    • AIMS: Valvular surgery has improved long-term prognosis in severe carcinoid heart disease (CaHD). Experience is limited and uncertainty remains about predictors for survival and strategy regarding single vs. double-valve surgery. The aim was to review survival and echocardiographic findings after valvular surgery for CaHD at our institution.METHODS AND RESULTS: Between 1986 and 2019, 60 consecutive patients, median age 64 years, underwent valve surgery for severe CaHD. Operations involved combined tricuspid valve replacement (TVR) and pulmonary valve replacement (PVR) in 42 cases, and TVR-only or TVR with pulmonary valvotomy (no PVR) in 18 patients. All implanted valves were bioprosthetic. Preoperative echocardiography, creatinine, NT-pro-brain natriuretic peptide (NT-pro-BNP), and 24-h urinary 5-hydroxyindoleacetic acid (5-HIAA) were obtained. 30-Day mortality was 12% (n=7), and 8% for the most recent decade 2010-2019. Median survival was 2.2 years and maximum survival 21 years. Patients undergoing combined TVR and PVR had significantly higher survival compared with operations without PVR (median 3.0 vs. 0.9 years, P = 0.02). Preoperative levels of NT-pro-BNP and 5-HIAA in the top quartile predicted poor survival. On preoperative echocardiograms, pulmonary regurgitation was severe in 51% and indeterminate in 17%. Postoperative echocardiography confirmed relatively good durability of bioprostheses, relative to the patients' limited oncological life expectancy.CONCLUSION: Valvular surgery in CaHD has an acceptable perioperative risk. Survival for combined TVR and PVR was significantly higher compared with operations without PVR. Bioprosthetic valve replacement appears to have adequate durability. Preoperative echocardiography may underestimate pulmonary pathology. Combined TVR and PVR should be considered in most patients.

Idé och koncept Lena Hellström-Färnlöf, regi Patrik Sörling, musikalisk ledning Bengt-Åke Lundin. Sångare från kammaroperautbildningen framförde konceptet. Arbetet utgick från idéer kring scenisk och musikalisk impuls. Både historiska och nutida frågeställningar om genus och corona berördes i den handling som genom improvisation med vissa förtecken växte fram tillsammans med teamet och sångstudenterna under våren 2020. Arbetet dokumenterades genom audio/video-inspelning.

  • Exploring pharmacists’ perspectives about substandard and falsified medical products through interviews

    • BackgroundThe problem with substandard and falsified (SF) medical products may grow in high-income countries when e-commerce of medicines increases. Unauthorized websites offer medicines of insufficient quality. This underscores the importance of evaluating how the problem with SF medical products can be prevented from escalating. However, little is known about what knowledge and experience professionals working primarily with medicines have about the phenomenon.ObjectiveThis study was conducted to explore purposively selected pharmacists' experience and knowledge about SF medical products.MethodsTwelve individual interviews were conducted with purposively selected pharmacists between May 2021 and September 2021. An interview guide was used with specific questions about e-commerce, which focused on exploring pharmacists' experience and knowledge about SF medical products. The interviews lasted, on average, 49 min and were analyzed using inductive qualitative content analysis.ResultsA main theme ‘Pharmacists as guardians of safe medicines’ emerged. This theme consisted of three categories pinpointing ‘risk factors’, ‘protective factors’, and ‘opportunities for improvement’ regarding SF medical products. Findings suggest that pharmacists can play a role in preventing the problem with SF medical products from escalating. Participants emphasized they were in this line of work to help patients and increase patient safety.ConclusionsPharmacists have the opportunity to empower the public with knowledge about SF medical products since they discuss medicines with many people every day. Awareness of risk factors for SF medical products enables pharmacists to guide patients to avoid risky purchases from unauthorized websites. To do this, better communication, and cooperation with patients and other healthcare professionals are needed.

  • Gas in Scattering Media Absorption Spectroscopy on Small and Large Scales: Towards the Extension of Lung Spectroscopic Monitoring to Adults

    • Numerous natural materials are porous, contain free gas, and are scattering light strongly. Scattering brings about a strong trapping of light and an associated prolonged transit time for photons through a medium. In contrast to the matrix materials, gas enclosures require very narrow‐band laser radiation for probing. We have in the present study used the gas in scattering media absorption spectroscopy (GASMAS) method to study free oxygen in thin (cm) samples utilizing a tunable diode laser, while a pulsed dye laser was employed in corresponding measurements on larger samples, up to the meter scale. Time‐resolved spectroscopy was in both cases used to assess the temporal distribution of the detected photons, mapping the path lengths through the media, which ranged between few centimeters up to 100 m. This study explores the feasibility to extend recent successful monitoring of gases in neonatal infant lungs to the case of larger children or even adults, which could have very important applications, for example, in ventilator setting optimization for severely ill patients, suffering, for example, from SARS‐CoV‐2. The conclusion of our work is that this goal most realistically can be reached by applying intra‐tracheal laser light illumination at the 1 W power level, employing a tapered amplifier, injected with a distributed feed‐back diode‐laser oscillator output and combined with wavelength‐modulation spectroscopy.

  • Development of an All-Marine 3D Printed Bioactive Hydrogel Dressing for Treatment of Hard-to-Heal Wounds

    • Current standard wound care involves dressings that provide moisture and protection; however, dressings providing active healing are still scarce and expensive. We aimed to develop an ecologically sustainable 3D printed bioactive hydrogel-based topical wound dressing targeting healing of hard-to-heal wounds, such as chronic or burn wounds, which are low on exudate. To this end, we developed a formulation composed of renewable marine components; purified extract from unfertilized salmon roe (heat-treated X, HTX), alginate from brown seaweed, and nanocellulose from tunicates. HTX is believed to facilitate the wound healing process. The components were successfully formulated into a 3D printable ink that was used to create a hydrogel lattice structure. The 3D printed hydrogel showed a HTX release profile enhancing pro-collagen I alpha 1 production in cell culture with potential of promoting wound closure rates. The dressing has recently been tested on burn wounds in Göttingen minipigs and shows accelerated wound closure and reduced inflammation. This paper describes the dressings development, mechanical properties, bioactivity, and safety.
