  • Natriuretic Peptide Biomarkers as Information Indicators in Elderly Patients With Possible Heart Failure Followed Over Six Years : A Head-to-Head Comparison of Four Cardiac Natriuretic Peptides

    • Background: Little is known about the differences between natriuretic peptides used to evaluate elderly patients with heart failure. The aim of the study was to evaluate the information and the power to predict cardiovascular mortality derived from an analysis of cardiac natriuretic peptides from the same study population and at the same time. Methods and Results: In all, 415 elderly patients (age 65-82 years) in primary health care were evaluated and followed for 6 years. All patients had symptoms of heart failure and were examined by a cardiologist. An electrocardiogram and chest x-rays were taken, and the systolic and diastolic functions were assessed using Doppler echocardiography. Brain natriuretic peptide (BNP), N-terminal proBNP, atrial natriuretic peptide (ANP), and N-terminal proANP were analyzed. All 4 peptides were associated with age, and only 1 of them showed any gender difference. Three of the 4 peptides (not ANP) provided important information for identifying patients with impaired systolic function and diastolic dysfunction (pseudonormal or restrictive filling pattern), and for assessing the risk of cardiovascular death. Conclusions: Cardiac natriuretic peptides are useful tools for evaluating elderly patients with heart failure. Three of the 4 peptides were very similar. ANP exhibits inferior properties and cannot be recommended in clinical practice. © 2007 Elsevier Inc. All rights reserved.

  • Design Space Exploration of Wireless Multihop Networks

    • This thesis explores the feasible design space of wireless multihop networks and identifies fundamental design parameters. In the process of exploring it is important to ignore all details and instead take a holistic view. This means that all protocol details are overseen, all details of radio wave propagation models are overseen and the system is modelled strictly on an architectural level. From a theoretical information perspective, there is a limit to the capacity that a certain bandwidth and a certain signal-to-noise ratio at the receiver can provide. This limit is approximated as a volume in the time-frequency-space domain. A single transmission is represented as an occupied volume in this domain. A wireless multihop network covers a spatial area, and the question is how multiple numbers of transmission volumes can be fit into a given limited spatial area. This volume fitting should be done in order to maximize the overall performance or to trade available resources to favour a specific characteristic in the wireless multihop network. The volume model is used for the design space exploration of a wireless multihop network. It is argued that the fault tolerance and the energy gain achieved in a multihop topology are its strength as compared to a single-hop architecture. It is further shown that the energy gain is achieved at the expense of delay and a greater end-to-end error probability. This indicates that these parameters must be very carefully balanced in order to gain in the global overall performance perspective. It can further be concluded that the overall spatial capacity is increased as a result of the spatial channel reuse in a multihop topology. On the other hand, it is also shown that the multihop topology introduces a rather stringent geometrical capacity limitation when the number of nodes of a wireless multihop network is increased. The dynamics (e.g. node mobility, changing radio channels etc.) of a large scale wireless multihop network is also a limiting factor. The nodes’ mobility creates a knowledge horizon beyond which very little can be known about the present network topology.

  • The use of clustered wireless multihop networks in industrial settings

    • This paper suggests a cluster collision avoidance mechanism and a dual transceiver architecture to be used in a clustered wireless multihop network. These two contributions make the clustered wireless multihop network the preferred architecture for future industrial wireless networks. The wireless multihop cluster consists of one master and several slaves, where some of the slaves will act as gateways between different clusters. Frequency hopping spread spectrum is used on a cluster level and to avoid frequency collisions between clusters a "neighbor cluster collision avoidance mechanism" is proposed and evaluated through simulations. To break up the dependence between the clusters, introduced by the gateway nodes, each node is equipped with two transceivers. The paper is concluded with a suggestion to use a clustered wireless multihop network with orthogonal hopping sequences for an industrial setting.

    • In this paper some well established wireless technologies are merged into a new concept solution for a future industrial wireless mesh network. The suggested clustered wireless mesh network can handle probabilistic quality of service guarantees and is based on a dual-radio node architecture using synchronized frequency hopping spread spectrum Bluetooth radios. The proposed architecture gives a heuristic solution to the inter-cluster scheduling problem of gateway nodes in clustered architectures and breaks up the dependence between the local medium access schedules of adjacent clusters. The dual-radio feature also enables higher network connectivity, implying, for example, that a higher link redundancy can be achieved.

    • There is a need for a more timely agenda for research on teaching and learning, and the use information technology in higher education. It is here claimed that many of the prior research issues within this area are obsolete. Information technology infrastructure previously provided solely by the institutions of higher education is now successively replaced by the students own technology which they bring to the educational context. The new agenda should orient towards (i) integration of students own technology in educational activities, and integration between on and of campus learning activities (ii) tools for supporting activities in classrooms such as presenting, visualizing and simulating, and (iii) issues of availability to participate in alternative ways, making educational activities available to new groups. The suggested agenda is discussed using a number of anecdotes collected in various educational settings.

    • The aim of the study was to examine the changes in attitudes towards mental illness after theorethical education and clinical placement among students from university programmes preparing for different kinds of health professions. Three different questionnaries were used, measuring the level of familaritiy with mental illness and attitudes towards mental illness in general and toward specific mental illnesses. The data were collected on two occasions, before the theorethical course and after the completed clinical placement. The result showed that the attitudes toward mental illnes in general had changed in a less stigmatising direction after the clinical placement. On the other hand, attitudes toward specific illnesses did not show any major changes.

  • Effect of Temperature Gradient and Sulphur Dioxide Addition on Erosion-corrosion of Iron- and Nickel-based Alloys

    • In this study, high temperature erosion-corrosion with trace amounts of added SO(2) and the role of isothermal or thermal gradient conditions are studied. The studies are focused on 304L as the model material, but results for other alloys (Alloy 625 and 9Cr1Mo-steel) are also included to support the results and trends observed. The experiments include 550 degrees C exposures in the erosion-corrosion rig under isothermal and thermal gradient conditions. The internal cooling used to create the thermal gradient meant that the 550 degrees C material temperature could be maintained, despite of the higher bed temperature of 750 degrees C. Laboratory oxidation experiments in a similar environment are also included as reference tests. The evaluation of material degradation was done by using a combination of different techniques such as scanning electron microscopy (SEM) including X-ray microanalyses, Auger and Xray electron spectroscopy. The results demonstrate three important aspects related to erosion-corrosion attack in fluidized bed combustion. First, provided oxide growth is rapid enough leading to thickness in the micrometer range, low material wastage is obtained as demonstrated for the 9Cr1Mo - steel and also for the 304L and Alloy 625 materials when exposed to air plus 50ppm added SO(2) in isothermal conditions. Because of the large catalytic surface in the test rig for the thermodynamically favored oxidation of SO(2) in air, significant presence of SO(3) is expected. The material wastage is then also associated with a shift in erosion pattern with maximum material wastage at the impact of erodent (called type B). Detailed microstructure investigation of oxide in question on the 304L showed that the inner part of the oxide then is dense and possibly isolated from the environment and also Cr-rich. Adding a temperature gradient then returns the wastage pattern to type A for both 304L and Alloy 625 and leads to increased wastage for the former material. The resulting oxide thickness is also smaller and always less than 0.2 mu m. The laboratory reference tests showed a similar range of average oxide thickness for air and air plus SO(2). However, in air plus 50ppm SO(2) the tendency for break away corrosion on 304L was less than for air exposed samples.

    • The use of radio frequency identification systems (RFID) is growing rapidly. Today, mostly "passive" RFID systems are used because no onboard energy source is needed on the transponders. However, "active " RFID with onboard power source gives a new range of opportunities not possible with passive systems. To obtain energy efficiency in an active RFID system a protocol should be designed that is optimized with energy in mind. This paper describes the on-going work of defining and evaluating such a protocol. The protocol's performance in terms of energy efficiency, aggregated throughput, delay, and number of air collisions is evaluated and compared to that of the medium-access layer in 802.15.4 Zigbee, and also to a commercially available protocol from Free2move.

    • The Wilms' tumour gene 1 (WT1) protein is highly expressed in most leukaemias. Co-expression of WT1 and the fusion protein AML1-ETO in mice rapidly induces acute myeloid leukaemia (AML). Mechanisms behind expression of WT1, as well as consequences thereof, are still unclear. Here, we report that the fusion protein BCR/ABL1 increases expression of WT1 mRNA and protein via the phosphatidylinositol-3 kinase (PI3K)-Akt pathway. Inhibition of BCR/ABL1 or PI3K activity strongly suppressed transcription from WT1 promoter/enhancer reporters. Forced expression of BCR/ABL1 in normal human progenitor CD34+ cells increased WT1 mRNA and protein, further supporting the notion of BCR/ABL1-driven expression of WT1 in human haematopoietic cells. Forced expression of WT1 in K562 cells provided protection against cytotoxic effects of the ABL1 tyrosine kinase inhibitor imatinib, as judged by effects on viability measured by trypan blue exclusion, metabolic activity, annexin V and DAPI (4', 6-diamidino-2-phenylindole) staining. None of the isoforms provided any detectable protection against apoptosis induced by arsenic trioxide and only very weak protection against etoposide, indicating that WT1 interferes with specific apoptotic signalling pathways. Our data demonstrate that WT1 expression is induced by oncogenic signalling from BCR/ABL1 and that WT1 contributes to resistance against apoptosis induced by imatinib.

    • The Wilms' tumor gene 1 (WT1) protein is a transcriptional regulator that is highly expressed in immature hematopoietic progenitor cells and in the majority of patients with acute and chronic myeloid leukemia. However, it is still unclear how WT1 exerts its function(s) in hematopoietic cells. The aim of this work was to investigate the function of WT1 as a transcription factor in human hematopoietic progenitor cells. To this end, an oligonucleotide array approach was used to study the gene expression in CD34(+) cells from human cord blood retrovirally transduced with WT1 or a control vector. We found that the expression of the putative tumor suppressor gene N-myc downstream regulated gene 2 (NDRG2) mRNA was induced by WT1 in CD34(+) cells and also in leukemic U937 cells. Furthermore, a novel transcription start site in the NDRG2 gene was identified in WT1-transduced cells, in addition to two previously reported transcription start sites. These results show that the expression of the NDRG2 gene is directly or indirectly induced by WT1, and provide the first insights into transcriptional regulation of the NDRG2 gene, including demonstration of a novel splice variant.
