    • The purpose of the dissertation is to study, describe and analyses which comparative measurement activities preschool children construct and use, and how they solve problems and communicate when they use the comparative measurement activities whilst playing with Lego (“the Lego play”). The measurement activities chosen are length/height, quantity and time.The empirical material is based on data from two preschool classes with children aged 2 – 5 year, It was collected through participant observation (video captures) of the children’s Lego play. The theoretical starting points in this study are grounded in childhood sociology and the sociocultural perspective. The study assumes the childhood sociology perspective by viewing the children as competent and active in creating meaning as well as controlling and influencing their own and others’ social environment. The sociocultural perspective gives prominence to development and learning, and its related tools and concepts are used to analyses the results of the study. That is, the Lego play is studied in a social context from the child’s perspective, and the sociocultural perspective describes and analyses the child’s use of mathematics and the acquisition of knowledge in the Lego play in a sociocultural context.The results show that children measuring length/height and quantity explored a store of measurement tools in order to make comparisons, and adapted these to the context in question. These were own body, other body, artefacts, numbers and counting. The measurements were used individually and with others, and the solving of the own or shared problems constituted a large share of the time spent constructing models during Lego play. By contrast, the time concept was used mainly as a tool when the children played with their finished Lego models. Thus, a time perspective was added to the child’s finished model, which inspired thoughts and reflections about time used in the Lego play. The children used the time concepts of the present, the past and the future, and also considered the concept of velocity in the context of the timescale. The children’s communication had a large impact on the Lego play, and they expressed their ideas verbally, physically and through action. The children’s use of mathematics was prominent and meaningful during the Lego play.

    • Amyloid precursor protein (APP) and its cleavage product amyloid beta (A beta) have been thoroughly studied in Alzheimer's disease. However, APP also appears to be important for neuronal development. Differentiation of induced pluripotent stem cells (iPSCs) towards cortical neurons enables in vitro mechanistic studies on human neuronal development. Here, we investigated expression and proteolytic processing of APP during differentiation of human iPSCs towards cortical neurons over a 100-day period. APP expression remained stable during neuronal differentiation, whereas APP processing changed. alpha-Cleaved soluble APP (sAPP alpha) was secreted early during differentiation, from neuronal progenitors, while beta-cleaved soluble APP (sAPP beta) was first secreted after deep-layer neurons had formed. Short A beta peptides, including A beta 1-15/16, peaked during the progenitor stage, while processing shifted towards longer peptides, such as A beta 1-40/42, when post-mitotic neurons appeared. This indicates that APP processing is regulated throughout differentiation of cortical neurons and that amyloidogenic APP processing, as reflected by A beta 1-40/42, is associated with mature neuronal phenotypes.

    • The notion of teaching is theoretically conceptualized in a manner relevant to the context of preschool. This concept is grounded in evolutionary and a pedagogical-psychological theory, with reference to Barnett, Rommetveit, Tomasello, and Vygotsky. The basis of teaching is found in people’s tendencies to make known to others what they themselves have seen/realized. Institutions such as preschool and school are understood as societal means for maintaining accumulated experience with the growing generation, something that puts teaching to the foreground. Empirical examples of language teaching in contemporary preschool are given and analyzed. Some important features of the concept of teaching as here theorized and exemplified are responsivity, language practices, the tension between intersub- jectivity and alterity, the variation of children’s experience, play and playfulness. The importance of basing theoretical accounts of teaching as a preschool activity on empirical research is emphasized.

    • I artikeln teoretiseras ett undervisningsbegrepp relevant för förskolans verksamhet. Teoretiskt grundas detta begreppsliggörande i såväl ett evolutionärt som i ett pedagogiskt-psykologiskt perspektiv. Några centrala teoretiker vi bygger på är Barnett, Rommetveit, Tomasello och Vygotskij. Vi lokaliserar grunden till undervisning i människors tendens att göra sådant de själva sett/insett synligt också för andra. Institutioner såsom förskolan och skolan förstås här som sätt för samhället att främja bevarandet av ackumulerande erfarenheter hos den växande generationen, något som gör undervisning centralt. Vi ger empiriska exempel på hur språkundervisning kan gestalta sig i dagens förskola; aktiviteter som vi analyserar utifrån de teoretiska begrepp vi introducerar. Några av de begrepp vi skriver fram för att teoretisera undervisning mer allmänt och språkundervisning mer specifikt i förskolan är responsivitet, kommunikativa praktiker, spänningsfältet mellan intersubjektivitet och alteritet, polyfoni och barns skilda erfarenheter, samt lek och lekfullhet. Vikten av att teoretisera undervisning i förskolan utifrån empirisk forskning betonas.

  • Body mass index influences the prognostic impact of combined nuclear insulin receptor and estrogen receptor expression in primary breast cancer

    • The prognostic importance of tumor-specific nuclear insulin receptor (InsR) expression in breast cancer is unclear, while membrane and cytoplasmic localization of InsR is better characterized. The insulin signaling network is influenced by obesity and may interact with the estrogen receptor a (ERα) signaling. The purpose was to investigate the interplay between nuclear InsR, ER, body mass index (BMI), and prognosis. Tumor-specific expression of nuclear InsR was evaluated by immunohistochemistry in tissue microarrays from 900 patients with primary invasive breast cancer without preoperative treatment, included in a population-based cohort in Sweden (2002-2012) in relation to prognosis. Patients were followed for up to 11 years during which 107 recurrences were observed. Nuclear InsR+ expression was present in 214 patients (23.8%) and increased with longer time between surgery and staining (P < 0.001). There were significant effect modifications by ER status and BMI in relation to clinical outcomes. Nuclear InsR+ conferred higher recurrence-risk in patients with ER+ tumors, but lower risk in patients with ER- tumors (Pinteraction = 0.003). Normal-weight patients with nuclear InsR+ tumors had higher recurrence-risk, while overweight or obese patients had half the recurrence-risk compared to patients with nuclear InsR- tumors (Pinteraction = 0.007). Normal-weight patients with a nuclear InsR-/ER+ tumor had the lowest risk for recurrence compared to all other nuclear InsR/ER combinations [HRadj 0.50, 95% confidence interval (CI): 0.25-0.97], while overweight or obese patients with nuclear InsR-/ER- tumors had the worst prognosis (HRadj 7.75, 95% CI: 2.04-29.48). Nuclear InsR was more prognostic than ER among chemotherapy-treated patients. In summary, nuclear InsR may have prognostic impact among normal-weight patients with ER+ tumors and in overweight or obese patients with ER- tumors. Normal-weight patients with nuclear InsR-/ER+ tumors may benefit from less treatment than normal-weight patients with other nuclear InsR/ER combinations. Overweight or obese patients with nuclear InsR-/ER- tumors may benefit from more tailored treatment or weight management.

    • Coffee is associated with decreased breast cancer risk, but the impact of body mass index (BMI) in combination with coffee consumption on prognosis is unclear. The suppressive effect of coffee constituents on the insulin-like growth factor receptor 1 (IGF1R) levels in breast cancer cells may play a role. The aim was to investigate the prognostic impact of coffee consumption and possible associations with tumor-specific IGF1R protein expression and BMI in a population-based cohort in Sweden, comprising 1,014 primary breast cancer patients without pretreatment enrolled 2002-2012 and followed for up to 13 years. Patients with higher coffee consumption had lower tumor IGF1R levels (P = 0.025), but only among the normal-weight patients (P = 0.005). Coffee did not impact the recurrence-risk overall. However, tamoxifen-treated patients with ER+ tumors drinking ≥ 2 cups of coffee/day had lower recurrence-risk [adjusted HR (HRadj) 0.57, 95% CI, 0.34-0.97] compared with patients with lower intake, although only among normal-weight patients (HRadj 0.37, 95% CI: 0.17-0.78; Pinteraction = 0.039). Similarly, coffee consumption ≥ 2 cups/day was associated with significantly lower recurrence-risk among the 640 radiotherapy-treated patients irrespective of BMI (HRadj 0.59, 95% CI 0.36-0.98) and in the 296 normal-weight patients (HRadj 0.36, 95% CI 0.17-0.76) but not in the 329 overweight or obese patients (HRadj 0.88, 95% CI 0.42-1.82) although the interaction was not significant (Pinteraction = 0.093). In conclusion, coffee consumption was negatively associated with tumor-specific IGF1R levels only among normal-weight patients. Though, IGF1R did not explain the association between coffee intake and improved prognosis among normal-weight tamoxifen- or radiotherapy-treated patients. Studies of IGF1R-targeting therapies may benefit from taking BMI and coffee consumption into account.

    • Clinical trials examining insulin-like growth factor-I receptor (IGF1R)-targeting strategies have emphasized that better predictive biomarkers are required to improve patient selection. Immunohistochemical tumor-specific protein expression of IGF1R, insulin receptor (InsR), and phosphorylated IGF1R/InsR (pIGF1R/InsR) individually and combined in relation to breast cancer prognosis was evaluated in a populationbased cohort of 1,026 primary invasive breast cancer patients without preoperative treatment diagnosed in Sweden. IGF1R (n = 923), InsR (n = 900), and pIGF1R/InsR (n = 904) combined cytoplasmic and membrane staining was dichotomized. IGF1Rstrong/InsRmod/strong/pIGF1R/InsRpos tumors were borderline associated with 2-fold risk for events, HRadj (2.00; 95%CI 0.96-4.18). Combined IGF1R and pIGF1R/InsR status only impacted prognosis in patients with InsRmod/strong expressing tumors (Pinteraction = 0.041). IGF1Rstrong expression impacted endocrine treatment response differently depending on patients' age and type of endocrine therapy. Phospho-IGF1R/InsRpos was associated with lower risk for events among non-endocrine-treated patients irrespective of ER status, HRadj (0.32; 95%CI 0.16-0.63), but not among endocrinetreated patients (Pinteraction = 0.024). In non-endocrine-treated patients, pIGF1R/InsRpos was associated with lower risk for events after radiotherapy, HRadj (0.31; 95%CI 0.12-0.80), and chemotherapy, HRadj (0.29; 95%CI 0.09-0.99). This study highlights the complexity of IGF hetero-and homodimer signaling network and its interplay with endocrine treatment, suggesting that combinations of involved factors may improve patient selection for IGF1R-targeted therapy.

  • High Estrogen Receptor β Expression Is Prognostic among Adjuvant Chemotherapy-Treated Patients-Results from a Population-Based Breast Cancer Cohort

    • PURPOSE: Isoform-specific tumor estrogen receptor β (ERβ) expression may hold prognostic information in breast cancer, especially among endocrine-treated breast cancer patients. The study's purpose was to evaluate ERβ isoform 1 (ERβ1) expression in relation to tumor characteristics, ESR2 genotypes, and prognosis in different treatment groups.EXPERIMENTAL DESIGN: A population-based prospective cohort of 1,026 patients diagnosed with primary invasive breast cancer in Lund, Sweden, between October 2002 and June 2012 was followed until June 2014 (median 5 years). Associations between immunohistochemical ERβ1 expression, patient and tumor characteristics, as well as outcome within treatment groups were analyzed.RESULTS: Tumor ERβ1 expression was available for 911 patients (89%) and was not associated with ESR2 genotypes. ERβ1 positivity, defined as >75% (ERβ175(+), 72.7%), was positively associated with established favorable tumor characteristics. Overall, ERβ175(+) was associated with lower risk of breast cancer events [HRadj = 0.60; 95% confidence interval (CI), 0.41-0.89]. The magnitude of the association was larger in patients with ERα(-) tumors (HRadj = 0.30; 95% CI, 0.12-0.76), compared with ERα(+) tumors (HRadj = 0.66; 95% CI, 0.42-1.03). Among the 232 chemotherapy-treated patients, ERβ175(+) tumors were associated with lower risk of breast cancer events compared with ERβ175(-) tumors (HRadj = 0.31; 95% CI, 0.15-0.64). Among the 671 chemonaïve patients, ERβ175 status was not associated with the outcome.CONCLUSION: High ERβ1 expression was a favorable prognostic marker in this breast cancer cohort, especially in chemotherapy-treated patients, but not in endocrine therapy-treated patients.   • Production and Products of Preschool Documentation : Entanglements of children, things, and templates

    • The objective of the present study is to produce knowledge about how children, things, documents, computers, teachers, templates and other entities take part in and produce documentation in preschool systematic quality development work. The thesis asks questions about how and what is produced as quality in preschool documentation. Through a posthumanist agential realist approach, the study engages empirically in video recordings of preschool documentation practices from one Swedish preschool, and in documentation from four other Swedish preschools.The results, presented in four articles, show that documentation practice is complex and entangled with a variety of entities, human and non-human, and spaces and time. The study shows how children’s participation in production of documentation does not depend on which documentation method is used. In the study, objects such as computers, images and coloured labels, together with teachers’ attitudes and agendas, have an impact on the way that children are involved in producing documentation. The results also show that the products of documentation, intra-acting with a local template, with past and present preschool traditions and policies, produce rather than represent certain elements of quality such as teachers’ actions, children’s interests and thematic work. The thesis produces knowledge about how the focus of preschool documentation shifts away from individual children and towards teachers’ activities, and creates possibilities for what preschool practices and quality may become.

I Fritidshemmets möjligheter – att arbeta fritidspedagogiskt presenterar författarna en rad viktiga områden om hur fritidshemmet kan bidra till barns allsidiga utveckling och lärande.I boken lyfts kärnfrågor som fritidshemmets sociala uppdrag, lek, föräldrasamverkan, fritidshemsutveckling. Här tas även ­nyare innehåll om fritidspedagogiskt arbete upp, som t.ex. hur man kan arbeta med språk, matematik, naturvetenskap och teknik.Fritidshemmets möjligheter – att arbeta fritidspedagogiskt ger en god grund för att skapa ett inspirerande innehåll för de många barn som tillbringar stora delar av sin tid på fritidshemmet. HMGCR expression in breast tumors has recently been proposed to hold prognostic and treatment-predictive information. This study aimed to investigate whether HMGCR expression in breast cancer patients was associated with patient and tumor characteristics and disease-free survival (DFS).

    • En stor andel svenska barn mellan sex och nio år gamla går efter skolan till ”fritids”. Fritidshem kännetecknas av både utbildning och omsorg, och därmed av både formella och informella aktiviteter. På senare år har verksamheten blivit mer och mer influerad av skolan och numera finns ett eget kapitel för fritidshem i den nationella läroplanen för grundskolan. Fritidshemmen genomgår därför en förändring som kommer att medföra att mer formella aktiviteter införs, exempelvis inom teknik, samtidigt som praktiskt taget ingen forskning har gjorts på detta. Syftet med det här kapitlet är att presentera några resultat från en pågående forskningsstudie om teknikens roll i aktiviteter på fritidshem. Observationerna fokuserade på de dagliga aktiviteterna på fritidshemmen där teknik hade en central roll. Vi använder begreppet gränsobjekt för att analysera teknik i aktiviteterna. Resultaten visar att det är en speciell sorts teknikundervisning som uppstår i fritidshem, på gränsen mellan den informella och formella verksamheten: från det informella lekrelaterade byggandet med Lego och träklotsar till den mer formella datorundervisningen. I dessa aktiviteter finns ett tydligt fritidsinslag, framför allt i form av ett fritt val av teknik och vad man vill lära sig. Det faktum att barn kan välja fritt pekar inte bara på att teknik i fritidshem är ett gränsobjekt med en stor tolkningsflexibilitet, utan också att teknikundervisning i fritidshem skulle kunna vara en lustfylld och effektiv väg till teknisk allmänbildning.

  • History of oral contraceptive use in breast cancer patients: impact on prognosis and endocrine treatment response.

    • The purpose was to study oral contraceptive (OC) use in relation to breast cancer events and endocrine treatment response in a prospective population-based cohort, because it is unclear whether history of OC use impacts on prognosis in breast cancer patients. Between 2002 and 2011, 994 primary breast cancer patients without preoperative treatment were enrolled in Lund, Sweden and followed until December 2012. History of OC use was obtained from preoperative questionnaires. Tumor characteristics, clinical data, and date of death were obtained from pathology reports, patient charts, and population registries. Among the 948 patients with invasive cancer and no metastasis detected on the post-operative screen, 74 % had ever used OCs. Patients were followed for up to nine years (median follow-up 3 years), and 100 breast cancer events were recorded. Ever OC use was not associated with prognosis, irrespective of duration. However, any OC use before age 20 was associated with a threefold increased risk for breast cancer events in patients <50 years but not in patients ≥50 years (P interaction = 0.009). In patients ≥50 years with estrogen receptor positive tumors, previous OC use at any age was associated with a significantly decreased risk of breast cancer events among patients who received aromatase inhibitors compared to patients who never used OCs (adjusted HR 0.37: 95 % CI 0.15-0.87). OC use was not associated with tamoxifen-response. If confirmed, history of OC use may yield valuable prognostic and treatment predictive information in addition to currently used criteria.

    • A majority of Swedish children between six and nine years old attend afterschool activities in a leisure-time centre, fritidshem, until their parents finish work for the day. Leisure-time centres are characterised by “educare” activities, that is, both educational and carerelated activities and thus both formal and informal learning settings. The centres are part of the Swedish educational system and activities should be related to the national curriculum for the compulsory school. The centres are right now in an interesting transition which will lead to the introduction of more formal learning activities, for instance, in technology, at the same time as virtually no research has been carried out on technology activities in the centres. The aim of this paper is therefore to study the role of technology in activities in leisure-time centres. The results are drawn from field work conducted in three Swedish leisure-time centres. The observations focused upon the daily activities in the leisure centres, specifically activities where technology play a central part. The data collection was completed with interviews with teachers. We use the concept of boundary object to analyse the technological activities. The findings indicate that there is a special technology education that emerges in the context of the leisuretime center – leisure-time technology education – at the intersection between informal and formal technology learning, which ranges from the informal play-related construction of Lego and wooden blocks to the more formal computer instruction. The amorphousness of leisure-time technology – the fact that children can choose what they want it to be and what to learn –points to it being a boundary object with a very open interpretative flexibility.

    • The overall aim of this article is to investigate Swedish preschool teachers’ perceptions of the interaction between home and institution in relation to children’s introduction to preschool. The focus of this article is on their talk about how they manage the gap between home and preschool in the introduction process. A discourse analysis is carried out, based on focus group interviews with seven preschool teacher teams that have started to use a more parent-active approach during the introduction of children to preschool. The results show that a parent-active introduction positions and governs parents to take a more self-regulative role in preschool from the beginning. The construction of the parent-active introduction discourse/practice produces new subject positions for the parents (and teachers) and creates expectations of intensified parental involvement in this institutional practice. Furthermore, the results indicate that the parents’ active introduction also changes the teachers’ own role and their attitudes toward the parents. The boundary work between the home and preschool seems to consist of negotiations and of the construction of an intermediate domain between home and preschool that draws on discourses of responsibility, performativity and efficiency.

The findings indicate that the life situation of intensive care survivors is changed, that recovery takes time and that the feeling of gratitude for having survived is strong.

    • The overall aim of this study is to describe the preschool setting that is constructed in the preschool teacher’s planning conversations with a special focus on the border context in which this particular preschool is located. In what ways are the geographic location noticeable and what conceptions are established in their conversations? The study has a social constructivist approach and is informed by borderland theory which means that the focus is on the context and the participants in this context. The staff's experience is, however, seen as historically and culturally influenced by the context. With the support of borderland theory the study explores how the preschool´s borderland location has relevance to the social construction in the preschool studied.This research project is an observation study of a two- sectionpreschool in Sweden close to the Norwegian border which started in the spring of 2013. The preschool teachers were observed in staff meetings, all together and in smaller groups. The target group consists of educators and other staff. The Swedish ethical guidelines for research on children have been observed in all respects.The result shows how the staff of the researched preschool construct themselves as a unified us as opposed to them, not in terms of national borders, but in relation to their joint decisions on what they share and have in common and on who do not – the others.Keywords:Preschool, kindergarten, preschool teachers, border/national border, staff meetings/planning conversations, curricula and constructions/social construction

    • Background:The association between smoking and breast cancer prognosis remains unclear. The purpose of this study was to investigate whether preoperative smoking was associated with prognosis in different treatment groups.Methods:This population-based cohort consisted of 1065 breast cancer patients without preoperative treatment included between 2002 and 2012 in Lund, Sweden. Smoking status was examined in relation to patient and tumour characteristics, and prognosis in different treatment groups.Results:At the preoperative visit, 21.0% smoked. Median follow-up time was 5.1 years. Overall, in the 1016 patients included in the survival analyses, there was no significant association between smoking and risk of breast cancer events (adjusted hazard ratio (adjHR): 1.45; 95% confidence interval (CI): 0.95–2.20). For the 309 aromatase inhibitor (AI)-treated patients ⩾50 years with oestrogen receptor-positive (ER+) tumours, smoking was associated with risk of breast cancer events (adjHR: 2.97; 95% CI: 1.44–6.13), distant metastasis (adjHR: 4.19; 95% CI: 1.81–9.72), and death (adjHR: 3.52; 95% CI: 1.59–7.81). Smoking was not associated with breast cancer events or distant metastasis in other treatment groups.Conclusions:Preoperative smoking was only associated with an increased risk for breast cancer events and distant metastasis in AI-treated patients.     • In Sweden the preschool is regarded as “the child´s first school” and a part of the educational system. Most children aged 1-5 attend preschool, where daycare and education are combined. The preschool is the start of the “lifelong learning” and the importance of an early start in this institution is underpinned by the government. Collaboration and communication between teachers and parents have during the last decades become increasingly highlighted in the Swedish preschool context. The aim of this study is to highlight how the preschool teachers’ view the interaction with parents in relation to children’s introduction to preschool, with focus on their talk about the parents’ position in the introduction process. The results are drawn from focus group interviews which have been conducted with seven preschool teams in Swedish preschools. Data was analyzed in the positioning theory tradition. The results show that the parents’ involvement is important for the teachers in order to create a professional relationship with the parents and to give them support in their parental role. Furthermore, the parents’ involvement during the introduction period is central for creating a complete picture of the child, and that it may be seen as an evaluation and co-operation practice for the parents and the preschool. New parents’ subject positions emerge: the parents are active actors and team members in the introduction process.

    • BACKGROUND: Endocrine resistance is a major obstacle to optimal treatment effect in breast cancer. Some genetic markers have been proposed to predict response to aromatase inhibitors (AIs) but the data is insufficient. The aim of the study was to find new genetic treatment predictive markers of AIs.METHODS: The ongoing population-based BC-blood study in Lund, Sweden includes women with primary breast cancer. This paper is based on AI-treated patients with estrogen receptor positive tumors who underwent breast cancer surgery in 2002-2008. First, an exploratory analysis of 1931 SNPs in 227 genes involved in absorption, distribution, metabolism, and elimination of multiple medications, using DMET™ chips, was conducted in a subset of the cohort with last follow-up in December 31(st) 2011 (13 cases, 11 controls). Second, selected SNPs from the first analysis were re-analyzed concerning risk for early breast cancer events in the extended cohort of 201 AI-treated with last follow-up in June 30(th) 2014. Clinical data were obtained from medical records and population registries.RESULTS: Only CYP1A2 rs762551 C-allele was significantly associated with increased risk for early events in the 24 patients (P = 0.0007) and in the extended cohort, adjusted Hazard ratio (HR) 2.22 (95 % CI 1.03-4.80). However, the main prognostic impact was found within five years, adjusted HR 7.88 (95 % CI 2.13-29.19). The impact of the CYP1A2 rs762551 C-allele was modified by a functional polymorphism in the regulator gene AhR Arg554Lys (G > A). Compared to patients who were homozygous for the major allele in both genes (CYP1A2 A/A and AhR G/G), a 9-fold risk for early events was found in patients who had at least one minor allele in both genes, adjusted HR 8.95 (95 % CI 2.55-31.35), whereas patients with at least one minor allele in either but not both genes had a 3-fold risk for early events, adjusted HR 2.81 (95 % CI 1.07-7.33). The impact of CYP1A2 rs762551 C-allele was also modified by the CYP19A1 rs4646 C/C, adjusted HR 3.39 (95 % CI 1.60-7.16) for this combination. This association was strongest within the first five years, adjusted HR 10.42 (95 % CI 3.45-31.51).CONCLUSION: CYP1A2 rs762551 was identified as a new potential predictive marker for early breast cancer events in AI-treated breast cancer patients. Moreover, combined genotypes of CYP1A2 rs762551 and CYP19A1 rs4646 or AhR Arg554Lys could further improve prediction of early AI-treatment response. If confirmed, these results may provide a way to more personalized medicine.

  • Integration of Tumor and Host Factors – Implications for Breast Cancer Prognosis

    • Breast cancer is the most common cancer among women worldwide, although the mortality rates have decreased in the last decades. Breast cancer is a heterogeneous disease, and treatment resistance is a significant clinical problem in spite of the overall high survival rates. Improved screening programs, novel surgical techniques, and adjuvant treatments have contributed to the improved survival, but more extensive detection of breast cancer and subsequent treatment also imply higher rates of overtreatment. Therefore, it is vitally important to identify new tumor markers or host factors for patients who are at risk of recurrence, as well as patients who would benefit from less treatment. This approach would lead to more personalized breast cancer treatment. The aim of this thesis is to elucidate whether combining host factors, including genetic constitution and lifestyle factors, with tumor characteristics could yield a more comprehensive understanding than either factor alone for the prognosis of breast cancer. In paper I, moderate to high coffee consumption was associated with higher frequency of ER-negative tumors but a lower risk of early breast cancer events among tamoxifen-treated patients with ER-positive tumors. Furthermore, the combinations of low coffee consumption with the germline CYP1A2 rs762551 C-allele and CYP2C8*3, respectively, were associated with a significantly increased risk of early breast cancer events, indicating that integrating genotype and lifestyle factors may impact the prognosis of breast cancer. In paper II, any preoperative or postoperative alcohol consumption was weakly associated with a lower risk of early breast cancer events. This association was modified by axillary lymph node status, in that any alcohol consumption was associated with a significantly lower risk of early breast cancer events among patients with axillary lymph node involvement but not in patients without axillary lymph node involvement. The results do not support recommending that all breast cancer patients abstain from low to moderate alcohol consumption. In paper III, the CYP1A2 rs762551 C-allele was strongly associated with a higher risk of breast cancer events among aromatase inhibitor-treated patients, and the main impact was found within the first five years. In addition, the impact of the CYP1A2 rs762551 C-allele was modified by genotypes of AhR Arg554Lys and CYP19A1 rs4646, and the combined genotypes could further improve the prediction of aromatase inhibitor response. If validated, these genotypes could be used as predictive markers for aromatase inhibitor response. In paper IV, tumor-specific COX-2 expression was associated with significantly less aggressive tumor characteristics and was independently associated with a lower risk of early breast cancer events. The association was modified by a history of oral contraceptive (OC) use, preoperative non-steroidal anti-inflammatory drug (NSAID) use, and tumor size. If the findings were validated in an independent prospective cohort or within a randomized trial, history of OC use and tumor size might need to be considered when designing or evaluating clinical outcomes in a randomized controlled trial of adjuvant NSAIDs or COX-2 selective inhibitors for breast cancer patients. In conclusion, a more comprehensive view of tumor characteristics combined with host factors could be beneficial when assessing breast cancer prognosis and may provide a method for more personalized medicine in the treatment of breast cancer patients.

Vilka bilder av arbetet i förskoleklass och vilken styrning av verksamheten framträder i detta material?I oktober 2014 gav Skolverket ut ett stödmaterial för förskoleklassen med titeln Förskoleklassen: Uppdrag, innehåll och kvalitet (Skolverket, 2014). Enligt Skolverket är syftet med materialet att ”bidra till ökad kunskap om och förståelse för förskoleklassens uppdrag, innehåll och kvalitetsutveckling med utgångspunkt i skollag och läroplan” (ibid. s. 3).Stödmaterialet ges ut 16 år efter det att förskoleklassen infördes som en egen skolform. Ett stödmaterial som detta kan ses som en del av den officiella retoriken kring utbildningsinstitutionen. Det kan ses som ett försök att åstadkomma forandrings--‐ processer som ska leda till det bättre. Detta material produceras för lärarna i förskoleklass som är de som ska realisera detta i de pedagogiska praktikerna. Produktionen av ett nytt stödmaterial för förskoleklassen innebär att det skapas en arena för förhandling, om bilden av barnet och vad det är för institution vi har att göra med. Vi behöver veta mer om vad det är för förhandlingar som uppstår och vilken verksamhet man vill åstadkomma.Syftet med denna artikel är att få kunskaper om vilka bilder av förskoleklassen och vilka styrningsmekanismer som framträder i Skolverkets stödmaterial (2014). För att göra detta har vi använt Foucaults teorier om styrning (governmentality). Vi vill på detta sätt lyfta fram de sätt på vilka förskoleklassen som utbildningsinstitution och förskoleklasslärarna blir föremål för styrning (Davies & Harré, 1990; Foucault, 1991, 2003; Rose, 1999).

    • The association between tumor cyclooxygenase 2 (COX-2) expression and breast cancer prognosis has been inconsistent. The purpose of this study was to evaluate the prognostic significance of COX-2 tumor expression according to adjuvant treatment, and potential effect modifications of non-steroid anti-inflammatory drug (NSAID) use, and other tumor and lifestyle factors. A prospective cohort of 1,116 patients with primary breast cancer in Lund, Sweden, included 2002-2012 was followed until June 2014 (median 5 years). Tumor-specific COX-2 expression was evaluated on tissue microarrays using immunohistochemistry. Associations between COX-2 intensity (negative, weak-moderate, high) and patient and tumor characteristics as well as prognosis were analyzed. Tumor-specific COX-2 expression was available for 911 patients and was significantly associated with higher age at diagnosis and less aggressive tumor characteristics. Higher COX-2 expression was associated with lower risk for breast cancer events during the first five years of follow-up, adjHR 0.60 (95%CI: 0.37-0.97), per category. The association between COX-2 expression and prognosis was significantly modified by oral contraceptive (OC) use (Pinteraction=0.048), preoperative NSAID use (Pinteraction=0.009), and tumor size (Pinteraction=0.039). COX-2 negativity was associated with increased risk for events during the first five years in ever OC users, adjHR 1.94 (1.01-3.72) and during the 11-year follow-up in preoperative NSAID users, adjHR 4.51 (1.18-11.44) as well as in patients with large tumors, adjHR 2.57 (1.28-5.15). In conclusion, this study, one of the largest evaluating COX-2 expression in breast cancer, indicates that the prognostic impact of COX-2 expression depends on host factors and tumor characteristics.

    • This study aims to examine how young children construct and master, with the help of artefacts, their very first institutional relations with peers, and how they handle the preschool practice during the transition period between the home and the preschool. The discourse analysis is based on ethnographic data from the introduction period of two children in the Swedish preschool. The results highlight that these young children are competent agents in their own transition. They use artefacts as tools for their own purposes; for negotiating places, creating relationships, and learning norms in their interactions with peers. The findings add to the understanding and significance of children’s perspective regarding their own transition and the role of the artefacts in it, which has conceptual and practical implications for researchers as well as practitioners worldwide.

    • Tuberculosis (TB) is the world’s most deadly infectious disease and causes enormous public health problems. The comorbidity with HIV and the rise of multidrug-resistant TB strains impede successful therapy through drug-drug interactions and the lack of efficient second-line treatments. The aim of this thesis was to support the improvement of anti-TB therapy through development of pharmacometric models, specifically focusing on the novel drug bedaquiline, pharmacokinetic interactions and methods for pooled population analyses.A population pharmacokinetic model of bedaquiline and its metabolite M2, linked to semi-mechanistic models of body weight and albumin concentrations, was developed and used for exposure-response analysis. Treatment response was quantified by measurements of mycobacterial load and early bedaquiline exposure was found to significantly impact the half-life of bacterial clearance. The analysis represents the first successful characterization of a concentration-effect relationship for bedaquiline.Single-dose Phase I studies investigating potential interactions between bedaquiline and efavirenz, nevirapine, ritonavir-boosted lopinavir, rifampicin and rifapentine were analyzed with a model-based approach. Substantial effects were detected in several cases and dose-adjustments mitigating the impact were suggested after simulations. The interaction effects of nevirapine and ritonavir-boosted lopinavir were also confirmed in patients with multidrug-resistant TB on long-term treatment combining the antiretrovirals and bedaquiline. Furthermore, the outcomes from model-based analysis were compared to results from conventional non-compartmental analysis in a simulation study. Non-compartmental analysis was found to consistently underpredict the interaction effect when most of the concentration-time profile was not observed, as commonly is the case for compounds with very long terminal half-life such as bedaquiline.To facilitate pooled analyses of individual patient data from multiple sources a structured development procedure was outlined and a fast diagnostic tool for extensions of the stochastic model components was developed. Pooled analyses of nevirapine and rifabutin pharmacokinetics were performed; the latter generating comprehensive dosing recommendations for combined administration of rifabutin and antiretroviral protease inhibitors.The work presented in this thesis demonstrates the usefulness of pharmacometric techniques to improve treatment of TB and especially contributes evidence to inform optimized dosing regimens of new and old anti-TB drugs in various clinical contexts.

Preoperative statin use, ABCB1 C3435T genotype, and HMGCR expression in relation to outcome were analyzed in 985 primary breast cancer patients from a population-based prospective cohort in Sweden from 2002 to 2012. Preoperative statin use (n = 80) was not associated with ABCB1 C3435T genotype (n = 576), HMGCR expression (n = 848), or clinical outcomes. ABCB1 C3435T TT-carriers had lower risk of breast cancer events than any C-carriers (adjusted hazard ratio (HRadj) 0.74; 95%CI 0.49, 1.12), but only in non-statin users (Pinteractio n = 0.042). Statin users with TT genotype had higher risk of distant metastasis (HRadj 4.37; 95%CI 1.20, 15.91; Pinteraction = 0.009) and shorter overall survival than other patients (HRadj 3.77; 95%CI 1.37, 10.39; Pinteractio n = 0.019). In conclusion, there were nominally significant interactions between ABCB1 genotype and preoperative statin use on clinical outcomes, while preoperative statin use was not associated with outcomes. Since this is an exploratory study of the impact of the ABCB1 genotype in relation to statin use and clinical outcomes in the breast cancer setting, the results should be interpreted with caution and warrant replication in an independent cohort, preferably in a randomized setting. Since statin use is common in breast cancer patients, it would be of interest to further elucidate the clinical impact of the ABCB1 genotype in breast cancer.

I intervjuer uttrycker föräldrar att de anser utvecklingssamtalen som meningsfulla på olika sätt. De kan säkerställa en känsla av tillit och trygghet med professionella pedagoger som i föräldrarnas ögon tycks framstå som ”barnexperter”. Vidare kan utvecklingssamtalen bidra till att bekräfta föräldrarnas egen bild av barnet och sitt föräldraskap.

    • Overweight and obesity are increasing worldwide, but the extent in breast cancer patients is unknown. The two aims were to study secular trends in preoperative body mass index (BMI), waist circumference, and breast volume and their impacts on clinical outcome. BMI, waist circumference, and breast volume were measured preoperatively in 24–99-year-old primary breast cancer patients (n = 640) in Sweden 2002–2016. The measurements were analyzed alone and combined in relation to recurrence and overall survival (OS). BMI, waist circumference, and breast volume increased 2002–2016 (ptrends < 0.0001). Of these, a breast volume ≥ 850 mL was associated with the strongest recurrence-risk (adjusted hazard ratio [adjHR] 1.67; 95% CI 1.17–2.39), especially combined with waist circumference ≥ 80 cm (adjHR 2.07; 95% CI 1.25–3.44), while BMI ≥ 25 kg/m2 or large waist circumference conferred almost a twofold risk for death (both Log-Rank p ≤ 0.0001). Chemotherapy seemed to counteract the negative impact of a high BMI or large waist circumference on OS. Large breast volume was the strongest predictor for recurrence in all treatment groups. In conclusion, preoperative BMI, waist circumference, and breast volume increased between 2002 and 2016. Larger body size negatively impacted breast cancer-free interval and OS. If confirmed, body measurements may help select patients requiring more individualized treatment.
