    • Förord av Björn Brorström, Prorektor Högskolan i Borås Ett professionslärosäte bedriver utbildning och forskning på vetenskaplig grund. Vetenskapliga metoder tillämpas och framkomna resultat prövas för att säkra tillförlitlighet och relevans. Förmågan att problematisera, formulera frågor och välja lämpliga metoder för att besvara frågor är egenskaper för god forskning och utmärkande för en stark utbildnings- och forskningsmiljö. Vid ett professionslärosäte finns samtidigt en stark strävan efter att i utbildning och forskning ta till vara den kunskap och de erfarenheter som professioner besitter och därigenom säkra en hög grad av praktisk relevans. Växelverkan mellan teori och praktik och erfarenhetsutbyte mellan forskare och praktiker är ständigt pågående. Det finns ingen avslutning på processen utan den pågår utan avbrott. En nödvändig förutsättning för ett fruktbart samarbete är en ömsesidig respekt och vilja att stödja och ifrågasätta varandra. Forskaren måste vara beredd på kritik för bristen på praktisk relevans och professionsföreträdaren måste vara beredd på att förtrogenhetskunskapens lämplighet ifrågasätts. Det är en fråga om med- och mothåll från båda håll i syfte att långsiktigt utveckla teori och praktik. Det är en utmaning att utveckla, etablera och därefter kontinuerligt försvara och förbättra professionslärosätet. Ett led i utvecklingen är att inom ramen för Högskolan i Borås föra en debatt om innebörden av ett professionslärosäte och vad vi behöver göra för att förbättra verksamheten. På uppdrag av rektor Lena Nordholm har medarbetare vid Bibliotekshögskolan utvecklat och ansvarat för en seminarieserie benämnd Profession och vetenskap. Seminarierna spände över ett brett fält. En av många viktiga frågor som diskuterades flitigt var huruvida vi ska forska om professioner, i för eller med, eller kanske alltihop samtidigt. Bidragen publiceras nu i denna antologi som ges ut inom ramen för Högskolan i Borås rapportserie Vetenskap för profession. Rapporten Profession och vetenskap – idéer och strategier för ett professionslärosäte, som redigerats av universitetsadjunkt Maria Lindh, kommer att få stor betydelse inom lärosätet och i vårt samarbete med företrädare för näringsliv, kulturliv och offentlig verksamhet. Jag ser det som en intressant fortsättning att anordna ett antal seminarier med inbjudna representanter för professioner där artiklarna i denna rapport kan ligga till grund för erfarenhetsutbyte och diskussioner.

    • Denna skrift om Svedala omfattar tre delar. Den första delen "Svedala igår" är en historisk översikt som beskriver Svedalas tillväxt framför allt från mitten av 1800-till början av 1900-talet. Då skedde den omvälvande ut­vecklingen i samband med järnvägarnas tillkomst och in­dustrins etablering. Den andra delen "Svedala idag" beskriver det nutida Sve­dala, dess bebyggelse och miljö, allt från gatu- och torgmiljöer till husdetaljer. I hus och detaljanalyser visar vi det typiska för epokens byggande vilket också idag präglar stora delar av Svedalas byggnadsbestånd. Med Svedalas historia och bebyggelse som bakgrund disku­terar vi i en tredje del "Svedala imorgon" Svedalas fram tid. Vi formulerar ett antal enkla teser med råd och an­visningar på plan- respektive husnivå till stöd för Sve­dalas bebyggelsemiljö nu och i framtiden.

  • Pimpa texten : En etnografisk studie av gymnasieelevers meningsskapande i dramatext

    • This ethnographic study is located in the field of literary didactics, covering an intersection of the subjects of Swedish and theatre. It investigates construction of text understanding while its participants work with staging Molière’s The affected ladies (Les précieuses ridicules, 1659). Studies of classroom interaction connected to reading drama text are rare in the Swedish field of literary didactics. The main research interest is the participants’ use of interwoven semiotic resources, including spoken language, body, voice and various artifacts. Based on a general need to understand how Swedish students develop reading skills (related to decreasing results internationally) and a growing research interest in the field for embodied knowledge connected to literacy competencies the study sets out to answer the following questions: How do the participants construct text understanding using semiotic resources with a focus on: a) matching of repertoires b) in-role re-presentation c) negotiations. The first aspect draws on theories used in literary didactics including McCormick’s (1994) literary and general repertoires and Langer’s (1995) envisioning literature. The second aspect draws on aesthetic learning and embodied knowledge  (Molander, 1996; Saar, 2005) and theatre semiotics (Heed, 2002). The third aspect draws on sociocultural theory (Vygotskij, 1978; Säljö, 2014) and sociocultural theory applied in the field of drama and theatre (Davis, Clemson & Ferholt, 2015). Seven upper secondary school students, a teacher of Swedish and a teacher of theatre were observed over a period of seven months, starting with the students’ first encounter with the drama text, lasting to the final performance of a one-hour theatre production. The researcher occasionally became participant in the creative process. Analyzed data include field notes, video and sound recordings. Major findings are that: a) Students continuously match their own repertoires with the repertoires of the drama text by references to popular culture, language and body expressions, thereby gradually constructing new understanding. b) By becoming co-creators of a fictional text in a process of aesthetic learning the students developed several perspectives on the literary text, which is one of the main goals in studies of literature in the subject of Swedish. c) The participants’ negotiations involving a number of semiotic resources showed a development of text understanding on an advanced level. It also showed knowing as something emerging out of social interaction where teachers play an essential role by providing aesthetic as well as traditional scaffolding. A central conclusion is that exploring ways of learning where various semiotic recourses are given focus in the process of shaping a final product, e.g. a theatre show, host potential for students’ development of text understanding. The potential for learning seems to relate to producing knowledge rather than reproducing. Verbal and physical expressions of text understanding form new understanding in an on-going spiral. A contribution of the study to the field is showing the potential of embodied knowledge in literary didactics without a master-servant relationship between school subjects. It is suggested that increased focus on embodied knowledge and aesthetic learning might help students to develop reading skills.

  • Individually tailored treatment with epirubicin and paclitaxel with or without capecitabine as first-line chemotherapy in metastatic breast cancer: a randomized multicenter trial

    • Anthracyclines and taxanes are active cytotoxic drugs in the treatment of early metastatic breast cancer. It is yet unclear whether addition of capecitabine to the combination of these drugs improves the treatment outcome. Patients with advanced breast cancer were randomized to first-line chemotherapy with a combination of epirubicin (Farmorubicin(A (R))) and paclitaxel (Taxol(A (R))) alone (ET) or in combination with capecitabine (Xeloda(A (R)), TEX). Starting doses for ET were epirubicin 75 mg/m(2) plus paclitaxel 175 mg/m(2), and for TEX epirubicin 75 mg/m(2), paclitaxel 155 mg/m(2), and capecitabine 825 mg/m(2) BID for 14 days. Subsequently, doses were tailored related to side effects. Primary endpoint was progression-free survival (PFS); secondary endpoints were overall survival (OS), time to treatment failure (TTF), objective response (OR), safety and quality of life (QoL). 287 patients were randomized, 143 to ET and 144 to TEX. Median PFS was 10.8 months for patients treated with ET, and 12.4 months for those treated with TEX (HR 0.84, 95% CI 0.65-1.07, P = 0.16); median OS was 26.0 months for women in the ET versus 29.7 months in the TEX arm (HR 0.84, 95% CI 0.63-1.11, P = 0.22). OR was achieved in 44.8% (ET) and 54.2% (TEX), respectively (chi(2) 3.66, P = 0.16). TTF was significantly longer for patients treated with TEX, 6.0 months, versus 5.2 months following ET (HR 0.73, 95% CI 0.58-0.93, P = 0.009). Severe hematological side effects related to epirubicin and paclitaxel were evenly distributed between the treatment arms, mucositis, diarrhea, and Hand-Foot syndrome were significantly more frequent in the TEX arm. Toxicity-adjusted treatment with ET and TEX showed similar efficacy in terms of PFS, OS, and OR. In this trial with limited power, the addition of capecitabine to epirubicin and paclitaxel as first-line treatment did not translate into clinically relevant improvement of the outcome.

    • The aim of the article is to demonstrate and discuss a method for analyzing reviews, emphasizing the emotional reactions and personal attempts by reviewers to position themselves. Paying attention to such structures is especially important during periods of intense aesthetic and ideological struggles in the field of theatre. The documents used in the study are three reviews of the opening night (15 October 1883) of Sanna kvinnor (True Women), by the Swedish playwright Anne Charlotte Leffler. The reviews constitute an interesting case, as both the play and the performance broke with dramaturgical conventions and strongly criticized the gender-ideology of the era, at a time when the advocates of idealism exercised hegemony and were challenged by various anti-idealist opponents.

    • Anne Charlotte Leffler’s play Elfvan (The Elf) (1880) depicts a young woman’s first alienating experience of marriage. The motif of the young woman’s difficulties to fit the role as the governor’s wife in a small Swedish town is connected to the motif of her growing up, including her first sensation of sexual desire. The aim of the article is to illuminate how the embodied experience of a young bourgeois woman is created in the play. The main character’s emotions and bodily sensations are highlighted. The analysis also deals with narration of the young girl’s desire to expose her uniqueness for others to recognize. It furthermore sheds light on the depiction of the emancipating power of the young girl’s access to her biography, which contributes to the widening of her narrow social space as a young bourgeois wife. In so doing Iris Marion Young’s and Adriana Cavarero’s different angles to feminist phenomenology are combined.

    • Alfhild Agrell, Victoria Benedictsson och Anne Charlotte Leffler var 1880-talets feministiska dramatiker på modet. Deras pjäser var populära på teatrar i hela Skandinavien. Samtidigt tillhörde de sin tids litterära avantgarde. I litteratur- och teaterhistorien har pjäserna benämnt efterklangsdramatik, dockhemslitteratur, indignationsdramer och tendensdramatik. I Som en vildfågel i en bur utgår Birgitta Johansson Lindh från en annan historiografi. Dramerna frigörs från sådana genrebestämningar och en ny läsart föreslås. I monografin visas hur melodramatiska inslag, som tillhörde tidens dramakonventioner, är del av en kompositionsteknik som gestaltar kvinnors känslomässiga och kroppsliga erfarenheter. Dessa erfarenhetsberättelser bidrar både till en genuskritik och till dramaturgiska normbrott i förhållande till den estetiska idealism som fortfarande på 1880-talet dominerade på teatrarna. De skapar ett ambivalent förhållningssätt till normen som möjliggör öppna tolkningar i förhållande till den estetiska idealismens moral- och konstsyn. Dramernas poetik mejslas fram genom jämförelser med annan 1800-talsdramatik. Genusteori med rötter i Hannah Arendts politiska och Simone de Beauvoirs fenomenologiska existentialism utgör det teoretiska fundamentet för analys och tolkning av de genuskritiska erfarenhetsberättelserna och deras alternativ till den rådande ordningen.

  • Anal cancer in Sweden 2015-2019. Implementation of guidelines, structural changes, national registry and early results

    • Background Squamous cell cancer of the anus is an uncommon malignancy, usually caused by human papilloma virus (HPV). Chemoradiotherapy (CRT) is the recommended treatment in localized disease with cure rates of 60-80%. Local failures should be considered for salvage surgery. With the purpose of improving and equalizing the anal cancer care in Sweden, a number of actions were taken between 2015 and 2017. The aim of this study was to describe the implementation of guidelines and organizational changes and to present early results from the first 5 years of the Swedish anal cancer registry (SACR). Methods The following were implemented: (1) the first national care program with treatment guidelines, (2) standardized care process, (3) centralization of CRT to four centers and salvage surgery to two centers, (4) weekly national multidisciplinary team meetings where all new cases are discussed, (5) the Swedish anal cancer registry (SACR) was started in 2015. Results The SACR included 912 patients with a diagnosis of anal cancer from 2015 to 2019, reaching a national coverage of 95%. We could show that guidelines issued in 2017 regarding staging procedures and radiotherapy dose modifications were rapidly implemented. At baseline 52% of patients had lymph node metastases and 9% had distant metastases. Out of all patients in the SACR 89% were treated with curative intent, most of them with CRT, after which 92% achieved a local complete remission and the estimated overall 3-year survival was 85%. Conclusions This is the first report from the SACR, demonstrating rapid nation-wide implementation of guidelines and apparently good treatment outcome in patients with anal cancer in Sweden. The SACR will hopefully be a valuable source for future research.

    • Objective:To evaluate treatment outcome in a large population-based cohort of patients with anal cancer treated according to Nordic guidelines.Material:Clinical data were collected on 1266 patients with anal squamous cell carcinoma diagnosed from 2000 to 2007 in Sweden, Norway and Denmark. 886 of the patients received radiotherapy 5464 Gy with or without chemotherapy (5-fluorouracil plus cisplatin or mitomycin). according to different protocols, stratified by tumor stage.Results:High age, male gender, large primary tumor, lymph node metastases, distant metastases, poor performance status, and non-inclusion into a protocol were all independent factors associated with worse outcome. Among patients treated according to any of the protocols, the 3-year recurrence-free survival ranged from 63% to 76%, with locoregional recurrences in 17% and distant metastases in 11% of patients. The highest rate of inguinal recurrence (11%) was seen in patients with small primary tumors, treated without inguinal irradiation.Conclusions:Good treatment efficacy was obtained with Nordic, widely implemented, guidelines for treatment of anal cancer. Inguinal prophylactic irradiation should be recommended also for small primary tumors. (C) 2014 Elsevier Ireland Ltd. All rights reserved.

    • The aim of this study was to compare the frequency of human leukocyte antigen (HLA) genotypes in 1-18-year-old patients with type 1 diabetes newly diagnosed in 1986-1987 (n = 430), 1996-2000 (n = 342) and in 2003-2005 (n = 171). We tested the hypothesis that the HLA DQ genotype distribution changes over time. Swedish type 1 diabetes patients and controls were typed for HLA using polymerase chain reaction amplification and allele specific probes for DQ A1* and B1* alleles. The most common type 1 diabetes HLA DQA1*-B1*genotype 0501-0201/0301-0302 was 36% (153/430) in 1986-1987 and 37% (127/342) in 1996-2000, but decreased to 19% (33/171) in 2003-2005 (P \ 0.0001). The 0501-0201/0501-0201 genotype increased from 1% in 1986-1987 to 7% in 1996-2000 (P = 0.0047) and to 5% in 2003-2005 (P > 0.05). This study in 1-18-year-old Swedish type 1 diabetes patients supports the notion that there is a temporal change in HLA risk.

  • A Complicit Reading Strategy: Exposing Censored Themes of Intimacy in Swedish Alfhild Agrell's Räddad

    • This article proposes an ethical reading position by taking the author’s socio–historic censorious situation into account and by adopting a complicit reading strategy in order to expose silenced themes of intimacy. It does so through textual references to the 1883 play Räddad [Saved] by the Swedish female playwright Alfhild Agrell. By taking the point of departure in Helen Freshwater’s idea of an inclusive model of censorship, Judith Butler’s theory of reiteration, and Toril Moi’s claims of the political potential of a reading mode starting in the concerns of the text, I establish the theoretical basis for a complicit reading strategy. I then present the gendered period-specific censorious situation of the 1880s in Sweden, in which the aesthetics of idealism influenced censorship in the theatres and also regulated playwriting. I demonstrate a vacillation between self-censoring and exposing themes of intimacy in Agrell’s play, proposing that this should be regarded as a specific rhetoric produced by an awareness of gendered censorship. Finally, by leaning on phenomenological and new materialist instruments of analysis, I present an embodied reading strategy for Agrell’s play in response to its specific individual poetics and concerns, to expose its full gender/feminist potential.

    • Swedish Alfhild Agrell and Anne Charlotte Leffler were contemporaries of Henrik Ibsen and August Strindberg. In the 1880s their plays were frequently staged at Scandinavian Theatres and translated into several European languages. Agrell’s and Leffler’s successes as playwrights did not last for very long though; by the early Twentieth century their plays were being judged as low quality, recognised as simply constructed melodramatic pieces of indignation and frustration. In my analyses of Agrell’s play Räddad (1883) and Leffler’s Elfvan (1883) however, the point of departure is from the very heart of that labelling of indignation, namely the melodramatic traits and their emotional effects. I reclaim the label, by showing that the melodramatic elements are vital components in a dramaturgy representing female experience of intimacy. I propose that the melodramatic and sentimental elements construct a phenomenological mimesis of the experience of the ‘body-in-situation’. By deploying melodramatic elements, the reactions, feelings and emotions shaped by the protagonists’ subordinated social position are dramatized. The analysis shows how these dramatizations are structured to criticise established gender norms and to advocate a widening of women’s agency. Furthermore, it elucidates how these playwrights, by giving voice to women’s experiences of intimacy, re-negotiate the dramaturgical norms of 19th century theatre.

    • Anne Charlotte Leffler’s Sanna Kvinnor (True Women 1883) is one of many gender-critical plays by Swedish women playwrights during the 1880s which linger in the shadows of Henrik Ibsen’s A Doll’s House. Sanna kvinnor certainly asks for a change of the prevailing gender norms in the 1880s, in other words, for a transformation into a better world, but how was this utopian drive staged at the theatres and how was it received by the theatre critics? By taking the theoretical point of departure in Slavoj Žižek’s notion of a radical utopia, the aim of my article is to illuminate the reviewers’ perception of the stagings in the first production at the Royal Dramatic Theatre in 1883 and in Albert Ranft’s touring production 1884–86. My archival sources mainly consist of reviews from which both the stagings and the reviewers’ reactions will be traced through a systematic method developed from the semiotics of Jurij Lotman.

  • Response prediction and treatment tailoring for chronic hepatitis C virus genotype 1 infection

    • We monitored early viral response during the treatment of hepatitis C virus (HCV) infection with the aim of identifying predictors of treatment outcome. We studied 53 patients with genotype 1 infection who received 180 microg/week pegylated interferon alfa-2a and 1,000 or 1,200 mg/day ribavirin depending on body weight and serially assessed HCV RNA in serum, using the Cobas TaqMan assay. Thirty-one patients (58%) achieved sustained viral response (SVR). SVR was obtained in 100% (10/10) of patients with pretreatment viremia concentrations below 400,000 IU/ml, in 100% (14/14) of patients with more than 1.5 log reduction of HCV RNA after 4 days of treatment, and in 95% (22/23) of patients with a rate of decline in viremia higher than 0.70 log units/week during the second phase. Non-SVR was seen in all patients with a second-phase decline rate lower than 0.35 log units/week. Patients with slopes between 0.50 and 0.80 log units/week achieved SVR (4/4) unless the treatment dose was modified (3/3). We conclude that the second-phase slope appears to be an accurate and useful predictor of treatment response. On the basis of these findings, we propose a model of tailored treatment which takes into account the second-phase slope and the amount of HCV RNA after 21 days of treatment.

    • The issue of this thesis concerns a selection of historical debates in which new Swedish drama is under discussion. The studied debates take place in the cultural and political fields and within the fields of theater and literature and deal with a recurring assumption in Swedish theatre history – that new Swedish drama is insufficient. The primary object of this thesis is to find explanations to: why is the Swedish new drama so often described as defective? The following questions, guiding the analysis, are: How are the crises described? What are the stakes? How has the dramatic text been influenced by being judged either as literary product or a product for the stage? How is the playwright’s role described, and perhaps changed, in the crises? The aim of the analysis is to understand how traditions and conventions are shaping the debates and contribute to perpetrate the myth of the malfunctioning Swedish new play.In a historical perspective several attempts have been made to govern new Swedish drama by legislative and political power. New Swedish drama has, for example, been viewed as a possible expression of the nation, as part of shaping the Swedish Welfare state or creating interactive communication with the audience. Despite its many uses, new Swedish drama continues to be describes as flawed.The study starts with King Gustav III:s Swedish theatre where the purpose was to produce Swedish original plays. The study ends with an analysis of a new government grant for new Swedish drama, which was installed in 1999. The chosen debates are analyzed with the help of concepts borrowed from the French sociologist Pierre Bourdieu, looking at each historical situation as a possible moment for the establishment of the field ”new Swedish drama”. The survey ends with eight interviews with playwrights, who are active today.The conditions for the new Swedish drama are the guiding line in this thesis. These conditions are found in the cultural, social and historical contexts that cooperate when a taste or convention is being shaped. They are part of the discourses in the field, where criteria for the new Swedish drama is formulated. In order to understand the significance of, for example, the expression, ”the newly written Swedish drama” research has been pursued in biographical material, historical surveys, and debates in the daily press and in professional journals.Without being a full bourdieuan analysis, the thesis is using concepts from Bourdieu. The work of British feminist theatre historian Tracy C Davis inspires the critical historic perspective.

    • In the NORDIC-7.5 trial, cetuximab administered every 2 weeks in 152 patients with Kirsten rat sarcoma viral oncogene homolog wild type metastatic colorectal cancer was investigated. After 4 months of induction therapy with Nordic FLOX (oxaliplatin, bolus 5-fluorouracil, and bolus folinic acid) and cetuximab, patients continued with cetuximab monotherapy. Cetuximab might have added to a longer chemotherapy-free interval, but needs to be validated in clinical trials. Background: In the NORDIC-7.5 trial, how cetuximab might safely and conveniently be added to an intermittent treatment strategy in patients with prospectively selected Kirsten rat sarcoma viral oncogene homolog wild type (KRASwt) metastatic colorectal cancer (mCRC) was investigated. Patients were treated in a multicenter phase II trial with cetuximab in combination with the Nordic bolus FLOX (oxaliplatin, 5-fluorouracil, and folinic acid) for 4 months followed by maintenance cetuximab. Patients and Methods: Patients had KRASwt, nonresectable mCRC, no previous chemotherapy, and Eastern Cooperative Group performance status of 0 to 2. Patients received 8 courses of Nordic FLOX (oxaliplatin 85 mg/m(2) over 1 hour on day 1, and 5-fluorouracil 500 mg/m(2) as a bolus injection, followed 30 minutes later with bolus folinic acid 60 mg/m(2) on days 1 and 2). Cetuximab was administered every 2 weeks at a dose of 500 mg/m(2) for 16 weeks followed by cetuximab as maintenance therapy until disease progression. Results: Between July 2008 and September 2010, 152 KRASwt patients were included. The response rate was 62% (95% confidence interval [CI], 54%-69%), median progression-free survival was 8.0 months (95% CI, 7.5-8.9) and median overall survival was 23.2 (95% CI, 18.1-27.4) months. Twenty-one patients (14%) had later R0-resection of metastasis. FLOX with cetuximab was reintroduced in 47 of 85 patients (55%). The most common Grade 3/4 nonhematologic adverse events were diarrhea in 14 patients (9%), skin rash in 13 patients (9%), infection without neutropenia in 11 patients (7%), and fatigue in 11 patients (7%). Conclusion: In a prospectively selected KRASwt population, biweekly cetuximab was safely integrated in an intermittent chemotherapy strategy and might have added to a longer chemotherapy-free interval. However, the combination of biweekly cetuximab with chemotherapy needs to be validated in trials using FOLFOX (oxaliplatin, fluorouracil, and leucovorin) or FOLFIRI (irinotecan, fluorouracil, and leucovorin).

    • BACKGROUND:Infections of the central nervous system (CNS) with herpes- or enterovirus can be self-limiting and benign, but occasionally result in severe and fatal disease. The polymerase chain reaction (PCR) has revolutionized the diagnostics of viral pathogens, and by multiple displacement amplification (MDA) prior to real-time PCR the sensitivity might be further enhanced. The aim of this study was to investigate if herpes- or enterovirus can be detected in cerebrospinal fluid (CSF) from patients without symptoms.METHODS:Cerebrospinal fluid (CSF) samples from 373 patients lacking typical symptoms of viral CNS infection were analysed by real-time PCR targeting herpesviruses or enteroviruses with or without prior MDA.RESULTS:In total, virus was detected in 17 patients (4%). Epstein-Barr virus (EBV) was most commonly detected, in general from patients with other conditions (e.g. infections, cerebral hemorrhage). MDA satisfactorily amplified viral DNA in the absence of human nucleic acids, but showed poor amplification capacity for viral DNA in CSF samples, and did not increase the sensitivity for herpes virus-detection with our methodology.CONCLUSIONS:Viral pathogens are rarely detected in CSF from patients without signs of CNS infection, supporting the view that real-time PCR is a highly specific method to detect symptomatic CNS-infection caused by these viruses. However, EBV may be subclinically reactivated due to other pathological conditions in the CNS.

  • Impact of the first tumor response at eight weeks on overall survival in metastatic breast cancer patients treated with first-line combination chemotherapy

    • The aim of this was to determine whether the change of size observed at the first response evaluation after initiation of first-line combination chemotherapy correlates with overall survival (OS) in patients with metastatic breast cancer (MBC). The change in size of tumors derived from measurements according to Response Evaluation Criteria In Solid Tumors (RECIST) at the first evaluation on computed tomography (CT) was obtained from a multicenter, randomized phase III trial ("TEX trial," n = 287) comparing treatment with a combination of epirubicin and paclitaxel alone or with capecitabine (TEX). Cox regression and Kaplan-Meier analyses were performed to evaluate the correlations between the first change in tumor size, response according to RECIST and OS. Data from CT evaluations of 233 patients were available. Appearance of new lesions or progression of non-target lesions (new/non-target) indicated short OS by univariable regression analysis (HR 3.76, 95 % CI 1.90-7.42, p andlt; 0.001). A decrease by andgt;30 % at this early time point was prognostic favorable (HR 0.69, 95 % CI 0.49-0.98, p = 0.04) and not significantly less than the best overall response according to RECIST. After adjustment for previous adjuvant treatment and the treatment given within the frame of the randomized trial, OS was still significantly shorter in patients with new/non-target lesions after a median 8 weeks of treatment (HR 4.41, 95 % CI 2.74-7.11, p andlt; 0.001). Disease progression at the first evaluation correlates with OS in patients with MBC treated with first-line combination chemotherapy. The main reason for early disease progression was the appearance of new lesions or progression of non-target lesions. These patients had poor OS even though more lines of treatment were available. Thus, these factors should be focused on in the response evaluations besides tumor size changes.

    • Aim: To assess the efficacy and safety of epirubicin, capecitabine and cisplatin (EXC) combination therapy in locally advanced breast cancer (LABC) and investigate the predictive value of selected biomarkers. Methods: Newly diagnosed LABC patients received four 3-weekly cycles of neoadjuvant EXC (epirubicin 60 mg/m2 day 1; capecitabine 1000 mg/m2 bid, days 1-14; cisplatin 60 mg/m2day 1) and two cycles of post-operative EXC. Results: Eight (17%) of 48 patients had inflammatory breast cancer. Overall response rate was 74% (95% CI: 59-86%), including complete responses in 13% (95% CI: 5-26%). Nine (22%; 95% CI: 11-38%) of 41 patients undergoing surgery achieved pathologic complete response (pCR), giving a pCR rate of 19% (95% CI: 9-33%) in the intent-to-treat population. Haematological toxicity was manageable. The most problematic toxicities were chemotherapy-induced nausea/vomiting and hypercoagulative disorders. None of the biomarkers investigated, including HER2, predicted response. Conclusion: EXC showed high efficacy in LABC, with high clinical response and pCR rate. Nausea and vomiting were unexpectedly frequent, and more aggressive prophylaxis and management of these side effects is recommended in future studies of this combination.

    • With increased autonomy in marine vessels, autonomous surface vessels (ASVs) and conventionally manned vessels need to coexist at sea. Any relocation needs to include collision avoidance according to the traffic rules at sea, COLREGs. Here, a local collision-avoidance planner for an archipelago environment is presented. The optimization-based local planner presented considers a predefined nominal path and upon detection of other vessels, a COLREGs-aware maneuver is computed. The maneuver adapts to the available space and includes a return to the nominal plan. The irregular available space for maneuvers is approximated with a local convex area. The planner was evaluated both in simulations and in field experiments.

    • The overall aims of this thesis are to describe women’s experiences of breast cancer from a gender perspective, describe women’s experiences of an art therapy intervention and the effects of an art therapy intervention on self-rated coping resources and quality of life. The thesis presents results from a study with 42 women with primary breast cancer without distance metastasis who participated in a randomized intervention study with art therapy carried out between 2001 and 2004. All women were recruited as they were referred to the Department of Oncology at Umeå University Hospital in northern Sweden for postoperative radiotherapy. They were randomized to an intervention group (n = 20) with individual art therapy for 1 hour/week during postoperative radiotherapy, or to a control group (n = 22). The women were between 37–69 years old and had a range of educational and socioeconomic backgrounds. All 42 women completed questionnaires in connection with three interview occasions during six months: at baseline (start of radiotherapy), two and six months later. The questionnaires assessed coping resources, quality of life, symptoms, and self-image. In addition, all women were interviewed about their experiences, and were asked to write a weekly diary about their experiences of breast cancer during the six months of participation. All women signed a written consent form in which they were assured voluntariness and that ending their participation would not affect the care or medical treatment in any way. The study protocol was approved by the Umeå University Ethical Committee at the Medical Faculty (archive number 99-386). In this thesis, part of results from the study is reported. The results presented in the first study show an overall increase in coping resources as measured by the Coping Resources Inventory (CRI) among women with breast cancer after taking part in the art therapy intervention. Significant differences were seen between the study group and the control group in the total score on the second occasion and in the social domain on the second and third occasions. The second study reports results of the art therapy intervention, with regard to quality of life aspects as measured by the instruments WHOQOL-BREF and EORTC QLQ-BR23, at baseline (start of radiotherapy) and two and six months later. Significant increases in total health, total quality of life, physical health and psychological health were found in the art therapy group compared to the control group. A significant positive difference was also found within the intervention group, concerning future perspectives, body image and systemic therapy side effects. The third study builds on previous quantitative results, drawing on gender theories and, taking a discursive approach in analyzing the women’s use of interpretative repertoires in diaries and interviews. The results show a connection between participation in art therapy, talking about protecting one’s own boundaries, and scoring higher on the CRI compared to the control group. A connection between the control group, repertoire conflicts, and lower scores on the CRI was also found. The fourth study presents further knowledge about women’s trajectories, in art therapy, towards helpful management of restraining boundaries. We were inspired by discursive psychology and the analytic concept of subject position. The result shows that art therapy helped women to get access to subject positions that enabled them to protect and strengthen their boundaries and put forward their own needs. Taken together, the results from all four studies support art therapy as a valuable complement in oncology care and rehabilitation of women with primary breast cancer in similar contexts.
