    • The purpose of this paper is to argue for rule extraction as an integral part of data mining. The paper contains two case studies where rule extraction is used for typical data mining tasks. More specifically, rule extraction is used both to explain existing opaque models and to produce concept description via an opaque model. The main result is that the rule extraction approach generally yields comprehensible models with higher accuracy than transparent models created directly from the data set by CART and See5. The implication is that rule extraction can enhance the capability of data mining for decision support.

    • When a sample is studied during loading in a tensile test machine, the sample is often exposed to rigid body motions during loading at the same time as it deforms due to tension. Therefore, the small deformation field is hard, or impossible, to measure when it is overlaid by a large motion. The large rigid body motions can be measured with methods like speckle correlation (also called digital speckle photography, DSP), but the results might be of too poor accuracy to resolve the deformation field. Interferometric methods on the other hand might measure the deformation field but the rigid body motion makes the fringes disappear. In this thesis a method is presented that makes it possible to master such measuring situations, by a combination of speckle correlation and speckle interferometry (also called TV holography, ESPI or DSPI). Both theory and experiments are presented. It is shown that speckle correlation can determine the speckle motion in the recording in order to determine the small deformation field in the interferometric algorithm. Speckle correlation can also be used to determine the amount of shear in shearography allowing a quantitative determination of the spatial derivative of the deformation field.

  • Exploratory mixed methods analysis of self-authored content from participants in a digital alcohol intervention trial

    • Background: Digital interventions readily permit data capture of participant engagement with them. If future interventions are intended to be more interactive, tailored, or a useful resource offered to users, it may be valuable to examine such data. One module available in a digital alcohol intervention recently tested in a randomised control trial offered participants the opportunity to self-author prompts that were sent to them by a text message at a time of their choosing. This study thus aimed to evaluate these self-authored prompts to increase knowledge on how individuals negotiate behaviour change and assess whether intervention content can be improved in the future. Methods: The self-authored prompts were evaluated qualitatively using a combination of content and thematic analysis. The identified themes and subcategories are exemplified using anonymized quotes, and the frequency that each identified theme was coded for among the prompts was calculated. Associations between baseline characteristics and the odds of authoring a prompt at all, as well as a prompt within each theme, were investigated using logistic regression. Results: Five themes were identified (Encouragement Style, Level of Awareness, Reminders of reasons to reduce/quit, Strategies to reduce/quit, and Timescale), all with several subcategories. The prompts module was more likely to be used by women and older individuals, as well as those for whom reducing alcohol consumption was perceived as important, or who felt they had the know-how to do so. Participants who had immediate access to the support tool (intervention group) were more than twice as likely to author a prompt (OR = 2.36; probability of association > 99%) compared to those with 4-month delayed access (control group). Conclusions: Individuals who engaged with the prompts module showed evidence of using the information provided in the support tool in an active way, with several showing goal setting and making plans to change their drinking behaviour. Individuals also used this opportunity to remind themselves of personal and specific reasons they wanted to change their drinking, as well as to encourage themselves to do so.

  • HeadCrowd : visual feedback for design

    • HeadCrowd is a collaboration between the School of Mathematical and Computer Sciences and the School of Textiles and Design at Heriot-Watt University. It investigates how rich web and mobile applications may be employed to provide designers with near instantaneous and highly visual feedback from thousands of potential customers, or crowds. We are exploring the use of state of the art rich media applications to add quantity, speed and statistical accuracy to the study of semiotics, and the use of visuals in fashion as communication. The project seeks to add to participatory design and market intelligence processes by enabling rapid and iterated co-design cycles between crowds and designers based on visual forms of communication so as to mirror the highly visual nature of fashion design inspiration. Such a scheme shows applications for sustainability in fashion if it can give crowds a concrete sense of ownership of the design process and provide enthusiastic target markets, thereby offering potential to significantly reduce the risks of producing unwanted product. The paper provides an analysis of prior knowledge before describing the first two stages of the project, in which a pilot browser has been constructed that allows observers to navigate a vocabulary of 500 images which have been ordered into 48 similarity stacks using a mixture of human and crowd sourced sorting techniques. A first test involved the presentation of 20 terms to observers and asking them to choose 3 images from the browser to represent each term. Analysis of the resulting pilot data has given insights into the communicative certainty that a selection of 3 images from a vocabulary of 500 can provide for certain types of terms, and amongst certain groups of testers. It has also prompted deeper analysis of the pilot browser. To put the communicative value of visual feedback to the test, the current research phase is preparing the reverse experiment of asking a fresh cohort of participants to associate images back to the original terms, and various interfaces are currently being constructed to facilitate the presentation of visual choices from phase 1. The similarity relationship between test images is investigated and visualized before a case is made for comparative experiments of raw selection data and versions of visual summaries in this second research phase, in order to test which way of data presentations best convey the intended visual communication.

  • Criteria influencing the choice of luminaires in office lighting

    • This study is the first to evaluate the different criteria presently used in Sweden for lighting design, and their importance when planning the visual environment. It considers the role of the Swedish lighting designer and the different evaluations of criteria in the office lighting design process. The study aims to investigate whether a designer with an academic education or longer experience would infer more value in perceptual criteria and incorporate research into designs to a larger extent than a designer with less education. The findings suggest that current research needs to be more readily available to practitioners to enhance differentiation of criteria and proposed lighting designs. Education and experience could not be proven to make a difference, but different gender brought with it a divergence in the evaluations.

    • The history of public music libraries, if defined as libraries lending music and music literature to the public without membership limitations or commercial interest, is generally regarded to commence with nineteenth-century institutions such as the Musikbibliothek Peters in Leipzig and the Civica Biblioteca Musicale Andrea della Corte in Turin, often with a background related either to private foundations or commercial publishing. This, however, fails to recognise the public availability of collections in governmental and royal professional institutions in a tradition quite separate from the commercial institutions, beginning already in the eighteenth century. One such example is the Library of the Swedish Royal Academy of Music, the collections of which were open on special conditions already by the turn of the eighteenth and nineteenth centuries and accessible to non-members from the middle of the nineteenth century onwards. This article examines principles and practice concerning acquisitions, loans, and cataloguing throughout the first hundred years of the Library, assessing how the methods and ideals of the Academy librarians relate to music librarianship today.

    • This chapter provides an overview of the most recent advances in the mechanistic study of hydrothermal carbonisation (HTC) and the strategies to improve the conversion by using carbon-based catalysts. HTC, although not a recent discovery, has lately been receiving increasingly attention by both academic and industrial sectors due to the possibility to exploit this process to perform a simple, green and inexpensive conversion of bio-derived waste material into valuable chemicals and advanced materials and, as such, this chapter will also look into the use of hydrochars formed in HTC and their application in catalysis, more specifically heterogeneous catalysis with a mention on electrocatalysis. The versatility and tuneability of these solids give rise to the great range of applicability in different fields. A detailed overview of the HTC process is presented and the main uses of hydrochars in catalysis is then shown, highlighting their use as solid acid catalysts, as pristine solid catalysts, as sacrificial agents in synthesis, since their removal through combustion is easy, and the niche application of these solids in electrocatalysis for future research perspective.

    • Background: Anorexia nervosa is a serious illness with a high mortality rate, a poor outcome, and no empirically supported treatment of choice for adults. Patients with anorexia nervosa strive for thinness in order to obtain self-control and are ambivalent toward change and toward treatment. In order to achieve a greater understanding of patients’ own understanding of their situation, the aim of this study was to examine the expectations of potential anorexic patients seeking treatment at a specialized eating-disorder unit.Methods: A qualitative study design was used. It comprised 15 women between 18 and 25 years of age waiting to be assessed before treatment. The initial question was, “What do you expect, now that you are on the waiting list for a specialized eating-disorder unit?” A content analysis was used, and the text was coded, categorized according to its content, and further interpreted into a theme. Results: From the results emerged three main categories of what participants expected: “treatment content,” “treatment professionals,” and “treatment focus.” The overall theme, “receiving adequate therapy in a collaborative therapeutic relationship and recovering,” described how the participants perceived that their expectations could be fulfilled./p>Discussion: Patients’ expectations concerning distorted thoughts, eating behaviors, a normal, healthy life, and meeting with a professional with knowledge and experience of eating disorders should be discussed before treatment starts. In the process of the therapeutic relationship, it is essential to continually address patients’ motivations, in order to understand their personal motives behind what drives their expectations and their desire to recover.

  • Prediction of weight increase in anorexia nervosa

    • Background: Anorexia nervosa (AN) is a serious psychiatric disorder with high mortality rates a poor outcome and no empirically supported treatment of choice for adults. Weight increase is essential for recovery from AN why research exploring important contributors is crucial.Aims: The current study examined the importance of motivation to change eating behaviour, treatment expectations and experiences, eating disorder symptomatology, self-image and treatment alliance for predicting weight increase. Methods: Female patients (n = 89) between 18 and 46 years of age with Lambda N were assessed pre-treatment and at 6- and 36-month follow-ups with interviews and self-report questionnaires. At the 6-month follow-up the response rates differed from n = 58 (65%) to 66 (74%), and at the 36-month follow-up the response rates differed from n = 71 (80%) to 82 (92%).Results: At treatment start, expressed motivation to change eating habits, social insecurity and self-neglect were predictors of weight increase from 0 to 6 months, while duration, the time from onset to entering treatment, body dissatisfaction and interoceptive awareness were predictors of weight increase from 0 to 36 months.Conclusions: In designing treatment for adult patients with AN, it is essential to include multifaceted interventions addressed to patients' motivation to change, social relations, negative self-image and body dissatisfaction in order to achieve weight increase. Early detection and thereby short duration is an additional important factor that contributes to weight increase.

    • Under sommaren 1993 har luftvårdsförbund och länsstyrelser medverkat i ett nationellt samarbete, 'Klöver - Sverige 1993' som initierades 1992 av IVL tillsammans med SLU, Röbäcksdalen, Umeå. I studien användes subklöver, Trifolium subterraneum, som bioindikator för marknära ozon. Under sommaren 1993 odlades subklöver i 15 län på 18 lokaler från Kalmar i Småland till Öjebyn i Norrbotten. Under sommarens försök har det på ett flertal platser varit svåra förhållanden för odling, d v s kallt och regnigt. Bäst förhållanden verkar ostkusten ha haft. Sammanfattningsvis kan sägas att ozonhalterna i Sverige under sommaren har varit ovanligt låga och skadorna på subklövern orsakade av ozon följaktligen varit av mycket begränsad omfattning.

  • Editor's choice - Safety of carotid endarterectomy after intravenous thrombolysis for acute ischaemic stroke : a case-controlled multicentre registry study

    • OBJECTIVE: Few studies have been published on the safety of carotid endarterectomy (CEA) after intravenous thrombolysis (IVT). Registry reports have been recommended in order to gather large study groups.DESIGN: A retrospective, registry based, case controlled study on prospectively gathered data from Sweden, the capital region of Finland, and from Denmark, including 30 days of follow up.METHODS: The study group was a consecutive series of 5526 patients who had CEA for symptomatic carotid artery stenosis during a 4.5 year period. Among these, 202 (4%) had IVT prior to surgery, including 117 having CEA within 14 days, and 59 within 7 days of thrombolysis. IVT as well as CEA were performed following established guidelines. The median time from index symptom to CEA was 12 days (range 0-130, IQR 7-21).RESULTS: The 30 day combined stroke and death rate was 3.5% (95% CI 1.69-6.99) for those having IVT + CEA, 4.1% (95% CI 3.46-4.39) for those having CEA without previous IVT (odds ratio 0.84 [95% CI 0.39-1.81]), 3.4% (95% CI 1.33-8.39) for those having IVT + CEA within 14 days, and 5.1% (95% CI 1.74-13.91) for those having IVT + CEA within 7 days.CONCLUSION: Data on the time from symptoms to CEA in patients not having IVT, Rankin score, degree of stenosis, and cerebral imaging were not available. Despite its weaknesses, this study reasserts that CEA can be performed within the recommended 2 weeks of the onset of symptoms and IVT without increasing the risk of peri-operative stroke or death. Centres and vascular registries are recommended to continue monitoring changes in patient characteristics, lead times, and major complications after CEA in general, with a special focus on those who have undergone a prior thrombolysis.

    • Different forms of trust in the contemporary organizational settings of virtual organizations and time management (e.g., just‐in‐time, lean production, and total quality management) are discussed in conjunction with some Greek philosophical notions of human action, namely theoria/episteme, poiesis/techne, andpraxis/phronesis, together with the two notions of time, chronos/kairos and their spatial counterparts,chora/topos. It is suggested that time management concepts in production line settings are frequently based upon asymmetric power‐relations and rigid time‐control making most forms of organizational trust instrumental and/or weak. Virtual organization settings, on the other hand, are more likely to contain trust that appears to be fragile and temporal, and in demand of communication based on right moments to act judiciously.

    • The long-term outcomes of reform processes in the public sector remain understudied inthe literature. This study investigates the Swedish Transport Administration (STA)employees’ and managers’ translation and internalization of their new role as societaldevelopers. Since the STA’s founding in 2010 and until 2018, the STA head officeneither guided nor centrally determined how to define and understand the STA’s role as asocietal developer. We examine this internalization process through the lens ofCzarniawska’s translation model of the distribution of ideas as a collective creationthrough local translation and adaptation. The study shows that the ongoing friction thatoccurs when the concept and role of a societal developer are discussed and disseminatedwithin an organization is influenced by prevailing identities and local action nets. It alsoshows that the translation of this new role eventually failed, due to either it beingsubmerged within already-existing concepts or it having a perceived lack of relevance.We conducted this mixed-method study over six years (2016–2021) using documentaryanalysis, workshop participation, interviews and a survey.

    • The application of activated carbon (AC) to the surface of contaminated sediments is a promising technology for sediment remediation in situ. Amendment with AC has proved to be effective in reducing bioavailability and sediment-to-water release of hydrophobic organic contaminants. However, AC may cause positive or negative biological responses in benthic organisms. The causes of these effects, which include changes in growth, reproduction, and mortality, are unclear but are thought to be related to the size of AC particles. The present study investigated biological response to AC ranging from ingestible powdered AC to noningestible granular AC in two benthic deposit feeders: the polychaete Marenzelleria spp. and the clam Limecola balthica (syn. Macoma balthica). In the polychaete, exposure to powdered AC (ingestible) reduced both dry weight and carbon assimilation, whereas exposure to granular AC (noningestible) increased both dry weight and carbon assimilation. Responses in the clam were similar but less pronounced, indicating that response levels are species-specific and may vary within a benthic community. In addition, worms exposed to the finest ingestible AC particles had reduced gut microvilli length and reduced gut lumen, indicating starvation. These results strongly suggest that biological responses to AC depend on particle ingestibility, whereby exposure to ingestible particles may cause starvation through reduced bioavailability of food coingested with AC or due to rejection of AC-treated sediment as a food source.

    • One of the serious obstacles preventing wide industrial use of additive manufacturing (AM) in metals and alloys is a lack of materials available for this technology. It is particularly true for the Electron Beam Melting (EBM®) process, where only a few materials are commercially available, which significantly limits the use of the method. One of the dominant trends in AM today is developing processes for technological materials already widely used by other methods and developed for other industrial applications, gaining further advantages through the unique value added by additive manufacturing. Developing new materials specifically for additive manufacturing that can utilize the properties and specifics of the method in full is still a research and development subject, and such materials are yet far from full scale industrial usage. Stainless steels are widely used in industry due to good mechanical properties, corrosion resistance and low cost of material. Hence, there is potentially a market for this material and one possible business driver compared with casting for example is that lead times could be cut drastically by utilizing an additive approach for one-off or small series production. This paper presents results from the additive manufacturing of components from the known alloy 316L using EBM®. Previously the samples of 316L were made by laser-based AM technology. This work was performed as a part of the large project with the long term aim to use additively manufactured components in a nuclear fusion reactor. Components and test samples successfully made from 316L stainless steel using EBM® process show promising mechanical properties, density and hardness compared to its counterpart made by powder metallurgy (hot isostatic pressing, HIP). As with the other materials made by EBM® process, 316L samples show rather low porosity. Present paper also reports on the hierarchical microstructure features of the 316L material processed by EBM® characterized by optical and electron microscopy.

  • The association between eczema, asthma and rhinitis -population studies of prevalence and risk factors among adults

    • Background: Allergic diseases such as asthma, rhinitis and eczema have increased significantly since the middle of the past century and are now common conditions among both children and adults. The increase was observed earlier and is more evident in Westernized countries but is now also apparent in urbanized areas in developing countries. The exact cause of this increase is still not fully explored, although several partly contradicting hypotheses exist, including the hygiene hypothesis. Asthma, rhinitis and eczema are common comorbidities and allergic sensitization is commonly seen as a unifying link. Research questions: The overall aim of this thesis was to investigate the prevalence of eczema; risk factors for eczema; overlapping risk factors for asthma, rhinitis and eczema; the prevalence of allergic sensitization and the impact of allergic sensitization on these diseases in an adult population. Additional objectives included a validation of the representativeness of the recruited population. Methodology: This thesis is based on a postal survey in Västra Götaland with 18 087 responders (62%) out of a real study sample of 29 218 individuals aged 16 to 75 years. Clinical examinations including structured interviews, lung function tests, anthropometric measures, skin prick tests and assessments of specific serum Immunoglobulin E were performed in 1172 randomly selected responders. A study of non-response was carried out among non-responders to the postal survey and 211 out of the 400 randomly selected subjects were successfully contacted by phone and agreed to participate. Results: Non-responders compared to responders to the postal survey tended to be younger, smoke, have male sex and live in the metropolitan area of Gothenburg but they did not differ in prevalence of asthma, airway symptoms, eczema and rhinitis. Ever having had eczema was reported by 40.7% in the postal survey and the prevalence of current eczema was 11.5%. Eczema was more common among women and associated with respiratory symptoms, asthma and rhinitis. Allergic sensitization, obesity, female sex and occupational exposure to gas, dust or fumes were significantly and independently associated with asthma. A risk factors for rhinitis but not for eczema was obesity and allergic sensitization was strongly associated with rhinitis but less so for eczema. Risk factors for eczema but not for rhinitis were female sex and occupational exposure to gas, dust or fumes. Farm childhood was negatively associated with rhinitis and eczema but not asthma. The prevalence of sensitization to at least one common airborne allergen was 29.7%. Sensitization to birch and dog was associated with asthma while rhinitis was associated with sensitization to birch and timothy. No significant association was found between allergic sensitization and current eczema. Conclusions: We conclude that non-response had minimal effect on the outcome in our study. Eczema was more common than anticipated and associated with asthma and rhinitis. There are different risk factor patterns for asthma, rhinitis and eczema in adults. Allergic sensitization is an important risk factor for asthma and rhinitis but less so for eczema among adults. Rhinitis is mainly associated with sensitization to outdoor allergens while asthma is related to both outdoor and indoor allergens.

    • Aims Little is known about the optimal right ventricular (RV) pacing site in cardiac resynchronization therapy (CRT). This study compares bi-ventricular pacing at the left ventricular (LV) free wall combined with two different RV stimulation sites: RV outflow tract (RVOT+LV) vs. RV-apex (RVA+LV). Methods and results Thirty-three patients (32 males) with chronic heart failure, NYHA class III-IV, optimal drug therapy, QRS-duration ≥150 ms, and chronic atrial fibrillation (AF) received CRT with two different RV leads, in the apex (RVA) or outflow tract (RVOT), together with an LV lead, all connected to a bi-ventricular pacemaker. Randomization to pacing in RVOT+LV or RVA+LV was made 1 month after implantation and cross-over to the alternate pacing configuration occurred after 3 months. The median age of patients was 69 ± 10 years, the mean QRS was 179 ± 23 ms, and 58% of patients had ischaemic heart disease. Seven patients had pacemaker rhythm at inclusion and 60% were treated with atrioventricular-junctional ablation before randomization. In the RVA+LV and RVOT+LV pacing modes, 67 and 63% (nonsignificant) responded symptomatically with a decrease of at least 10 points in the Minnesota Living with Heart Failure score. The secondary end-points (6-min walk test, peak oxygen uptake, N-Terminal fragment of B-type Natriuretic Peptide, and left ventricular ejection fraction) showed significant improvement between baseline and CRT, but not between RVOT+LV and RVA+LV. Conclusion In this randomized controlled study, the exact RV pacing site, either apex or outflow tract, did not influence the benefits of CRT in a group of patients with chronic heart failure and AF.     • This paper presents results from a research project studying prequalification for restricted developer competitions aimed at housing in Sweden. The methodology includes an inventory of competitions, case studies, document reviews, and interviews with members in the organizers’ selection committees. The case studies comprise three restricted developer competitions in Danderyd, Nacka, and Trelleborg organized by the respective municipalities. The design teams are selected by prequalification in restricted competition. The organizer starts prequalification by inviting candidates to the competition. The organizer’s invitation contains a short description of the competition task, the aim of the competition and the conditions, submission requirements, and criteria for the evaluation of applications. The essential demands are general and based on Swedish law on public procurement. The evaluation criteria are experience-based and reflect professional merits for the competition. The three developer competitions generated a total of sixteen applications from candidates, teams of constructors, and developers in cooperation with architect firms. The lead applicant was the constructor or developer – not the architect firm. Eleven design teams were invited to the developer competitions after prequalification. Winners in developer competitions receive building permits and can implement their proposal, either by purchasing the land or acquiring the leasehold. Building costs (economy) and design quality (aesthetics) are key factors for organizers. The invited teams generally take part in the developer competitions at their own expense. This was the case for two of the competitions studied here, while in one the invited teams each received 50,000 SEK for their design proposals, which is very low compensation for design proposals as compared with architecture competitions. The organizers were pleased with the information in the candidates’ applications and the selection committees were easily able to choose teams for the competitions, which can be explained by the low number of applications. According to the selection committees, the prequalification process worked well, although the organizers had expected wider interest from the building sector and more applications from constructors and developers.

  • Markanvisningstävlingen som verktyg i strategisk utveckling: Del 2. Arrangörens erfarenheter

    • FoU-projektet syftar till att utveckla kunskap om gestaltning, hållbarhet och klimatomställning i samhällsbyggandet. Markanvisningstävlingen används som empiri och utgångspunkt. Projektet finansieras av CMB, Centrum för Management i Byggsektorn. Bakgrunden är kommunernas nyckelroll i byggandet som tävlingsarrangör, markägare och planmyndighet. Som arrangör svarar kommunen för tävlingsprogram och utser vinnare. I egenskap av markägare kan kommunen överföra vinnande kvaliteter till markanvisningsavtal och marköverlåtelseavtal. I rollen som planmyndighet reglerar kommunen markens användning i detaljplaner och kan följa upp vinnande förslag i samband med bygglov. Denna studie behandlar tre markanvisningstävlingar i Kinna, Mellbystrand och Varberg som fokuserar på gestaltning och hållbarhet. Insamlade data består av intervjuer med nyckelpersoner och analyser av dokument. Kommunernas markanvisningspolicy fördelar ansvaret mellan politik och förvaltning. Politiken beslutar om riktlinjer för markanvisningar och tilldelning av mark till byggherren bakom vinnande förslag. Förvaltningen tar fram beslutsunderlag, planerar tävlingsprocesser och utser vinnare. Kommunernas markanvisningspolicyer förmedlar en förenklad bild av tävlingen. Mark kan anvisas via tävling, anbud och direktanvisning. Tävlingen i kommunernas affärsmodell innehåller en serie strategiska överväganden som kommer till uttryck som tävlingsform, tävlingsuppgift, bedömningskriterier, inlämningskrav och utvärderingsmetodik. Tävlingsförslag och juryutlåtanden är offentliga handlingar är tillgängliga i sin helhet efter förfrågan till respektive kommun.

    • BACKGROUND AND PURPOSE: Leptin, important for body weight regulation, may be involved in the pathogenesis of the insulin resistance syndrome, associated with cardiovascular disease. We tested to determine whether leptin is a risk marker for first-ever stroke in a nested case-referent study.METHODS: We identified 113 patients with first-ever stroke (94 with ischemic and 19 with hemorrhagic stroke) who, before the stroke, had participated in population-based health surveys in northern Sweden. Referents were matched for sex, age, date and type of health survey, and geographic region. Blood pressure (BP), body mass index (BMI), and presence of smoking, diabetes, and hypertension were recorded. Total cholesterol, insulin, and leptin were analyzed in stored samples. Risk markers for first-ever stroke were analyzed by conditional logistic regression analysis.RESULTS: Patients with hemorrhagic stroke had higher levels of BMI and systolic and diastolic BPs. Leptin levels were 72% and 59% higher in males and females, respectively, with hemorrhagic stroke versus referents. Patients with ischemic stroke more often had hypertension, diabetes mellitus, and higher fasting glucose and insulin levels. A diagnosis of hypertension and elevated systolic and diastolic BPs were significant risk markers for first-ever hemorrhagic stroke in univariate analysis. High leptin (OR=20.55; 95% CI, 1.12 to 376.7) levels together with hypertension (OR=16.28; 95% CI, 1.49 to 177.3) remained as significant risk markers in a multivariate model. The combination of high leptin and high systolic or diastolic BP were associated with a profoundly increased risk for hemorrhagic stroke (OR=22.11; 95% CI, 1.57 to 310.9). Diabetes, hypertension, and obesity (BMI >/=27), together with high levels of insulin, glucose, systolic and diastolic BP, were significant risk markers for first-ever ischemic stroke in univariate analysis. Hypertension (OR=2.10; 95% CI, 1.14 to 3.86) remained as an independent risk marker in a multivariate model.CONCLUSIONS: Plasma leptin is strongly associated with an increased risk for first-ever hemorrhagic stroke, independent of other risk markers for cardiovascular disease. Leptin may be an important link in the development of cardiovascular disease in obesity.

    • Background. Fibromyalgia (FM) is a chronic pain syndrome, which affects mostly middle-aged women. The syndrome is poorly understood and treatment is mainly palliative. The diagnosis is established from diagnostic criteria. Living with FM means living a life greatly influenced by the illness in various ways for people affected. Aim of the study. The aim of this study was to describe the experiences of living with a woman with FM from the husbands' perspective. Methods. Five men married to women with FM were interviewed using a narrative approach. The interviews were analysed using qualitative thematic content analysis. Findings. The analysis resulted in the following seven themes: increasing responsibility and work in the home; being an advocate for and supporting the wife; learning to see the woman's changing needs; changing relationship between spouses; changing relationship with friends and relatives; deepening relationship with the children and lacking information and knowledge about FM. The findings show that the women's illness had a great impact on husbands' lives, and that husbands lacked information about the woman's illness. Conclusion. This study shows that it is not only the women with FM who experience a changed life; the whole family life is influenced and limited by FM. The husband's role in the family changes, first and foremost concerning responsibility and workload within the family. This must be taken into consideration in care planning. This study also highlighted the need of information and knowledge about FM expressed by the participants, information that health care personnel have a great responsibility to give.

    • Background: Obesity is associated with increased risk of cardiovascular disease. However, there is still a debate whether accumulation of fat in certain depots modifies this risk. Using data from the CArdioPulmonary bioImage Study (SCAPIS), we investigated if anthropometric measurements of obesity (waist and hip) modifies the risk of coronary artery calcification. Methods: In the first 15,810 participants in SCAPIS (mean age 58 years, 52% women), data on coronary artery calcification score (CACS) and anthropometry were recorded and traditional cardiovascular risk factors were measured. Body mass index (BMI) was categorized as; <25, 25-30, 30-35 and >35 kg/m2 , quartiles of waist and hip circumferences were constructed within each BMI category and compared using the lowest quartile as reference. Results were adjusted for site, age, smoking and diabetes status. Results: Obesity (BMI >30 kg/m2 ) was found in 21.9% of men and in 20.5% of women. In both sexes the odds ratio (OR) for CACS >0 increased with increasing BMI categories: comparing <25 and >35 kg/m2 , OR = 2.1 (95% CI: 1.6-2.7) for men and OR = 1.4 (1.2-1.8) for women. In addition, increasing quartiles of waist significantly increased the prevalence of CACS >0 for men [p = 0.05; OR = 1.2 (1.0-1.4) for highest quartile] and women [p = 0.005; OR = 1.3 (1.1-1.5)] while increasing quartiles of hip significantly decreased the prevalence for men [p = 0.005; OR = 0.8 (0.6-0.9)] and women [p = 0.04; OR = 0.8 (0.7-0.9)]. Data on education level and physical activity did not affect the model. Conclusion: Increased BMI is associated with increased prevalence of coronary artery calcification and the distribution of fat modifies this risk. Our results suggest that gluteofemoral adipose tissue (hip) counteracts the negative effects associated with BMI and abdominal adipose tissue (waist).
