  • APOE influences on neurosychological function after mild head injury : within-person comparisons

    • Objective: To examine the relationship between neuropsychological outcome following mild head injury (MHI) and APOE genotype. Methods: Data from a population-based longitudinal study (n = 3,500) were used to identify 34 adults who experienced MHI during the course of the study. Their pre- and postinjury performances on a battery of nine neuropsychological tests were compared within person, and the postinjury performance was compared with that of age- and gender-matched control subjects. Results: The within-person comparisons showed that participants with at least oneAPOE ε4 allele (n = 11) had a significantly decreased postinjury performance on three of the tests, whereas the postinjury performance for APOE ε4-negative participants (n = 23) was unchanged. There was no significant difference in postinjury performance between participants with/without the ε4 allele, and neither group was impaired relative to controls. Conclusions: APOE genotype may influence the outcome following an MHI. Pre/postinjury within-person comparisons seem more sensitive than control group comparisons for detecting injury-related effects.

  • Det hotade kollektivet : Neolitiseringsprocessen ur ett östmellansvenskt perspektiv

    • This dissertation questions the established view on the social process in connection with Neolithisation. Therefore it has been necessary to discuss more general principles, on how to understand this process of change, when iterant hunter-gatherers became settled farmers. The dissertation begins with a theoretical discussion on the foundations for the interpretation and most important a discussion on differences between the interpretations presented and the treatment of the empirical material on which they are based. A necessary point of departure for understanding the implications an incorporation of domesticates in prehistoric society, i.e. emergence of Funnel Beaker Culture in Sweden, is a principal discussion, social mechanisms of hunters-gatherers on one hand and peoples reactions to changes threatening the social ideology, on the other. Four case studies are used to discuss people’s reaction to change. They clearly indicate that the reaction involved the lifting up of blurred and semiconscious structures to a conscious, ideological level. Inherent in this awareness process has been an active use of material culture, both in the production of symbols and in communication. The most important reason for stating that the Neolithisation must have meant a clear break with the earlier existence is the abandonment of itinerant way of life, both geographically and socially. This abandonment resulted in reactionary process involving material culture. Thus the material culture of the Funnel-Beaker period can be perceived as instruments of reproduction of a historically well-anchored egalitarian ideology. In the dispersed settlement system of autonomous individual farmsteads the collective aggregation sites are given a focal role of the discussion on social reproduction. The social mechanisms of the Early Neolithic society of Eastern Central Sweden are investigated on a local settlement level by an analysis of the production of locally available raw material. This study involve a petrological investigation showed a system of local management in relation to raw material extraction, production and consumption. This system is considered as one way of upholding the social ideology historically situated in the life style of hunters and gatherers.

  • A chip for linearization of RF power amplifiers using predistortion based on a bit-parallel complex multiplier

    • This paper presents a custom chip for linearization of RF power amplifiers using digital predistortion. The chip has been implemented in a standard digital 0.8 mu m CMOS process with standard static cells and single-phase clocking. A systolic complex multiplier based on distributed arithmetic constitutes the core of the chip. The nonlinear function is realized with a look-up table containing complex gain factors applied to the complex multiplier. Maximum clock frequency was found by means of simulation to be 105 MHz corresponding to 21 Msamples/s throughput with 3 W power consumption using 5 V supply voltage. The fabricated chip is fully functional and has been measured up to 60 MHz clock frequency with 825 mW power consumption with 3.3 V supply voltage. Operation at 1.5 V supply voltage allows 10 MHz clock frequency with 35 mW power consumption.

    • Alzheimer's disease (AD) is a primary degenerative disease of the brain. The prevalence increases with age, with devastating consequences for the individual and society. The aim of this study was to evaluate whether patients with early AD show an altered regional cerebral blood flow (rCBF) compared with control persons. Furthermore, we aimed to investigate the correlation between rCBF in sublobar volumes of the brain and performance on memory tests. Memory tests were chosen to evaluate episodic and semantic memory. Fourteen patients (aged 75.2+/-8.8 years) with early AD and 15 control persons (aged 71.4+/-3.2 years) were included. rCBF measurements with single-photon emission tomography (SPET) using technetium-99m hexamethylpropylene amine oxime (HMPAO) were performed. The rCBF (99m)Tc-HMPAO SPET images were spatially transformed to fit a brain atlas and normalised for differences in rCBF (Computerised Brain Atlas software). Cortical and subcortical volumes of interest (VOIs) were analysed and compared. Compared with the controls, AD patients showed a significantly lower rCBF ratio in temporoparietal regions, including the left hippocampus. The diagnostic sensitivity and specificity for AD were high in temporoparietal regions. AD patients had significantly reduced performance on semantic and, in particular, episodic memory tests compared with age-matched normative data, and their performance on several episodic tests correlated with rCBF ratios in parietal and temporal regions, including the left hippocampus. The correlation between rCBF ratio and level of episodic memory performance suggests that abnormalities in rCBF pattern underlie impaired episodic memory functioning in AD.

    • BACKGROUND: The stroke volume to pulse pressure ratio (SV/PP), a measure of total arterial compliance, predicts adverse cardiovascular events in hypertensive subjects. The relations to cardiovascular risk factors and its predictive capacity in the general population are not known. METHOD AND RESULTS: In 1970-73, all 50-year-old men living in Uppsala County, Sweden, were invited to a health survey assessing cardiovascular risk factors. At a reinvestigation 20 years later, 470 subjects underwent an echocardiographic examination, hyperinsulinaemic euglycaemic clamp, oral glucose tolerance test and measurements of blood pressure and lipids. They were thereafter followed for a median of 7.2 years. Serum triglycerides and post-load glucose and insulin levels at age 50 were predictors of SV/PP ratio measured 20 years later (P < 0.05-0.001). At age 70, SV/PP was related to serum non-esterified fatty acids, post-load glucose and insulin levels and insulin sensitivity (P < 0.05-0.001). SV/PP was reduced in subjects with concentric left ventricular hypertrophy (LVH, P < 0.01), and in subjects with a low E-wave to A-wave (E/A) ratio (P < 0.001). The SV/PP ratio predicted mortality from coronary heart disease [hazard ratio 0.54, 95% confidence interval 0.30-0.97 for a one standard deviation (1SD) increase in ln(SV/PP)] independently of left ventricular mass and other major cardiovascular risk factors. Pulse pressure or total peripheral resistance were not significant predictors for future mortality from coronary heart disease. CONCLUSION: The SV/PP ratio was related to main components of the insulin resistance syndrome, concentric LVH and a low E/A ratio. Furthermore, the SV/PP ratio was an independent predictor of mortality from coronary heart disease in a community-based sample of men aged 70.

    • Adaptive predistortion is one of the most promising methods to linearize RF power amplifiers (PAs). Its linearity performance is, however, limited by various memory effects existing in the system. In this paper, we study the effects of anti-aliasing filters in the feedback path of adaptive predistortion. Different filter approximations are examined with a class AB power amplifier by means of simulations. Of the filters studied, Butterworth filters perform better than Chebyshev filters when they have the same noise-equivalent bandwidth (NEB). Simulation results also show that the cut-off frequency of the anti-aliasing filter affects both the convergence speed of the adaptation algorithm and the level of mean square error (MSE) between the desired and the feedback signals. To a certain degree, the higher the cut-off frequency of the filter, the less the number of iterations needed by the adaptation process

    • The LINC transmitter is an architecture that provides linear amplification using nonlinear but power efficient amplifiers. One crucial signal processing function of LINC is the signal component separator (SCS) which forms two constant-amplitude phase-modulated signals from the source signal. This paper presents an analog SCS chip implemented in a 0.35 mum CMOS process using a novel design based on the so-called voltage-translinear circuit principle. An experimental LINC transmitter was built with the SCS chip, nonlinear amplifiers and a power combiner. Test results showed that all output spurious levels some -55 dBc and -48 dBc could be obtained with a North American Digital Cellular (NADC) signal and an IS-95 signal, respectively. This implies a high degree of linearity.

A simple approximate formula is derived, linking the instantaneous frequency of the CALLUM signals to the modulation depth of the information signal. Further, the effect of the frequency mismatch between the VCOs present in the CALLUM architecture is examined, and the need for frequency synchronization via a phase-locked loop is discussed.

    • This article presents an analog implementation of the signal component generator (SCG) of the CALLUM2 linear transmitter architecture. The proposed SCG is suited for integration in a standard 0.35 μm CMOS process, and has from simulations proven to be adequate when operating on an EDGE modulated baseband signal with a data rate of 270.833 ksymb/s. The total current consumption of the SCG is 2.0 mA from a 3.3 V supply. A variable-gain amplifier (VGA) with common-mode (CM) control is presented, and the VGA is inserted in between the SCG and the voltage-controlled oscillator (VCO) to adjust the loop gain, which has strong influence on the stability and spectral performance of the linear transmitter architecture. © 2004 IEEE.

    • BACKGROUND:-Left ventricular hypertrophy (LVH) is a common risk factor for cardiovascular mortality. Causes other than hypertension have not previously been investigated longitudinally. The aim of the present study was to determine hemodynamic, metabolic, and psychosocial predictors at 50 years of age for the prevalence of echocardiographic LVH and geometric subtypes at age 70 by use of a large sample of men from the general population followed up for 20 years. Methods and Results-In 1970 to 1973, all men born from 1920 to 1924 and residing in Uppsala County, Sweden, were invited to participate in a health survey aimed at identifying risk factors for cardiovascular disease. At a reinvestigation 20 years later, echocardiographic left ventricular mass index was determined in 475 subjects. A 1-SD increase in body mass index, systolic or diastolic blood pressure, fasting LDL/HDL cholesterol, serum triglycerides, or the serum cholesterol ester proportion of several saturated fatty acids or oleic acid at age 50 significantly increased the odds of having LVH at age 70 by 27% to 41%, whereas an increase in linoleic acid proportion was protective. Almost all metabolic predictors were independent of ischemic heart disease, valvular disease, and use of antihypertensive medication at age 70. CONCLUSIONS:-Dyslipidemia and indices of a low dietary intake of linoleic acid and high intake of saturated and monounsaturated fats, as well as hypertension and obesity, at age 50 predicted the prevalence of LVH 20 years later in this prospective longitudinal cohort study, thereby suggesting that lipids may be important in the origin of LVH.

    • BACKGROUND:The increased risk associated with left ventricular hypertrophy (LVH) diagnosed echocardiographically (Echo-LVH) or electrocardiographically (ECG-LVH) is well known, but the clinically relevant question of how much additional prognostic information would be provided by echocardiographically assessing LVH if a subject's ECG-LVH and hypertension status are known has not been addressed. Methods and RESULTS:We investigated whether Echo-LVH and ECG-LVH predicted total and cardiovascular mortality and morbidity independently of each other and of other cardiovascular risk factors by using a population-based sample of 475 men investigated at age 70 with a median follow-up time of 5.2 years. Echocardiographic left ventricular mass index (LVMI) predicted total mortality (hazards ratio [HR] 1.44, 95% CI 1.09 to 1.92, for a 1-SD increase in LVMI) and cardiovascular mortality (HR 2.38, 95% CI 1.52 to 3.73) independently of ECG-LVH and other cardiovascular risk factors. ECG-LVH, defined as Cornell product >244 microV. s, predicted total mortality (HR 2.89, 95% CI 1.41 to 5.96) independently of LVMI and other cardiovascular risk factors. Thus, Echo-LVH and ECG-LVH provided complementary prognostic information, especially in hypertensive subjects. CONCLUSIONS:Echo-LVH and ECG-LVH predict mortality independently of each other and of other cardiovascular risk factors, implying that Echo-LVH and ECG-LVH in part carry different prognostic information. Therefore, to fully assess the increased risk associated with these conditions, both ECG and echocardiography should be performed.

    • BACKGROUND: Associations between left ventricular (LV) geometry and the insulin resistance syndrome have been found, mostly in small studies of middle-aged hypertensives. The purpose of this study was to elucidate these associations through the use of a large sample of elderly men. METHODS AND RESULTS: We investigated 475 men (157 hypertensives) 71 years of age who were attending a population-based health survey in Uppsala County with echocardiography, oral glucose tolerance test (OGTT), hyperinsulinemic euglycemic clamp, and lipid and 24-hour ambulatory blood pressure monitoring. LV relative wall thickness was significantly related to clamp insulin sensitivity index (r=-0.14), fasting insulin, 32-33 split proinsulin, triglycerides, nonesterified fatty acids, OGTT glucose and insulin levels, waist-to-hip ratio, body mass index, 24-hour blood pressure, and heart rate (r=0.10 to 0.22). Only 24-hour systolic pressure (r=0. 15), OGTT 2-hour insulin (r=-0.10), and heart rate (r=-0.14) were significantly related to LV mass index. Comparing subjects with various LV geometry (normal, concentric remodeling and concentric and eccentric hypertrophy) showed that 24-hour heart rate, OGTT glucose and insulin levels, waist-to-hip ratio, and body mass index were significantly higher (P<0.001 to 0.05) and clamp insulin sensitivity index was significantly lower (P<0.01) in the concentric remodeling geometry group than in the normal LV geometry group. The 24-hour blood pressure was significantly higher in the concentric hypertrophy group than in the normal LV geometry group (P<0.001). CONCLUSIONS: Several components of the insulin resistance syndrome were related to thick LV walls and concentric remodeling but less to LV hypertrophy in this population-based sample of elderly men.

    • Whole-body insulin sensitivity has been shown to be impaired in subjects with increased left ventricular relative wall thickness (RWT) and in hypertensive subjects with left ventricular hypertrophy, but the relation between myocardial insulin sensitivity and RWT or left ventricular mass index (LVMI) in normotension is not known. We measured myocardial and skeletal muscle glucose uptake with [18F]fluorodeoxyglucose and positron emission tomography during hyperinsulinemic euglycemic clamp in nine men with wide ranges of echocardiographic RWT and LVMI. The subjects were male, 72-74 years old, normotensive and free from medication or history of heart disease. RWT correlated inversely with skeletal muscle glucose uptake (r = -0.69, p = 0.04), borderline significantly directly with myocardial glucose uptake (r = 0.62, p = 0.07), and directly with the ratio between myocardial and skeletal muscle glucose uptake (r = 0.77, p = 0.02) during hyperinsulinemic euglycemic clamp. LVMI was not related to insulin-mediated myocardial or skeletal muscle glucose uptake or the ratio between myocardial and skeletal muscle glucose uptake. In conclusion, RWT was inversely related to insulin sensitivity in skeletal muscle and borderline significantly directly related to insulin sensitivity in the myocardium in healthy normotensive elderly men, whereas LVMI was not related to myocardial or skeletal muscle insulin sensitivity.

    • The Department of Human Work Sciences at Lulea University of Technology, Sweden, has, since the autumn of 1991, been conducting a research project that offers a free advisory service regarding matters of the working environment to companies with fewer than 50 employees. The model used has offered the companies 16 hours' free advisory service and an additional 75 hours at half price. The project is financed by the Swedish Council for Work Life Research. This service is offered to all types of small company - both those that produce goods and those that produce services. A total of 148 small companies have contacted the service. At present the improvement process in these companies is being evaluated. The material presented in this paper represents only a part of the evaluation. In the evaluation the following questions are considered: why is the company interested in issues concerning the working environment? what advantages does the company derive from an improved working environment? does an improved working environment, with the introduction of new technology, result in any financial gain.

  • Polymorphism in the P2X7 receptor gene and survival in chronic lymphocytic leukaemia

    • BACKGROUND: The P2X7 receptor is a ligand-gated cation channel that mediates ATP-induced apoptotic death in haemopoietic and chronic lymphocytic leukaemia (CLL) cells. We aimed to investigate the clinical effect of a single nucleotide polymorphism in the P2X7 receptor gene (1513A-->C) and correlate findings with the immunoglobulin heavy chain variable (V(H)) gene mutation status, a prognostic marker in CLL. METHODS: We investigated tumour DNA in 170 patients with CLL using PCR-RFLP analysis with HhaI restriction enzyme cleavage to screen for the polymorphism in the P2X7 receptor gene. The VH gene mutation status was assessed in 165 patients by PCR amplification and nucleotide sequencing. We correlated the findings of the P2X7 receptor genotype with the V(H) gene mutation data and overall survival and screened for the P2X7 receptor polymorphism in 200 healthy controls. FINDINGS: Of the 170 patients, 35 (21%) were heterozygous and one (1%) homozygous for the 1513C allele; whereas 134 (79%) had the 1513A/A genotype of the P2X7 receptor gene. Overall survival was significantly longer for patients with CLL heterozygous for the 1513C allele than those with the 1513A/A genotype. Median survival for heterozygous patients was 104 months (range 33-467) and 72 months (1-190) for homozygous patients (p=0.009). Of the 165 patients with CLL in whom we assessed the V(H) gene mutation status, the V(H) genes were mutated in 71 (43%) patients and unmutated in 94; 18 (25%) of the 71 patients with mutated genes had 1513C allele compared with 17 (18%) of 94 who had unmutated genes. In patients with mutated VH genes, those with CLL who were 1513C positive had 53 months' longer median survival than did those with the 1513A/A genotype (151 vs 98 months, p=0.011). INTERPRETATION: The P2X7 polymorphism could affect clinical outcome in CLL, especially in patients with mutated V(H) genes. Studies are necessary to elucidate the biological role of the P2X7 polymorphism in CLL in vivo.

    • Most previous studies of associations between insulin sensitivity and common carotid artery (CCA) atherosclerosis have been conducted in small samples, have not used direct measurement of insulin sensitivity, and have yielded inconclusive results. We investigated associations of CCA intima-media thickness (IMT) and diameter (CCA-D) measured by B-mode ultrasound and insulin sensitivity measured by the euglycemic hyperinsulinemic clamp test together with risk factors of the insulin resistance syndrome in a community-based sample of 493 elderly men. The clamp glucose disposal rate was an independent predictor of CCA-IMT in multivariate models adjusting for blood pressure, smoking, serum cholesterol, and body mass index (1% decrease in CCA-IMT for a 1 unit increase in glucose disposal rate, P=0.009). Glucose disposal rate was significantly related to CCA-D in univariate (r=-0.11, P=0.02) but not in multivariate models. In conclusion, this study is the first to establish impaired insulin sensitivity, measured by the euglycemic hyperinsulinemic clamp test, as an independent predictor of CCA-IMT in a population-based sample of elderly men.
