    • Individualization of therapy based on a person’s specific type of diabetes is one key element of a “precision medicine” approach to diabetes care. However, applying such an approach remains difficult because of barriers such as disease heterogeneity, difficulties in accurately diagnosing different types of diabetes, multiple genetic influences, incomplete understanding of pathophysiology, limitations of current therapies, and environmental, social, and psychological factors. Monogenic diabetes, for which single gene mutations are causal, is the category most suited to a precision approach. The pathophysiological mechanisms of monogenic diabetes are understood better than those of any other form of diabetes. Thus, this category offers the advantage of accurate diagnosis of nonoverlapping etiological subgroups for which specific interventions can be applied. Although representing a small proportion of all diabetes cases, monogenic forms present an opportunity to demonstrate the feasibility of precision medicine strategies. In June 2019, the editors of Diabetes Care convened a panel of experts to discuss this opportunity. This article summarizes the major themes that arose at that forum. It presents an overview of the common causes of monogenic diabetes, describes some challenges in identifying and treating these disorders, and reports experience with various approaches to screening, diagnosis, and management. This article complements a larger American Diabetes Association effort supporting implementation of precision medicine for monogenic diabetes, which could serve as a platform for a broader initiative to apply more precise tactics to treating the more common forms of diabetes.

  • Regulation of the Drosophila Enhancer of split and invected-engrailed gene complexes by sister chromatid cohesion proteins

    • The cohesin protein complex was first recognized for holding sister chromatids together and ensuring proper chromosome segregation. Cohesin also regulates gene expression, but the mechanisms are unknown. Cohesin associates preferentially with active genes, and is generally absent from regions in which histone H3 is methylated by the Enhancer of zeste [E(z)] Polycomb group silencing protein. Here we show that transcription is hypersensitive to cohesin levels in two exceptional cases where cohesin and the E(z)-mediated histone methylation simultaneously coat the entire Enhancer of split and invected-engrailed gene complexes in cells derived from Drosophila central nervous system. These gene complexes are modestly transcribed, and produce seven of the twelve transcripts that increase the most with cohesin knockdown genome-wide. Cohesin mutations alter eye development in the same manner as increased Enhancer of split activity, suggesting that similar regulation occurs in vivo. We propose that cohesin helps restrain transcription of these gene complexes, and that deregulation of similarly cohesin-hypersensitive genes may underlie developmental deficits in Cornelia de Lange syndrome.

  • Solid and fluid segments within the same molecule of stratum corneum ceramide lipid

    • The outer layer of the skin, stratum corneum (SC) is an efficient transport barrier and it tolerates mechanical deformation. At physiological conditions, the majority of SC lipids are solid, while the presence of a small amount of fluid lipids is considered crucial for SC barrier and material properties. Here we use solid-state and diffusion nuclear magnetic resonance to characterize the composition and molecular dynamics of the fluid lipid fraction in SC model lipids, focusing on the role of the essential SC lipid CER EOS, which is a ceramide esterified omega-hydroxy sphingosine linoleate with very long chain. We show that both rigid and mobile structures are present within the same CER EOS molecule, and that the linoleate segments undergo fast isotropic reorientation while exhibiting extraordinarily slow self-diffusion. The characterization of this unusual self-assembly in SC lipids provides deepened insight into the molecular arrangement in the SC extracellular lipid matrix and the role of CER EOS linoleate in the healthy and diseased skin.

    • Catalytic converters have substantially contributed to the improvement of air quality in populated areas in the last decades by abatement of the emission of hazardous componentssuch as CO, unburned hydrocarbons and NOx. However, due to the exposure to high temperature and different components in the exhaust gas, the performance of the catalysts degrades with time. An understanding of the microstructural and chemical changes of the catalysts during different operating conditions is required in order to develop strategies to reduce the degradation.In the present work, a bimetallic Pt-Pd nanoparticle catalyst supported on gamma-aluminawas investigated using high resolution electron microscopy and spectroscopy. The studies included both fresh and aged catalyst systems. In order to allow a quantitativeevaluation of the particle size distributions and locations on the support, a new TEM specimen preparation method was developed. The new method allows for monitoring theevolution of the particle size distribution as well as the spatial difference in the porous oxide support. These microstructural observations were complemented by investigations of the chemical composition and alloying of the nanoparticles. Finally, CO oxidation experimentswere performed to correlate the observed microstructural and compositional changes to the catalytic activity.

  • Anionic nanoplastic contaminants promote Parkinson’s disease–associated α-synuclein aggregation

    • Recent studies have identified increasing levels of nanoplastic pollution in the environment. Here, we find that anionic nanoplastic contaminants potently precipitate the formation and propagation of α-synuclein protein fibrils through a high-affinity interaction with the amphipathic and non-amyloid component (NAC) domains in α-synuclein. Nanoplastics can internalize in neurons through clathrin-dependent endocytosis, causing a mild lysosomal impairment that slows the degradation of aggregated α-synuclein. In mice, nanoplastics combine with α-synuclein fibrils to exacerbate the spread of α-synuclein pathology across interconnected vulnerable brain regions, including the strong induction of α-synuclein inclusions in dopaminergic neurons in the substantia nigra. These results highlight a potential link for further exploration between nanoplastic pollution and α-synuclein aggregation associated with Parkinson’s disease and related dementias.

    • Following the completion in May 2011 of the shutdown for the installation of the beryllium wall and the tungsten divertor, the first set of JET campaigns have addressed the investigation of the retention properties and the development of operational scenarios with the new plasma-facing materials. The large reduction in the carbon content (more than a factor ten) led to a much lower Z(eff) (1.2-1.4) during L- and H-mode plasmas, and radiation during the burn-through phase of the plasma initiation with the consequence that breakdown failures are almost absent. Gas balance experiments have shown that the fuel retention rate with the new wall is substantially reduced with respect to the C wall. The re-establishment of the baseline H-mode and hybrid scenarios compatible with the new wall has required an optimization of the control of metallic impurity sources and heat loads. Stable type-I ELMy H-mode regimes with H-98,H-y2 close to 1 and beta(N) similar to 1.6 have been achieved using gas injection. ELM frequency is a key factor for the control of the metallic impurity accumulation. Pedestal temperatures tend to be lower with the new wall, leading to reduced confinement, but nitrogen seeding restores high pedestal temperatures and confinement. Compared with the carbon wall, major disruptions with the new wall show a lower radiated power and a slower current quench. The higher heat loads on Be wall plasma-facing components due to lower radiation made the routine use of massive gas injection for disruption mitigation essential.

    • Antibiotic residues are being continuously recognized in the aquatic environment and in food. Though the concentration of antibiotic residues is typically low, adverse effects on the environment and human health have been observed. Hence, an efficient method to determine numerous antibiotic residues should be simple, inexpensive, selective, with high throughput and with low detection limits. Liquid-based extractions have been exceedingly used for clean-up and preconcentration of antibiotics prior to chromatographic analysis. In order to make methods more green and environmentally sustainable, conventional hazardous organic solvents can be replaced with green solvents. This review presents sampling strategies as well as comprehensive and up-to-date methods for chemical analysis of antibiotic residues in different sample matrices. Particularly, solvent-based sample preparation techniques using green solvents are discussed along with applications in antibiotic residue analysis.

    • The pursuit of commercialization is at the core of economic activity, and limited commercialization success is generally ascribed to the lower scalability or potential of the venture. Drawing on the anti-profit-taking socio-institutional perspective and innovation resistance theory, commercialization of a value-creating technology could be resisted from the demand side. We use a 3D surgical video technology developed at a Swedish hospital along with Potentially All Pairwise RanKings of all possible alternatives (PAPRIKA) based on conjoint analysis completed by 1437 elderly individuals. We find that, despite the value benefits for the surgeon and the overall social benefits of the technology, the most preferred part-worth utility (i.e., weight) of relative importance is a preference for not commercializing the technology for profit. These findings carry implications for entrepreneurship and institutional theory related to medical innovations in countries with universal healthcare.

    • Persistent cognitive impairment in the euthymic state of bipolar disorder is increasingly recognized. Mounting evidence also suggests an association between neuroinflammation and cognitive dysfunction. The purpose of this study was to test if cerebrospinal fluid (CSF) markers of neuroinflammation could account for cognitive impairment in bipolar disorder. Hierarchical linear regression models were applied to account for performance in five cognitive domains using CSF neuroinflammatory biomarkers as predictors in patients with bipolar disorder type I and II (N=78). The associations between these biomarkers and cognition were further tested in healthy age- and sex-matched controls (N=86). In patients with bipolar disorder, the CSF biomarkers accounted for a significant proportion of the variance in executive functions (42.8%, p=<.0005) independently of age, medication, disease status, and bipolar subtype. The microglial marker YKL-40 had a high impact (beta=-.99), and was the only biomarker that contributed individually. CSF biomarkers were not associated with cognitive performance in healthy controls. The CSF neuroinflammation biomarker YKL-40 is associated with executive performance in euthymic bipolar disorder, but not in healthy controls.

    • A new method based on summing of phase controlled AC voltages makes it possible to compare thermal voltage converters with a ratio of 2:1 without introducing any error due to level dependence. The method is used to determine the level dependence of thermal voltage converters up to 1 kV and 100 kHz. The level dependence is compared to the change of AC-DC transfer difference due to power dissipation in the range resistor and due to DC-effects in the thermal converter. The results for two investigated TVC's indicate that the power dissipation is the only cause of level dependence in the range resistor of a high voltage TVC.

    • Theoretical investigations on the micellization of mixtures of (i) amino acid-based anionic surfactants [AAS: N-dodecyl derivatives of aminomalonate, -aspartate, and -glutamate] and (ii) hexadecyltrimethylammonium bromide (HTAB), were carried out at different mole ratios. Variation in the theoretical values of critical micelle concentration (CMC), mole fraction of surfactants in the micellar phase (X), at the interface (X-sigma), interaction parameters at the bulk/interface (beta(R)/beta(sigma)), ideality/nonideality of the mixing processes, and activity coefficients (f) were evaluated using Rubingh, Rosen, Motomora, and Sarmoria-Puvvada-Blankschtein models. CMC values significantly deviate from the theroretically calculated values, indicating associative interaction. With increasing mole fraction of AAS (alpha(AAS)), the magnitude of the (beta(R)/beta(sigma)) values gradually decreased, considered to attributable to hydrophobic interactions. With increasing alpha(AAS), the micellar mole fraction of HTAB (X-2) decreased insignificantly and X-2 values were higher than those compared to AAS for all combinations, due to the dominance of HTAB in micelles. Micellar mole fraction at the ideal state of AAS (X1ideal) differed from micellar mole fraction of AAS (X-1), indicating nonideality in the mixed micellization process. Gibbs free energy of micellization ( increment G(m)) values are more negative than the free energy of micellization for ideal mixing ( increment Gmideal), indicating the micellization process is spontaneous. With increasing alpha(AAS), the enthalpy of micellization (Delta H-m) and entropy of micellization (Delta S-m) values gradually increased, which indicates micellization is exothermic. The different physicochemical parameters of the mixed micelles are correlated with the variation in the spacer length between the two carboxylate groups of AAS.

    • The formation of planets with gaseous envelopes takes place in protoplanetary accretion discs on time scales of several million years. Small dust particles stick to each other to form pebbles, pebbles concentrate in the turbulent flow to form planetesimals and planetary embryos and grow to planets, which undergo substantial radial migration. All these processes are influenced by the underlying structure of the protoplanetary disc, specifically the profiles of temperature, gas scale height, and density. The commonly used disc structure of the minimum mass solar nebula (MMSN) is a simple power law in all these quantities. However, protoplanetary disc models with both viscous and stellar heating show several bumps and dips in temperature, scale height, and density caused by transitions in opacity, which are missing in the MMSN model. These play an important role in the formation of planets, since they can act as sweet spots for forming planetesimals via the streaming instability and affect the direction and magnitude of type-I migration. We present 2D simulations of accretion discs that feature radiative cooling and viscous and stellar heating, and they are linked to the observed evolutionary stages of protoplanetary discs and their host stars. These models allow us to identify preferred planetesimal and planet formation regions in the protoplanetary disc as a function of the disc's metallicity, accretion rate, and lifetime. We derive simple fitting formulae that feature all structural characteristics of protoplanetary discs during the evolution of several Myr. These fits are straightforward for applying to modelling any growth stage of planets where detailed knowledge of the underlying disc structure is required.

    • In this paper we look at various options for calculating target-measurement association probabilities and updating the state estimates in the Joint Probabilistic Data Association Filter (JPDAF). In addition to the "standard" methods, we look at other methods that try to improve the estimation accuracy by coupling the states, discarding certain joint association events, or by applying random finite set theory to change how the states are updated. We compare the performance of trackers based on several of these concepts to each other and to the PMHT, the MHT, and the GNN tracker. We also single out approaches that are "snake oil", in that they are either not suited for practical use, or that their complexity is higher than that of calculating the probabilities exactly. Additionally we show how the JPDAF* can be implemented to have a lower worst-case complexity than the regular JPDAF when the number of targets and/or observations is large. We also review some oft overlooked references on gating that are useful for implementations in real systems.

    • The presence of cold work material (blechmantel) smeared out on cylinder liners by faulty honing is undesirable as it seals the oil-bearing honing grooves. It is also believed to correlate with increased bore wear, presumably due to its loosening and together with the carbon build-up from the piston’s top land form an increased quantity of abrasive particles. An attempt has been made to estimate the extent of removal of blechmantel for different wear regimes present at the middle and top region (near the top dead centre) of the liner surface using the unworn bottom region as a reference for comparison. A number of truck cylinder liners were axially sectioned after varying periods of engine running under similar conditions of load, engine speed and lubrication. 3D surface measurements were taken at the three regions and a range of standard parameters was extracted. Combining the profile and image analysis, an algorithm was developed to identify and quantify the blechmantel covering the grooves that is most likely to break and act as abrasive wear particles and at the same time it represents the part of blechmantel which covers the most of the surface. It was found out that a mere portion of the blechmantel from the middle and bottom section was removed, while the blechmantel from the top section was greatly removed and thereby it represents one of the possible causes for wear. Axial wear scratches more emphasized on the thrust side of the liner were observed not only through the whole stroke area, but also in the bottom region. The fact that the most of the blechmantel is not removed from the running surface of the liner (except for the very small portion of the polished areas at the dead centres), points out that the blechmantel plays only a minor role on wear of the cylinder liner surfaces.

    • The use of data and metrics on a professional and personal level has led to considerable discourse around the performative power and politics of 'big data' and data visualization, with academia being no exception. We have developed a university system, ResViz, which publicly visualizes the externally funded research projects of academics, and their internal collaborations. We present an interview study that engages 20 key stakeholders, academics and administrators who are part of the pilot release for the first version of this system. In doing so, we describe and problematize our design space, considering the implications of making metrics visible and their social use within a large organization. Our findings cut across the way people communicate, review and manage performance with metrics. We raise seven design issues in this space practical considerations that expose the tensions in making metrics available for public contestation.

    • OBJECTIVES: To evaluate the long-term clinical and economic outcomes of biphasic insulin aspart 70/30 (BIAsp 70/30) treatment vs. insulin glargine in insulin naïve, type 2 diabetes patients failing oral antidiabetic drugs in a Swedish setting.METHODS: A published and validated computer simulation model (the CORE Diabetes Model) was used to project life expectancy, quality-adjusted life expectancy (QALE) and costs over patient lifetimes. Cohort characteristics [54.5% male, mean age 52.4 years, 9 years mean diabetes duration, mean glycosylated haemoglobin (HbA1c) 9.77%] and treatment effects were based on results from the Initiate Insulin by Aggressive Titration and Education (INITIATE) clinical trial. Direct medical costs were accounted in 2006 Swedish Kronor (SEK) and economic and clinical benefits were discounted at 3% per annum.RESULTS: Biphasic insulin aspart 70/30 treatment when compared with insulin glargine treatment was associated with improvements in discounted life expectancy of 0.21 years (13.10 vs. 12.89 years) and QALE of 0.21 quality-adjusted life years (QALYs) (9.16 vs. 8.96 QALYs). Reductions in the incidence of diabetes-related complications in the BIAsp 70/30 treatment arm led to reduced total costs of SEK 10,367 when compared with insulin glargine (SEK 396,475 vs. SEK 406,842) over patient lifetimes. BIAsp 70/30 treatment was projected to be dominant (cost and lifesaving) when compared with insulin glargine in the base case analysis.CONCLUSIONS: Biphasic insulin aspart 70/30 treatment was associated with improved clinical outcomes and reduced costs compared with insulin glargine treatment over patient lifetimes. These results were driven by improved HbA1c levels associated with BIAsp 70/30 compared with insulin glargine and the accompanying reduction in diabetes-related complications despite increases in body mass index.

    • TOUGH2/ECO2N was used to simulate CO2 injection into a saline aquifer in the Baltic Sea and the effect of different amounts of CO2 injection on the seismic response. The Biot-Gassmann model was used to convert the simulated saturation and densities to seismic velocities and synthetic seismic responses before and after injection were compared. The results show that the amplitude changes in the seismic response are detectable even for small amounts of injected CO2, while noticeable signs of velocity pushdown, as a signature of the CO2 substitution, could only be observed if the injection rate is high enough.

    • A major constraint in deployments of resource-limited networks is the energy consumption related to the battery lifetime of the network nodes. To this end, power efficient digital modulation techniques such as On-Off keying (OOK) are highly attractive. In this paper, a novel complete probabilistic description of the Direct Sequence - Coded Division Multiple Access (DS-CDMA) system with random signatures employing OOK modulation is presented. The system scenario considers simultaneously transmitting nodes in Rayleigh fading conditions. Numerical simulations are provided to support the derived results.

    • Objective: Curative gastric cancer surgery entails removal of the primary tumor with adequate margins including regional lymph nodes. European randomized controlled trials with recruitment in the 1990’s reported increased morbidity and mortality for D2 compared to D1. Here, we examined the extent of lymphadenectomy during gastric cancer surgery and the associated risk for postoperative complications and mortality using the strengths of a population-based study. Methods: A prospective nationwide study conducted within the National Register of Esophageal and Gastric Cancer. All patients in Sweden from 2006 to 2013 who underwent gastric cancer resections with curative intent were included. Patients were categorized into D0, D1, or D1+/D2, and analyzed regarding postoperative morbidity and mortality using multivariable logistic regression. Results: In total, 349 (31.7%) patients had a D0, 494 (44.9%) D1, and 258 (23.4%) D1+/D2 lymphadenectomy. The 30-d postoperative complication rates were 25.5%, 25.1% and 32.2% (D0, D1 and D1+/D2, respectively), and 90-d mortality rates were 8.3%, 4.3% and 5.8%. After adjustment for confounders, in multivariable analysis, there were no significant differences in risk for postoperative complications between the lymphadenectomy groups. For 90-d mortality, there was a lower risk for D1 vs. D0. Conclusions: The majority of gastric cancer resections in Sweden have included only a limited lymphadenectomy (D0 and D1). More extensive lymphadenectomy (D1+/D2) seemed to have no impact on postoperative morbidity or mortality.

  • Impact of surgical resection rate on survival in gastric cancer : nationwide study

    • BACKGROUND: There are marked geographical variations in the proportion of patients undergoing resection for gastric cancer. This study investigated the impact of resection rate on survival. METHODS: All patients with potentially curable gastric cancer between 2006 and 2017 were identified from the Swedish National Register of Oesophageal and Gastric Cancer. The annual resection rate was calculated for each county per year. Resection rates in all counties for all years were grouped into tertiles and classified as low, intermediate or high. Survival was analysed using the Cox proportional hazards model. RESULTS: A total of 3465 patients were diagnosed with potentially curable gastric cancer, and 1934 (55.8 per cent) were resected. Resection rates in the low (1261 patients), intermediate (1141) and high (1063) tertiles were 0-50.0, 50.1-62.5 and 62.6-100 per cent respectively. The multivariable Cox analysis revealed better survival for patients diagnosed in counties during years with an intermediate versus low resection rate (hazard ratio (HR) 0.81, 95 per cent c.i. 0.74 to 0.90; P < 0.001) and high versus low resection rate (HR 0.80, 0.73 to 0.88; P < 0.001). CONCLUSION: This national register study showed large regional variation in resection rates for gastric cancer. A higher resection rate appeared to be beneficial with regard to overall survival for the entire population.

    • This paper suggests that research on innovation in the cultural economy should lookbeyond firms and industrial formations in seeking to understand how values and trendsare produced. Sometimes, dynamic externalities are constructed in creative milieus byamateurs and consumers. One example is Comiket, the largest event for amateur comics inthe world. Twice a year half a million people gather in Tokyo to participate in shopping,sharing and enjoying. The event, and the subculture around it, is examined in an attemptto understand its role in innovation processes and the production of trends. The paperDifferentiates between three different characteristics of Comiket. Firstly, it is a marketplace where goods are distributed and sold; more specifically, it is a female-dominated,self-organized, bottom-up market where large mainstream publishers only have amarginal presence. Secondly, it is a space where trends are constructed and mediated –innovation, in this sense, being something negotiated among users and producers ratherthan the outcome of research or laboratory work. Moreover, plagiarism and provocationsof mainstream culture are driving forces of these creative processes. Thirdly, Comiket is aplayground for leisure and exhibition: an unusual physical meeting place for an economicand cultural sphere, where the internet is rapidly becoming the terra firma.

    • In an attempt to evaluate the possible relationship between the microbial flora in the stallion ejaculate and its ability to freeze, three ejaculates from five stallions were frozen using a standard protocol. Before freezing, an aliquot was removed for bacteriological analysis. Bacterial growth was observed in all the ejaculates studied. The isolated microorganisms were: Staphylococcus spp. and Micrococcus spp. (in all the stallions), beta-haemolytic Streptococcus (in stallions 3 and 4), Corynebacterium spp. (in stallions 1, 3-5), Rhodococcus spp. (in stallion number 2), Pseudomonas spp. (in stallion number 1) and Klebsiella spp. (in stallions 1, 3 and 5). The presence and richness of Klebsiella and beta-haemolytic Streptococcus in the ejaculate were related to two sperm variables post-thaw, namely the proportion of dead spermatozoa (ethidium+ cells; r = 0.55, p less than 0.05) and the amplitude of lateral displacement of the sperm head (ALH, mu m; r = -0.56, p less than 0.05), respectively. The degree of growth of Corynebacterium spp. in the ejaculate was positively correlated with the percentage of spermatozoa showing high caspase activity post-thaw (r = 0.62, p less than 0.05). The presence and number of colonies of beta-haemolytic Streptococcus were negatively correlated (r = -0.55, p less than 0.05) with low sperm caspase activity. It is concluded that the microbial flora of the equine ejaculate may be responsible for some of the sublethal damage experimented by the spermatozoa during cryopreservation.

    • The rheology of a fluid flowing in an industrial process pipe can be calculated by combining the pressure drop and the velocity profile that the fluid develops across the tube diameter. The profile is obtained noninvasively through an ultrasound Doppler investigation. Unfortunately, at present, no system capable of real-time velocity profile assessment is available for in-line industrial rheological measurements, and tests are operated by manually moving fluid specimens to specialized laboratories. In this work, we present an embedded system capable of in-line and real-time measurement of velocity profile and pressure drop, which enables the automatic rheological characterization of non-Newtonian fluids in process pipes. The system includes all the electronics for the ultrasound front-end, as well as the digital devices for the real-time calculation of the velocity profile. The proposed system is highly programmable, low-noise, and specifically targeted for industrial use. It is shown capable of producing, for example, 512-point velocity profiles at 45 Hz rate. An application is presented where a sludge fluid, flowing at 600 L/min in a 48 mm diameter high-grade stainless steel pipe, is characterized in real-time with a ±5% accuracy.

    • Light-charged particles emitted in proton-induced fission reactions on 208 Pb have been measured at different kinetic energies: 370A, 500A, and 650A MeV. The experiment was performed by the SOFIA collaboration at the GSI facilities in Darmstadt (Germany). The inverse kinematics technique was combined with a setup especially designed to measure light-charged particles in coincidence with fission fragments. The data were compared with different model calculations to assess the ground-to-saddle dynamics. The results confirm that transient and dissipative effects are required for an accurate description of the fission observables.

    • The chapter presents the topic of “historicity” (Geschichtlichkeit) as a core concern for phenomenological thinking in the intersection with hermeneutics. It is first coined as a philosophical term by Dilthey and Yorck von Wartenburg as a way to capture the unique way in which humans exist historically and belong to history. Through their correspondence published posthumously in 1923 it enters the orbit of Heidegger’s existential phenomenology, as he quotes extensively from these letters in Being in Time. For Heidegger, historicity was the key to transforming Husserlian phenomenology into hermeneutical ontology. In his reappraisal of hermeneutic thinking, Gadamer also locates historicity at the center of his magnum opus Truth and Method. The chapter also shows how Husserl was a thinker of historicity. This is brought out in particular in Derrida’s early interpretations of Husserl, where the deconstructive approach emerges literally from the problem of the historicity of ideal objects.

    • Leishmania infantum causes visceral leishmaniasis, a deadly vector-borne disease introduced to the Americas during the colonial era. This non-native trypanosomatid parasite has since established widespread transmission cycles using alternative vectors, and human infection has become a significant concern to public health, especially in Brazil. A multi-kilobase deletion was recently detected in Brazilian L. infantum genomes and is suggested to reduce susceptibility to the anti-leishmanial drug miltefosine. We show that deletion-carrying strains occur in at least 15 Brazilian states and describe diversity patterns suggesting that these derive from common ancestral mutants rather than from recurrent independent mutation events. We also show that the deleted locus and associated enzymatic activity is restored by hybridization with non-deletion type strains. Genetic exchange appears common in areas of secondary contact but also among closely related parasites. We examine demographic and ecological scenarios underlying this complex L. infantum population structure and discuss implications for disease control.

    • Familial primary hyperparathyroidism (HPT) is a rare disorder that is treated surgically and mostly occurs in association with tumor-susceptibility syndromes, like multiple endocrine neoplasia and the hyperparathyroidism-jaw tumor syndrome. Familial hypercalciuric hypercalcemia (FHH) is another cause of hereditary hypercalcemia that generally is considered to require no treatment and is genetically and pathophysiologically distinct from HPT. Inactivating mutations in the calcium receptor gene cause FHH, whereas the down-regulated expression of the CaR in HPT never has been coupled to CaR gene mutations.Family screening revealed a hitherto unknown familial condition with characteristics of both FHH and HPT. The hypercalcemia was mapped to a point mutation in the intracellular domain of the CaR gene that was coupled to relative calcium resistance of the PTH release by transient expression in HEK 294 cells. Unusually radical excision of parathyroid glands was required to normalise the hypercalcemia. The mildly enlarged parathyroid glands displayed hyperplasia with nodular components. Frequent allelic loss on especially 12q was found and contrasts to findings in HPT. Allelic loss was also seen in loci typical for primary HPT like 1p, 6q and 15q, but not 11q13. Quantitative mRNA analysis showed that the glands had mild increase in a proliferation index (PCNA/GAPDH mRNA ratio) and mild reduction in genes important to parathyroid cell function, like CaR, PTH, VDR and LRP2.A previously unrecognized variant of hypercalcemia is explored that could be one explanation for persistent hypercalcemia after apparently typical routine operations for HPT. It also raises the issue of possibilities to treat FHH with parathyroidectomy provided it is radical enough.

    • The goal of this study is to investigate putative molecular dynamic changes in cerebrospinal fluids (CSFs) collected from individuals with mild cognitive impairment (MCI) and Alzheimer's disease (AD) as compared to healthy controls.The CSF samples from 12 subjects comprised of four cognitively normal individuals and eight patients with MCI and AD, respectively. Two aliquots of each CSF samples (total 1 mL) of each participant are used for this study. Endogenous peptide separations are performed using 10000 molecular weight cut-off filters followed by LC-MS/MS identification and quantitation while lectin-enrichment chromatography is used to enrich glycoproteins in CSF followed by trypsin digestion and subsequent LC-MS/MS for shotgun identification and label-free quantitation.Using an optimized submicrogram peptide separation with molecular weight cut-off filtration and an in house-constructed database, 645 peptides are identified. Glycoproteins are enriched by lectin affinity chromatography, resulting in 795 identified proteins. The discovery and alterations of proSAAS-derived peptides and transthyretin are described and their roles in AD are discussed.Comprehensive identification of endogenous CSF peptidome is achieved. Fifteen proteins are found to be differentially expressed among the three groups. The dynamic changes of transthyretin are reported for the first time.
