    • The care for persons with dementia is involving a diversity of social and health care interventions. The systems for dementia care and services vary across European countries. However, there is a lack of knowledge about the complex system for dementia care on a European level. A more comprehensive view of dementia care provision in Europe may provide knowledge that can serve as a basis for the development of best practice in the care for persons with dementia. The objective of the study is to describe and analyse the European health and social care and welfare systems, advocacy and informal caregiver support systems for people with dementia. The presentation will focus on how data will be collected and how collaboration within the consortium will be performed.A consortium including members from eight European countries is invited in the study. The research teams that will collect the data represent Estonia, Finland, France, Germany, Netherlands, Spain, Sweden and United Kingdom. The Swedish research team coordinates the study. The consortium members will review a suggested terminology for overall issues regarding the care system for people with dementia as well as national directives and nancing. Further, data will comprise the terminology regarding descriptions of stages in the dementia trajectory, types of care provided and care providers. From this process, an overall agreement on the meaning of relevant terminology is going to be obtained, and this will make it possible to describe the health and social care system for older persons with dementia in a European perspective. A developed template will be used to collect data regarding health and social care and welfare system from each country included in the study.

    • There are numerous of challenges confronting social work with older people. In Sweden, this is scarcely discussed, which is interesting in light of the radical organisational change in 1992, when all eldercare became a municipal responsibility governed by the Social Service Act. At this point in time, a decisive change was also introduced in England. The legislative changes have had a major impact on the roles and tasks of social work with older people. The overarching aim of this paper is to discuss the role of social work in cross-professional teamwork focused on older people. A multidisciplinary older peoples team in the English Midlands, consisting of fifteen members from different professions, was followed. Theories of cross-professional teamwork and inter-professional rivalries were used. The findings demonstrated a collaborative cross-professional teamwork and the different roles were genuinely complementary and not competitive. Social work was an integrated part; thus, it seemed to have an impact on health care for older people in this context. It suggests that it might be time to move on from the excessively pessimistic view and we hope that this statement may also be valid for the situation in Sweden.

The aim was to explore the staff's view on the illness dementia; on older peoplesuffering from dementia; the care provided and the impact of various views on the care. The carewas studied through observations at 22 care settings in the three countries and the staff'sattitudes were explored by means of interviews with 79 people. A relation-orientedphenomenological approach was used. Great differences appeared regarding the various staffgroups' views. The findings also indicated that the staff's view; the cultural context and theorganizational environment might have an impact on the care provided. However the impact ofprofessional affiliation and the way of collaborating seemed to be of greater significance than othersimilarities or differences. An interdisciplinary based starting point, with close collaborationbetween medical based health care and social care of older people suffering from dementia inintegrative care models, is therefore of vital importance.

  • Changes in outdoor mobility when becoming alone in the household in old age

    • The aim of this article is to analyze reported changes in outdoor mobility, increased/unchanged/decreased, for a sample of older people (>62 years) in two regions in Sweden, who have transitioned from a two-person to a single-person household during the two years since the study was conducted. The target group (N=162) consists of all people who had transitioned to a single-person household in a random sample of 2033 people. The predominant results reveal that the stressful life event of transitioning into a single-person household in old age means reduced outdoor mobility for certain sub-groups. All modes of transport are used similarly regardless of reported changes in mobility (except for walking). Our results suggest that society must put more effort into offering good walking conditions, since (a) walking seems to be the most important mode of transport for outdoor mobility and (b) walking is valuated almost as high as car after becoming alone in the household regardless if the population in our study reported unchanged, decreased or increased mobility. Further, illuminating another result, namely that special transport service 3 (STS) came out as especially important for people with increased activity, society also needs to invest in the provision of STS to keep the most vulnerable group of people mobile when other modes of transport are no longer a reality.

    • The aim of this article is to reveal how family members act, react and reason when their elderly relative considers relocation to a residential home. Since family members are usually involved in the logistics of their elderly relative's relocation, yet simultaneously expected not to influence the decision, the focus is on how family members experience participation in the relocation process in a Swedish context. 17 family members are included in 27 open, semi-structured interviews and follow-up contacts. Prominent features in the findings are firstly the family members' ambition to tone down their personal opinions, even though in their minds their personal preferences are clear, and secondly, the family members' ambivalence about continuity and change in their everyday lives. Family members are found to apply the adapting, the representing, or the avoiding strategy, indirectly also influencing their interaction with the care manager. Siblings applied the adapting strategy, spouses the representing strategy, while family members in the younger generation at times switched between the strategies.

    • Based on older people's perspectives, the aim of this article is to reveal how the culture of independence influences the decision-making process preceding relocation to a residential home. Since there is a predominant ideology of ageing in place in Sweden like in many other welfare states, the focus is on how a continued life in ordinary housing is justified versus how relocation to a residential home is excused. Twenty-one older people have been included in open semi-structured interviews and in follow-up contacts. The findings show that the value of independence, originally intended to protect the position of older people, in practice led to stigmatizing processes. In order to bridge the gap between values and declining capacities, expectations and actions, older people develop individual-oriented, family-oriented, and public-oriented justifications and excuses, so-called ‘accounts.’

    • Self-control and autonomy are not guaranteed when people age and are in need of help from others, especially in institution-based housing. This occurs despite the fact that it is a generally accepted belief that care should be delivered with the greatest possible degree of independence and autonomy. This article discusses older people's experiences with and the opportunities for autonomy in institution-based housing. Through focus group interviews and observations, the daily lives of residents at two nursing homes in Sweden were studied through a comparative approach using the theoretical framework of organizational culture. Three themes emerged showing how autonomy was not a reality for the residents. The first theme revealed an ambivalent mission, indicating ambiguity as to whether the nursing home was a place to live in or a place in which to be cared for. The second theme was symbolic power, which encompassed the staff's power embedded in the organization. The third theme was the ageist approach to care, which was noted in the way staff considered the residents to be old people who were unable and unwilling to strive for autonomy. The three themes were embedded in the organizational culture and were created and recreated in the interaction between residents and staff. A key question for further research is how to change the culture to strengthen the residents’ autonomy.
