    • The potential for using deep inelastic reactions to populate high-spin states in neutron-rich nuclei is studied in a series of experiments using GAMMASPHERE for gamma-ray detection and a silicon strip detector for measuring the angles of projectilelike and targetlike fragments. In three experiments 61 new transitions up to a maximum spin of 22 (h) over bar in 12 neutron-rich rare-earth nuclei were found. We observe that gamma-ray yields as a function of spin are flatter for all neutron transfer products than for inelastic excitation of either the projectile or target nucleus. Calculations are presented which indicate that this difference cannot be accounted for by quasielastic processes, but more likely are the result of larger energy loss processes, such as deep inelastic reactions.     • A study was conducted on utilization of and satisfaction with the health care and medical services among 527 Scandinavian children (aged 4-18) with myelomeningocele (MMC). Data were collected from two sources: a questionnaire distributed to parents in 1984, and patients' medical records. Comparisons were made with a control group comprised of 7,792 children. Over a period of one year, 52% of the MMC group and 6% of the controls were hospitalized. Over 3 months, children with MMC had more contact with specialist care services than had the controls, while contact with primary health care was the same for both groups. Continuity of care and satisfaction with time spent with the physician were both greater among children with MMC than in the control group. Dissatisfaction with medical services was expressed by approximately 10% of the parents of both categories. Such dissatisfaction was found significantly more frequently among Danish parents and well educated mothers. Dissatisfaction with care was not related to either the child's age or the severity of its disability. Danish children with MMC received treatment relatively more frequently from primary care physicians than from specialists. In Sweden, where satisfaction was the greatest, families with children with MMC were supported by local habilitation teams.

    • BACKGROUND: Laparoscopic procedures of increasing difficulty and duration are becoming more and more common. This may cause significant challenges to the circulatory system and possibly influence thrombogenicity.METHODS: Experimental study of carbon dioxide pneumoperitoneum in pigs.RESULTS: Inferior caval vein blood flow remained unchanged, whereas inferior caval vein pressure increased during pneumoperitoneum. Inferior caval vein, pulmonary, and systemic vascular resistance increased during pneumoperitoneum and remained increased after exsufflation.CONCLUSIONS: Pneumoperitoneum leads to an increased inferior caval vein pressure, which could cause a dilation of peripheral veins. The similar patterns of vascular resistance in the inferior caval vein, pulmonary artery, and systemic arteries (a gradual increase remaining elevated after exsufflation) suggest a common humoral factor or increased sympathetic nerve activity.

  • Incidence of thromboembolic complications after laparoscopic cholecystectomy : review of the literature.

    • The purpose of this study was to quantify the risk of thromboembolic complications after laparoscopic cholecystectomy by a survey of the literature. We reviewed 60 laparoscopic cholecystectomy series consisting of 153,832 patients. The average mortality was 0.08%. The average rate of fatal pulmonary embolism was 0.02% and total pulmonary embolism 0.06%. The average rate of reported deep vein thrombosis was 0.03%. We conclude that laparoscopic cholecystectomy is a safe procedure, and the rate of clinically evident postoperative thromboembolic complications is probably lower than after conventional cholecystectomy. A lingering bias due to the overrepresentation of young and healthy patients early in the era of laparoscopic cholecystectomy could, however, still affect these figures. An underreporting of the lesser complications is likely. The risk is not negligible, though, and some authors have recommended thromboembolism prophylaxis, although further studies are necessary to find the optimal prophylaxis strategy. The true incidence is possible to establish only by using objective diagnostic methods for surveillance.

    • In a recent paper [Phys. Rev. B 48, 17 806 (1993)] Cunha, Canuto, and Fazzio reported ab initio Hartree-Fock calculations on nitrogen impurities in group-IV semiconductors. In their paper it is suggested that nitrogen pairs form from substitutional atoms on adjacent lattice sites. However, the experimentally observed configuration in silicon and germanium is different from this. The aim of this Comment is to clarify the situation of the nitrogen pair in these materials.

    • The nitrogen-pair oxygen defect in silicon has been studied by infrared absorption spectroscopy on samples implanted with various combinations of 14N, 15N, 16O and 17O. The measurements give direct evidence for the involvement of nitrogen and oxygen in the defect and show that the impurity atoms comprising the defect are only weakly coupled. Ab initio cluster calculation on several models of the nitrogen-pair oxygen defect have been performed and are compared with experiment. Based on these investigations a model consisting of a bridging oxygen atom adjacent to the nitrogen pair is suggested

    • Nitrogen-oxygen complexes in silicon have been studied by infrared absorption spectroscopy on samples implanted with various combinations of O and N isotopes as well as O-rich samples doped with N during growth. Local vibrational modes at 805, 999 and 1030 cm-1 (14N, 16O) are directly shown to belong to a defect consisting of a N-pair interacting with an O interstitial. A recently proposed model of this NNO defect is further investigated by ab initio cluster calculations and found to account for the dynamical properties of the defect. A mode around 730 cm-1 predicted by the calculation is observed giving further support to the model.

    • TB PNA FISH is a new fluorescence in situ hybridization (FISH) method using peptide nucleic acid (PNA) probes for differentiation between species of the Mycobacterium tuberculosis complex (MTC) and nontuberculous mycobacteria (NTM) in acid-fast bacillus-positive (AFB+) cultures is described. The test is based on fluorescein-labelled PNA probes that target the rRNA of MTC or NTM species applied to smears of AFB+ cultures for microscopic examination. Parallel testing with the two probes serves as an internal control for each sample such that a valid test result is based on one positive and one negative reaction. TB PNA FISH was evaluated with 30 AFB+ cultures from Denmark and 42 AFB+ cultures from Thailand. The MTC-specific PNA probe showed diagnostic sensitivities of 84 and 97%, respectively, and a diagnostic specificity of 100% in both studies, whereas the NTM-specific PNA probe showed diagnostic sensitivities of 91 and 64%, respectively, and a diagnostic specificity of 100% in both studies. The low sensitivity of the NTM-specific PNA probe in the Thai study was due to a relatively high prevalence of Mycobacterium fortuitum, which is not identified by the probe. In total, 63 (87%) of the cultures were correctly identified as MTC (n = 46) or NTM (n = 17), whereas the remaining 9 were negative with both probes and thus the results were inconclusive. None of the samples were incorrectly identified as MTC or NTM; thus, the predictive value of a valid test result obtained with TB PNA FISH was 100%.
