    • Det sociala arbetets praktik handlar om att ge hjälp och stöd och förhindra exkludering och främja delaktighet. Socialt arbete handlar till stor del om att tillämpa befintliga lagar utifrån det tolkningsutrymme som finns. I varje enskilt fall vägs en rad individuella omständigheter samman med tolkningar av denna lagstiftning. Det är bl.a. inom ramen för detta handlingsutrymme som de sociala rättigheterna görs tillgängliga och blir till. Därför har kopplingen mellan rättigheter och socialt arbete stor betydelse för det sociala arbetet, både som praktik och som forskningsfält. I den här antologin riktar författarna uppmärksamhet mot sociala rättigheter utifrån olika teoretiska infallsvinklar, metoder och empiriska fokuspunkter. Med stöd i exempel från en svensk kontext behandlar författarna tolkning och tillämpning av sociala rättigheter. Hur får de olika innebörder beroende på vilket system som ska säkerställa dem? Hur återskapas de och ges delvis nytt innehåll när en domstol uttolkar ett begrepp? Bokens avslutande kapitel handlar om förhandlingar om olika sociala rättigheter, t.ex. rätten till meningsfulla fritidsaktiviteter för barn och unga med intellektuell funktionsnedsättning, ansvar och rätten till vård och omsorg när det gäller anhörigvård samt rätten till språkutbildning för nyanlända. Magnus Dahlstedt (red.) är professor i socialt arbete vid Linköpings universitet. Hans forskning rör frågor om välfärd och medborgarskapande, med särskilt fokus på unga, inkludering och exkludering i migrationens tid. Anna Lundberg (red.) är professor i välfärdsrätt vid Linköpings universitet och professor i rättssociologi vid Lunds universitet. Hennes forskning rör rätten och välfärden i migrationens tidsålder med särskilt fokus på rättigheter. Dimitris Michailakis (red.) är professor emeritus i socialt arbete vid Linköpings universitet.     • Det är viktigt att förstå att rättigheter kan etableras och utvecklas i en viss tid, för att i en annan tid utmanas, urholkas och ersättas av skyldigheter. Olika rättighetskategorier kan få olika tyngd i förhållande till varandra. Ett exempel belyses i detta kapitel, om de mänskliga rättigheternas framväxt i ett internationellt sammanhang. Bland annat behandlar kapitlet hur det kommer sig att ekonomiska och sociala rättigheter har fått en svagare ställning än politiska och medborgerliga rättigheter rent juridiskt. Skillnaden i juridisk tyngd mellan olika rättighetskategorier har på olika sätt betydelse för hur rättigheter institutionaliseras, och i förlängningen även för hur de används i det sociala arbetet och dess vardagliga praktik. Det är till exempel svårare att driva frågor i rättsliga processer som inte har en tydlig juridisk tyngd att falla tillbaka på.

    • Nancy Frasers arbete är ett exempel på kritik som en rad teoretiker har riktat mot Marshall, och inte minst mot hans förgivettagande att nationen är den givna ram inom vilken medborgarskap, det vill säga rättigheter av olika slag, anordnas och tillhandahålls. Även om vi i dag lever i en tid av globalisering och världsomspännande migration där rörlighet är mer regel än undantag, så är nationalismens ideal om tillhörighet till gemenskapen – grundad på kultu- rell homogenitet och enhetlighet – inte på något vis överspelade. Tvärtom lever sådana ideal fortfarande kvar, tas ofta för givna och är i många länder på frammarsch. Nationalstaten sätter på så sätt fortsatt prägel på samtida politiska debatter och bidrar på olika vis till att forma medborgarskap och dess rättigheter – både i Sverige och i många andra länder runt om i världen

  • Childbirth rates in women with myeloproliferative neoplasms

    • Myeloproliferative neoplasms (MPN) are associated with inferior pregnancy outcome, however, little is known about fertility and childbearing potential in women with MPN. In this study we aimed to describe reproductive patterns, as well as to quantify risk of miscarriage and stillbirth. Women aged 15-44 years with an MPN diagnosis 1973-2018, were identified in Swedish health care registers, and age-matched 1:4 to population controls. We identified 1141 women with MPN and 4564 controls. Women with MPN had a lower rate of childbirth (hazard ratio [HR] with 95% confidence interval was 0.78 (0.68-0.90)). Subgroup analysis showed that the rate was not significantly reduced in essential thrombocythemia, HR 1.02 (0.86-1.22) while the HR was 0.50 (0.33-0.76) in PV and 0.45 (0.28-0.74) in PMF. The risk of miscarriage was not significantly increased before MPN diagnosis, the HR during follow-up after diagnosis was 1.25 (0.89-1.76). Women with MPN were more likely to have had a previous stillbirth. Women with MPN had fewer children at diagnosis, and fewer children in total. In conclusion, the childbirth rate was lower among women with MPN than controls, but not among women with essential thrombocythemia.

    • Pregnancy and childbirth in women with myeloproliferative neoplasms (MPN) are reported to be associated with maternal thrombosis, hemorrhage, and placental dysfunction. To assess the risks of adverse events in pregnancy in women with MPN, we performed a large population-based study using Swedish health care registers, and included all pregnancies that had reached gestational week 22 (prior to 2008, week 28) during the years 1973-2017 in women with MPN. Control pregnancies were matched 1:1 for age, calendar year, and parity. We identified 342 pregnancies in 229 women with MPN. Preterm birth was significantly increased in pregnancies in MPN, 14% compared to 4% of pregnancies in controls (p < 0.001). Correspondingly, low birth weight (<2500 g) was also significantly increased in MPN pregnancies (p = 0.042). Stillbirth was rare, with two events (0.6%) in MPN, none in controls. Maternal thrombotic complications occurred in three (1%) of the pregnancies in MPN patients, compared to none in controls. Pregnancy-related bleeding affected 14% of pregnancies in MPN and 9% in controls (p < 0.110). Cesarean section was significantly more common in pregnancies in MPN. Incidence was 12.2 per 100.000 pregnancies. In summary, preterm birth was an important complication in MPN pregnancies, while maternal complications were less common than previously reported.

    • The article examines temporal governing in the Swedish asylum bureaucracy through a qualitative analysis of the case files of ten asylum-seeking families. The documents studied include more than 5,000 pages of asylum investigation protocols, decisions, court judgments, certificates, appeals, submissions, personal letters, memos, and other material. The study focuses on decisions made by the Swedish Migration Agency and the oral investigations documented by the agencys officers. Three inherent and overlapping contradictions related to the temporal governance of the Migration Agency are identified: 1) migration officials insist that asylum seekers provide a strict chronological narrative, whereas what they have lived through has been chaotic, 2) migration officials rush asylum seekers through the asylum process, but the applicant is then subjected to a stressful period of prolonged waiting, and 3) migration officials discount any speculation by asylum seekers when they explain why they are seeking protection, although speculation is unavoidable when conveying fears for the future. The contradictions affect the procedure for judging the asylum claim as well as the outcome of the assessment. The article also highlights consequences of time management paradoxes for asylum seekers and on the societal level in general

    • Artikeln undersöker temporal styrning i den svenska asylbyråkratin genom en kvalitativ analys av tio familjers asylakter. Dessa består av över 5 000 sidors dokumentation: asylutredningsprotokoll, beslut, domar, intyg, överklaganden, inlagor, personliga brev, minnesnoteringar med mera. Studien fokuserar särskilt på Migrationsverkets beslut och de muntliga utredningar som myndighetens handläggare har protokollfört. Tre inneboende och överlappande motsättningar, som är relaterade till Migrationsverkets temporala styrning, identifieras i studien: 1) jakten på den kronologiska berättelsen i asylutredningen å ena sidan och ett kaosartat förflutet å den andra, 2) en frenetisk tidsanvändning som tar sig uttryck i en hetsig process samtidigt som den asylsökande är försatt i långvarig väntan, 3) en framåtsyftande redogörelse som inte får vara spekulativ samtidigt som spekulation, åtminstone delvis, är en ofrånkomlig del av att försöka göra sannolikt att man är i behov av skydd. Motsättningarna har anknytning till såväl proceduren för bedömning av asylansökan som utfallet i ärendet. Artikeln pekar även på några av konsekvenserna av tidsstyrningens paradoxer såväl för de asylsökande i Sverige som på samhällsnivå.

    • Background: Optimal oxygen targets in patients resuscitated after cardiac arrest are uncertain. The primary aim of this study was to describe the values of partial pressure of oxygen values (PaO2) and the episodes of hypoxemia and hyperoxemia occurring within the first 72 h of mechanical ventilation in out of hospital cardiac arrest (OHCA) patients. The secondary aim was to evaluate the association of PaO2 with patients’ outcome. Methods: Preplanned secondary analysis of the targeted hypothermia versus targeted normothermia after OHCA (TTM2) trial. Arterial blood gases values were collected from randomization every 4 h for the first 32 h, and then, every 8 h until day 3. Hypoxemia was defined as PaO2 < 60 mmHg and severe hyperoxemia as PaO2 > 300 mmHg. Mortality and poor neurological outcome (defined according to modified Rankin scale) were collected at 6 months. Results: 1418 patients were included in the analysis. The mean age was 64 ± 14 years, and 292 patients (20.6%) were female. 24.9% of patients had at least one episode of hypoxemia, and 7.6% of patients had at least one episode of severe hyperoxemia. Both hypoxemia and hyperoxemia were independently associated with 6-month mortality, but not with poor neurological outcome. The best cutoff point associated with 6-month mortality for hypoxemia was 69 mmHg (Risk Ratio, RR = 1.009, 95% CI 0.93–1.09), and for hyperoxemia was 195 mmHg (RR = 1.006, 95% CI 0.95–1.06). The time exposure, i.e., the area under the curve (PaO2-AUC), for hyperoxemia was significantly associated with mortality (p = 0.003). Conclusions: In OHCA patients, both hypoxemia and hyperoxemia are associated with 6-months mortality, with an effect mediated by the timing exposure to high values of oxygen. Precise titration of oxygen levels should be considered in this group of patients. Trial registration: clinicaltrials.gov NCT02908308, Registered September 20, 2016.

    • Purpose: The optimal ventilatory settings in patients after cardiac arrest and their association with outcome remain unclear. The aim of this study was to describe the ventilatory settings applied in the first 72 h of mechanical ventilation in patients after out-of-hospital cardiac arrest and their association with 6-month outcomes. Methods: Preplanned sub-analysis of the Target Temperature Management-2 trial. Clinical outcomes were mortality and functional status (assessed by the Modified Rankin Scale) 6 months after randomization. Results: A total of 1848 patients were included (mean age 64 [Standard Deviation, SD = 14] years). At 6 months, 950 (51%) patients were alive and 898 (49%) were dead. Median tidal volume (VT) was 7 (Interquartile range, IQR = 6.2–8.5) mL per Predicted Body Weight (PBW), positive end expiratory pressure (PEEP) was 7 (IQR = 5–9) cmH20, plateau pressure was 20 cmH20 (IQR = 17–23), driving pressure was 12 cmH20 (IQR = 10–15), mechanical power 16.2 J/min (IQR = 12.1–21.8), ventilatory ratio was 1.27 (IQR = 1.04–1.6), and respiratory rate was 17 breaths/minute (IQR = 14–20). Median partial pressure of oxygen was 87 mmHg (IQR = 75–105), and partial pressure of carbon dioxide was 40.5 mmHg (IQR = 36–45.7). Respiratory rate, driving pressure, and mechanical power were independently associated with 6-month mortality (omnibus p-values for their non-linear trajectories: p < 0.0001, p = 0.026, and p = 0.029, respectively). Respiratory rate and driving pressure were also independently associated with poor neurological outcome (odds ratio, OR = 1.035, 95% confidence interval, CI = 1.003–1.068, p = 0.030, and OR = 1.005, 95% CI = 1.001–1.036, p = 0.048). A composite formula calculated as [(4*driving pressure) + respiratory rate] was independently associated with mortality and poor neurological outcome. Conclusions: Protective ventilation strategies are commonly applied in patients after cardiac arrest. Ventilator settings in the first 72 h after hospital admission, in particular driving pressure and respiratory rate, may influence 6-month outcomes.

  • Between Fire and Ice : Experiences of the Persian Fire Festival in a Nordic Setting

    • This exploratory research seeks to gain insight into the visitors of Persian Fire Festival in Stockholm, their motivation, level of satisfaction and loyalty to the festival. The overall methodology is mixed methods consisting of participant observation and 280 completed online surveys including close-ended and open-ended questions. A Grounded theory analysis has been employed. The most important reason behind the participation has been to strive for the recognition of ethnic cultural identity; to keep the ancient culture alive; to maintain their ancestors’ legacy and transfer it to the next generation. Most of the visitors experienced positive feelings, showed satisfaction and loyalty, believed that the festival fulfilled their expectations and it should be celebrated officially every year. A minority showed dissatisfaction mostly due to the lack of accessibility to the event venue and might are not loyal to the festival. The first conclusion here is that these motivators are totally different than those mentioned by earlier research (e.g., escape, socialization, novelty, rest, prestige, excitement). Even though the desire to preserve the ancient culture and tradition is very strong, a lack of accessibility to the venue would constitute disloyalty and a barrier to re-attend in such a festival.

    • The purpose of this study is to investigate how the open-air museum ofSkansen in Stockholm as venue for celebration of Persian Fire Festival is assessed by thefestival’s visitors. The study is based on a delimited part of a larger online survey with280 completed questionnaires including both close-ended and open-ended questions,and a qualitative thematic analysis method has been used in this paper. The resultsshow a high level of appreciation of the Skansen venue that gives the visitors positiveemotions, meanings, place attachment, place identity and a sense of place. Contributingfactors have been the high status of the venue that has given the visitors pride anddignity; security and safety of the venue; size of the venue; and the natural beauty of thevenue. The most important disadvantage factor has been the lack of accessibilitycombined with lack of sufficient public transport. The results have implications for theSwedish society, for the policy makers in Stockholm city and for both the host and theorganiser of the festival and gives rise to new debates on immigrants’ socio-culturalintegration into Swedish society. This paper gives a contribution to existing literature onfestival venue and its impact on the visitor’s overall assessment of the festival.

    • This report represents INFRACOMS deliverable D2.1 Appraisal Methodology. It builds upon the deliverables of INFRACOMS Work Package 1 which identified the information needs, gaps and priorities of NRAs in terms of their approach to data collection and monitoring, and a list of current and emerging measurement technologies. This report includes a review of several commonly-used appraisal methodologies that can be used to evaluate the effectiveness, suitability and potential impact of new technologies for an organisation. These methodologies include Technology Readiness Levels (TRLs), Cost Benefit Analysis (CBA), Life Cycle Cost Analysis (LCCA), Risk Assessment, and Multi-Criteria Decision Analysis (MCDA). Elements of these commonly used methodologies are included in the INFRACOMS Appraisal Methodology. The report also includes key highlights from a workshop with NRAs conducted in January 2023 which also fed into the design of the appraisal methodology. The INFRACOMS Appraisal Methodology described here is designed around the technology use case, that is, a particular application of a technology by a NRA. It incorporates three core processes for Pre-Evaluation, Evaluation and Case Studies of technology use cases. It also includes processes for NRAs to define their strategic and technical priorities so that the appraisal process can be tailored to addressing their individual requirements, as identified from Work Package 1.

The approach has been to establish existing knowledge via a review of previous projects, current best practices and standards in data collection and inspection, and a review of current business processes, NRA strategies around data collection and digitalisation etc. The report identifies a set of key imperatives for carriageway and bridge assets covering Availability, Reliability, Environment, Economy and Safety. Each of these is supported by the collection of key condition data, which is used to report technical parameters and performance indicators that can be combined to assess the ability of the asset to meet its key imperatives. A wide range of technologies are identified, which are currently applied to collect the data that supports this assessment.The consultation shows that there are also gaps between the desired and the current capability for the assessment of these assets. These include gaps in the data, challenges in the ability to collect the data, gaps in the application of the data that is already collected etc. A review of emerging technologies shows that there are tools and technologies that could help to fill these gaps. These could overcome the limitations of current technologies, better integrate new data sources, provide greater flexibility in using current and new data, and provide better analysis. They include remote sensing, Internet of Things (IoT), crowdsourcing, and advanced data processing/visualisation.

    • Vägens friktion är troligen den egenskap hos vägen som har störst betydelse för trafiksäkerheten. När en ny beläggning läggs varnas trafikanterna normalt genom skyltning med varningsmärket A10 ”Varning för slirig väg”, med tilläggstavla T22, ”Vid våt vägbana”. Skyltningen utförs som en säkerhetsåtgärd. Huvudsyftet med detta projekt har varit att verifiera resultaten från den första studien, för att veta hur friktionen förändras efter trafikpåsläpp på en nyasfalterad väg. Ambitionen var att kunna avgöra om nylagda vägavsnitt har nedsatt friktion för att kunna ge rekommendationer hur skyltning till trafikanter ska ske i samband med och efter beläggningsarbeten och nybyggnationer. I denna kompletterande studie mättes friktion och makrotextur på vägar med olika nylagda beläggningar från strax innan trafikpåsläpp till dess makrotextur- och friktionsnivåerna hade stabiliserats. De tre beläggningstyper som studerades var Asfaltbetong stenrik (ABS), Asfaltbetong tät (ABT) och Mjukbitumenbundet grus med oljegrusgradering (MJOG). Ett av objekten specialstuderades genom upprepade avgjutningar och fotografering av en specifik kontrollyta för att försöka avgöra hur vägytan förändras över tid. Denna studie bekräftar mycket av vad som framkom i den tidigare studien. En ny beläggning har den högsta friktionen i samband med att trafiken släpps på. Därefter minskar friktionen under de kommande veckorna innan den börjar öka igen för att vara stabiliserad efter ungefär fem veckor. Detta var fallet för två av beläggningstyperna (ABS och ABT). Sträckan med MJOG var stabil redan efter tre veckor. Den normala tiden innan makrotexturnivåerna stabiliserades var mellan fyra och fem veckor. Även där fanns det en viss variation mellan de olika beläggningstyperna. Det visade sig även att det kan skilja sig i friktion mellan en konventionell beläggning och en remixad beläggning.

    • Effective analysis and visualisation of data is critical for the efficient application of the data provided by carriageway and bridge condition monitoring technologies. It supports better decisions in relation to asset reliability, availability, safety, economy and environment. This report discusses the link between the data provided by monitoring technologies on the properties of assets and how the collected data can be analysed and visualised to provide value in decision support. The next step in the report is to use this understanding to develop an appraisal system which could enable technologies in the INFRACOMS technology database to be appraised (scored) in relation to their abilities for data analysis, visualisation, integration and use in decision support.The presented system is referred to as the D3.1 scoring system. It consists of four components covering data visualisation, data analysis, integration within current data architectures and potential for practical decision-making. The present D3.1 report primarily examines the components pertaining to data visualisation and data analysis, while the exploration of the other two components, data architecture and decision support, will be carried out in the D3.2 report. It is proposed that the D3.1 scoring system could be used to appraise the capability of monitoring technologies to support asset management decisions, and would become an integral component of the INFRACOMS Appraisal Toolkit. It will also be used to further filter the current INFRACOMS Technology Database 2.0 technologies as part of the Appraisal Toolkit as INFRACOMS completes the development of the toolkit/database within WP2.

  • Web-based exercise versus supervised exercise for decreasing visceral adipose tissue in older adults with central obesity : a randomized controlled trial

    • BACKGROUND: Visceral adipose tissue (VAT) is a strong risk factor for cardiovascular disease and increases with age. While supervised exercise (SE) may be an effective approach, web-based exercise (WE) have other advantages such as being more readily accessible. Therefore, we evaluated the effects of WE on VAT, body composition and cardiometabolic risk markers in centrally obese older adults and compared the effects of WE to SE. We also explored the feasibility of WE.METHODS: In a randomized controlled trial conducted in Umeå, Sweden during January 2018 - November 2018, N = 77, 70-year-old men and women with central obesity (> 1 kg VAT for women, > 2 kg for men) were randomized to an intervention group (n = 38) and a wait-list control group (n = 39). The intervention group received 10 weeks of SE while the wait-list control group lived as usual. Following a 10-week wash-out-period, the wait-list control group received 10 weeks of WE. The primary outcome was changes in VAT. Secondary outcomes included changes in fat mass (FM), lean body mass (LBM), blood lipids, fasting blood glucose. Additionally, we explored the feasibility of WE defined as adherence and participant experiences.RESULTS: WE had no significant effect on VAT (P = 0.5), although it decreased FM by 450 g (95% confidence interval [CI], 37 to 836, P < 0.05). The adherence to WE was 85% and 87-97% of the participants rated aspects of the WE intervention > 4 on a scale of 1-5. Comparing SE to WE, there was no significant difference in decrease of VAT (Cohen's δ effect size [ES], 0.5, 95% CI, - 24 to 223, P = 0.11), although SE decreased FM by 619 g (ES, 0.5, 95% CI, 22 to 1215, P < 0.05) compared to WE.CONCLUSIONS: Ten weeks of vigorous WE is insufficient to decrease VAT in centrally obese older adults, but sufficient to decrease FM while preserving LBM. The high adherence and positive experiences of the WE intervention implies that it could serve as an alternative exercise strategy for older adults with central obesity, with increased availability for a larger population.TRIAL REGISTRATION: ClinicalTrials.gov (NCT03450655), retrospectively registered February 28, 2018.

    • Background: The clinical-stage drug candidate EBL-1003 (apramycin) represents a distinct new subclass of aminoglycoside antibiotics for the treatment of drug-resistant infections. It has demonstrated best-in-class coverage of resistant isolates, and preclinical efficacy in lung infection models. However, preclinical evidence for its utility in other disease indications has yet to be provided. Here we studied the therapeutic potential of EBL-1003 in the treatment of complicated urinary tract infection and acute pyelonephritis (cUTI/AP). Methods: A combination of data-base mining, antimicrobial susceptibility testing, time-kill experiments, and four murine infection models was used in a comprehensive assessment of the microbiological coverage and efficacy of EBL-1003 against Gram-negative uropathogens. The pharmacokinetics and renal toxicology of EBL-1003 in rats was studied to assess the therapeutic window of EBL-1003 in the treatment of cUTI/AP. Findings: EBL-1003 demonstrated broad-spectrum activity and rapid multi-log CFU reduction against a phenotypic variety of bacterial uropathogens including aminoglycoside-resistant clinical isolates. The basicity of amines in the apramycin molecule suggested a higher increase in positive charge at urinary pH when compared to gentamicin or amikacin, resulting in sustained drug uptake and bactericidal activity, and consequently in potent efficacy in mouse infection models. Renal pharmacokinetics, biomarkers for toxicity, and kidney histopathology in adult rats all indicated a significantly lower nephrotoxicity of EBL-1003 than of gentamicin. Interpretation: This study provides preclinical proof-of-concept for the efficacy of EBL-1003 in cUTI/AP. Similar efficacy but lower nephrotoxicity of EBL-1003 in comparison to gentamicin may thus translate into a higher safety margin and a wider therapeutic window in the treatment of cUTI/API.     • Objective Idiopathic inflammatory myopathies (IIMs) are a heterogeneous group of complex autoimmune conditions characterized by inflammation in skeletal muscle and extramuscular compartments, and interferon (IFN) system activation. We undertook this study to examine the contribution of genetic variation to disease susceptibility and to identify novel avenues for research in IIMs.Methods Targeted DNA sequencing was used to mine coding and potentially regulatory single nucleotide variants from ~1,900 immune-related genes in a Scandinavian case–control cohort of 454 IIM patients and 1,024 healthy controls. Gene-based aggregate testing, together with rare variant– and gene-level enrichment analyses, was implemented to explore genotype–phenotype relations.Results Gene-based aggregate tests of all variants, including rare variants, identified IFI35 as a potential genetic risk locus for IIMs, suggesting a genetic signature of type I IFN pathway activation. Functional annotation of the IFI35 locus highlighted a regulatory network linked to the skeletal muscle–specific gene PTGES3L, as a potential candidate for IIM pathogenesis. Aggregate genetic associations with AGER and PSMB8 in the major histocompatibility complex locus were detected in the antisynthetase syndrome subgroup, which also showed a less marked genetic signature of the type I IFN pathway. Enrichment analyses indicated a burden of synonymous and noncoding rare variants in IIM patients, suggesting increased disease predisposition associated with these classes of rare variants.Conclusion Our study suggests the contribution of rare genetic variation to disease susceptibility in IIM and specific patient subgroups, and pinpoints genetic associations consistent with previous findings by gene expression profiling. These features highlight genetic profiles that are potentially relevant to disease pathogenesis.

    • Featured Application Herein, an innovative methodology, called X-ray chronoscopy, is proposed for exploration of ultrafast processes in matter with attosecond precision using current XFEL sources. The method is based on measuring the change in an X-ray pulse temporal profile induced by interaction with a medium. X-ray free electron lasers (XFELs) have provided scientists opportunities to study matter with unprecedented temporal and spatial resolutions. However, access to the attosecond domain (i.e., below 1 femtosecond) remains elusive. Herein, a time-dependent experimental concept is theorized, allowing us to track ultrafast processes in matter with sub-fs resolution. The proposed X-ray chronoscopy approach exploits the state-of-the-art developments in terahertz streaking to measure the time structure of X-ray pulses with ultrahigh temporal resolution. The sub-femtosecond dynamics of the saturable X-ray absorption process is simulated. The employed rate equation model confirms that the X-ray-induced mechanisms leading to X-ray transparency can be probed via measurement of an X-ray pulse time structure.

    • BackgroundStreptococcus pyogenes bacteremia is a severe condition with high mortality. Time to blood culture positivity (TTP) is known to predict the outcome in bacteremia with other pathogens. This study aimed to determine the association between TTP and outcome in S pyogenes bacteremia.MethodsThis retrospective observational cohort study comprised adults with S pyogenes bacteremia, identified through the laboratory database between 2015 and 2018, in the Region of Skåne, Sweden. Correlations between TTP and outcomes were investigated. Primary outcome was death within 30 days, and secondary outcomes were presence of sepsis or disease deterioration within the first 48 hours.ResultsA total of 347 episodes of S pyogenes bacteremia were identified, of which 61 were excluded, resulting in 286 included episodes. Median TTP was 10.4 (interquartile range, 8.4–11.4) hours. Thirty-day mortality was 10%. Median TTP was shorter in patients who died within 30 days compared to survivors (8.6 vs 10.4 hours; P < .001). In a multivariable logistic regression, shorter TTP was associated with 30-day mortality when adjusting for age, Charlson Comorbidity Index, and focus of infection (odds ratio, 3.7 [95% confidence interval, 1.2–11.3]; P = .02). There was no statistically significant difference in TTP between patients with sepsis within 48 hours and those who did not have sepsis. Additionally, there was no statistically significant difference in TTP between patients with disease deterioration compared to those who did not deteriorate.ConclusionsKnowledge on TTP might be a tool to determine the prognosis of a given patient with S pyogenes bacteremia.

Time to positivity from blood cultures (TTP) has been shown to be a prognostic indicator in bloodstream infections caused by other beta-haemolytic streptococci. This study aimed to determine any possible association between TTP and outcome in invasive infections due to S. dysgalactiae.Materials and methodsEpisodes of S. dysgalactiae bacteraemia, that occurred during 2015–2018 in the Region of Skåne, Sweden, were identified through the laboratory database and retrospectively studied. Any association with TTP and the primary outcome, death within 30 days and the development of sepsis or disease deterioration within 48 hours from blood culturing as secondary outcomes were investigated.ResultsAmong the 287 episodes of S. dysgalactiae bacteraemia, 30-day mortality rate was 10% (n = 30). Median TTP was 9.3 h (interquartile range 8.0–10.3 h). Median TTP was statistically significantly shorter in patients who died within 30 days compared to surviving patients (7.7 vs 9.3 h, p = .001, Mann–Whitney U test). Short TTP(≤ 7.9 h) was still associated with 30-day mortality when adjusting for age, (OR 4.4, 95% CI 1.6–12.2, p = .004). Associations between secondary outcomes and levels of TTP were not observed.ConclusionTTP may be an important prognostic indicator for 30-day mortality in patients with bloodstream infections due to S. dysgalactiae.

    • Background People with dementia who live in ordinary housing need to perform activities outside the home such as visiting friends, talking walks and doing grocery shopping. This article identifies and examines characteristics that may influence accessibility in the space of a grocery shop as perceived by people with dementia. Methods This is a qualitative study with a grounded theory approach. The data collection was done with two different methods. It started with photo documentation and continued with focus group interviews in combination with photo elicitation. Data from both photo documentation and focus groups were analysed according to a grounded theory approach. Results The categories "illogical arrangement", "overload of products, information and people", "visual illusions" and "intrusive auditory stimuli" showed characteristics in the grocery shop that influenced how accessible and usable the informants experienced a shop to be. Furthermore, personal capacities in relation to the specific characteristics of the grocery shop space had an influence on how accessible and usable the informants experienced the grocery shop to be. Capacities to find, stay focused and concentrated, meet stress, remember, interpret and discriminate sensory impressions through hearing and sight came to the fore as important. Conclusions Characteristics of both the shop and the person need to be taken into account when supporting people with dementia in grocery shopping.

    • ObjectivesTo evaluate the diagnostic performance of ultrasound guided attenuation parameter (UGAP) for evaluating liver fat content with different probe forces and body positions, in relation to sex, and compared with proton density fat fraction (PDFF).MethodsWe prospectively enrolled a metabolic dysfunction-associated steatotic liver disease (MASLD) cohort that underwent UGAP and PDFF in the autumn of 2022. Mean UGAP values were obtained in supine and 30 degrees left decubitus body position with normal 4 N and increased 30 N probe force. The diagnostic performance was evaluated by the area under the receiver operating characteristic curve (AUC).ResultsAmong 60 individuals (mean age 52.9 years, SD 12.9; 30 men), we found the best diagnostic performance with increased probe force in 30 degrees left decubitus position (AUC 0.90; 95% CI 0.82-0.98) with a cut-off of 0.58 dB/cm/MHz. For men, the best performance was in supine (AUC 0.91; 95% CI 0.81-1.00) with a cut-off of 0.60 dB/cm/MHz, and for women, 30 degrees left decubitus position (AUC 0.93; 95% CI 0.83-1.00), with a cut-off 0.56 dB/cm/MHz, and increased 30 N probe force for both genders. No difference was in the mean UGAP value when altering body position. UGAP showed good to excellent intra-reproducibility (Intra-class correlation 0.872; 95% CI 0.794-0.921).ConclusionUGAP provides excellent diagnostic performance to detect liver fat content in metabolic dysfunction-associated steatotic liver diseases, with good to excellent intra-reproducibility. Regardless of sex, the highest diagnostic accuracy is achieved with increased probe force with men in supine and women in 30 degrees left decubitus position, yielding different cut-offs.Clinical relevance statementThe ultrasound method ultrasound-guided attenuation parameter shows excellent diagnostic accuracy and performs with good to excellent reproducibility.     • Imaging is a powerful approach for studying protein expression and has the advantage over other methodologies in providing spatial informationin situat single cell level. Using immunofluorescence and confocal microscopy, detailed information of subcellular distribution of proteins can be obtained. While adherent cells of different tissue origin are relatively easy to prepare for imaging applications, non-adherent cells from hematopoietic origin, present a challenge due to their poor attachment to surfaces and subsequent loss of a substantial fraction of the cells. Still, these cell types represent an important part of the human proteome and express genes that are not expressed in adherent cell types. In the era of cell mapping efforts, overcoming the challenge with suspension cells for imaging applications would enable systematic profiling of hematopoietic cells. In this work, we successfully established an immunofluorescence protocol for preparation of suspension cell lines, peripheral blood mononucleated cells (PBMC) and human platelets on an adherent surface. The protocol is based on a multi-well plate format with automated sample preparation, allowing for robust high throughput imaging applications. In combination with confocal microscopy, the protocol enables systematic exploration of protein localization to all major subcellular structures.

    • Background: It is unknown whether renal pathology lesions in immunoglobulin A nephropathy (IgAN) correlate with renal outcomes over decades of follow-up.Methods: In 1130 patients of the original Validation Study of the Oxford Classification for IgA Nephropathy (VALIGA) cohort, we studied the relationship between the MEST score (mesangial hypercellularity, M; endocapillary hypercellularity, E; segmental glomerulosclerosis, S; tubular atrophy/interstitial fibrosis, T), crescents (C) and other histological lesions with both a combined renal endpoint [50% estimated glomerular filtration rate (eGFR) loss or kidney failure] and the rate of eGFR decline over a follow-up period extending to 35 years [median 7 years (interquartile range 4.1-10.8)].Results: In this extended analysis, M1, S1 and T1-T2 lesions as well as the whole MEST score were independently related with the combined endpoint (P < 0.01), and there was no effect modification by age for these associations, suggesting that they may be valid in children and in adults as well. Only T lesions were associated with the rate of eGFR loss in the whole cohort, whereas C showed this association only in patients not treated with immunosuppression. In separate prognostic analyses, the whole set of pathology lesions provided a gain in discrimination power over the clinical variables alone, which was similar at 5 years (+2.0%) and for the whole follow-up (+1.8%). A similar benefit was observed for risk reclassification analyses (+2.7% and +2.4%).Conclusion: Long-term follow-up analyses of the VALIGA cohort showed that the independent relationship between kidney biopsy findings and the risk of progression towards kidney failure in IgAN remains unchanged across all age groups and decades after the renal biopsy.

    • The aim of the present study was to assess the psychometric properties of a Swedish translation of the Sexual Orientation Microaggressions Scale (SOMS) in a convenience sample of 267 Swedish LGB+ people (Mean age = 36.41). Testing suggested some strengths in terms of factor structure and 2-week test-retest reliability (ICC > .79). Also, internal consistency (α = .80–.91) and convergent validity were supported for most subscales. However, the Assumption of Deviance subscale was associated with low response variability and internal consistency (α = .35), and the correlational pattern between the Environmental Microaggressions subscale and mental health variables diverged from the overall trend. Furthermore, measurement invariance between homo- and bisexual participants was not supported for most subscales, and although microaggressions would be theoretically irrelevant to a small comparison sample of heterosexual people (N = 76, Mean age = 40.43), metric invariance of the Environmental Microaggressions subscale was supported in comparison to LGB+ people. We argue that these limitations suggest a restricted applicability of the SOMS in a Swedish context, and this has consequences for the definition and operationalization of the construct of microaggressions as a whole. Therefore, more research on the latent properties of microaggressions in Swedish as well as in other contexts is required.

Materialet är hämtat från projektet Insamling av forskningsdata för granskning av lagstiftning, lagtillämpning och rättssäkerhet för människor som sökte asyl i Sverige under perioden 2015-2017. Det är ett 1-årigt så kallat akutprojekt finansierat av Formas (Forskningsrådet för miljö, areella näringar och samhällsbyggande). Akutprojekt av detta slag syftar till att samla in data och att sammanställa denna som underlag för senare vetenskaplig forskning.

    • This article contributes to scholarly discussions about sanctuary enactments. We analyse transversal sanctuary enactments, i.e. when migrants in search of protection are provided with basic welfare and an opportunity to remain, in contestation of the state's migration politics. The study draws on empirical data collected through a transdisciplinary research initiative in Sweden, the Asylum Commission. The analysis is based on interviews with 90 people, asylum seekers and people voluntarily or professionally involved in support work. The article explores bottom-up transversal processes paralleled and intertwined with a growing institutional disenchantment. By analysing practices and challenges in sanctuary enactments and their implications for individuals and institutions, we argue that such disenchantment may have broader societal implications in terms of fundamental changes in relationships between public institutions and civil society actors. Furthermore, our study points to the importance of seeing beyond the city as sites for sanctuary enactments and acknowledging sanctuary enactments initiated in Sweden's rural areas.

    • The point of departure for this paper is the opportunities and challenges experienced in the work of the Swedish Asylum Commission – Commission for review of legislation, law enforcement and legal security for people who applied for asylum in Sweden during the period 2015-17. The Asylum Commission was formed in the spring of 2019, by researchers and people with extensive knowledge about asylum in Sweden (e.g. people who have experiences as asylum seekers, social workers, teachers, and members of NGO’s and other networks in the civil society). The aim of the commission is to initiate critical enquiries of changes in the Swedish asylum process, based on asylum seekers’ own lived experiences and perspectives. The background to the Commission are changes in Sweden’s migration management in recent years and recurring testimonies in various settings depicting a very difficult situation for children, adolescents, and adult asylum seekers. These testimonies concern unpredictable and degrading decision-making processes, increasing homelessness, mental illness, broken families and more violent deportations to countries affected by armed conflict. The Asylum Commission aims to initiate academic and activist work departing from asylum seekers’ own perspectives and lived experiences. The work, inspired by participatory action research (lisahunter et al. 2013), is carried out in close collaboration between asylum seekers, researchers, professionals. (e.g. social workers, teachers), the voluntary sector and civil society actors (e.g. god man, Vi står inte ut, Ensamkommandes förbund). Thus, one important ambition with the work is to rely upon the expert knowledge of all participants in the commission and working within a collective for shared understandings, analysis and action.

  • Impact of general anaesthesia on breast cancer survival: a 5-year follow up of a pragmatic, randomised, controlled trial, the CAN-study, comparing propofol and sevoflurane

    • Background Anaesthesia may impact long-term cancer survival. In the Cancer and Anaesthesia study, we hypothesised that the hypnotic drug propofol will have an advantage of at least five percentage points in five-year survival over the inhalational anaesthetic sevoflurane for breast cancer surgery. Methods From 2118 eligible breast cancer patients scheduled for primary curable, invasive breast cancer surgery, 1764 were recruited after ethical approval and individual informed consent to this open label, single-blind, randomised trial at four county- and three university hospitals in Sweden and one Chinese university hospital. Of surveyed patients, 354 were excluded, mainly due to refusal to participate. Patients were randomised by computer at the monitoring organisation to general anaesthesia maintenance with either intravenous propofol or inhaled sevoflurane in a 1:1 ratio in permuted blocks. Data related to anaesthesia, surgery, oncology, and demographics were registered. The primary endpoint was five-year overall survival. Data are presented as Kaplan-Meier survival curves and Hazard Ratios based on Cox univariable regression analyses by both intention-to-treat and perprotocol. EudraCT, 2013-002380-25 and ClinicalTrials.gov, NCT01975064. Findings Of 1764 patients, included from December 3, 2013, to September 29, 2017, 1670 remained for analysis. The numbers who survived at least five years were 773/841 (91.9% (95% CI 90.1-93.8)) in the propofol group and 764/829 (92.2% (90.3-94.0)) in the sevoflurane group, (HR 1.03 (0.73-1.44); P = 0.875); the corresponding results in the per-protocol-analysis were: 733/798 (91.9% (90.0-93.8)) and 653/710 (92.0% (90.0-94.0)) (HR = 1.01 (0.71-1.44); P = 0.955). Survival after a median follow-up of 76.7 months did not indicate any difference between the groups (HR 0.97, 0.72-1.29; P = 0.829, log rank test). Interpretation No difference in overall survival was found between general anaesthesia with propofol or sevoflurane for breast cancer surgery. Copyright (c) 2023 The Author(s).     • Background: Retrospective studies indicate that the choice of anesthetic can affect long-term cancer survival. Propofol seems to have an advantage over sevoflurane. However, this is questioned for breast cancer. We gathered a large cohort of breast cancer surgery patients from seven Swedish hospitals and hypothesized that general anesthesia with propofol would be superior to sevoflurane anesthesia regarding long-term breast cancer survival.Methods: We identified all patients who were anaesthetized for breast cancer surgery between 2006 and 2012. The patients were matched to the Swedish Breast Cancer Quality Register, to retrieve tumor characteristics, prognostic factors, and adjuvant treatment as well as date of death. Overall survival between patients undergoing sevoflurane and propofol anesthesia was analyzed with different statistical approaches: (a) multiple Cox regression models adjusted for demographic, oncological, and multiple control variables, (b) propensity score matching on the same variables, but also including the participating centers as a cofactor in a separate analysis.Results: The database analysis identified 6305 patients. The 5-year survival rates were 91.0% and 81.8% for the propofol and sevoflurane group, respectively, in the final model (P = .126). Depending on the statistical adjustment method used, different results were obtained, from a non-significant to a "proposed" and even a "determined" difference in survival that favored propofol, with a maximum of 9.2 percentage points higher survival rate at 5 years (hazard ratio 1.46, 95% CI 1.10-1.95).Conclusions: It seems that propofol may have a survival advantage compared with sevoflurane among breast cancer patients, but the inherent weaknesses of retrospective analyses were made apparent.

    • Background: Otomicroscopy and pneumatic methods are superior to otoscopy alone in diagnosing acute otitis media (AOM). There is a lack of knowledge regarding the use of different diagnostic methods for AOM in primary health care in Sweden and Norway. Methods: This cross-sectional study included a questionnaire completed by general practitioners (GPs) and specialist trainees (STs/residents/registrars) working in primary care in Sweden and Norway. Multivariable binary logistic regressions were performed to evaluate the use of diagnostic methods and written advice adjusted for educational level, sex and country. Results: Otoscopy was the most frequently used method. Sweden had greater access to the more accurate diagnostic methods. In Norway, the following methods were used to a lesser extent: pneumatic otoscopy, adjusted OR 0.15 (95% CI 0.10-0.23; p < .001), otomicroscopy, adjusted OR 0.013 (95% CI 0.070-0.027; p < .001), pneumatic otomicroscopy, adjusted OR 0.028 (95% CI 0.010-0.078; p < .001) and tympanometry, adjusted OR 0.31 (95% CI 0.21-0.45; p < .001). Written advice was used to a greater extent in Norway, adjusted OR 4.5 (95% CI 3.1-6.7; p < .001). The STs used pneumatic otoscopy and pneumatic otomicroscopy to a lesser extent, adjusted OR 0.65 (95% CI 0.45-0.93; p = .019) and 0.63 (95% CI 0.43-0.92; p = .016). Conclusions: Swedish physicians both used and had greater access to the significantly better diagnostic methods compared with Norwegian physicians while the opposite applied to the use of written information. The GPs used pneumatic otoscopy and pneumatic otomicroscopy to a greater extent than STs. Compared with 2012, the Swedish physicians now more frequently used pneumatic otoscopy.

    • Purpose Since 2006, higher education institutions (HEIs) in Sweden, should according to the Higher Education Act, promote sustainable development (SD). In 2016, the Swedish Government asked the Swedish higher education authority to evaluate how this study is proceeding. The authority chose to focus on education. This paper aims to produce a report on this evaluation. Design/methodology/approach All 47 HEIs in Sweden were asked to write a self-evaluation report based on certain evaluation criteria. A panel was appointed consisting of academics and representatives for students and working life. The panel wrote an evaluation of each HEI, a report on general findings and recommendations, and gave an overall judgement of each HEI in two classes as follows: the HEI has well-developed processes for integration of SD in education or the HEI needs to develop their processes. Findings Overall, a mixed picture developed. Most HEIs could give examples of programmes or courses where SD was integrated. However, less than half of the HEIs had overarching goals for integration of SD in education or had a systematic follow-up of these goals. Even fewer worked specifically with pedagogy and didactics, teaching and learning methods and environments, sustainability competences or other characters of education for SD. Overall, only 12 out of 47 got a higher judgement. Originality/value This is a unique study in which all HEIs in a country are evaluated. This provides unique possibilities for identifying success factors and barriers. The importance of the leadership of the HEIs became clear.
