  • A control oriented strategy of disruption prediction to avoid the configuration collapse of tokamak reactors

    • The objective of thermonuclear fusion consists of producing electricity from the coalescence of light nuclei in high temperature plasmas. The most promising route to fusion envisages the confinement of such plasmas with magnetic fields, whose most studied configuration is the tokamak. Disruptions are catastrophic collapses affecting all tokamak devices and one of the main potential showstoppers on the route to a commercial reactor. In this work we report how, deploying innovative analysis methods on thousands of JET experiments covering the isotopic compositions from hydrogen to full tritium and including the major D-T campaign, the nature of the various forms of collapse is investigated in all phases of the discharges. An original approach to proximity detection has been developed, which allows determining both the probability of and the time interval remaining before an incoming disruption, with adaptive, from scratch, real time compatible techniques. The results indicate that physics based prediction and control tools can be developed, to deploy realistic strategies of disruption avoidance and prevention, meeting the requirements of the next generation of devices.

  • A Standard Set of Value-Based Patient-Centered Outcomes and Measures of Overall Health in Adults

    • Background The definition of population-specific outcomes is an essential precondition for the implementation of value-based health care. We developed a minimum standard outcome set for overall adult health (OAH) to facilitate the implementation of value-based health care in tracking, comparing, and improving overall health care outcomes of adults across multiple conditions, which would be of particular relevance for primary care and public health populations. Methods The International Consortium for Health Outcomes Measurement (ICHOM) convened an international panel (patients, clinicians, and topic experts). Following the development of a conceptual framework, a modified Delphi method (supported by public consultations) was implemented to identify, in sequence, the relevant domains, the best instruments for measuring them, the timing of measurement, and the relevant adjustment variables. Findings Outcomes were identified in relation to overall health status and the domains of physical, mental, and social health. Three instruments covering these domains were identified: PROMIS Scale v1.2-Global Health (10 items), WHO Wellbeing Index (5 items), and the WHO Disability Assessment Schedule 2.0 (12 items). Case-mix variables included a range of sociodemographic and biometric measures. Yearly measurement was proposed for all outcomes and most case-mix variables. Interpretation The ICHOM OAH Standard Set has been developed through consensus-based methods based on predefined criteria following high standards for the identification and selection of high-quality measures The involvements of a wide range of stakeholders supports the acceptability of the set, which is readily available for use and feasibility testing in clinical settings.

  • The prediction of crack propagation in coarse grain RR1000 using a unified modelling approach

    • The polycrystalline nickel-base superalloy RR1000 is used as turbine rotor material in Rolls-Royce aero engines and has to withstand a wide variety of load and temperature changes during operation. In order to maximize the potential of the material and to improve component design, it is of great interest to understand, and subsequently be able to accurately model the crack propagation caused by thermo-mechanical fatigue conditions. In this work, experimental data is analysed and used to inform unified modelling approaches in order to predict the crack propagation behaviour of RR1000 under a variety of stress-controlled thermo-mechanical fatigue conditions.

    • Cyclic hysteresis loops generated during high-temperature, isothermal, low-cycle fatigue testing of the nickel-based superalloy RR1000 revealed a gradual decrease in elastic modulus of up to 25 % compared to the initial value in dependence of the accumulated plastic strain. Based on the experimental observations, a material model was developed and implemented within a finite element solver to investigate the effect of degrading stiffness on crack growth laws and crack growth predictions. It was shown that the stress intensity factor diminished by 6 to 8 % and crack growth rates were 14 to 20 % higher when stiffness degradation was accounted for.

  • Global, regional, and cryptic population structure in a high gene-flow transatlantic fish

    • Lumpfish (Cyclopterus lumpus) is a transatlantic marine fish displaying large population sizes and a high potential for dispersal and gene-flow. These features are expected to result in weak population structure. Here, we investigated population genetic structure of lumpfish throughout its natural distribution in the North Atlantic using two approaches: I) 4,393 genome wide SNPs and 95 individuals from 10 locations, and II) 139 discriminatory SNPs and 1,669 individuals from 40 locations. Both approaches identified extensive population genetic structuring with a major split between the East and West Atlantic and a distinct Baltic Sea population, as well as further differentiation of lumpfish from the English Channel, Iceland, and Greenland. The discriminatory loci displayed similar to 2-5 times higher divergence than the genome wide approach, revealing further evidence of local population substructures. Lumpfish from Isfjorden in Svalbard were highly distinct but resembled most fish from Greenland. The Kattegat area in the Baltic transition zone, formed a previously undescribed distinct genetic group. Also, further subdivision was detected within North America, Iceland, West Greenland, Barents Sea, and Norway. Although lumpfish have considerable potential for dispersal and gene-flow, the observed high levels of population structuring throughout the Atlantic suggests that this species may have a natal homing behavior and local populations with adaptive differences. This fine-scale population structure calls for consideration when defining management units for exploitation of lumpfish stocks and in decisions related to sourcing and moving lumpfish for cleaner fish use in salmonid aquaculture.

    • The current paper describes TMF crack growth behaviour in an advanced nickel-based superalloy. Changes in behaviour are examined which occur as a function of the phase angle between applied stress and temperature. The fractography of the failed specimens reveals changes from transgranular to intergranular growth between high and low phase angle tests as a result of the onset of high temperature damage mechanisms. More targeted testing has also been undertaken to isolate the contributions of these mechanisms, with specific transitions in behaviour becoming clear in 90 degrees diamond cycles, where dynamic crack growth and oxidation strongly interact.

    • Background Globally, experiences of menarche and subsequent menstruation are embedded in social and cultural beliefs, norms and practices. Menarche is an important developmental milestone in sexual and reproductive health (SRH) for females. Menarche is intertwined with socio-cultural norms, beliefs and practices, which can impact on women's ability to manage menstruation with dignity. This paper reviews the social and cultural factors that affect women's ability to effectively manage their menstrual health and hygiene (MHH) in Pacific Island Countries and Territories (PICTs). Methods A scoping review was conducted following PRISMA scoping review guidelines and inclusion/exclusion criteria. An online search was conducted for peer-reviewed publications in Medline/OVID; Medline/PubMED; PsycINFO; CINAHL; Scopus and JSTOR, and Google Scholar. A search for grey literature was conducted in Google Scholar and websites of international and local organizations. Experts in the field also contributed additional references. Extracted data were summarised in an Excel spreadsheet. Searches were conducted between May and June, 2019, and then repeated in July, 2020. Results A total of 11 studies were included; 10 qualitative and one mixed methods study. Studies were conducted in Melanesian (n = 9), Polynesian (n = 1) and Micronesian (n = 1) PICTs. All 11 studies reported elements of societal and personal factors; ten studies reported evidence relating to interpersonal factors; nine studies reported elements relating to environmental factors; and two studies presented evidence linked to biological factors. Managing menstrual health with dignity is challenging for many women and girls because menstruation is associated with menstrual taboos and shame. Conclusion This review found that the MHH experiences of women in PICTs are affected by social and cultural beliefs, norms and practices. Beliefs, norms and practices about menarche need to be incorporated in SRH planning, programs and education in order to be relevant to diverse village and urban settings. Plain language Summary Globally, menarche is an important developmental milestone for females.   • Timing and causes of the mid-Cretaceous global plate reorganization event

    • Global plate reorganization events (GPREs) can have profound impact on plate velocities, climate and the biotic cycle but the timing and triggers for GPREs remain debated. The timing and causes of the mid-Cretaceous GPRE, one of only two GPREs since the breakup of Pangea, remains poorly constrained because it occurred during the Cretaceous Normal Superchron. Here, we provide a new plagioclase Ar-40/Ar-39 age of 103.82 +/- 0.54 Ma (2 sigma) at DSDP site 256 along the world's clearest curved fracture zones in the Wharton Basin, off NW Australia. Using Monte Carlo simulations, we compute that the mid-Cretaceous GPRE commenced at 111.2(-1.2)(+1.3) Ma (95% confidence). The mid-Cretaceous GPRE was associated with a significant plate deceleration (minimum similar to 26%), similar to the Eocene GPRE, implying that this process is fundamental to plate reconfigurations. We propose that the mid-Cretaceous GPRE was caused by slab-breakoff and cessation of the south-dipping subduction zone in the Mesotethys Ocean at ca. 111 Ma. Ultimately, we posit that subduction zone initiations and cessations are the primary triggers for both post-Pangean GPREs and that top-down processes may be the fundamental drivers for GPREs.     • Assessing multiple domains of health in older adults requires multidimensional and large datasets. Consensus on definitions, measurement protocols and outcome measures is a prerequisite. The Physical Activity and Nutritional INfluences In Ageing (PANINI) Toolkit aims to provide a standardized toolkit of best-practice measures for assessing health domains of older adults with an emphasis on nutrition and physical activity. The toolkit was drafted by consensus of multidisciplinary and pan-European experts on ageing to standardize research initiatives in diverse populations within the PANINI consortium. Domains within the PANINI Toolkit include socio-demographics, general health, nutrition, physical activity and physical performance and psychological and cognitive health. Implementation across various countries, settings and ageing populations has proven the feasibility of its use in research. This multidimensional and standardized approach supports interoperability and re-use of data, which is needed to optimize the coordination of research efforts, increase generalizability of findings and ultimately address the challenges of ageing.

  • Cobalt containing glass fibres and their synergistic effect on the HIF-1 pathway for wound healing applications

    • Introduction and Methods: Chronic wounds are a major healthcare problem, but their healing may be improved by developing biomaterials which can stimulate angiogenesis, e.g. by activating the Hypoxia Inducible Factor (HIF) pathway. Here, novel glass fibres were produced by laser spinning. The hypothesis was that silicate glass fibres that deliver cobalt ions will activate the HIF pathway and promote the expression of angiogenic genes. The glass composition was designed to biodegrade and release ions, but not form a hydroxyapatite layer in body fluid.Results and Discussion: Dissolution studies demonstrated that hydroxyapatite did not form. When keratinocyte cells were exposed to conditioned media from the cobalt-containing glass fibres, significantly higher amounts of HIF-1α and Vascular Endothelial Growth Factor (VEGF) were measured compared to when the cells were exposed to media with equivalent amounts of cobalt chloride. This was attributed to a synergistic effect of the combination of cobalt and other therapeutic ions released from the glass. The effect was also much greater than the sum of HIF-1α and VEGF expression when the cells were cultured with cobalt ions and with dissolution products from the Co-free glass, and was proven to not be due to a rise in pH. The ability of the glass fibres to activate the HIF-1 pathway and promote VEGF expression shows the potential for their use in chronic wound dressings.

    • The current paper presents work on identification and evaluation of a range of factors influencing accuracy and comparability of data generated by three laboratories carrying out stress-controlled thermo-mechanical fatigue crack growth tests. It addresses crack length measurements, heating methods and temperature measurement techniques. It also provides guidance for pre-cracking and use of different specimen geometries as well as Digital Image Correlation imaging for crack monitoring. The majority of the tests have been carried out on a coarse grain polycrystalline nickel-base superalloy using two phase angles, Out-of-Phase and In-Phase cycles with a triangular waveform and a temperature range of 400-750 degrees C.

  • OPTIKNEE 2022 : Consensus recommendations to optimise knee health after traumatic knee injury to prevent osteoarthritis

    • The goal of the OPTIKNEE consensus is to improve knee and overall health, to prevent osteoarthritis (OA) after a traumatic knee injury. The consensus followed a seven-step hybrid process. Expert groups conducted 7 systematic reviews to synthesise the current evidence and inform recommendations on the burden of knee injuries; risk factors for post-traumatic knee OA; rehabilitation to prevent post-traumatic knee OA; and patient-reported outcomes, muscle function and functional performance tests to monitor people at risk of post-traumatic knee OA. Draft consensus definitions, and clinical and research recommendations were generated, iteratively refined, and discussed at 6, tri-weekly, 2-hour videoconferencing meetings. After each meeting, items were finalised before the expert group (n=36) rated the level of appropriateness for each using a 9-point Likert scale, and recorded dissenting viewpoints through an anonymous online survey. Seven definitions, and 8 clinical recommendations (who to target, what to target and when, rehabilitation approach and interventions, what outcomes to monitor and how) and 6 research recommendations (research priorities, study design considerations, what outcomes to monitor and how) were voted on. All definitions and recommendations were rated appropriate (median appropriateness scores of 7-9) except for two subcomponents of one clinical recommendation, which were rated uncertain (median appropriateness score of 4.5-5.5). Varying levels of evidence supported each recommendation. Clinicians, patients, researchers and other stakeholders may use the definitions and recommendations to advocate for, guide, develop, test and implement person-centred evidence-based rehabilitation programmes following traumatic knee injury, and facilitate data synthesis to reduce the burden of knee post-traumatic knee OA.

  • Toward designing human intervention studies to prevent osteoarthritis after knee injury : A report from an interdisciplinary OARSI 2023 workshop

    • Objective: The global impact of osteoarthritis is growing. Currently no disease modifying osteoarthritis drugs/therapies exist, increasing the need for preventative strategies. Knee injuries have a high prevalence, distinct onset, and strong independent association with post-traumatic osteoarthritis (PTOA). Numerous groups are embarking upon research that will culminate in clinical trials to assess the effect of interventions to prevent knee PTOA despite challenges and lack of consensus about trial design in this population. Our objectives were to improve awareness of knee PTOA prevention trial design and discuss state-of-the art methods to address the unique opportunities and challenges of these studies. Design: An international interdisciplinary group developed a workshop, hosted at the 2023 Osteoarthritis Research Society International Congress. Here we summarize the workshop content and outputs, with the goal of moving the field of PTOA prevention trial design forward. Results: Workshop highlights included discussions about target population (considering risk, homogeneity, and possibility of modifying osteoarthritis outcome); target treatment (considering delivery, timing, feasibility and effectiveness); comparators (usual care, placebo), and primary symptomatic outcomes considering surrogates and the importance of knee function and symptoms other than pain to this population. Conclusions: Opportunities to test multimodal PTOA prevention interventions across preclinical models and clinical trials exist. As improving symptomatic outcomes aligns with patient and regulator priorities, co-primary symptomatic (single or aggregate/multidimensional outcome considering function and symptoms beyond pain) and structural/physiological outcomes may be appropriate for these trials. To ensure PTOA prevention trials are relevant and acceptable to all stakeholders, future research should address critical knowledge gaps and challenges.

    • The EU Waste Framework Directive 2008/98/EC states that all member states should take all necessary measures in order to achieve at least 70% re-use, recycling or other recovery of non-hazardous Construction and Demolition Waste (CDW) by 2020. In response, the Horizon 2020 RE4 project consortium (REuse and REcycling of CDW materials and structures in energy efficient pREfabricated elements for building REfurbishment and construction) consisting of 12 research and industrial partners across Europe, plus a research partner from Taiwan, was set up. For its success, the approach of the Project was manifold, developing sorting technologies to first improve the quality of CDW-derived aggregate. Simultaneously, CDW streams were assessed for quality and novel applications developed for aggregate, timber and plastic waste in a variety of products including structural and non-structural elements. With all products considered, innovative building concepts have been designed in a bid to improve future reuse and recycling of the products by promoting prefabricated construction methods and modular design to ease future recycling and increase value of the construction industry. The developed technologies and products have been put to the test in different test sites in building a two-storey house containing at least 65% of CDW. © 2019, © 2019 The Author(s)..
