    • This case study explored how a researcher–community partnership contributed to program adaptations when implementing person-centered group-based health promotion services to older people who have migrated to Sweden. The study was conducted over 3 years and various data sources were used: focus groups, individual interviews, documents, and archive material. Findings from different data sources and partners’ perspectives were triangulated to an overall case description using an iterative process. Adaptations were shaped through a dynamic process, negotiating toward suitable solutions that culminated in actions taken to adapt or inhibit adaptations. The negotiations were driven by the interplay within and between three reasons to adapt. The partners’ opportunities to influence the negotiation process depended on establishing common ground to shape adaptations. Practical implications are provided on how to move from knowledge to action when implementing person-centered group-based health promotion to support optimal aging in the context of migration.

  • Towards a screening tool for housing accessibility problems: a reduced version of the Housing Enabler.

    • To present a reduced version of the Housing Enabler, useful as a screening tool in practice contexts and in research, this study identifies the core items of the environmental component of the instrument, that is, the items most important in terms of the instrument's overall validity to measure accessibility problems. Utilizing cross-national data and interdisciplinary knowledge through an iterative process, a set of core items was identified. The environmental component of the reduced version comprises 61 items, compared to 188 in the original instrument. The rank correlation between the accessibility scores generated by the original and reduced versions was r ≥ .97 (p < .001). Based on comprehensive empirical research in three countries, the reduced version of the Housing Enabler is a time-efficient and valid screening instrument. It is a powerful tool for research and interventions focusing on housing accessibility problem identification.

    • The study aimed to explore how home help service staff described their role in improving the abilities of older people, in particular, older women with chronic pain who are dependent on formal care, to perform everyday activities. Three focus group interviews were conducted, and a qualitative inductive thematic content analysis was used. The analysis resulted in one theme: struggling to improve the care recipients' opportunities for independence but being inhibited by complex environmental factors. By encouraging the care recipients to perform everyday activities, the staff perceived themselves to both maintain and improve their care recipients' independence and quality of life. An important goal for society and health care professionals is to improve older people's abilities to "age in place" and to enable them to age independently while maintaining their quality of life. A key resource is home help service staff, and this resource should be utilized in the best possible way.

    • In Sweden, many older adults receive care in their own homes. However, their perceptions of the experience of receiving home care has not been sufficiently examined. This study aimed to explore older adults' experiences of receiving care and services in their homes and their perceptions of the care that they had received. In-depth interviews were conducted with 29 older adults. There were individual differences in their level of participation, but they mostly perceived their participation in the planning and provision of home care to be limited. Furthermore, their needs (and wishes), especially those that pertained to different aspects of time, were not always gratified. Organizational factors and care providers' lack of competence and high levels of time pressure influenced their perceptions of care. It is important to consider the perspectives of older care recipients when developing home care.

    • The development of effective interventions for mild cognitive impairment (MCI) in older adults has been limited by extensive variability in the conceptualization and definition of MCI, its subtypes, and relevant diagnostic criteria within the neurocultural, pharmaceutical, and gerontological communities. A scoping review was conducted to explore the conceptual development of MCI and identify the resulting ethical, political, and technological implications for the care of older adults with MCI. A comprehensive search was conducted between January and April 2013 to identify English-language peer-reviewed articles published between 1999 and 2013. Our analysis revealed that the MCI conceptual debate remains unresolved, the response to ethical issues is contentious, the policy response is limited, and one-dimensional and technological interventions are scarce. Reflections on the conceptual, ethical, and policy responses in conjunction with the identification of the needs of older adults diagnosed with MCI highlight significant opportunities for technological interventions to effectively reposition MCI in the aging care discourse.

  • Social Citizenship Through Out-of-Home Participation Among Older Adults With and Without Dementia

    • There is limited empirical knowledge about how older adults living with dementia enact their social citizenship through out-of-home participation. This study aimed: (a) to investigate out-of-home participation among older adults with and without dementia in four countries and (b) to compare aspects of stability or change in out-of-home participation. Using a cross-sectional design, older adults with mild-to-moderate dementia and without dementia, aged 55 years and over, were interviewed using the Participation in ACTivities and Places OUTside the Home questionnaire in Canada (n = 58), Sweden (n = 69), Switzerland (n = 70), and the United Kingdom (n = 128). Data were analyzed using descriptive statistics and a two-way analysis of variance. After adjustment for age, diagnosis of dementia and country of residence had significant effects on total out-of-home participation (p <.01). The results contribute to policies and development of programs to facilitate social citizenship by targeting specific activities and places.

    • Long-term musculoskeletal pain is a global health problem among older adults. Yet little is known about how these older adults actually deal with daily life. This studys aim is to describe the ways these older adults dealt with daily life at home. Phenomenography is used to collect and analyze data from semistructured interviews with 19 older adults and to identify the range in which they dealt with daily life. Findings consist of an outcome space that encompassed four categories: ignore, struggle, adjust, and resign. The different ways older adults dealt with daily life when living with long-term pain at home strengthen the importance of individualized plans of care in the home and a holistic perspective. Findings contribute to enhanced understanding of a common health problem among older adults that can be used to promote quality of care and improve the quality of life of older adults.

  • Swedish health care professionals' view of frailty in older persons

    • There is a paucity of research literature concerning frailty in older persons from the health care professionals' perspective. Consequently, the purpose of this study was to elucidate health care professionals' view of frailty in older persons. An explorative, qualitative design was selected and four focus groups comprising 21 health care professionals were conducted, audiotaped, transcribed verbatim, and analyzed. Frailty was found to consist of seven dimensions: "being bodily weak and ill," "being negatively influenced by personal qualities," "lacking balance in everyday activities," "being dependent in everyday life," "not being considered important," "being hindered by the physical milieu and defective community service," and "having an inadequate social network." The results showed that health care professionals' view of frailty in older persons differed from the current state of knowledge on frailty. This implies that the seven dimensions found to constitute frailty could contribute to a more comprehensive understanding of the concept.

  • Factors associated with psychotropic drug use among the elderly living at home

    • The use of psychotropic drugs has been widely studied; however, few studies have focused on factors associated with psychotropic drug use among the elderly. The aim of this study was to analyze the relationship between sociodemographic variables, socioeconomic status, social network, health, and physician visits and the use of psychotropic drugs among the elderly living at home. The study area was a municipality in mideastern Sweden with a population of about 21,000 inhabitants. Data from interviews with and health assessments of noninstitutionalized people age 75 and older (N = 421) were linked to an individual-based research register of prescription drug purchases. There were no differences in psychotropic drug use between men and women or between different age groups. The multivariate analyses (LISREL) showed that mental health and physician visits were associated with psychotropic drug use. An indirect relationship was also found between psychotropic drugs and loneliness and chronic morbidity, respectively.

    • The self is constructed in cooperation with other people and social context influences how people perceive and express it. People with Alzheimer’s disease (AD) often receive insufficient support in constructing their preferred selves, but little is known about how they express themselves together with other people with AD. In accordance with Harré’s social constructionist theory of self, this study aimed to describe how five people with mild and moderate AD express their Self 2 (i.e., their personal attributes and life histories) in a support group with a facilitator experienced in communicating with people with AD. The participants’ expressions of their Self 2 were analyzed with qualitative abductive content analysis and interpreted in terms of agency and communion and a lack of agency and communion. The findings highlight the importance of supporting a sense of agency and communion when assisting people with AD in constructing their self.

    • The aim was to explore the use of an activity-based approach to determine the validity of a set of housing standards addressing accessibility. This included examination of the frequency and the extent of accessibility problems among older people with physical functional limitations who used no mobility device (n = 10) or who used a wheelchair (n = 10) or a rollator (n = 10). The setting was a kitchen designed according to present housing standards. The participants prepared lunch in the kitchen. Accessibility problems were assessed by observation and self-report. Differences between the three participant groups were examined. Performing well-known kitchen activities was associated with accessibility problems for all three participant groups, in particular those using a wheelchair. The overall validity of the housing standards examined was poor. Observing older people interacting with realistic environments while performing real everyday activities seems to be an appropriate method for assessing accessibility problems.

    • The ambition underlying this study is to provide a valid and efficient screening tool targeting accessibility problems in the ordinary housing stock. The study aims to test the feasibility and interrater agreement of a screening-tool version of the Housing Enabler in a real estate company practice context. Two rater pairs administer the screening in 35 apartments in a Swedish municipality. The overall agreement for the rater pairs was 83% and 78%. For 29 of the 61 items of the tool, the agreement was > 80% for both rater pairs; for 8 items, it was < 80%. The findings show that the tool is feasible for use in a real estate practice context but that there is a need for more rater training and an instrument manual. This study contributes to the development of research-based strategies for the identification of accessibility problems in ordinary housing, with the long-term aim to increase accessibility in the housing stock, ultimately supporting activity and participation among senior citizens.

  • Psychosocial Work Environment and Well-Being of Direct-Care Staff Under Different Nursing Home Ownership Types: A Systematic Review

    • This systematic review investigated the psychosocial work environment and well-being of direct-care staff under different nursing home ownership types. Databases searched: Scopus, Web of Science, Cinahl, and PubMed, 1990–2020. Inclusion criteria: quantitative or mixed-method studies; population: direct-care staff in nursing homes; exposure: for-profit and non-profit ownership; and outcomes: psychosocial work environment and well-being. In total, 3896 articles were screened and 17(n = 12,843 participants) were assessed using the Joanna Briggs Institute Critical Appraisal tools and included in the narrative synthesis. The results were inconsistent, but findings favored non-profit over for-profit settings, for example, regarding leaving intentions, organizational commitment, and stress-related outcomes. There were no clear differences concerning job satisfaction. Job demands were higher in non-profit nursing homes but alleviated by better job resources in one study. The result highlights work environment issues, with regulations concerning for-profit incentives being discussed in terms of staff benefits.

  • License suspension revisited: A 3-year follow-up study of older drivers

    • This 3-year follow-up study addressed changes in health, cognitive functioning, and driving status among 37 older drivers and 37 individually matched controls. Initially, the study group had suspended driver’s licenses due to traffic violations. The in-person follow-up medical and neuropsychological examinations concerned 20 case participants and 22 controls. Mortality tended to be higher with case participants than with controls (p = .085), and there was more dementia or cognitive impairment with case participants (5/37) than with controls (0/37,p = .027). Initially, crash-involved case participants performed consistently worse on measures of cognitive functioning than did controls and noncrashed case participants and showed greater deterioration over time. Compared to controls, more crash-involved drivers had died (p = .019) or had stopped driving (p = .040). Because some older drivers with unsafe driving behavior may be in early phases of dementing processes or serious medical conditions, they should be medically and cognitively assessed.

    • This study examines the association between nursing assistants' perceptions of their psychosocial work environment and satisfaction among older people receiving care in nursing homes and home care. Cross-sectional surveys were conducted among people receiving care (N= 1,535) and nursing assistants (N= 1,132) in 45 nursing homes and 21 home care units within municipal old-age care. Better psychosocial work environment was related to higher satisfaction in old-age care among the recipients. Significant and stronger associations were more common in nursing homes than in home care. Perception of mastery and positive challenges at work were associated with higher recipient satisfaction both in home care and in nursing homes: social climate, perception of group work, perception of mastery, and positive challenges at work only in nursing homes. Findings suggest that recipient satisfaction may be increased by improving the psychosocial work environment for nursing assistants, both in nursing homes and in home care.

    • Autonomy, especially of nursing home residents, concerns the essence of values such as personal freedom and independence. This study examines Collopy's six polar dimensions of autonomy from the perspective of 202 nursing home personnel from a sample of 15 nursing homes in West Central Florida. Using regression analysis, autonomy was examined with respect to four catego ries of variables: (a) nursing home staff background factors, for example, education; (b) nursing home staff attitudinal issues, for example, job satisfaction; (c) staffing issues, for example, turn over rates of nurses; and (d) formal facility and resident characteristics, for example, percentage of residents physically restrained. The results confirm that autonomy is a complex construct with considerable subtlety. The most predictive set of variables overall include race, educational at tainment, and employee type (i.e., certified nursing assistants vs. others). The results are dis cussed with respect to their implications for care.

    • This study elucidates the oldest olds’ leisure repertoire and how this repertoire varies due to gender, where they live, and cognitive status. A validated 20-item leisure interest checklist with four subscales was used to measure leisure participation and investigate the leisure repertoire among participants. The oldest old were most likely to be interested in, perform, be motivated for, and perceive well-being from social activities, cultural activities, and TV/video/movies. The respondents were least likely to be interested in, perform, be motivated for, and perceive well-being from equipment sports and ball games. Some gender, geographic differences, and differences in cognitive status were found. The oldest old were more likely to endorse the same activities across all subscales of the checklist, but the linear magnitude varied across sub-scales. The relationships between performance and the other subscales suggested that a sense of engagement or participation is related to actual performance.

    • The purpose of this study was to enhance an earlier identified and briefly described social self category of integrity and how it affects the quality of care in patients older than 70 years. The two methods used in the earlier research were qualitative interviews and participant observations. From the data analysis, two themes were identified: social exchange and social experiences. The results indicate that improving care for geriatric patients includes offering more time to these patients for therapeutic social interactions such as listening, talking, storytelling, and reminiscing. Improving care for geriatric patients also includes providing information from the world outside from such sources as newspapers, radio, television, and communication from fellow patients and significant others. An increased awareness of the “social self” category in older adults by health care professionals will provide a more comprehensive foundation for holistic geriatric care.

    • Technology is believed to have a potential for supporting significant others of people with dementia but little is known of their experiences and views of technology. The aim of this study is to explore how significant others relate to technology and to their relatives with dementia as technology users. The focus is on both their own use of technology as significant others and the use of technology by their relatives with dementia. Individual interviews and focus group discussions were undertaken and analyzed using a grounded theory approach. The significant others showed an overall readiness toward using technology in their present roles. Technology use in daily activities was perceived to be an important means to keeping retained abilities exercised but could also be perceived as a possible threat to health if activities were oversimplified. The significant others asked for flexible technology not perceived as stigmatizing, to be integrated into existing habits.

    • Objective: We explored post-Affordable Care Act hospital and skilled nursing facility (SNF) perspectives in discharge and admission practices. Method: Interviews were conducted with 138 administrative personnel in 16 hospitals and 25 SNFs in eight U.S. markets and qualitatively analyzed. Results: Hospitals may use prior referral rates and patients' geographic proximity to SNFs to guide discharges. SNFs with higher hospital referral rates often use licensed nurses to screen patients to admit more preferred patients. While SNFs with lower hospital referral rates use marketing strategies to increase admissions, these patients are often less preferred due to lower reimbursement or complex care needs. Conclusion: An unintended consequence of increased hospital-SNF integration may be greater disparity. SNFs with high hospital referral rates may admit well-reimbursed or less medically complex patients than SNFs with lower referral rates. Without policy remediation, SNFs with lower referral rates may thus care for more medically complex long-term care patients.

    • The aim of this qualitative interview study was to delineate the meaning of preparing, cooking, and serving meals among retired single living and cohabiting women. Sixty-three women living in two Swedish cities and their rural surroundings participated. The findings showed that the profound meaning was to do something for others. The whole procedure of preparing a meal could be seen as preparing a gift. Four phases were identified: finding out what to serve, cooking with fresh ingredients, presenting the gift in a beautiful manner, and enjoying the gift in commensality. Cohabiting women went on cooking with duty and joy as they had done before retirement as long as their power was maintained. For widows, especially those who had recently lost their spouse, the whole meaning of cooking and eating was lost, and among these women there is a risk of poor nutritional intake.

  • Pilot Outcomes of a Multicomponent Fall Risk Program Integrated Into Daily Lives of Community-Dwelling Older Adults

    • Objectives: To evaluate whether a fall prevention intervention reduces fall risk in older adults who have previously fallen. Design: Randomized controlled pilot trial. Setting: Participants’ homes. Intervention: LIVE-LiFE, adapted from Lifestyle-Intervention Functional Exercise (LiFE) integrates strength and balance training into daily habits in eight visits over 12 weeks. The adaptations to LiFE were to also provide (a) US$500 in home safety changes, (b) vision contrast screening and referral, and (c) medication recommendations. Control condition consisted of fall prevention materials and individualized fall risk summary. Measurement: Timed Up and Go (TUG) and Tandem stand. Falls efficacy, feasibility, and acceptability of the intervention. Results: Sample (N = 37) was 65% female, 65% White, and average 77 years. Compared with the control group, each outcome improved in the intervention. The LIVE-LiFE intervention had a large effect (1.1) for tandem stand, moderate (0.5) in falls efficacy, and small (0.1) in the TUG. Conclusion: Simultaneously addressing preventable fall risk factors is feasible.

    • It is increasingly possible for couples with differing care needs to move to nursing homes together. To our knowledge, this is the first study about spousal caregiving in this context. This study explores spouse and staff experiences when relatively healthy spouses cohabit with and provide care to their partners in Swedish nursing homes. The overarching theme, ?being in-between,? reflects the spouse?s overlapping role transition and the staff?s unfamiliarity with the spouse?s role. The spouses are both ?insiders? and ?outsiders? and are torn between maintaining and letting go of caregiving. Meanwhile, the staff express concern about the spouse?s need for respite yet being constantly present, and struggle to balance the staff?s and the spouse?s control of the caregiving situation. This article provides insight into the challenges experienced by spouses who accompany their partners to residential care and how to best support them in this unfamiliar role.

    • The purpose of this study was to explore and describe older adults' opinions regarding actions to prevent falls and to analyze differences in the opinions of highly versus less physically active older adults. An open-ended question was answered by 262 individuals aged 75 to 98 years living in the community. The answers were analyzed using qualitative content analysis, and differences in the categories were compared between highly and less physically active persons. Physical activity was measured according to a five-level scale. The content analysis resulted in eight categories: assistive devices, avoiding hazards, behavioral adaptive strategies, being physically active, healthy lifestyle, indoor modifications, outdoor modifications, and seeking assistance. Behavioral adaptive strategies were mentioned to a greater extent by highly active people, and indoor modifications were more often mentioned by less active older adults. Support for active self-directed behavioral strategies might be important for fall prevention among less physically active older adults.

In this subgroup of pre-frail older adults, more frequent support by personnel may be required to secure efficient intensity and progression in the exercises performed on your own at home.

    • To understand health and well-being in later life, it is vital to consider the meaning of subjective aging. This study aimed to explore how perceived health, self-perceptions of aging, and participation in social activities relate to each other among older persons in the Bothnia region and Åland islands in Finland. Data were analyzed using Spearman’s and polychoric correlation and multinomial logistic regression analyses. The perceived good health and the younger physical, psychological, and social dimensions of subjective age were found to be associated with each other and with participation in social activities outside one's home.

    • The growing ageing population in the Nordic region calls for increased focus on health promotion work. To enhance multi-professional understanding and further develop strategies for promoting healthy ageing, it is vital to consider the perspectives of those working with health promotion. The aim of this study was to explore a wide spectrum of practitioners' experiences of community-level health promotion targeting older adults in Finland and Sweden. Nine focus group interviews (34 informants) were conducted in 2019-2020. "Seeing the person" emerged as the ideal for health promotion targeting older adults, but this ideal was not always realized in current practice. Barriers related to organizational structures and the practitioner role were identified. However, work methods connected to user involvement and technology-based tools were considered key facilitators, enabling tailored health promotion initiatives.

    • Background: Older adults manage increasing numbers of everyday technologies to participate in home and community activities. Purpose: We investigated how assessing use of everyday technologies enhanced predictions of overall needed assistance among urban older adults. Method: We used a cross-sectional design to analyze responses from 114 participants completing the Everyday Technology Use Questionnaire, the Montreal Cognitive Assessment, and a sociodemographic questionnaire. We estimated overall needed assistance based on definitions in the Assessment of Motor and Process Skills. We created logistic regression models and receiver operator characteristic curves to analyze variables predicting overall needed assistance. Findings: With high specificity and sensitivity, the Everyday Technology Use Questionnaire and the Montreal Cognitive Assessment were the strongest predictors of overall needed assistance.

    • Falls are associated with considerable morbidity and mortality in aged care services and falls risk increasing drugs (FRIDs) are often overlooked as a contributor to falls. This study aims to investigate the association between the risk of falling and use of FRIDs from aged care services. Inverse-probability-weighted multinomial logistic regression was used to estimate the association between falls risk and regular FRIDs in 383 residents from six Australian aged care services. Overall, residents at high and low falls risk had similar prevalence of FRIDs. Prevalence of antipsychotics and sedative-hypnotics was low. Residents at high falls risk had higher adjusted odds of using >= 2 psychotropic medications (odds ratio [OR] = 1.75, 95% confidence interval [CI] = 1.17-2.61) and >= 2 medications that cause/worsen orthostatic hypotension (OR = 3.59, 95% CI = 2.27-5.69). High prevalence of FRIDs was mainly attributable to medications for which residents had clinical indications. Clinicians appeared to have largely avoided FRIDs that explicit criteria deem potentially inappropriate for high falls risk.

    • The current study investigates the relevance of positive attitudes toward Internet technologies for psychological well-being and social well-being in old age. A sample of 245 elderly people (Mean age = 70; SD =9.1) filled in the Psychological Well-Being Questionnaire, the Social Well-Being Questionnaire, and Attitudes Toward Technologies Questionnaire (ATTQ). Favorable attitudes toward Internet technologies showed positive correlations with overall social well-being and all its components with the exception of social acceptance. Positive correlations with overall psychological well-being and two of its components, namely, personal growth and purpose in life, were also found. Two hierarchical multiple regression models underscored that positive attitudes toward Internet technologies constitute the most important predictor of social well-being, and it appears to be a significant predictor for psychological well-being as well. Results are discussed and integrated into the Positive Technology theoretical framework that sustains the value of technological resources for improving the quality of personal experience and well-being.

  • Support Needs Among Older Tenants Living in Public Housing in Sweden : Perspectives of Janitors and Maintenance Staff

    • Neighborhood support can improve aging in place for older adults, but research on the role of public housing staff in supporting older tenants is lacking. Twenty-nine participants (janitors, n = 11; maintenance staff, n = 18) collected data about critical situations among older tenants residing in apartments in Sweden. Modifying the Critical Incident Technique (CIT) and applying a mixed-methods design, quantitative and qualitative data were collected and analyzed with descriptive statistics and thematic analysis, integrated through narrative. We found that older tenants asked staff for help with daily tasks. The staff identified CI management dilemmas in meeting older tenants' support needs while following the housing company's regulations, maintaining professional responsibilities, respecting individual work attitudes and preferences, and experienced a lack of competencies in some situations. Staff members were responsive to offering support in simple, practical, and emotional situations and in addressing matters they perceived as deficits in social and health services.
