Patterns and resectability of colorectal cancer recurrences: outcome study within the COLOFOL trial

- BACKGROUND: Improvements in surgery, imaging, adjuvant treatment, and management of metastatic disease have led to modification of previous approaches regarding the risk of recurrence and prognosis in colorectal cancer. The aims of this study were to map patterns, risk factors, and the possibility of curative treatment of recurrent colorectal cancer in a multimodal setting. METHODS: This was a cohort study based on the COLOFOL trial population of patients who underwent radical resection of stage II or III colorectal cancer. The medical files of all patients with recurrence within 5years after resection of the primary tumour were scrutinized. Follow-up time was 5years after the first recurrence. Primary endpoints were cumulative incidence, site, timing, and risk factors for recurrence, and rate of potentially curative treatment. A secondary endpoint was survival. RESULTS: Of 2442 patients, 471 developed recurrences. The 5-year cumulative incidence was 21.4 (95 per cent c.i. 19.5 to 23.3) per cent. The median time to detection was 1.1years after surgery and 87.3 per cent were detected within 3years. Some 98.2 per cent of patients who had potentially curative treatment were assessed by a multidisciplinary tumour board. A total of 47.8 per cent of the recurrences were potentially curatively treated. The 5-year overall survival rate after detection was 32.0 (95 per cent c.i. 27.9 to 36.3) per cent for all patients with recurrence, 58.6 (51.9 to 64.7) per cent in the potentially curatively treated group and 7.7 (4.8 to 11.5) per cent in the palliatively treated group. CONCLUSION: Time to recurrence was similar to previous results, whereas the 21.4 per cent risk of recurrence was somewhat lower. The high proportion of patients who received potentially curative treatment, linked to a 5-year overall survival rate of 58.6 per cent, indicates that it is possible to achieve good results in recurrent colorectal cancer following multidisciplinary assessment.

Inside the Ideal Home : The Changing Values of Apartment Living and the Promotion of Consumption in Sweden, 1950-1970

- This dissertation explores the image of the ideal home in Sweden, an idea closely bound to the spatial dynamics of building norms and the outfitting of the domestic household, from the 1950s through the 1960s. By examining official, commercial, and consumer-cooperative ideals of housing and home, I attempt to understand and analyze correlations between various visions of the ideal apartment home and the social, political, economic, and cultural contexts in which they were conceived during the heyday of the Swedish welfare state. This subject has been extensively researched for the first years of the phenomenon, the 1930s and 1940s, but is relatively ignored for the years following World War II, when the new housing policy was formulated and the Record Years began. The study ends in 1970, when the construction of apartments and the standard of living peaked, followed by a focus on building single-family houses and the questioning of building norms. I argue that the values and visions of an ideal home were expressed in the rhetoric and representations of state, consumer-cooperative, and commercial publications. While scholars have studied individual aspects of this context, I maintain that the interrelationship between them produced a widely circulated vision of domesticity between 1950 and 1970. I highlight how commercial actors—primarily Ikea and the interior design magazine Allt i Hemmet (Everything in the Home)—interacted with state institutions in creating and promoting a discourse of the ideal home.

- For years, scientists from various disciplines have studied the effects of endocrine disrupting chemicals (EDCs) on the health and wellbeing of humans and wildlife. Some studies have specifically focused on the effects of low doses, i.e. those in the range that are thought to be safe for humans and/or animals. Others have focused on the existence of non-monotonic dose-response curves. These concepts challenge the way that chemical risk assessment is performed for EDCs. Continued discussions have clarified exactly what controversies and challenges remain. We address several of these issues, including why the study and regulation of EDCs should incorporate endocrine principles; what level of consensus there is for low dose effects; challenges to our understanding of non-monotonicity; and whether EDCs have been demonstrated to produce adverse effects. This discussion should result in a better understanding of these issues, and allow for additional dialog on their impact on risk assessment.

In the past 10years, the research community has produced a significant number of design notations to represent security properties and concepts in a design artifact. These notations are aimed at documenting and analyzing security in a software design model. The fragmentation of the research space, however, has resulted in a complex tangle of different techniques. Hence, practitioners are confronted with the challenging task of scouting the right approach from a multitude of proposals. Similarly, it is hard for researchers to keep track of the synergies among the existing notations, in order to identify the existing opportunities for original contributions. This paper presents a systematic literature review that inventorizes the existing notations and provides an in-depth, comparative analysis for each.
