  • Overview of the JET results with the ITER-like wall

    • Following the completion in May 2011 of the shutdown for the installation of the beryllium wall and the tungsten divertor, the first set of JET campaigns have addressed the investigation of the retention properties and the development of operational scenarios with the new plasma-facing materials. The large reduction in the carbon content (more than a factor ten) led to a much lower Z(eff) (1.2-1.4) during L- and H-mode plasmas, and radiation during the burn-through phase of the plasma initiation with the consequence that breakdown failures are almost absent. Gas balance experiments have shown that the fuel retention rate with the new wall is substantially reduced with respect to the C wall. The re-establishment of the baseline H-mode and hybrid scenarios compatible with the new wall has required an optimization of the control of metallic impurity sources and heat loads. Stable type-I ELMy H-mode regimes with H-98,H-y2 close to 1 and beta(N) similar to 1.6 have been achieved using gas injection. ELM frequency is a key factor for the control of the metallic impurity accumulation. Pedestal temperatures tend to be lower with the new wall, leading to reduced confinement, but nitrogen seeding restores high pedestal temperatures and confinement. Compared with the carbon wall, major disruptions with the new wall show a lower radiated power and a slower current quench. The higher heat loads on Be wall plasma-facing components due to lower radiation made the routine use of massive gas injection for disruption mitigation essential.

  • Overview of the JET results

    • Since the last IAEA Conference JET has been in operation for one year with a programmatic focus on the qualification of ITER operating scenarios, the consolidation of ITER design choices and preparation for plasma operation with the ITER-like wall presently being installed in JET. Good progress has been achieved, including stationary ELMy H-mode operation at 4.5 MA. The high confinement hybrid scenario has been extended to high triangularity, lower ρ*and to pulse lengths comparable to the resistive time. The steady-state scenario has also been extended to lower ρ*and ν*and optimized to simultaneously achieve, under stationary conditions, ITER-like values of all other relevant normalized parameters. A dedicated helium campaign has allowed key aspects of plasma control and H-mode operation for the ITER non-activated phase to be evaluated. Effective sawtooth control by fast ions has been demonstrated with3He minority ICRH, a scenario with negligible minority current drive. Edge localized mode (ELM) control studies using external n = 1 and n = 2 perturbation fields have found a resonance effect in ELM frequency for specific q95values. Complete ELM suppression has, however, not been observed, even with an edge Chirikov parameter larger than 1. Pellet ELM pacing has been demonstrated and the minimum pellet size needed to trigger an ELM has been estimated. For both natural and mitigated ELMs a broadening of the divertor ELM-wetted area with increasing ELM size has been found. In disruption studies with massive gas injection up to 50% of the thermal energy could be radiated before, and 20% during, the thermal quench. Halo currents could be reduced by 60% and, using argon/deuterium and neon/deuterium gas mixtures, runaway electron generation could be avoided. Most objectives of the ITER-like ICRH antenna have been demonstrated; matching with closely packed straps, ELM resilience, scattering matrix arc detection and operation at high power density (6.2 MW m-2) and antenna strap voltages (42 kV). Coupling measurements are in very good agreement with TOPICA modelling. © 2011 IAEA, Vienna.

    • I samband med av- och påstigning av buss utsätts passagerare för risker från övrig trafik. Barn är speciellt utsatta i samband med färd till och från skolan och det finns därför starka skäl att försöka förmå de fordon som passerar en stillastående skolskjuts att sänka hastigheten rejält. Med erfarenheterna från 30-projektet som bakgrund beslutades att föreslå ett införande av en 30-gräns vid stillastående skolskjuts i hela landet på vägar med hastighetsgränser 70 km/h och lägre. Inför en sådan eventuell lagändring är det viktigt att utforma en skolskjutsskylt som respekteras av alla trafikanter. I ett laboratorietest jämfördes hur iögonfallande fyra skyltar vars budskap varierade var, jämfört med fyra skyltar som hela tiden visade ett och samma budskap. Delvis med laboratoriestudiens resultat som bakgrund, testades i ett fältförsök två skyltar med varierande budskap mot tre skylar som visade en enda symbol. I en tredje studie undersöktes efterlevnaden av några av de skyltar som studerats tidigare. Hastigheten mättes för fordon som passerade en stillastående buss försedd endera med en digital 30-skylt med röd bård, en digital 30-skylt utan bård, en digital skylt med varierande symbol och en traditionell skolskjutsskylt.

  • The European Integrated Tokamak Modelling (ITM) effort: achievements and first physics results

    • A selection of achievements and first physics results are presented of the European Integrated Tokamak Modelling Task Force (EFDA ITM-TF) simulation framework, which aims to provide a standardized platform and an integrated modelling suite of validated numerical codes for the simulation and prediction of a complete plasma discharge of an arbitrary tokamak. The framework developed by the ITM-TF, based on a generic data structure including both simulated and experimental data, allows for the development of sophisticated integrated simulations (workflows) for physics application.The equilibrium reconstruction and linear magnetohydrodynamic (MHD) stability simulation chain was applied, in particular, to the analysis of the edgeMHDstability of ASDEX Upgrade type-I ELMy H-mode discharges and ITER hybrid scenario, demonstrating the stabilizing effect of an increased Shafranov shift on edge modes. Interpretive simulations of a JET hybrid discharge were performed with two electromagnetic turbulence codes within ITM infrastructure showing the signature of trapped-electron assisted ITG turbulence. A successful benchmark among five EC beam/ray-tracing codes was performed in the ITM framework for an ITER inductive scenario for different launching conditions from the equatorial and upper launcher, showing good agreement of the computed absorbed power and driven current. Selected achievements and scientific workflow applications targeting key modelling topics and physics problems are also presented, showing the current status of the ITM-TF modelling suite.

    • The presence of genes encoding organellar proteins in different cellular compartments necessitates a tight coordination of expression by the different genomes of the eukaryotic cell. This coordination of gene expression is achieved by organelle-to-nucleus communication. Stress-induced perturbations of the tetrapyrrole pathway trigger large changes in nuclear gene expression. In order to investigate whether the tetrapyrrole Mg-ProtoIX itself is an important part of plastid-to-nucleus communication, we used an affinity column containing Mg-ProtoIX covalently linked to an Affi-Gel matrix. The proteins that bound to Mg-ProtoIX were analyzed by sodium dodecyl sulfate polyacrylamide gel electrophoresis combined with nano liquid chromatography–mass spectrometry (MS)/MS. Thus, we present a novel proteomic approach to address the mechanisms involved in cellular signaling and we identified interactions between Mg-ProtoIX and a large number of proteins associated with oxidative stress responses. Our approach revealed an interaction between Mg-ProtoIX and the heat shock protein 90-type protein, HSP81-2 suggesting that a regulatory complex including HSP90 proteins and tetrapyrroles controlling gene expression is evolutionarily conserved between yeast and plants. In addition, our list of putative Mg-ProtoIX-binding proteins demonstrated that binding of tetrapyrroles does not depend on a specific amino acid motif but possibly on a specific fold of the protein.

    • The need to develop biological effects monitoring to facilitate a reliable assessment of hazardous substances has been emphasized in the Baltic Sea Action Plan of the Helsinki Commission. An integrated chemical-biological approach is vitally important for the understanding and proper assessment of anthropogenic pressures and their effects on the Baltic Sea. Such an approach is also necessary for prudent management aiming at safeguarding the sustainable use of ecosystem goods and Services. The BEAST project (Biological Effects of Anthropogenic Chemical Stress: Tools for the Assessment of Ecosystem Health) set out to address this topic within the BONUS Programme. BEAST generated a large amount of quality-assured data on several biological effects parameters (biomarkers) in various marine species in different sub-regions of the Baltic Sea. New indicators (biological response measurement methods) and management tools (integrated indices) with regard to the integrated monitoring approach were suggested.

  • Which sign is more visible? : Measuring the visibility of traffic signs through the conspicuity index method

    • Purpose: A new method of measuring visibility taking context into account introduced by Alexander Wertheim has been validated in this study. The purpose was to see if a simple and practical technique could render robust and useful quantitatively measurable results. Methods: An ordinary traffic sign, placed against different backgrounds, was used as a target. Subjects were told to fixate the sign and then slowly deviate with their gaze until they could no longer see the sign in the corner of their eye. This angle was then used as an index for conspicuity. A simple paper and pen method was tested as well as a computer based method. Results: Results for the paper and pen method showed low variance among subjects and a clear correlation between the conspicuity index and intuitive judgments of the visibility of the signs against different backgrounds. For the computer based method the variability was higher but the results still significant between the two groups of dynamic versus static signs. Conclusions: The conclusion is that the conspicuity index method is a simple and useful method that renders quantitative measurements of the visibility of targets taken context into account.

  • Influence of Life Style Factors on Barretts Oesophagus

    • Background. Since the incidence of adenocarcinoma of the oesophagus is rising, the prognosis is poor, and surveillance programs are expensive and mostly cost ineffective, there is a need to increase the knowledge of risk factors in Barretts oesophagus and oesophageal cancer in order to be able to give attention to medical prevention and/or surveillance programs. Aim. To study if there is a correlation between the development of Barretts oesophagus and GOR (gastro oesophageal reflux), family history of GOR, and life style factors, such as alcohol, smoking habits, and mental stress. Methods. Fifty-five consecutively selected patients with Barretts oesophagus (BO) examined at Linkoping University Hospitals Oesophageal Laboratory were matched by sex, age, and duration of reflux symptoms with 55 GOR patients without Barretts oesophagus at the Oesophageal Laboratory. The medical charts in respective groups were examined for comparison of life style factors, mental stress, medication, duration of gastroesophageal acid reflux at 24 hr-pH-metry, and incidence of antireflux surgery and of adenocarcinoma of the oesophagus (ACO). Also, potential gender differences and diagnosis of ACO were studied. Results. Mean percentage reflux time on 24 hr-pH-metry was higher for the Barretts oesophagus group, 18% for women and 17% for men compared to 4% for women and 4% for men in the control group (P less than 0.05). Family history of GOR was more frequent in Barretts oesophagus patients (62%) than in the control group (35%) (P less than 0.05). Male patients with Barretts oesophagus had medical therapy for their GOR symptoms to a higher extent (38%) than male controls (65%) (P less than 0.05). No difference was found in the number of tobacco users or former tobacco users between Barretts oesophagus patients and controls. Barretts oesophagus patients had the same level of alcohol consumption and the same average BMI as the control subjects. Female patients with Barretts oesophagus rated themselves as more mentally stressed (67%) than the female controls (38%) (P less than 0.05). In the five-year medical chart follow-up, five of 55 patients developed adenocarcinoma among the Barretts oesophagus patients, none in the control group. Conclusions. Long reflux time and family clustering of GOR seem to influence the development of Barretts oesophagus. Smoking habits, alcohol consumption and BMI do not seem to have any impact on the development of Barretts oesophagus.

  • Perilous Silences and Counterproductive Narratives Pertaining to HIV/AIDS in the Ugandan, Lesotho and Namibian Press

    • Research on Western mainstream media’s framing of HIV/AIDS in the 1980’s, showed that media narratives influenced audiences’ understanding of the epidemic, as well as society’s responses. Subsequently, by analyzing a society’s mass media and its framing of HIV/AIDS, it is possible to explore what understandings are given preferential treatment in that society, as well as explore what social change those narratives indirectly or directly facilitate. Such an analysis is particularly important in Sub-Saharan Africa, the continent most affected by HIV/AIDS and which has struggled to reverse the course of the epidemic. This dissertation has in five separate articles, not only identified and described media narratives on HIV/AIDS and the closely related topic of same-sex sexuality in three countries hard-hit by the epidemic –Lesotho, Namibia and Uganda – but also discussed the potential effects of persistent silences, as well as narratives that are counterproductive to the countries’ ability to respond to their epidemics. The research uses a combination of quantitative and qualitative approaches: content analysis of independent and government-controlled print media products, semi-structured interviews with media practitioners and representatives from organizations that seek to influence the media agenda, as well as analysis of legislative and policy documents.The articles discuss a range of persistent silences and counterproductive narratives on HIV/AIDS in the three countries. Overall, the media is found to largely fail in providing its readers with narratives that contain many of the particular factors – economic, social, cultural, biological, as well as those related to stigma and discrimination –that fuel their epidemics. The research however also finds differences between the countries and the types of media. In particular privately-owned media is found to play important role in terms of acknowledging the existence of same-sex sexuality as well as relevance in relation to HIV/AIDS prevention and treatment services in Namibian and Ugandan.

  • Variations in the length of stay of intensive care unit nonsurvivors in three scandinavian countries

    • Introduction: The length of stay (LOS) in intensive care unit (ICU) nonsurvivors is not often reported, but represents an important indicator of the use of resources. LOS in ICU nonsurvivors may also be a marker of cultural and organizational differences between units. In this study based on the national intensive care registries in Finland, Sweden, and Norway, we aimed to report intensive care mortality and to document resource use as measured by LOS in ICU nonsurvivors. Methods: Registry data from 53,305 ICU patients in 2006 were merged into a single database. ICU nonsurvivors were analyzed with regard to LOS within subgroups by univariate and multivariate analysis (Cox proportional hazards regression). Results: Vital status at ICU discharge was available for 52,255 patients. Overall ICU mortality was 9.1%. Median LOS of the nonsurvivors was 1.3 days in Finland and Sweden, and 1.9 days in Norway. The shortest LOS of the nonsurvivors was found in patients older than 80 years, emergency medical admissions, and the patients with the highest severity of illness. Multivariate analysis confirmed the longer LOS in Norway when corrected for age group, admission category, sex, and type of hospital. LOS in nonsurvivors was found to be inversely related to the severity of illness, as measured by APACHE II and SAPS II. Conclusions: Despite cultural, religious, and educational similarities, significant variations occur in the LOS of ICU nonsurvivors among Finland, Norway, and Sweden. Overall, ICU mortality is low in the Scandinavian countries.

  • The Experience of Patients With Neoplasm Metastasis in the Spine During a Magnetic Resonance Imaging Examination

    • The aim of the study was to explore the experience of a magnetic resonance imaging (MRI) examination by patients with neoplasm metastasis in the spine. MRI is the most accurate method to diagnose and evaluate suspected metastatic disease in the spine. Patients may experience anxiety because of the fear of pain, fear of the unknown, and the apprehension about what the test might reveal. The study had a qualitative design, and the collected data were analyzed by means of latent content analysis. Twelve semistructured in-depth interviews were carried out starting with the question “Can you tell me about your experience of the MRI examination?” Four themes were identified: “motivation,” “worry and anxiety,” “insecurity,” and “security.” The patients were highly motivated to be examined by MRI, although most of them did experience some degree of worry or anxiety. The level of worry or anxiety was generated by the perception that an MRI examination was unpleasant, uncomfortable, or by the fear of what the result would show. All participants experienced some degree of insecurity, but in different ways, the insecurity of the patient could be reduced and the patients could experience a greater degree of security. The feelings of insecurity or security could be influenced by the radiographer, patients themselves, and MRI equipment. This study shows that most patients usually experience worry and anxiety. If the patients are motivated, they can manage to go through the examination in spite of the previously mentioned adverse feelings. Patients' feelings tend to fluctuate between a sense of insecurity and one of security. © 2014 Association for Radiologic & Imaging Nursing.
