    • OBJECTIVES: Allogeneic blood transfusion and reoperation for postoperative bleeding after the coronary artery bypass grafting have a negative impact on the patient outcome. This study aimed at evaluating the effects of reduced doses of heparin and protamine on the patient outcome, using a heparin-coated mini-cardiopulmonary bypass (CPB) system. METHODS: Sixty patients undergoing elective first-time CPB were prospectively randomized either to have a reduced systemic heparinization [activated clotting time (ACT) = 250 s] or to a control group perfused with a full heparin dose (ACT = 420 s). Blood transfusions, ventilation time, early postoperative bleeding, ICU stay, reoperations for bleeding, postoperative cognitive status and the level of mobilization were registered. RESULTS: Twenty-nine patients were randomized to the control group, 27 patients to the low-dose group and 4 patients were excluded because of protocol violations. Four patients in the control group received a total of 10 units of packed red blood cells, and in the low-dose group, no transfusions were given, P = 0.046. No patient was reoperated because of bleeding. The ICU stay was significantly shorter in the low-dose group (8.4 vs 13.7 h, P = 0.020), less dependent on oxygen on the first postoperative day (78 vs 97%, P = 0.034), better mobilized (89 vs 59%, P = 0.006) and had less pain (visual analogue scale 2.0 vs 3.5, P = 0.019) compared with the control group. CONCLUSIONS: The use of a mini-CPB system combined with a low dose of heparin reduced the need for blood transfusions and may facilitate the faster mobilization of the patients.

    • The discolouration of hydrogen-peroxide bleached Norway spruce mechanical pulp during storage in mill systems was studied and the contributions of process water, iron and dyes were evaluated over the visible spectrum. Washing of the pulp made it less sensitive to storage, possibly due to the removal of extractives, lignin-like substances, iron and pulp fines. Storage in white water gave extensive discolouration with a shoulder in the absorption spectrum at around 550-650 nm. Most of the colour was associated with pulp fines or filler but some colour was also found in smaller fractions and in the water phase. The addition of ferric ions increased the light absorption coefficient during storage, initially at short wavelengths and then over the whole spectrum, but could not explain the increased absorption at 550-650 nm and could not be the only cause of the darkening in the mill system. A cationic basic violet dye gave a shoulder in the absorption spectrum similar to that in the mill system, but the absorption in this area did not increase during storage. Model calculations indicate that ferric ions together with violet and red dyes could explain a major part, but not all, of the colour observed in the mill system after storage. The darkening not accounted for at longer wavelengths and around 550-650 nm is suggested to be related to fines and fillers including dissolved and colloidal substances associated with these particles.

  • Storage of Hydrogen Peroxide Bleached Mechanical Pulp : Reduction in Reflectance over the Visible Spectrum

    • The objective of this thesis is to determine possible causes of the darkening of hydrogen peroxide bleached mechanical pulp over the visible spectrum and their relative contributions. It focuses on both process conditions and the composition of the pulp and the dilution water, including additions or losses of material along the process line from the bleach tower to the paper machine.A mapping of the optical properties of the pulp along the process showed that the fine fraction of the pulp darkened more than the long fibre fraction. Simulation of retention times of different fractions showed that the main part of the fine material is retained in the paper within a few hours, a small part might circulate for considerably longer time and may therefore be strongly coloured.Storage trials were mainly performed using a hydrogen peroxide bleached mechanical pulp intended for SC paper made of Norway spruce (Picea abies), sampled on one occasion and stored in a freezer. Unwashed or well-washed pulp was stored in distilled water or in different process waters. Some complementary trials were included, e.g. unbleached pulp.Time and temperature were the process variables that gave the strongest darkening of the pulp, as expected, both in a clean and a more process-like system, whereas pH only had an effect in the presence of process waters; the highest brightness stability was seen at a pH around 5.5–6.0.The darkening was due to an increase in the light absorption coefficient (k) beginning at short wavelengths, but after longer storage times the increase in kλ also became noticeable at longer wavelengths. The colour (CIE L*, a*, b*) of the pulp changed towards red and yellow, initially more towards red and then more towards yellow. These changes were clearly visible.Washing of the bleached pulp made it less sensitive to storage; possibly due to the removal of extractives, lignin-like substances, metals and pulp fines. This washing had little effect before storage and the amount of material removed was small.The pulp darkened more when stored in process waters compared to distilled water. Apart from fibres, most of the colour was associated with pulp fines or filler but some colour was also found in the dissolved and colloidal fractions. At an increased pulp consistency, the increase in k460 was smaller.Storage in white water from the paper machine gave extensive discolouration with a shoulder in the absorption spectrum around 550–650 nm, which increased with time. The addition of ferric ions increased the light absorption coefficient during storage, but could not explain the increased absorption at 550–650 nm nor could it be the only cause of the darkening in the mill system. It is approximately six times faster than standard methods, reduces the risk of additional darkening of the sample and can be used with small pulp quantities.The deviation from the expected linear behaviour of the light scattering coefficient, s, at wavelengths corresponding to strong light absorption has been studied using the Kubelka-Munk model and the angular resolved DORT2002 radiative transfer solution method. The decrease in s could not be explained by errors introduced in the Kubelka-Munk modelling by anisotropic scattering.

    • To evaluate the causes behind the formation of colour in mill systems, the effect of different process waters on the optical properties of hydrogen-peroxide bleached Norway spruce mechanical pulp during storage was studied. The influence of process water type, temperature, time, pulp consistency and pH was evaluated over the visible range of wavelengths. The darkening was more pronounced when pulp was stored in process waters compared to in distilled water. Increased temperature and prolonged storage times increased the darkening as expected, whereas a higher storage consistency gave less darkening than a lower storage consistency (15% compared to 5%). The pH value that gave the lowest brightness reduction and a minimum in Delta k(460) was found to be 5.5-6.0. Storage of pulp in white water from the paper machine resulted in a broad shoulder in the k spectra indicating colour formation related to the particles in this water. Part of this shoulder in absorption spectra was related to added dyes, but the increase in absorption with time is likely to be caused by some other colour formation. The colour measured as a*, b* showed that the storage changed the colour of the pulp towards red and yellow, and initially more towards red.

It reduces the risk of contamination and discoloration and can be used with small pulp quantities. The deviation from the expected linear behaviour of the light scattering coefficient, s, at wavelengths corresponding to strong light absorption, has been studied using the Kubelka-Munk model and the angular resolved DORT2002 radiative transfer solution method. This decrease in s could not be explained by errors introduced in the Kubelka-Munk modelling by anisotropic scattering. Linear extrapolation of s can therefore not be justified as a way to obtain a more correct light absorption coefficient, k. For thepulps studied, the decrease in s at short wavelengths had little effect on k at 457 nm.

  • Strategies to reduce heat-induced darkening for enhanced bleachability of mechanical pulps

    • Under mechanical pulping conditions, elevated temperature initiates reactions in the wood constituents resulting in the formation of coloured structures. In an attempt to reduce the formation of colour, a number of additives have been examined to assess their ability to inhibit heat-induced darkening of a Norway spruce (Picea abies) stone groundwood (SGW) pulp. SGW pulp was used as a model pulp with the purpose of imitating a thermomechanical pulp (IMP). The heat-treated SGW pulp, both with and without additives present, was subsequently bleached with 4.5% hydrogen peroxide at high pulp consistency and with varying alkali charges in order to examine the hydrogen peroxide bleachability of such pulps. Heat treatment at 170 °C resulted in a loss of brightness of about 4% ISO after heating for 60 seconds. The reduction in brightness could only partly be regained through high consistency hydrogen peroxide bleaching; the SGW pulp displayed a brightness of 83.7% ISO whereas the heat-treated SGW pulp reached a brightness of 80.4% ISO. Heat treatment with a low addition rate (1 %) of alkaline hydrogen peroxide, sodium bisulfite or sodium sulfite, with a subsequent hydrogen peroxide bleaching stage, resulted in an improved bleached brightness of about 81.6% ISO, i.e. the loss of brightness due to heat was still approximately 2% ISO. Sodium borohydride (1 %) did not suppress the thermal darkening but improved the bleached brightness by approximately 2% ISO compared to the heat-treated SGW pulp and resulted in a brightness of 82.6% ISO. Addition of a chelating agent (DTPA, 0.11 and 0.45%) and the radical scavengers phthalic hydrazide (1 %) and 1 -hexanol (1 %) suppressed the heat-induced darkening somewhat but did not improve the hydrogen peroxide bleachability to any noticeable extent. Addition of citric acid (0.6%) did not suppress the thermal darkening and resulted in impaired bleachability.

    • The pH-profile in the hydrogen peroxide bleaching stage of a Norway spruce thermomechanical pulp was levelled out by using carbon dioxide in combination with sodium or magnesium based alkali sources. Addition of carbon dioxide when magnesium hydroxide was used as the alkali source increased the pulp brightness with up to 1.5 brightness units when the bleaching was performed at 70 °C. When the temperature was increased from 70°C to 90 °C the brightness was increased with about two brightness units, addition of carbon dioxide increased it further. The residual hydrogen peroxide and the chemical oxygen demand (COD) levels were not affected as a consequence of the carbon dioxide addition. In the experiments where sodium hydroxide was used as the alkali source, addition of carbon dioxide lowered the pH that resulted in a decreased COD level, a slightly lower brightness level and, to some extent, an increased residual hydrogen peroxide level.

  • The Scandinavian multicenter hemodynamic evaluation of the SJM Regent aortic valve

    • Background: 112 patients who received small and medium sized St.Jude Regent heart valves (19-25 mm) at 7 Scandinavian centers were studied between January 2003 and February 2005 to obtain non-invasive data regarding the hemodynamic performance at rest and during Dobutamine stress echocardiography (DSE) testing one year after surgery. Material and methods: 46 woman and 66 men, aged 61.8 +/- 9.7 (18-75) years, were operated on for aortic regurgitation (17), stenosis (65), or mixed dysfunction (30). Valve sizes were 19 mm (6), 21 mm (33), 23 mm (41), 25 mm (30). Two patients receiving size 27 valves were excluded from the hemodynamic evaluation. Pledgets were used in 100 patients, everted mattress in 66 and simple interrupted sutures in 21. Valve orientation varied and was dependent on the surgeons' choice. 34 patients (30.4%) underwent concomitant coronary artery surgery. Results: There were two early deaths (1.8%) and three late deaths, one because of pancreatic cancer. Late events during follow-up were: non structural dysfunction (1), bleeding (2), thromboembolism (2). At one year follow up 93% of the patients were in NYHA classes 1-2 versus 47.8% preoperatively. Dobutamine stress echocardiography (DSE) was performed in a total of 66 and maximal peak stress was reached in 61 patients. During DSE testing, the following statistically significant changes took place: Heart rate increased by 73.0%, cardiac output by 85.5%, left ventriclular ejection fraction by 19.6%, and maximal mean prosthetic transvalvular gradient by 133.8%, whereas the effective orifice area index did not change. Left ventricular mass fell during one year from 215 +/- 63 to 197 +/- 62 g (p < 0.05). Conclusion: The Dobutamine test induces a substantial stress, well suitable for echocardiographic assessment of prosthesis valve function and can be performed in the majority of the patients. The changes in pressure gradients add to the hemodynamic characteristics of the various valve sizes. In our patients the St. Jude Regent valve performed satisfactory at rest and under pharmacological stress situation.

  • Common genetic determinants of lung function, subclinical atherosclerosis and risk of coronary artery disease.

    • Chronic obstructive pulmonary disease (COPD) independently associates with an increased risk of coronary artery disease (CAD), but it has not been fully investigated whether this co-morbidity involves shared pathophysiological mechanisms. To identify potential common pathways across the two diseases, we tested all recently published single nucleotide polymorphisms (SNPs) associated with human lung function (spirometry) for association with carotid intima-media thickness (cIMT) in 3,378 subjects with multiple CAD risk factors, and for association with CAD in a case-control study of 5,775 CAD cases and 7,265 controls. SNPs rs2865531, located in the CFDP1 gene, and rs9978142, located in the KCNE2 gene, were significantly associated with CAD. In addition, SNP rs9978142 and SNP rs3995090 located in the HTR4 gene, were associated with average and maximal cIMT measures. Genetic risk scores combining the most robustly spirometry-associated SNPs from the literature were modestly associated with CAD, (odds ratio (OR) (95% confidence interval (CI95) = 1.06 (1.03, 1.09); P-value = 1.5×10-4, per allele). In conclusion, our study suggests that some genetic loci implicated in determining human lung function also influence cIMT and susceptibility to CAD. The present results should help elucidate the molecular underpinnings of the co-morbidity observed across COPD and CAD.

    • Cancer-associated fibroblasts (CAF) attract increasing attention as potential cancer drug targets due to their ability to stimulate, for example, tumor growth, invasion, angiogenesis, and metastasis. However, the molecular mechanisms causing the tumor-promoting properties of CAFs remain poorly understood. Forkhead box F1 (FoxF1) is a mesenchymal target of hedgehog signaling, known to regulate mesenchymal-epithelial interactions during lung development. Studies with FoxF1 gain- and loss-of-function fibroblasts revealed that FoxF1 regulates the contractility of fibroblasts, their production of hepatocyte growth factor and fibroblast growth factor-2, and their stimulation of lung cancer cell migration. FoxF1 status of fibroblasts was also shown to control the ability of fibroblasts to stimulate xenografted tumor growth. FoxF1 was expressed in CAFs of human lung cancer and associated with activation of hedgehog signaling. These observations suggest that hedgehog-dependent FoxF1 is a clinically relevant lung CAF-inducing factor, and support experimentally the general concept that CAF properties can be induced by activation of developmentally important transcription factors.

    • The effect of treating thermomechanical pulp with Fenton chemistry was evaluated in a model study. Ferrous ions and chelated ferric ions were compared as catalysts while initial pH was varied. The results showed that a catalyst of ferrous sulphate had a greater impact on the pulp (increased total fibre charge and carbonyl groups, more dissolved organic material in filtrate) than ferric ions chelated with EDTA. If using ferric-EDG as catalyst, the hydrogen peroxide consumption was higher and more hydroxyl radicals were detected compared to if using ferrous sulphate. Ferric-EDG, however, gave similar or less effect on the pulp.

    • The influence of different types of iron salts (i.e., ferrous or ferric cations with sulphate, nitrate or chloride anions) on the reaction between coarse thermomechanical pulp and acid hydrogen peroxide (Fenton chemistry) was studied when the initial pH was 3.2 and 5.3. Also, ferric ions chelated with EDTA or EDG at different molar ratios were compared with ferrous sulphate when the initial pH was extended from about 3 to 8. Different anions of ferric ion salt gave a similar catalytic effect. At an initial pH of 7–8, the ferric-EDTA catalyzed reaction resulted in similar or higher hydrogen peroxide consumption and more detectable hydroxyl radicals than the ferrous sulphate catalyzed reaction, but less reaction with the pulp was indicated. Between pH 5–8, using Fe-EDG as a catalyst gave higher hydrogen peroxide consumption and more detectable hydroxyl radicals than if using ferrous sulphate; however, the measured effect on the pulp was similar or less.

  • The use of Fenton chemistry for reducing the refining energy during TMP production : the effect of free ferrous and free or chelated ferric ions

    • The potential of using acid hydrogen peroxide under Fenton conditions to lowerthe electrical energy consumed during the production of Black spruce (Piceamariana) thermomechanical pulp (TMP) was investigated in pilot scale. Thechemical system, which consisted of ferrous sulphate, hydrogen peroxide andoptionally an enhancer (such as a chelating agent), was evaluated as an inter-stagetreatment. The produced TMPs were thoroughly characterised in order to explainthe effect of the chemical system on fibre development and to be able to propose amechanism for the impact on refining energy reduction. The possibility to improvethe optical properties by washing, chelating and sodium dithionite or hydrogenperoxide bleaching the treated pulps was evaluated. The system of lignocellulosicmaterial, a Norway spruce (Picea abies) TMP, and Fenton chemistry was alsoevaluated in a model study to understand more about how conditions such as e.g.initial pH, dissolved organic material and reaction time affect the reactions.Ferrous and ferric ions (free and chelated) and different anions were evaluated.Moreover, it was examined whether hydroxyl radicals could be detected andmonitored.The results obtained in pilot scale showed that it is possible to significantly reducethe specific energy consumption by approximately 20% and 35% at a freenessvalue of 100 ml CSF or a tensile index of 45 Nm/g by using 1% and 2% hydrogenperoxide respectively. The energy reduction was obtained without any substantialchange to the fractional composition of the pulp, although tear strength wasslightly reduced, as were brightness and pulp yield. No major differences betweenthe reference pulp and the chemically treated pulps were found with respect tofibre length, width or cross-sectional dimensions. However, the acid hydrogenperoxide-treated pulps tended to have more collapsed fibres, higher flexibility, alarger specific surface area and a lower coarseness value. The yield lossiiiaccompanying the treatment was mainly a consequence of degradedhemicelluloses. It was also found that the total charge of the chemically treatedpulps was higher compared to the reference pulps; something that may haveinfluenced the softening behaviour of the fibre wall.A washing or chelating procedure could significantly reduce the metal ion contentof the chemically treated TMPs. The amount of iron could be further reduced to alevel similar to that of untreated pulps by performing a reducing agent-assistedchelating stage with dithionite. The discoloration could not, however, becompletely eliminated. The brightness decrease of the treated pulps was thereforenot only caused by the greater iron content in the pulp, but was also dependent onthe type of iron compound and/or other coloured compounds connected with theacid hydrogen peroxide treatment. Oxidative bleaching using hydrogen peroxidewas more effective than reductive bleaching using sodium dithionite in regainingthe brightness that was lost during the energy reductive treatment.From the model study and by using a chemiluminescence method, it could beconcluded that hydroxyl radicals were present in the system of Fenton chemicalsand lignocellulosic material (TMP). Initial pH, retention time, pulp consistency,type of catalyst (free or chelated) and dissolved organic material had an impact onthe reactions between TMP and acid hydrogen peroxide. Different anions(sulphate, nitrate and chloride) of ferric ion salt gave a similar catalytic effect.There appeared to be more reactions with the TMP when there was less dissolvedorganic material in the liquid phase from the start. A catalyst of ferrous sulphatehad a greater impact on the pulp (increased total fibre charge and carbonyl groups,more dissolved organic material in filtrate) than ferric ions chelated withethylenediaminetetraacetic acid at an initial pH of about 3-7. If using ferric-EDG(ethanol diglycinic acid) as catalyst, the measured effect on the pulp was similar orless compared to using ferrous sulphate. Ferric-EDG, however, gave higherhydrogen peroxide consumption and more detectable hydroxyl radicals than usingferrous sulphate (initial pH 5-8). It is likely that the iron catalyst must bind to theTMP, or be in close proximity to it, for the hydroxyl radicals to be able to react withthe material.A mechanism was proposed: the hydroxyl radicals generated in the Fentonreaction will probably attack and oxidise the available outer fibre surfaces,weakening these layers, and simultaneously dissolve some of the organic material.This can facilitate fibre development, give a better bonding pulp and reduce theelectrical energy required during refining.
