  • Surgical repositioning of intraocular lenses after late dislocation: Complications, effect on intraocular pressure, and visual outcomes

    • PURPOSE: To study outcomes after surgery for late intraocular lens (IOL) dislocation and, more specifically, to evaluate different surgical techniques to find predictors of worse visual outcomes, describe postoperative complications, and analyze the effect on intraocular pressure (IOP). SETTING: Department of Ophthalmology, Sahlgrenska University Hospital, Molndal, Sweden. METHODS: Medical records from cataract surgery, IOL repositioning, and follow-up examinations were reviewed. RESULTS: Ninety-one eyes with in-the-bag (80) or out-of-the-bag (11) late IOL dislocation were consecutively included. In 94% of eyes, the IOLs were repositioned using scleral sutures; 76% of cases were operated on with a posterior approach, including pars plana vitrectomy. The median follow-up was 17 months. Pseudoexfoliation was detected in 57% of eyes. A significant decrease in IOP (mean 3.0 mm Hg) from preoperative values (P=.028) was seen in glaucoma patients. Thirteen eyes had additional surgical procedures. Three cases of retinal detachment occurred. Of the eyes, 59% obtained a Snellen corrected distance visual acuity (CDVA) of 0.5 or more at followup; 23% of eyes had worse CDVA during the follow-up than preoperatively. CONCLUSIONS: Repositioning surgery for late IOL dislocation with a posterior pars plana approach using scleral suturing of the preexisting IOL appears to be a safe and effective method for restoring visual acuity. Postoperative complications were comparable to previous findings in this field. Patients with glaucoma may have improved IOP regulation. (C) 2013 ASCRS and ESCRS

  • Factors predicting pain reduction in chronic back and neck pain after multimodal treatment.

    • OBJECTIVES: To determine whether treatment related pain reduction on the short- and long-term is predicted by different baseline variables, and with different accuracy, in patients with chronic low back pain as compared with those with chronic neck pain. DESIGN AND METHODS: A single blinded prospective cohort study based on patients with chronic musculoskeletal pain in the lower back (N = 167) or the neck (N = 136) who completed a 4-week multimodal rehabilitation program. At admission, each patient was evaluated on 17 potential predictors, including pain characteristics and physical, sociodemographic, and psychosocial-behavioral variables. Changes in self-reported pain intensity in the lower back or the neck between the pretreatment evaluation and those performed immediately after, and 12 months after the rehabilitation program, were assessed. RESULTS: Logistic regression models revealed that change in pain intensity could be predicted with good specificity but with poor sensitivity both for patients with chronic low back pain and chronic neck pain. Significant predictors among the neck pain patients were high endurance, low age, high pain intensity, few other symptoms, low need of being social, to do things with others, and to be helped, along with optimistic attitudes on how the pain will interfere with daily life. Among the low back pain patients, high pain intensity, low levels of pain severity, and high affective distress were important predictors. Variables such as sex, sick leave history, working status, accident, pain duration, and depressive symptoms demonstrated no predictive value. Short- and long-term pain outcome was equally predictable and predicted by almost the same variables. CONCLUSIONS: Patients who reported unchanged or increased pain after multimodal treatment could be predicted with good accuracy, whereas those who reported decreased pain were more difficult to identify. Treatment-related pain alteration in chronic low back pain seems to be predicted by partly different variables than in chronic neck pain.

  • Guiding the future energy transition to net-zero emissions : Lessons from exploring the differences between France and Sweden

    • Despite similarities in their current energy mixes, France and Sweden's pathways have been very different since the 1970s, when both systems were highly dependent on fossil fuels. After the oil crisis, both countries chose to reduce their oil consumption by developing nuclear power. However, Sweden pursued a more diversified energy policy that has subsequently allowed it to reduce its CO2 emissions. Today, both countries have committed to a carbon neutrality goal: by 2045 for Sweden and 2050 for France. In order to understand the key factors that can drive energy transition toward a carbon neutrality goal, we propose to compare the past energy transitions in France and Sweden, two countries that have significantly reduced their CO2 emissions and fossil fuel dependency. To assess the impacts of the current energy system and its regulations on the feasibility of meeting carbon neutrality, we use TIMES bottom-up energy system optimization models. The results show that France faces more challenges in transforming its energy system than Sweden i.e. an increase in power production, a decrease in gas consumption, the replacement of heating systems in buildings, and the electrification of industry, thus confirming that its energy policy has to be driven with a long-run perspective.

  • Simulation of a suspended droplet under evaporation with Marangoni effects

    • We investigate the Marangoni effects in a hexane droplet under evaporation and close to its critical point. A lattice Boltzmann model is used to perform 3D numerical simulations. In a first case, the droplet is placed in its own vapor and a temperature gradient is imposed. The droplet locomotion through the domain is observed, where the temperature differences across the surface is proportional to the droplet velocity and the Marangoni effect is confirmed. The droplet is then set under a forced convection condition. The results show that the Marangoni stresses play a major role in maintaining the internal circulation when the superheated vapor temperature is increased. Surprisingly, surface tension variations along the interface due to temperature change may affect heat transfer and internal circulation even for low Weber number. Other results and considerations regarding the droplet surface are also discussed.

  • Trochanteric fracture-implant motion during healing - A radiostereometry (RSA) study

    • Cut-out complication remains a major unsolved problem in the treatment of trochanteric hip fractures. A better understanding of the three-dimensional fracture-implant motions is needed to enable further development of clinical strategies and countermeasures. The aim of this clinical study was to characterise and quantify three-dimensional motions between the implant and the bone and between the lag screw and nail of the Gamma nail. Radiostereometry Analysis (RSA) analysis was applied in 20 patients with trochanteric hip fractures treated with an intramedullary nail. The following three-dimensional motions were measured postoperatively, at 1 week, 3, 6 and 12 months: translations of the tip of the lag screw in the femoral head, motions of the lag screw in the nail, femoral head motions relative to the nail and nail movements in the femoral shaft. Cranial migration of the tip of the lag screw dominated over the other two translation components in the femoral head. In all fractures the lag screw slid laterally in the nail and the femoral head moved both laterally and inferiorly towards the nail. All femoral heads translated posteriorly relative to the nail, and rotations occurred in both directions with median values close to zero. The nail tended to retrovert in the femoral shaft. Adverse fracture-implant motions were detected in stable trochanteric hip fractures treated with intramedullary nails with high resolution. Therefore, RSA method can be used to evaluate new implant designs and clinical strategies, which aim to reduce cut-out complications. Future RSA studies should aim at more unstable fractures as these are more likely to fail with cut-out. (C) 2018 Elsevier Ltd. All rights reserved.

  • Back To The Roots : Inherited Ownership Effects and Spawns’ Performance

    • This paper analyzes inherited ownership effects, defined as socialization benefits that employees acquire while working in privately-held firms that may be transferred to a spawn. We test for inherited ownership effects in the population of Swedish entrepreneurial spawns competing in medium-high and high technology manufacturing industries. Preliminary results show that spawns of family-owned businesses survive at a higher rate than spawns of non-family-owned businesses. Moreover, spawns that share family ties with the parent company survive at a higher rate than spawns started by non-family members. This findings suggest the existence of inherited ownership effects that influence the performance of new firms.

  • Management of obstructive sleep apnea in Europe

    • Objectives: In Europe, the services provided for the investigation and management of obstructive sleep apnoea (OSA) varies from country to country. The aim of this questionnaire-based study was to investigate the current status of diagnostic pathways and therapeutic approaches applied in the treatment of OSA in Europe, qualification requirements of physicians involved in diagnosis and treatment of OSA, and reimbursement of these services. Methods: Two questionnaires were sent to 39 physicians in 22 countries in Europe. In order to standardize the responses, the questionnaire was accompanied by an example. Results: Sleep centers from 21 countries (38 physicians) participated. A broad consistency among countries with respect to the following was found: pathways included referral to sleep physicians/sleep laboratories, necessity for objective diagnosis (primarily by polysomnography), use of polygraphic methods, analysis of polysomnography (PSG), indications for positive airway pressure (PAP) therapy, application of standard continuous PAP (CPAP) therapy (100% with an CPAP/APAP ratio of 2.24:1), and the need (90.5%) and management of follow-up. Differences were apparent in reimbursement of the diagnostic procedures and follow-up, in the procedures for PAP titration from home APAP titration with portable sleep apnea monitoring (38.1%) up to hospital monitoring with PSG and APAP (85.7%), and in the qualification requirements of sleep physicians. Conclusions: Management of OSA in different European countries is similar except for reimbursement rules, qualification of sleep specialists and procedures for titration of the CPAP treatment. A European network (such as the one accomplished by the European Cooperation in Science and Technology [COST] B26 Action) could be helpful for implementing these findings into health-service research in order to standardize management in a cost effective perspective.
