    • The instrumentation in Hall A at the Thomas Jefferson National Accelerator Facility was designed to study electro-and photo-induced reactions at very high luminosity and good momentum and angular resolution for at least one of the reaction products. The central components of Hall A are two identical high resolution spectrometers, which allow the vertical drift chambers in the focal plane to provide a momentum resolution of better than 2 x 10(-4). A variety of Cherenkov counters, scintillators and lead-glass calorimeters provide excellent particle identification. The facility has been operated successfully at a luminosity well in excess of 10(38) CM-2 s(-1). The research program is aimed at a variety of subjects, including nucleon structure functions, nucleon form factors and properties of the nuclear medium. (C) 2003 Elsevier B.V. All rights reserved.

  • High-order-harmonic generation: towards laser-induced phase-matching control and relativistic effects

    • We present a review of some recent results on high-order-harmonic generation, aiming at optimizing the photon flux to allow for future applications in extreme-ultra-violet non-linear optics. We first present new schemes to control phase matching of high harmonics in gases, by using the effect of the spatially varying atomic phase displayed by the high harmonics. An enhancement by a factor of 50 is observed in neon in conditions for which the gradient of the atomic dispersion balances the electronic dispersion. A new scheme to manipulate the laser field was demonstrated, and shown to improve phase matching. We then turn to high-harmonic generation by solid targets, and show that high harmonics generated by an intense 30-fs laser pulse remain collimated even at the threshold of the relativistic regime.

  • Psychosocial and geriatric correlates of functional status after total hip replacement

    • Objective. To determine whether psychosocial factors, chronic diseases, and common geriatric problems are associated with poor physical function 3 years after primary total hip replacement (THR). Methods. We studied a sample of Medicare recipients in Ohio, Pennsylvania, and Colorado (n = 922) who underwent primary THR in 1995 (mean +/- SD age 73.1 +/- 5.6 years, 32% men). Participants completed a questionnaire regarding lifestyle factors, medical history, and quality of life similar to3 years after the surgery. Physical function was measured using the function subscale of the Western Ontario and McMaster Universities Osteoarthritis Index. We assessed the relationship between functional outcome 3 years postsurgery and 4 predictor domains: pain or complications in the operated hip, other musculoskeletal comorbidity, medical factors (obesity, chronic medical comorbidity, rheumatoid arthritis, and such common geriatric problems as falls, poor balance, or incontinence), and psychosocial factors (mental health, regular alcohol consumption, smoking, provider role, living alone, and education). Results. Ten percent of subjects had poor functional status. In a logistic regression model controlling for sex and age, the following factors were associated with an increased risk for poor functional status (in order of importance): pain in the back or lower extremity, severe pain in the operated hip, poor mental health, more than 1 common geriatric problem, obesity, and less than college education. Conclusion. Pain in the operated hip was strongly associated with poor functional status 3 years after THR. However, other factors associated with poor functional status were not related to the hip. Our results suggest that a comprehensive assessment of functional status in elderly THR patients should include assessment of common geriatric problems, mental health status, and weight.

  • Analysis of the effects of outdoor and accelerated ageing on the optical properties of reflector materials for solar energy applications

    • Lifetime tests of solar reflector materials are eligible prior to application, to prove the stability of the optical properties. In this work, six reflector materials were aged outdoors and in a climatic test chamber. The surfaces of the samples were inspected using profilometry and optical microscopy. The total and specular reflectance spectra were measured using spectrophotometry and the weighted solar reflectance values were calculated before and after ageing. Reflectors of silvered glass, anodised aluminium, thin film-coated anodised aluminium, and lacquered rolled aluminium withstood accelerated testing well, while a laminated evaporated aluminium reflector, which was specular initially, became diffuse. Laminated and lacquered reflectors withstood outdoor ageing better than expected from accelerated ageing and better than unprotected thin film-coated and anodised aluminium, which degraded significantly outdoors. Thus, optical degradation depends on climatic conditions and on the protective layer, if applicable. The discrepancy between results from outdoor and accelerated ageing shows that a thorough understanding of corrosion processes is necessary for drawing conclusions about long-term performance from accelerated ageing tests.     • An optical measurement system composed of an optical microscope (Olympus BX60) and an optical multichannel analyzer (EGG OMA 1460) has been assembled and tested. The optical microscope allows the user to make measurements on a small and well defined area of the sample. The light source, a 100 W halogen lamp, and the diode array detector, result in high sensitivity in the wavelength region of 450-750 nm. The spectral resolution of the instrument is listed as 0.59 nm/channel. The full width at half maximum (FWHM) of the strongest peaks in calibration measurements on a mercury lamp is 5 channels corresponding to 3 nm. Quasifractal clusters of gold particles have been produced with electron beam lithography. The clusters consists of different numbers of particles, giving a cluster size variation from 1.6 m to 50 m. The individual gold particles are 50 nm in diameter each. The measurement system has been used to measure both absolute transmittance and the relative transmittance using the uncoated substrate as a reference

    • A newly developed aluminium-polymer-laminated steel reflector for use in solar concentrators was evaluated with respect to its optical properties, durability, and reflector performance in solar thermal and photovoltaic systems. The optical properties of the reflector material were investigated using spectrophotometer and scatterometry. The durability of the reflector was tested in a climatic test chamber as well as outdoors in Alvkarleby (60.5degreesN, 17.4degreesE), Sweden. Before ageing, the solar weighted total and specular reflectance values were 82% and 77%, respectively, and the reflector scattered light isotropically. After I year's outdoor exposure, the total and specular solar reflectance had decreased by less than 11%. However, after 2000 h in damp heat and 1000 W/m(2) simulated solar radiation, the optical properties had changed significantly: The light scattering was anisotropic and the total and specular solar reflectance values had decreased to 75% and 42%, respectively. The decrease was found to be due to degradation of the protective polyethylene terephthalate (PET) layer, caused by UV radiation and high temperature. The conclusions are that the degradation is climate dependent and that PET is not suitable as a protective coating under extreme conditions, such as those in the climatic test chamber. However, the results from outdoor testing indicate that the material withstands exposure in a normal Swedish climate.

    • We address the simple question, whether Hartree-Fock atoms are smaller or larger than exact (Schrodinger) atoms. As a measure, we use . We study the ground state of the atoms He-Kr. The unrestricted Hartree-Fock method is used. To obtain the Schrodinger values we use the finite field CASPT2 approach, and the full CI scheme where possible. CASSCF calculations are also reported. Very large basis sets are employed. We find that for those atoms for which the CASSCF wavefunction is distinct from the HF wavefunction, the Schrodinger values are distinctly smaller than the HF values. Most other atoms are also smaller than the HF values, (or the same within a numerical uncertainty), the exceptions being the hard atoms N-Ne, and Cl, Ar and Kr. We interpret our results in terms of categories of correlation contribution. It is also of interest to test the performance of density functional theory (DFT); we find on the whole that the predictions are good, with B3LYP giving close agreement with finite field CASPT2 for nearly all atoms, in particular for the transition metals.

  • Introducing qualitative perinatal audit in a tertiary hospital in Sudan

    • In a 3-month period, May to August 2000, the perinatal mortality rate at Omdurman Maternity Hospital (OMH), Sudan, was 8.2%. Two groups of perinatal deaths, intrapartum deaths of non-malformed infants and neonatal deaths of mature infants above 34 weeks, both considered to be potentially avoidable by improved care, were in excess when compared with other regions. It was therefore decided to perform in-depth assessment of cases in these two groups. An interdisciplinary internal audit was designed in collaboration with two external obstetricians. The audit activity was preceded by a 2-day workshop at the hospital. Individual assessments based on 43 detailed narratives were followed by regular consensus meetings. This structure seemed useful for interdisciplinary discussions, and the audit process resulted in several specific suggestions for quality improvement in data collection, interdisciplinary collaboration, and obstetric and neonatal care. The present audit activity is not very resource demanding and therefore a good starting point for quality assurance in a developing country. However, since adverse outcome audit only focuses on selected cases and may encourage interventions without considering the full impact on the population, it should not stand alone. Audit of perinatal deaths should be combined with other quantitative and qualitative quality assessment activities for improvement of perinatal care.

    • The lower excited states of 2-benzoylthiophene have been studied using ab initio quantum chemical methods based on multiconfigurational wave functions. Six singlet and six triplet excited states have been characterized. The geometry has been optimized for the two lowest triplet states, which are responsible for the photoreactivity of the chromophore in the photosensitizing drug tiaprofenic acid. The T1(π → π*) and T2(n → π*) states have been found to be close in energy with the π → π* state slightly lower. The excited states have been characterized using density difference and spin density plots. The different photochemical behaviour of the two triplet states can be rationalized from the theoretical data.

    • In nuclear reactors plutonium and transplutonium isotopes are produced by multiple neutron capture of uranium and plutonium and are important for the energy production and their composition reflects the core burnout. Under normal operation these elements are not released to the environment in significant amounts. There are accordingly very few areas or source terms where exotic transplutonium elements, such as curium isotopes, can be studied in the environment. The Chernobyl accident provided a complex spectrum of fission and activation products in fallout while the relative amounts, compared to the core inventory, of refractory elements such as transuranium and transplutonium elements were small. The major alpha-activity consisted of Cm-242 (T-1/2=163 d) that would have decayed after a few years. In this study we have demonstrated the presence of so called "supported Cm-242" from the long-lived Am-242(m) (T-1/2=141 a) in environmental samples, following fallout from the Chernobyl accident. It has also been possible to assess the core bum up by using the data obtained for the Cm isotopes. The curium isotopes Cm-243 (T-1/2=29.1 a) and Cm-244 (T-1/2=18.1 a) cannot be resolved by conventional alpha-spectrometry. The assessment of these isotopes in environmental samples contaminated from the Chernobyl accident has been made by studying the effective half-life of the mixture of the isotopes. The data are compared with those previously obtained by high-resolution alpha-spectrometry and spectral deconvolution.

    • The immunoglobulin V-H gene mutation status can divide B-cell chronic lymphocytic leukaemia (CLL) into two entities with a different clinical course. Cases with unmutated V-H genes, considered to evolve from pregerminal centre (GC) cells, have a worse outcome compared to cases showing mutated VH genes, that is, post-GC derived. Also, telomere length has been reported to be of prognostic significance in CLL. Interestingly, telomerase becomes activated during the GC reaction and an elongation of the telomeres occurs in GC B cells. We performed telomere length and VH gene analysis in a series of 61 CLL cases, in order to investigate if the unique telomere lengthening shown in GC B cells could reflect the telomere status in the two subsets of mutated and unmutated CLL. A novel association was found between VH gene mutation status and telomere length, since significantly shorter telomeres were demonstrated in the unmutated group compared to the mutated group (mean length 4.3 vs 63 kbp). Shorter telomeres also constituted a subgroup with a worse prognosis than cases with longer telomeres (median survival 59 vs 159 months), Furthermore, the I-g gene sequence data revealed that samples with high mutations frequency (> 6%) had long telomeres (similar to 8 kbp). Thus, both the telomere and VH gene mutation status in CLL appear linked, which may reflect the proliferative history of the clonal cells with regard to the GC reaction. (C) 2003 Cancer Research UK.

    • The scaling of surface roughness in thin spray pyrolyzed fluorinated tin dioxide films of different thicknesses was obtained from atomic force microscopy. The data show that, within experimental uncertainties, the effective dimensionality of the surface is 2; hence no evidence of fractal surface roughness was found. Other methods – based upon light scattering and cyclic voltammetry – gave additional information on the surface topography. Cyclic voltammetry measurements show that the reaction sites on the surface are distributed in a fractal structure and may be identified with hillocks seen in surface reliefs.

    • OBJECTIVES: To investigate prospectively long term patient relevant outcomes after unilateral total hip replacement (THR) for osteoarthritis (OA). To identify non-responders to this intervention and patient related predictors of unsatisfactory outcome.METHODS: A case-control study comparing health related quality of life of 219 patients (mean age 71) after THR with that of a matched reference group of 117 subjects without hip complaints recruited from the community. Patients and reference group answered SF-36 and WOMAC questionnaires preoperatively, at 3, 6, 12 months, and at 3.6 years (range 26-65 months) postoperatively. Supplementary questions were asked at the final follow up.RESULTS: 198/211 (94%) of the patients and 83/109 (76%) of the reference group participated at the final follow up. At follow up, the only difference between the two groups in the SF-36 was physical function, where patients scored worse. Patients also reported worse WOMAC function. 31% of the patients had improved by <10/100 WOMAC score points for pain and/or function at final follow up, compared with preoperatively. More pain preoperatively and higher age and postoperative low back pain predicted a worse outcome in WOMAC function.CONCLUSION: 3.6 years after THR for OA, health related quality of life was similar for patients and reference group except for function, where patients had worse function. Higher age and more pain preoperatively predicted a poor outcome. Patients with hip OA with musculoskeletal comorbidities, such as low back pain and OA of the non-operated hip, have less long term functional improvement after THR.

    • OBJECTIVE: To investigate whether patients with severe radiographic osteoarthritis (OA) have a different outcome at one year after total hip replacement than patients with moderate radiographic OA. To investigate sex related differences in preoperative radiographic and self reported status and in postoperative outcome.METHODS: 184 patients (96 women) with a mean age at surgery of 71.4 years (50-92), with primary OA of the hip were investigated preoperatively and six and 12 months postoperatively with two self administered questionnaires, SF-36 and WOMAC. The radiographs were evaluated by two independent radiologists using an atlas. Minimal joint space narrowing, osteophytes, cysts, sclerosis, and deformity were assessed. A summary grade 0-3 was made, based on joint space, where 3 is severe OA. The reference population for SF-36 consisted of 2901 subjects matched for age and sex from the general Swedish population.RESULTS: 162 patients fulfilled the study criteria. 113 had grade 3, 47 grade 2, and two grade 1 radiographic OA. There was no difference in preoperative or postoperative pain and physical impairment between patients with moderate and severe radiographic OA. There were no sex related differences in preoperative radiographic status, or in postoperative outcome. Neither were any differences in preoperative radiographic status of OA found in patients with previous total hip replacement of the contralateral hip, compared with those who had not been operated on before. All patients, regardless of preoperative radiographic OA stage, showed significant postoperative improvement and at one year achieved a health related quality of life similar to that of the reference group.CONCLUSION: The severity of radiographic changes indicating OA often weighs heavily in the surgeons decision to perform a total hip replacement. Yet, the findings of this study emphasise that the preoperative radiographic stage of OA has no correlation with the postoperative outcome after one year. Furthermore, this study failed to detect any sex related differences in preoperative radiographic and self reported status or in postoperative outcome of hip replacement.

    • Theoretical investigations of the structure and function of the blue copper proteins are described. We have studied the optimum vacuum geometry of oxidised and reduced copper sites, the relative stability of trigonal and tetragonal Cu(II) structures, the relation between the structure and electronic spectra, the reorganisation energy, and reduction potentials. Our calculations give no support to the suggestion that strain plays a significant role in the function of these proteins; on the contrary, our results show that the structures encountered in the proteins are close to their optimal vacuum geometries (within 7 kJ/mol). We stress the importance of defining what is meant by strain and of quantifying strain energies or forces in order to make strain hypotheses testable.

    • A group of 133 primary oral squamous cell carcinomas were studied concerning a relationship between exposure factors and tumour biological parameters with a focus on the TP53 gene and p53 protein status. Tumours were evaluated using immunohistochemistry (IHC) for expression of p53, PCNA, Ki-67 and bcl-2 proteins. The TP53 gene was studied for mutations using PCR amplification of exons 5-9 and single strand conformation polymorphism (SSCP) analysis. The collected data were correlated to the exposure factors smoking, oral snuff, liquor, oral infections, dental factors, dental X-ray and iron deficiency. When compared with matched controls only oral infections, and reported HSV-infections in particular, gave statistically significant ORs (odds ratio) for all tumours (OR 8.0) as well as for the group of IHC p53 positive tumours (OR 12). No association between smoking and p53 positivity was found (OR 1.0).

    • Different factors influencing recycling and burial rates of organic carbon (OC) were investigated in the continental margin sediments of the Skagerrak (NE North Sea). Two different areas, one in the southern and one in the northeastern part of the Skagerrak were visited shortly after a spring bloom (March 1999) and in late summer (August 2000). Results suggested that: (1) Organic carbon oxidation rates (C-ox) (2.2-18 mmol Cm-2 d(-1)) were generally larger than the O-2 uptake rates (1.9-25 mmol m(-2) d(-1)). Both rates were measured in situ using a benthic lander. A mean apparent respiration ratio (C-ox:O-2corr) of 1.3 +/- 0.5 was found, indicating some long-term burial of reduced inorganic substances in these sediments. Measured O-2, fluxes increased linearly with increasing C-ox rates during the late summer cruise but not on the, early spring cruise, indicating a temporal uncoupling of anaerobic mineralization and reoxidation of reduced substances. (2) Dissolved organic carbon (DOC) fluxes (0.2-1.0 mmol Cm-2 d(-1)) constituted 3-10% of the C-ox rates and were positively correlated with the latter, implying that net DOC production rates were proportional to the overall sediment OC remineralization rates. (3) Chlorophyll a (Chl-a) concentrations in the sediment were significantly higher in early spring compared to late summer. The measured C-ox rates, but not O-2 fluxes, showed a strong positive correlation with the Chl-a inventories in the top 3 cm of the sediment. (4) Although no relationship was found between the benthic fluxes and the macrofaunal biomass in the chambers, total in situ measured dissolved inorganic carbon (C-T) fluxes were 1-5.4 times higher than diffusive mediated C-T fluxes, indicating that macrofauna have a significant impact on benthic exchange rates of OC remineralization products in Skagerrak sediments. (5) OC burial fluxes were generally higher in northeastern Skagerrak than in the southern part. The same pattern was observed for burial efficiencies, with annual means of similar to62% and similar to43% for the two areas respectively. (6) On a basin-wide scale, there was a significant positive linear correlation between the burial efficiencies and sediment accumulation rates. (7) The calculated particulate organic carbon (POC) deposition, from benthic flux and burial measurements, was only 24-78% of the sediment trap measured POC deposition, indicating a strong near-bottom lateral transport and resuspension of POC. (8) A larger fraction of the laterally advected material of lower quality seemed to settle in the northeastern Skagerrak rather than in the southern Skagerrak. (9) Skagerrak sediments, especially in the northeastern part, act as an efficient net sink for organic carbon, even in a global continental margin context.
