    • BackgroundMaintaining a good nutritional status during the hematopoietic cell transplantation (HCT) procedure is challenging in the pediatric population. MethodsIn a multicentric retrospective study, we compared the outcome of nutritional status and HCT-related parameters in 227 pediatric patients during and after HCT between 2005 and 2015. 112 patients received a gastrostomy before the start of HCT (GS group), and 115 did not receive a gastrostomy (NGS). Data collection was performed at HCT, 3, 6, and 12 months post-HCT. ResultsAt time point of HCT the Standard Deviation Score (SDS) of weight was 0.17 in the NGS group, and 0.71 in the GS group (p = .01) Patients in the NGS group lost more weight during the first 3 months after HCT than patients in the GS group. At 12 months, patients in the NGS remained at a lower weight, while patients in the GS group slightly increased their weight.There were no differences between the groups in the incidence of acute graft-versus-host-disease (GvHD), overall survival, and non-relapse mortality. However, the number of febrile episodes requiring intravenous treatment with antibiotics, was higher in the GS group as compared to the NGS group, during the first 3 months post-HCT (p < .001). ConclusionsOur results indicate that gastrostomy can be utilized in children undergoing HCT without any negative effects on mortality. Therefore, the use of a gastrostomy appears to be a safe option to maintain a good nutritional status during the HCT procedure.

  • Measurement of the tt¯ production cross-section and lepton differential distributions in eμ dilepton events from pp collisions at √s=13TeV with the ATLAS detector

    • The inclusive top quark pair (tt¯) production cross-section σtt¯ has been measured in proton–proton collisions at s=13TeV, using 36.1 fb- 1 of data collected in 2015–2016 by the ATLAS experiment at the LHC. Using events with an opposite-charge eμ pair and b-tagged jets, the cross-section is measured to be: σtt¯=826.4±3.6(stat)±11.5(syst)±15.7(lumi)±1.9(beam)pb,where the uncertainties reflect the limited size of the data sample, experimental and theoretical systematic effects, the integrated luminosity, and the LHC beam energy, giving a total uncertainty of 2.4%. The result is consistent with theoretical QCD calculations at next-to-next-to-leading order. It is used to determine the top quark pole mass via the dependence of the predicted cross-section on mtpole, giving mtpole=173.1-2.1+2.0GeV. It is also combined with measurements at s=7TeV and s=8TeV to derive ratios and double ratios of tt¯ and Z cross-sections at different energies. The same event sample is used to measure absolute and normalised differential cross-sections as functions of single-lepton and dilepton kinematic variables, and the results are compared with predictions from various Monte Carlo event generators. © 2020, CERN for the benefit of the ATLAS collaboration.

  • Aerosol particle dry deposition to canopy and forest floor measured by two-layer eddy covariance system

    • We present, to our knowledge for the first time, results from subcanopy (forest trunk space) aerosol flux measurements with the eddy covariance (EC) technique. The measurements were performed at the SMEAR II measurement station located in a Scots pine forest in southern Finland during spring 2003 when the ground was snow covered and in situ biogenic particle formation took place almost every day. Spectral analysis showed that the EC method can be applied to estimate subcanopy aerosol fluxes and thereby ground deposition in a forest. By using the two-layer EC measurements we were able to investigate the fraction of the total aerosol deposition taking place in the canopy and on the forest floor. We found that the ratio of subcanopy to above- canopy flux is dependent on the strength of turbulence. When the friction velocity was low (u(*) < 0.25 m s(-1)), approximately 35% of the depositing particles penetrated the canopy and deposited on the ground. When u* increased, the fractional deposition on the forest floor decreased almost linearly, and in strongly turbulent conditions (u(*)> 1.0 m s(-1)) the ground deposition contributed only around 10% to the total deposition. Thus, it seems that increased turbulence enhances the importance of the canopy for aerosol deposition and the relative importance of the forest floor to the total deposition diminishes.

    • Background: It has been recognized that a clinically significant portion of patients with coronary artery disease (CAD) continue to experience anginal and other related symptoms that are refractory to the combination of medical therapy and revascularization. The Euro Heart Survey on Revascularization (EHSCR) provided an opportunity to assess pharmacological treatment and outcome in patients with proven CAD who were ineligible for revascularization. Methods: We performed a secondary analysis of EHS-CR data. After excluding patients with ST-elevation myocardial infarction and those in whom revascularization was not indicated, 4409 patients remained in the analyses. We selected two groups: (1) patients in whom revascularization was the preferred treatment option (n = 3777, 86%), and (2) patients who were considered ineligible for revascularization (n = 632, 14%). Results: Patient ineligible for revascularization had a worse risk profile, more often had a total occlusion (59% vs. 37%, p < 0.001), were treated more often with ACE-inhibitors (65% vs. 55%, p < 0.001) but less likely with aspirin (83% vs. 88%, p < 0.001). Overall, they had higher case-fatality at 1-year (7.0% vs. 3.7%, p < 0.001). Regarding self-perceived health status, measured via the EuroQol 5D (EQ-5D) questionnaire, these same patients reported more problems on all dimensions of the EQ-5D. Furthermore, in the revascularization group we observed an increase between discharge and 1-year follow up (utility score from 0.85 to 1.00) whereas patients ineligible for revascularization did not improve over time (utility score remained 0.80). Conclusion: In this large cohort of European patients with CAD, those considered ineligible for revascularization had more co-morbidities and risk factors, and scored worse on self-perceived health status as compared to revascularized patients in the revascularization group. With the exception of ACE-inhibitors and aspirin, there were no major differences regarding drug treatment between the two groups. Given these clinically significant observations, there appears to be a role for nurse-led, multidisciplinary, rehabilitation teams that target clinically vulnerable patients whose symptoms remain refractory to standard medical care. © 2006 European Society of Cardiology.

  • Clinical relevance of endoscopy with histopathological assessment in children with suspected gastrointestinal graft-versus-host disease

    • Endoscopy with histopathological assessment is an established practice to confirm gastrointestinal graft-versus-host disease (GI-GVHD). However, the clinical relevance of this approach in children is incompletely evaluated. In a retrospective cohort study, we investigated the frequency of treatment changes in response to histopathological findings in all children (<18 years) in Sweden who underwent endoscopy for suspected GI-GVHD (2000-2013) after receiving hematopoietic stem cell transplantation. Sixty-eight children with ninety-one endoscopic occasions were enrolled. At the time of endoscopy, anti-GI-GVHD treatment was ongoing in 71% (65/91). In 18% (12/65) with ongoing treatment, no histopathological evidence of GI-GVHD or another cause to justify anti-GI-GVHD treatment was found. In 48% (44/91), endoscopy with histopathological assessment led to changes in the treatment regimen. Re-endoscopy was more frequent among those with treatment changes, versus unchanged treatment, 39% (17/44) and 13% (6/47), respectively (P = .007). Histopathological findings generating treatment changes were as follows: GI-GVHD in 68% (30/44), normal histology in 25% (11/44), and an alternative diagnosis in 7% (3/44). In conclusion, this study supports that endoscopy with histopathological assessment should be considered in all children with suspected GI-GVHD.

    • Several observables sensitive to the fragmentation of b quarks into b hadrons are measured using 36 fb-1 of s=13 TeV proton-proton collision data collected with the ATLAS detector at the LHC. Jets containing b hadrons are obtained from a sample of dileptonic tt¯ events, and the associated set of charged-particle tracks is separated into those from the primary pp interaction vertex and those from the displaced b-decay secondary vertex. This division is used to construct observables that characterize the longitudinal and transverse momentum distributions of the b hadron within the jet. The measurements have been corrected for detector effects and provide a test of heavy-quark-fragmentation modeling at the LHC in a system where the top-quark decay products are color connected to the proton beam remnants. The unfolded distributions are compared with the predictions of several modern Monte Carlo parton-shower generators and generator tunes, and a wide range of agreement with the data is observed, with p values varying from 5×10-4 to 0.98. These measurements complement similar measurements from e+e- collider experiments in which the b quarks originate from a color singlet Z/γ∗. © 2022 CERN.

    • The LAPBIAT measurement campaign took place in the Varrio SMEAR I measurement station located in Eastern Lapland in the spring of 2003 between 26 April and 11 May. In this paper we describe the measurement campaign, concentrations and fluxes of aerosol particles, air ions and trace gases, paying special attention to an aerosol particle formation event broken by a air mass change from a clean Arctic air mass with new particle formation to polluted one approaching from industrial areas of Kola Peninsula, Russia, lacking new particle formation. Aerosol particle number flux measurements show strong downward fluxes during that time. Concentrations of coarse aerosol particles were high for 1-2 days before the nucleation event (i.e. 28-29 April), very low immediately before and during the observed aerosol particle formation event (30 April) and increased moderately from the moment of sudden break of the event. In general particle deposition measurements based on snow samples show the same changes. Measurements of the mobility distribution of air ions showed elevated concentrations of intermediate air ions during the particle formation event. We estimated the growth rates in the nucleation mode size range. For particles <10 nm, the growth rate increases with size on 30 April. Dispersion modelling made with model SILAM support the conclusion that the nucleation event was interrupted by an outbreak of sulphate-rich air mass in the evening of 30 April that originated from the industry at Kola Peninsula, Russia. The results of this campaign highlight the need for detailed research in atmospheric transport of air constituents for understanding the aerosol dynamics.

  • The effect of dosing strategies on the therapeutic efficacy of artesunate-amodiaquine for uncomplicated malaria : a meta-analysis of individual patient data

    • Background: Artesunate-amodiaquine (AS-AQ) is one of the most widely used artemisinin-based combination therapies (ACTs) to treat uncomplicated Plasmodium falciparum malaria in Africa. We investigated the impact of different dosing strategies on the efficacy of this combination for the treatment of falciparum malaria. Methods: Individual patient data from AS-AQ clinical trials were pooled using the WorldWide Antimalarial Resistance Network (WWARN) standardised methodology. Risk factors for treatment failure were identified using a Cox regression model with shared frailty across study sites. Results: Forty-three studies representing 9,106 treatments from 1999-2012 were included in the analysis; 4,138 (45.4%) treatments were with a fixed dose combination with an AQ target dose of 30 mg/kg (FDC), 1,293 (14.2%) with a non-fixed dose combination with an AQ target dose of 25 mg/kg (loose NFDC-25), 2,418 (26.6%) with a non-fixed dose combination with an AQ target dose of 30 mg/kg (loose NFDC-30), and the remaining 1,257 (13.8%) with a co-blistered non-fixed dose combination with an AQ target dose of 30 mg/kg (co-blistered NFDC). The median dose of AQ administered was 32.1 mg/kg [IQR: 25.9-38.2], the highest dose being administered to patients treated with co-blistered NFDC (median = 35.3 mg/kg [IQR: 30.6-43.7]) and the lowest to those treated with loose NFDC-25 (median = 25.0 mg/kg [IQR: 22.7-25.0]). Patients treated with FDC received a median dose of 32.4 mg/kg [IQR: 27-39.0]. After adjusting for reinfections, the corrected antimalarial efficacy on day 28 after treatment was similar for co-blistered NFDC (97.9% [95% confidence interval (CI): 97.0-98.8%]) and FDC (98.1% [95% CI: 97.6%-98.5%]; P = 0.799), but significantly lower for the loose NFDC-25 (93.4% [95% CI: 91.9%-94.9%]), and loose NFDC-30 (95.0% [95% CI: 94.1%-95.9%]) (P < 0.001 for all comparisons). After controlling for age, AQ dose, baseline parasitemia and region; treatment with loose NFDC-25 was associated with a 3.5-fold greater risk of recrudescence by day 28 (adjusted hazard ratio, AHR = 3.51 [95% CI: 2.02-6.12], P < 0.001) compared to FDC, and treatment with loose NFDC-30 was associated with a higher risk of recrudescence at only three sites. Conclusions: There was substantial variation in the total dose of amodiaquine administered in different AS-AQ combination regimens. Fixed dose AS-AQ combinations ensure optimal dosing and provide higher antimalarial treatment efficacy than the loose individual tablets in all age categories.

    • Drift is one of the most serious impairments afflicting gas sensors. It can be seen as a gradual change in the sensor response over a long period of time when the external conditions an constant. This paper presents a new simple drift counteraction method based on PCA and PLS. The basic idea is to remove the drift direction component from the measurements. The direction of the drift, p, is calculated from measurements for a reference gas. Projecting the sample gas measurements on this vector gives the score vector t. The drift component tp(T) can then he removed from the sample gas data, which we call component correction (CC). The method is tested on a data set based on a reduced factorial design with four gases and a concentration gradient of hydrogen. It is found that the method works efficiently for both cases.     • Ligands with improved potency and selectivity for free fatty acid receptor 2 (FFA2R) have become available, and we here characterize the neutrophil responses induced by one such agonist (Cmp1) and one antagonist (CATPB). Cmp1 triggered an increase in the cytosolic concentration of Ca2+, and the neutrophils were then desensitized to Cmp1 and to acetate, a naturally occurring FFA2R agonist. The antagonist CATPB selectively inhibited responses induced by Cmp1 or acetate. The activated FFA2R induced superoxide anion secretion at a low level in naive blood neutrophils. This response was largely increased by tumor necrosis factor alpha (TNF-alpha) in a process associated with a recruitment of easily mobilizable granules, but neutrophils recruited to an aseptic inflammation in vivo were nonresponding. Superoxide production induced by Cmp1 was increased in latrunculin A-treated neutrophils, but no reactivation of desensitized FFA2R was induced by this drug, suggesting that the cytoskeleton is not directly involved in terminating the response. The functional and regulatory differences between the receptors that recognize short-chain fatty acids and formylated peptides, respectively, imply different roles of these receptors in the orchestration of inflammation and confirm the usefulness of a selective FFA2R agonist and antagonist as tools for the exploration of the precise role of the FFA2R.

    • In this work Al(2)O(3) and SiO(2) coatings are tested as Xe diffusion barriers on plastic scintillator substrates. The motivation is improved beta-gamma coincidence detection systems, used to measure atmospheric radioxenon within the verification regime of the Comprehensive Nuclear-Test-Ban Treaty. One major drawback with the current setup of these systems is that the radioxenon tends to diffuse into the plastic scintillator material responsible for the beta detection, resulting in an unwanted memory effect. Here, coatings with thicknesses between 20 and 900 nm have been deposited onto plastic scintillators, and investigated using two different experimental techniques. The results show that all tested coatings reduce the Xe diffusion into the plastic. The reduction is observed to increase with coating thickness for both coating materials. The 425 nm Al(2)O(3) coating is the most successful one, presenting a diffusion reduction of a factor 100, compared to uncoated plastic. In terms of memory effect reduction this coating is thus a viable solution to the problem in question.

    • A cylindrical plastic scintillator cell, used for radioxenon monitoring within the verification regime of the Comprehensive Nuclear-Test-Ban Treaty, has been coated with 425 nm Al2O3 using low temperature Atomic Layer Deposition, and its performance has been evaluated. The motivation is to reduce the memory effect caused by radioxenon diffusing into the plastic scintillator material during measurements, resulting in an elevated detection limit. Measurements with the coated detector show both energy resolution and efficiency comparable to uncoated detectors, and a memory effect reduction of a factor of 1000. Provided that the quality of the detector is maintained for a longer period of time, Al2O3 coatings are believed to be a viable solution to the memory effect problem in question.

    • Surface and interface properties of Cu thin films (1–4 monolayers) deposited on Ni(100) have been extracted by means of x-ray absorption spectroscopy and analyzed in combination with ab initio density-functional calculations. An unoccupied Cu surface state is identified in an x-ray absorption spectra and studied as a function of film thickness. Experimental data is supported by calculations of the layer-resolved density of states and the results from this combined theoretical-experimental effort show that the surface state is almost entirely located on the atomic layer closest to the vacuum. Our results also indicate strong hybridization between unoccupied states at the Cu/Ni interface boundary.

A standard aphasia test was performed. <i>Results:</i> In 60 patients (67%) at baseline and in 100% at 6 months, comprehension allowed a reliable DSM-IV diagnosis. Among these patients MADRS was feasible in 95% at baseline and in 100% at 6 months. The assistance of relatives and staff increases the feasibility and decreases the validity. Depression was identified in 24% during the 6 months. <i>Conclusion:</i> Depression diagnosis and severity rating can reliably be made in the acute phase in at least two thirds of aphasic patients, and feasibility increases over time.

  • Microbial dustiness and particle release of different biofuels

    • Exposure to organic dust originating from biofuels can cause adverse health effects. In the present study we have assessed the dustiness in terms of microbial components and particles of various biofuels by using a rotating drum as a dust generator. Microbial components from straw, wood chips, wood pellets and wood briquettes were quantified by several methods. Excellent correllations (r greater than or equal to 0.85, P < 0.0001) were found: between lipopolysaccharide (LPS) (as determined by 3-hydroxy fatty acid analysis) and endotoxin (as determined by a Limulus test), cultivable bacteria, total number of bacteria and muramic acid; between endotoxin and cultivable bacteria, total number of bacteria and muramic acid; between total number of bacteria and muramic acid; between cultivable fungi and total number of fungi. Straw was dustier than the other biofuels in terms of actinomycetes, bacteria, muramic acid, endotoxin, LPS, particle mass and number of particles. One of the wood chips studied and the straws had comparatively high dustiness In terms of fungi, while both wood pellets and wood briquettes had comparatively low dustiness in terms of all microbial components. An initially high particle generation rate of straw and wood chips decreased over time whereas the particle generation rate of wood briquettes and wood pellets increased during a 5 min rotation period. Particles of non-microbial origin may be the determining factor for the health risk in handling briquettes and pellets. Straw dust contained significantly more microorganisms per particle than did wood chip dust, probably because bacteria were most abundant in straw dust. The concentrations of endotoxin and fungi were high in wood and straw dust; dust from one of the straws contained 3610 EU/mg and dust from one of the chips contained 7.3 x 10(6) fungal spores/mg. An exposure to 3 mg of straw or wood chips dust/m(3) (the Swedish and Danish OEL of unspecific inhalable dust) could cause exposures to endotoxin and fungi higher than levels were health symptoms are seen to develop. The very different levels of dustiness in terms of particles and microbial components of different biofuels shows that dustiness is an important health-relevant factor to consider when choosing among biofuels and when designing worksites for handling of biofuels.

    • XAFS and X-ray Photoelectron Spectroscopy (XPS) are element specific techniques used in a great variety of research fields. The near edge regime of XAFS provides information on the unoccupied electronic states of a system. For the detailed interpretation of the XAFS results, input from XPS is crucial. The combination of the two techniques is also the basis for the so called core-hole clock technique. One of the important aspects of photoelectron spectroscopy is its chemical sensitivity and that one can obtain detailed information about the composition of a sample. We have for a series of carbon based model molecules carefully investigated the relationship between core level photoelectron intensities and stoichiometry. We find strong EXAFS-like modulations of the core ionization cross sections as function of photon energy and that the intensities at high photon energies converge towards values that do not correspond to the stoichiometric ratios. The photoelectron intensities are dependent on the local molecular structure around the ionized atoms. These effects are well described by molecular calculations using multiple scattering theory and by considering the effects due to monopole shake-up and shake-off as well as to intramolecular inelastic scattering processes.

  • The continuity of continuous integration: Correlations and consequences

    • The practice of continuous integration has firmly established itself in the mainstream of the software engineering industry, yet many questions surrounding it remain unanswered. Prominent among these is the issue of scalability: continuous integration has been reported to be possible to scale, but with difficulties. Understanding of the underlying mechanisms causing these difficulties is shallow, however: what is it about size that is problematic, which kind of size, and what aspect of continuous integration does it impede? Based on quantitative data from six industry cases encompassing close to 2000 engineers, complemented by interviews with engineers from five companies, this paper investigates the correlation between the continuity of continuous integration and size. It is found that not only is there indeed a correlation between the size and composition of a development organization and its tendency to integrate continuously; there is evidence that the size of the organization influences ways of working, which in turn correlate with the degree of continuity, raising the question of software manufacturability. It is further observed that developer behavior in ostensibly continuously integrating cases does not necessarily match expectations, and that frequent integration of the product itself does not automatically imply that each individual developer commits frequently.

  • Leachability testing of sludge from street gullies

    • Sludge from gullies, on two types of streets with different traffic intensity, was investigated using two recommended EU methods for leachability testing of waste: a two-stage batch test and an up-flow percolation test. The main purpose of this investigation was to gain more knowledge about these leaching test methods to be able to make future decisions on the general applicability of the proposed tests. A number of parameters were determined in the sludge as well as in the eluates obtained from the two leaching tests. These include pH, conductivity, dissolved organic carbon and inorganic ions as chloride ions. A number of metals as Cd, Cr, Cu, Hg, Ni and Zn, were determined by inductive coupled plasmamass spectrometry and organic compounds were screened by high performance liquid chromatography-diode array UV detection and gas chromatography-mass spectrometry. It was found that the concentrations of metals and organic compounds in the sludge were several orders of magnitudes higher than the actual eluate concentrations. For all metals the concentrations were well below the proposed limit values for non-hazardous waste included in the Council decision document 2003/33/EC. Generally, concentrations obtained in batch test were equal or higher than from percolation tests. The repeatability of the percolation and the batch test were in average 28 and 17%, respectively.
