett brett panorama som inkluderar andraspråksundervisningens koppling till fakta om vuxnas andraspråksinlärning i ett individ- och samhällsperspektiv, lärande och språkanvändning i olika kontexter samt språkdidaktiska perspektiv. De aspekter på andraspråksinlärning som tas upp här är specifikt relaterade till kritiken av sfi i dess roll som integrationsinstrument, arbetsmarknadsinstrument och kvalificerad språkundervisning. Det påvisas att slitningar omkring sfi, vilka har sin grund i en marknadsanpassad agenda och bristande samsyn mellan aktörer på nationell och kommunal nivå, motverkar möjligheterna att utforma en kvalificerad språkutbildning som är specifikt anpassad till de studerandes starkt varierande individuella förutsättningar. En professionalisering av området är beroende av en relevant lärarutbildning och kompetensutveckling för nu verksamma lärare. Forskningen omkring andraspråksinlärning och andraspråksanvändning utanför klassrummet blottlägger ett behov av ökad kunskap om språkbruk och språkets roll i dagens alltmer komplexa och flerspråkiga arbets- och samhällsliv. De flesta yrken ställer krav på högstvarierande produktiva och receptiva språkfärdigheter. Även om språkundervisningens roll i olika sammanhang ifrågasätts visar forskningen att enbart kontakt med och användning av språket i kommunikation inte leder till en optimal utveckling och en tillfredsställande slutlig nivå. Det finns därför inom forskningen en relativt stor enighet om behovet av undervisning och ett medvetet fokus på språklig form och grammatik för att språkutvecklingen inte ska avstanna på en alltför elementär nivå. Även synen på vilken metod/pedagogik som bäst leder till en ändamålsenlig och framgångsrik andraspråksundervisning har varierat över tid. Här kan utvecklingen beskrivas som en rörelse från metodbaserad pedagogik via postmetodisk pedagogik till s.k. kritiska angreppssätt och en strävan efter en funktionell och samhällsrelevant undervisning. Forskningen har också bekräftat att en enskild metod inte generellt kan sägas vara överlägsen en annan eftersom undervisning är en dynamisk, interaktionell och högst kontextberoende process, i vilken lärarens egna värderingar, attityder och förhållningssätt spelar en avgörande roll.

The causality between creativity, knowledge development and innovation ascribes creative knowledge environments an important role as work settings in which people produce new knowledge. The Creative Knowledge Environment approach reaches beyond the Triple Helix model—promoted in innovation policy and research—in its potential to acknowledge creative environments outside academia, industry and government. This is a relevant contribution since the public funding of Triple Helix constellations has been criticised for paradoxically consolidating old structures rather than opening up for creative change. In this article, the model of Women Resource Centres (WRCs)—developed in Sweden and internationalized throughout Europe—serves as an example of how creativity in the organization of joint action networks can make new knowledge and innovation prosper. These centres were initiated in order to promote gender equality in regional development policy, enhancing women’s realization of business ideas and innovations. The model of Women Resource Centres illustrates the need for further development of predominant models for promoting innovation. Suggested concepts such as Creative Knowledge Environments and Quadruple Helix have the potential to increase the diversity of actors and areas being acknowledged as important in the expanding knowledge economy, by including the civil society and creative industries. However, the example of WRCs exposes an aspect of creativity and innovation that is not properly addressed in the suggested models, namely the aspect of gender.

  • Emerging educational action? Diploma project in Maritime higher education

    • This paper is framed within a 3-year R&D-project at the Åland University of Applied Sciences (ÅUAS), aiming at changing the current practice related to the final diploma project (a thesis of 15 credit points) for the bachelor degree. A survey preceding the project showed that a majority of students within the Maritime programme never completed their studies. This has become a problem on three levels: firstly, the individuals experience failure in their studies and thus they don’t get the job they where aiming at by attending the education. Secondly, on the organisational level the student drop-out is defined as a quality problem. Thirdly, the shipping industry cannot, due to international security regulations, employ these students as anything but seamen. Well aware of the problem, ÅUAS had already made efforts to improve their education. Rhetorically, the thesis (although a problem) was considered an important part of the education. Based on an activity theoretical perspective the diploma project was presented as an educational action central to the Maritime programme (Engeström, 1987; Leont’ev, 1978). Being described as a central action the problems can be seen as somewhat contradictory (Engeström, 1987).The R&D-project, here presented, was initiated by ÅUAS. A short-term aim was to help this group of students to complete their thesis and thereby finishing their studies whit a bachelor degree as master mariners. The long-term aim was to identify needs and initiate changes in the educational practice.The project started with an investigation aiming at mapping the problem from the perspective of central actors. Teachers and management ascribed the problem as related to academization of the education, resulting in a diploma project “of no use” for students’ future career as master mariners on the one hand, and in students’ “lack of motivation and laziness” on the other (Berthén, Eriksson & Lindberg, 2006; Haggis, 2003; Hagström, 2005; Mulligan & Kirkpartick, 2000).Interviews with students, however, gave a different picture, indicating that the diploma project was peripheral to the programme. Students were scheduled for the diploma project during three semesters in total, but mostly for random free periods and a few occasional days. Further, these periods and days were cancelled when time was needed for something else. Also, it was common that students had no idea of who their supervisor was and no supervision was scheduled. According to the students, asking for help meant increasing the burden of the teachers.Based on these findings, intervention studies on three levels were designed: hands-on teachers’ supervision work (intervention in action), in-service course on diploma projects and supervision as part of polytechnics/professional higher education (intervention for action), and organising for diploma project (organisational level, intervention for action).The issue of this paper is to discuss the indications of change that we have identified at the end of the project. The preliminary result shows that teachers’ conceptions of the diploma project have changed. This is shown in their talk and actions when meeting the students. Both those enrolled in the project and those that still are participating in the education. The diploma project is now seen as a possibility instead of as a problem. In addition, the teachers’ have started to address the management with requests of time – scheduled supervision with students, time for teachers to develop their supervision collaboratively. Based on these indicators, we argue that the project has contributed to the diploma project have (at least partly) becoming a central educational action. Instead of leaving the students to their fate when it comes to accomplishing a thesis – on their spare-time and beside the regular programme – it seems to becoming a part of the programme and supervision something more than certifying whether the students have managed on their own or not.

  • What makes a teaching practice an epistemic one? : Examining a mathematicteaching project

    • This presentation is related to the praxis-developmental collaborative research project“Farstaprojektet” conducted 2004-2006. The project was developed as a collaborative projectwith teachers, educational researchers and mathematical researchers. The aim for the projectwas to explore the meaning of the goals to strive for (~subject specific qualities of knowing)stated in the National mathematic syllabus, in order to design and try out experimentalteaching related to these goals, i.e. a capability to solve mathemati-cal problems. The dataused in this presentation was produced during the second project year. The content chosen wasequation, since the teachers experienced that students had difficulties to master equations asa method in problem solving assignments. The collabo-rative work was divided into to parts.Firstly, we analysed mathematical textbooks in or-der to explore which assignments actuallyrequested equation as a method for solving them. The result was that most assignments wereeasier to solve using other methods. Secondly, the collaborative work was directed towardsdesign of experimental teaching. In this work we drew upon Davydov’s ideas of Developmentalteaching. In relation to this we aimed at developing assignments that, in their construction,require these capabilities here a confidant use of equations. This type of assignments wenamed key-assignments or epistemic assignments. In this paper we describe teachers’ use ofone assignment, which, according to our judgement, had the potential of becoming a keyassignment.A conclusion is that assignments cannot be designed as epistemic assignments;instead they become (or will not become) epistemic as a consequence of the teaching practice.

The Swedish fieldtrial includes 700 patients at four healthcare centres in Norrbotten. Patients with Type 2 Diabetes and CardioVascular diagnoses utilize new eHealth products and services. The new applications are provided througha national Patient Portal available for all Swedish citizens.

    • Purpose - To describe staffs’ perceptions of digital support for medication administration (DSM) and out of the perceptions interpret underlying values. Design/methodology/approach - Twenty-two persons working in elder-care participated in the study. The study had a qualitative approach and focus-group interviews were used to collect data. To analyze the manifest content a phenomenographic method was used. An interpretation of perceptions was then undertaken aimed at identifying underlying values. Findings - Three descriptive categories, "Utility", "Impact on working environment" and "Economic impact" were the result of the manifest analysis. The values of having a "Good working environment", "Benefits" and "Good economy" were interpreted as guidance for staffs’ acceptance or rejection of the DSM.Originality/value - This study had a twofold approach with the intention of going beyond descriptions. To gain a deeper understanding a normative interpretation was completed. Ethical conflicts are frequently characterized as conflicts between at least two values. In this study staffs expressed fear of losing prerequisites needed to perform their work well. Prerequisites that were identified as values and these values were threatened by the DSM

    • This study was undertaken to describe staff expectations prior to implementation of new electronic applications in a changing organization. Changes are a part of human existence; changes based on implementation of technology and information and communication technology are taking place in the health care sector globally. The Swedish public health care and social care sector is taking a similar path. A qualitative approach with group interviews of 23 staff divided in 5 groups was performed. Latent content analysis was used to analyze the transcribed interviews. The theme, taking standpoint today in relation to the past, emerged from 3 areas of discussion: "distance holding," "ruled by the organization," and "health care development in the future." New restraints on staff affect the caregiving process. Managers should consider whether a particular change is revolutionary or evolutionary and act in the change process according to the possible psychological impact of the change.

Interviews were recorded, transcribed, and then analysed using qualitative content analysis.Findingsfour categories were identified: to consider and accept the decision; to lack understanding about the abortion procedure; to be in need of support and information; to have memories for life. Findings show that information and support during the whole abortion process is important. Women found it difficult to make the decision and going through abortion left memories for life.Conclusioninformation and support is of great importance for women in this vulnerable situation. The need for further support points out the need to have follow-up contacts with women after an induced second trimester abortion.

    • ObjectiveTo describe midwives' experience of encountering women with posttraumatic stress symptoms after childbirth.Study designSemi-structured interviews with eight midwives with experiences of encountering women with posttraumatic stress symptoms after childbirth. Data were analyzed using thematic content analysis.ResultThe midwives encountered women with severe experiences after a childbirth, which had affected their lives. The midwives thought that they had to reflect on their own attitude and use their gentleness as they responded to the women's stories. By listening to and enabling the women to express their feelings and tell their stories, the midwives thought that they could confirm the women's experience. The midwives thought that they could support women who had lost confidence to give birth but also thought that cesarean section did not solve the women's problems.ConclusionTraining of midwives is necessary to increase awareness of the cause of women's posttraumatic stress symptoms. It is urgent to optimize the implementation of new research into practical reality to raise the quality of care of women with fear of childbirth. Childbirth is an important experience that can influence a woman's well-being and her connection to the child. Posttraumatic stress symptoms should be identified in an early stage/pregnancy to allow early treatment on the right level of care.
