    • Aims To investigate vibrotactile sense (large fibre neuropathy) at different frequencies in index and little fingers (median and ulnar nerves, respectively) of subjects with diabetes, or impaired (IGT) or normal glucose tolerance (NGT). Methods Vibration thresholds (tactilometry at seven frequencies (8, 16, 32, 64, 125, 250 and 500 Hz)) and median nerve function (electrophysiology) were examined in men (age 73.4 +/- 0.12 years; n = 58, mean +/- sd) with persistent NGT (n = 28) or IGT (n = 7) or with Type 2 diabetes mellitus (T2DM) (n = 23) for > 15 years. Results HbA(1c) was increased and vibrotactile sense (sensibility index) was impaired in index and little fingers in men with T2DM. Vibration thresholds were particularly increased at 16, 250 and 500 Hz in the little finger (ulnar nerve). T2DM subjects showed electrophysiological (gold standard) signs of neuropathy in the median nerve. Although subjects with persistent IGT had higher HbA(1c), vibrotactile sensation and electrophysiology remained normal. HbA(1c) did not correlate with vibrotactile sense or electrophysiology, but the latter two correlated with respect to Z-score (sign of neuropathy) in forearm (NGT) and at wrist level (NGT and DM). Conclusions Vibration thresholds are increased in the finger pulps in T2DM subjects, particularly at specific frequencies in ulnar nerve innervated finger pulps. Neuropathy is not present in IGT. Tactilometry, with a multi-frequency approach, is a sensitive technique to screen for large fibre neuropathy in T2DM. Frequency-related changes may mirror dysfunction of various receptors.

  • Cylinder Liner Honed Surface Optimisation - a Manufacture-Characterisation-Function Study

    • In order to comply with increasingly stringent emission standards, engine manufacturers strive to reduce the oil consumption and frictional losses. One of the major contributors to it is the cylinder liner surface. Therefore, it is important to investigate what are the optimal machining parameters to manufacture liner topography with an optimal function. Numerous liners were slide honed, varying the base honing pressure and plateau honing time. The samples of the liners were measured on stylus and interferometer, and characterized by 2D and 3D parameters. The interference measurements were inputted in a flow simulation program and the mean oil flow and hydrodynamic shear stress were calculated for different nominal separations between the ring and liner. The analytical results show that a relatively low base honing pressure and longer plateau honing time give a surface with the lowest mean oil flow and the highest shear stress. A good correlation among the oil flow, shear stress, base honing pressure, Sa, Sk, Svk, and other roughness parameters was also found.

    • Cylinder liner surface has a direct impact on the oil consumption and frictional losses in the internal combustion engines which are in common interest to be as lower as possible. Thus, the optimisation of the liner surface, its function and manufacture is often on the agenda of the automotive industry. Since the liner surface finish is a subject of improving, there is a need of improving and facilitating of its 3D characterisation preliminary when the parameter control limits are unknown, as well as later when it is experimentally verified and the limits are better determined. For that purpose a method for quality control in 3D of cylinder liners is proposed here. A tool was developed and implemented for rating of cylinder liner surfaces, computing of 3D groove parameters (groove width, height and distance between grooves) and determination of a needed number of measurements.

  • Hepatic encephalopathy verified by psychometric testing and EEG in cirrhotic patients : Effects of mesocaval interposition shunt or sclerotherapy

    • Background. The aim of this randomised prospective study was to evaluate hepatic encephalopathy after mesocaval interposition shunt operation and after repeated endoscopic sclerotherapy. Methods. Forty-five patients with bleeding oesophageal varices due to liver cirrhosis were randomised to the two treatment groups, 24 to the shunt group and 21 to the sclerotherapy group. The patients were evaluated preoperatively regarding blood tests, hepatic encephalopathy as measured by electroencephalogram with spectral analysis and by a battery of psychometric tests. The direction of portal flow in the shunt group was investigated by shunt phlebography and ultrasonography with Doppler. During follow-up the same investigations were performed twice at median 6.7 and 14.7 months after operation. Results. No statistically significant difference was found during follow-up regarding blood tests and electroencephalography with spectral analysis. Although the preoperative psychometric tests showed that the shunt group performed significantly better than the sclerotherapy group, the first follow-up showed that the shunt group performed statistically worse than the sclerotherapy group in seven of the tests: Synonyms (measuring verbal ability), Block Design Test (measuring visuo-spatial ability), Memory for Design Test, Error Score (measuring memory function), Revised Visual Retention Test, correct answers and the same test error answers (measuring visuo-spatial memory, ability and immediate memory), Digit Symbol Test (measuring perceptual ability) and Trial Making Test B (measuring cognitive motor abilities). Conclusions. Patients treated by mesocaval interposition shunt showed a progressive general reduction in psychometric performance compared with patients treated with repeated sclerotherapy, in whom a general intellectual improvement was observed. This finding corresponds to the reverse direction of the preoperative portal flow to a hepatofugal pattern at first follow-up and at 12 months among two-thirds of the patients. © 2005 Taylor & Francis Group Ltd.

    • There is a demand for more efficient and tissue-specific MRI contrast agents and recent developments involve the design of substances useful as molecular markers and magnetic tracers. In this study, nanoparticles of gadolinium oxide (Gd2O3) have been investigated for cell labeling and capacity to generate a positive contrast. THP-1, a monocytic cell line that is phagocytic, was used and results were compared with relaxivity of particles in cell culture medium (RPMI 1640). The results showed that Gd2O3-labeled cells have shorter T1 and T2 relaxation times compared with untreated cells. A prominent difference in signal intensity was observed, indicating that Gd2O3 nanoparticles can be used as a positive contrast agent for cell labeling. The r1 for cell samples was 4.1 and 3.6 s-1 mm-1 for cell culture medium. The r2 was 17.4 and 12.9 s-1 mm-1, respectively. For r1, there was no significant difference in relaxivity between particles in cells compared to particles in cell culture medium, (pr1 = 0.36), but r2 was significantly different for the two different series (pr2 = 0.02). Viability results indicate that THP-1 cells endure treatment with Gd2O3 nanoparticles for an extended period of time and it is therefore concluded that results in this study are based on viable cells.

    • Objetive: Sweden experienced a marked epidemic of celiac disease between 1984 and 1996 in children younger than 2 years of age, partly explained by changes in infant feeding. The objective of this study was to determine the prevalence of celiac disease in 12-year-olds born during the epidemic (1993), including both symptomatic and screening detected cases.Patients and methods: All sixth-grade children in participating schools were invited (n = 10,041). Symptomatic and, therefore, previously diagnosed celiac disease cases were ascertained through the National Swedish Childhood Celiac Disease Register and/or medical records. All serum samples were analyzed for antihuman tissue transglutaminase (tTG)-IgA (Celikey), and serum-IgA, and some for tTG-IgG and endomysial antibodies. A small intestinal biopsy was recommended for all children with suspected undiagnosed celiac disease.Results: Participation was accepted by 7567 families (75%). Previously diagnosed celiac disease was found in 67 children; 8.9/1000 (95% confidence interval [CI] 6.7-11). In another 192 children, a small intestinal biopsy was recommended and was performed in 180. Celiac disease was verified in 145 children, 20/1000 (95% CI 17-23). The total prevalence was 29/1000 (95% CI 25-33).Conclusions: The celiac disease prevalence of 29/1000 (3%)-with two thirds of cases undiagnosed before screening-is 3-fold higher than the usually suggested prevalence of 1%. When these 12-year-olds were infants, the prevailing feeding practice was to introduce gluten abruptly, often without ongoing breast-feeding, which might have contributed to this unexpectedly high prevalence.

    • In vitro studies suggest that the G protein-coupled receptor (GPR) 30 is a functional estrogen receptor. However, the physiological role of GPR30 in vivo is unknown, and it remains to be determined whether GPR30 is an estrogen receptor also in vivo. To this end, we studied the effects of disrupting the GPR30 gene in female and male mice. Female GPR30((-/-)) mice had hyperglycemia and impaired glucose tolerance, reduced body growth, increased blood pressure, and reduced serum IGF-I levels. The reduced growth correlated with a proportional decrease in skeletal development. The elevated blood pressure was associated with an increased vascular resistance manifested as an increased media to lumen ratio of the resistance arteries. The hyperglycemia and impaired glucose tolerance in vivo were associated with decreased insulin expression and release in vivo and in vitro in isolated pancreatic islets. GPR30 is expressed in islets, and GPR30 deletion abolished estradiol-stimulated insulin release both in vivo in ovariectomized adult mice and in vitro in isolated islets. Our findings show that GPR30 is important for several metabolic functions in female mice, including estradiol-stimulated insulin release.

    • Hand prostheses that are currently available on the market are used by amputees to only a limited extent, partly because of lack of sensory feedback from the artificial hand. We report a pilot study that showed how amputees can experience a robot-like advanced hand prosthesis as part of their own body. We induced a perceptual illusion by which touch applied to the stump of the arm was experienced from the artificial hand. This illusion was elicited by applying synchronous tactile stimulation to the hidden amputation stump and the robotic hand prosthesis in full view. In five people who had had upper limb amputations this stimulation caused referral touch sensation from the stump to the artificial hand, and the prosthesis was experienced more like a real hand. We also showed that this illusion can work when the amputee controls the movements of the artificial hand by recordings of the arm muscle activity with electromyograms. These observations indicate that the previously described "rubber hand illusion" is also valid for an advanced hand prosthesis, even when it has a robotic-like appearance.

  • Estimation of fibre segment orientation using steerable filtering

    • This work investigates the combined approach of sheet splitting by heat-seal pouch lamination and fibre segment orientation analysis by steerable filtering to characterize both in-plane and z-directional variations in local fibre orientation. The use of steerable filters allows the orientation of individual fibre segments to be estimated from a series of rotated versions of a two-dimensional fibre template. The performance of steerable filtering is compared to the widely spread method based on local gradients in the image. The results are based on a 60 g/m2 oriented handsheet formed with a dynamic sheet former from decrilled softwood kraft pulp. The sheet was split in 10 z-directional layers and the fibre orientation was estimated on a 10×—10 mm2 sub-area. Despite an observed visual difference in the 2D filter response between the two techniques, the result showed only small differences in the estimated fibre orientation angle and anisotropy. The effect of a decreased signal-to-noise ratio, due to e.g. fines and filler aggregates, was simulated by adding a statistical small-scale noise. The fibre orientation distribution estimated by steerable filtering was more or less unaffected by the introduced noise, whereas the estimation based on gradient filtering showed a decreased anisotropy by 35%. The results indicate that, for routine measurements of local fibre orientation, the more selective properties of steerable filtering do not motivate the more complex computations involved. For most cases, gradient filtering should give similar results. The use of steerable filtering is however recommended as an alternative for non-standard applications, e.g. in the presence of a high "noise" level or to minimize problems related to scanning imperfections. The theoretical possibility to use steerable filtering to determine fibre width distributions is also discussed.

  • Refined myoelectric control in below-elbow amputees using artificial neural networks and data glove

    • Purpose: To develop a system for refined motor control of artificial hands based on multiple electromyographic (EMG) recordings, allowing multiple patterns of hand movements. Methods: Five subjects with traumatic below-elbow amputations and 1 subject with a congenital below-elbow failure of formation performed 10 imaginary movements with their phantom hand while surface electrodes recorded the EMG data. In a training phase a data glove with 18 degrees of freedom was used for positional recording of movements in the contralateral healthy hand. These movements were performed at the same time as the imaginary movements in the phantom hand. An artificial neural network (ANN) then could be trained to associate the specific EMG patterns recorded from the amputation stump with the analogous specific hand movements synchronously performed in the healthy hand. The ability of the ANN to predict the 10 imaginary movements off line, when they were reflected in a virtual computer hand, was assessed and calculated. Results: After the ANN was trained the subjects were able to perform and control 10 hand movements in the virtual computer hand. The subjects showed a median performance of 5 types of movement with a high correlation with the movement pattern of the data glove. The subjects seemed to relearn to execute motor commands rapidly that had been learned before the accident, independent of how old the injury was. The subject with congenital below-elbow failure of formation was able to perform and control several hand movements in the computer hand that cannot be performed in a myoelectric prosthesis (eg, opposition of the thumb). Conclusions: With the combined use of an ANN and a data glove, acting in concert in a training phase, amputees rapidly can learn to execute several imaginary movements in a virtual computerized hand, this opens promising possibilities for motor control of future hand prostheses.

  • Disruption of interleukin-18, but not interleukin-1, increases vulnerability to preterm delivery and fetal mortality after intrauterine inflammation

    • Preterm birth is a major contributor of adverse perinatal outcome. Clinical data suggest that an inflammatory response is important in the process leading to preterm labor. By using a recently introduced mouse model of localized intrauterine lipopolysaccharide-induced inflammation, the effect of interleukin (IL)-18 gene disruption and/or IL-18 neutralization as well as combined IL-1alpha/beta gene disruption on inflammation-induced fetal loss was investigated. The frequency of preterm fetal loss was significantly higher in IL-18 knockout mice (58.9%) and in mice administered IL-18-binding protein (59.7%) compared to wild-type controls (34.7%). The rate of fetal loss was not affected by IL-1alpha/beta gene deficiency (38.7%). Decreased IL-18 protein expression combined with elevated IL-12 protein expression in uterine tissue of IL-18 knockout mice and IL-18-binding protein-treated animals was noticed. These data demonstrate that preterm pregnancy loss in response to intrauterine inflammation was enhanced by disruption of the IL-18 gene and/or IL-18 neutralization, events that may relate to exaggerated Th1 responses because of an increased IL-12/IL-18 ratio.
