    • ObjectivesIn Phase 1 of developing new hand osteoarthritis (OA) classification criteria, features associated with hand OA were identified in a population with hand complaints. Radiographic findings could better discriminate patients with hand OA and controls than clinical examination findings. The objective of Phase 2 was to achieve consensus on the features and their weights to be included in three radiographic criteria sets of overall hand OA, interphalangeal OA and thumb base OA.MethodsMultidisciplinary, international expert panels were convened. Patient vignettes were used to identify important features consistent with hand OA. A consensus-based decision analysis approach implemented using 1000minds software was applied to identify the most important features and their relative importance influencing the likelihood of symptoms being due to hand OA. Analyses were repeated for interphalangeal and thumb base OA. The reliability and validity of the proposed criteria sets were tested.ResultsThe experts agreed that the criteria sets should be applied in a population with pain, aching or stiffness in hand joint(s) not explained by another disease or acute injury. In this setting, five additional criteria were considered important: age, morning stiffness, radiographic osteophytes, radiographic joint space narrowing and concordance between symptoms and radiographic findings. The reliability and validity were very good.ConclusionRadiographic features were considered critical when determining whether a patient had symptoms due to hand OA. The consensus-based decision analysis approach in Phase 2 complemented the data-driven results from Phase 1, which will form the basis of the final classification criteria sets.

  • The Hamburg/ESO R-process enhanced star survey (HERES) IV. Detailed abundance analysis and age dating of the strongly r-process enhanced stars CS 29491-069 and HE 1219-0312

    • We report on a detailed abundance analysis of two strongly r-process enhanced, very metal-poor stars newly discovered in the HERES project, CS 29491-069 ([Fe/H] = -2.51, [r/Fe] = +1.1) and HE 1219-0312 ([Fe/H] = -2.96, [r/Fe] = +1.5). The analysis is based on high-quality VLT/UVES spectra and MARCS model atmospheres. We detect lines of 15 heavy elements in the spectrum of CS 29491-069, and 18 in HE 1219-0312; in both cases including the Th II 4019 angstrom line. The heavy-element abundance patterns of these two stars are mostly well-matched to scaled solar residual abundances not formed by the s-process. We also compare the observed pattern with recent high-entropy wind (HEW) calculations, which assume core-collapse supernovae of massive stars as the astrophysical environment for the r-process, and find good agreement for most lanthanides. The abundance ratios of the lighter elements strontium, yttrium, and zirconium, which are presumably not formed by the main r-process, are reproduced well by the model. Radioactive dating for CS 29491-069 with the observed thorium and rare-earth element abundance pairs results in an average age of 9.5 Gyr, when based on solar r-process residuals, and 17.6 Gyr, when using HEW model predictions. Chronometry seems to fail in the case of HE 1219-0312, resulting in a negative age due to its high thorium abundance. HE 1219-0312 could therefore exhibit an overabundance of the heaviest elements, which is sometimes called an "actinide boost".

  • Physical activity after cardiac arrest; protocol of a sub-study in the Targeted Hypothermia versus Targeted Normothermia after Out-of-Hospital Cardiac Arrest trial (TTM2)

    • Aims: The primary aim of this study is to investigate whether out-of-hospital cardiac arrest (OHCA) survivors have lower levels of self-reported physical activity compared to a non-cardiac arrest (CA) control group who had acute myocardial infarction (MI). Additional aims are to explore potential predictors of physical inactivity (older age, female gender, problems with general physical function, global cognition, mental processing speed/attention, anxiety symptoms, depression symptoms, kinesiophobia, fatigue), and to investigate the relationship between self-reported and objectively measured physical activity among OHCA-survivors. Methods: The Targeted Hypothermia versus Targeted Normothermia after Out-of-Hospital Cardiac Arrest trial (TTM2-trial) collects information regarding age, gender, self-reported physical activity, general physical function, global cognition and mental processing speed/attention at 6 months after OHCA. In this TTM2-trial cross-sectional prospective sub-study, participants at selected sites are invited to an additional follow-up meeting within 4 weeks from the main study follow-up. At this meeting, information regarding anxiety symptoms, depression symptoms, kinesiophobia and fatigue is collected. The OHCA-survivors are then provided with an objective measure of physical activity, a hip-placed accelerometer, to wear for one week, together with a training diary. At the end of the week, participants are asked to once again answer two self-reported questions regarding physical activity for that specific week. MI-controls attend a single follow-up meeting and perform the same assessments as the OHCA-survivors, except from wearing the accelerometer. We aim to include 110 OHCA-survivors and 110 MI-controls in Sweden, Denmark and the United Kingdom. Conclusion: The results from this sub-study will provide novel information about physical activity among OHCA-survivors.

  • Thin-film design of amorphous hafnium oxide nanocomposites enabling strong interfacial resistive switching uniformity

    • A design concept of phase-separated amorphous nanocomposite thin films is presented that realizes interfacial resistive switching (RS) in hafnium oxide-based devices. The films are formed by incorporating an average of 7% Ba into hafnium oxide during pulsed laser deposition at temperatures <= 400 degrees C. The added Ba prevents the films from crystallizing and leads to similar to 20-nm-thin films consisting of an amorphous HfOx host matrix interspersed with similar to 2-nm-wide, similar to 5-to-10-nm-pitch Ba-rich amorphous nanocolumns penetrating approximately two-thirds through the films. This restricts the RS to an interfacial Schottky-like energy barrier whose magnitude is tuned by ionic migration under an applied electric field. Resulting devices achieve stable cycle-to-cycle, device-to-device, and sample-to-sample reproducibility with a measured switching endurance of >= 10(4) cycles for a memory window >= 10 at switching voltages of +/- 2 V. Each device can be set to multiple intermediate resistance states, which enables synaptic spike-timing-dependent plasticity. The presented concept unlocks additional design variables for RS devices.

    • Aim Fruit selection by animal dispersers with different mobility directly impacts plant geographical range size, which, in turn, may impact plant diversification. Here, we examine the interaction between fruit colour, range size and diversification rate in palms by testing two hypotheses: (1) species with fruit colours attractive to birds have larger range sizes due to high dispersal ability and (2) disperser mobility affects whether small or large range size has higher diversification, and intermediate range size is expected to lead to the highest diversification rate regardless of disperser.Location Global.Time Period Contemporary (or present).Major Taxa Studied Palms (Arecaceae).Methods Palm species were grouped based on likely animal disperser group for given fruit colours. Range sizes were estimated by constructing alpha convex hull polygons from distribution data. We examined disperser group, range size or an interaction of both as possible drivers of change in diversification rate over time in a likelihood dynamic model (Several Examined State-dependent Speciation and Extinction [SecSSE]). Models were fitted, rate estimates were retrieved and likelihoods were compared to those of appropriate null models.Results Species with fruit colours associated with mammal dispersal had larger ranges than those with colours associated with bird dispersal. The best fitting SecSSE models indicated that the examined traits were not the primary driver of the heterogeneity in diversification rates in the model. Extinction rate complexity had a marked impact on model performance and on diversification rates.Main Conclusions Two traits related to dispersal mobility, range size and fruit colour, were not identified as the main drivers of diversification in palms. Increased model extinction rate complexity led to better performing models, which indicates that net diversification should be estimated rather than speciation alone. However, increased complexity may lead to incorrect SecSSE model conclusions without careful consideration. Finally, we find palms with more mobile dispersers do not have larger range sizes, meaning other factors are more important determinants of range size.

    • Point-of-care diagnostic devices typically require six distinct qualities: they must deliver at least the same sensitivity and selectivity, and for a cost per assay no greater than that of today's central lab technologies, deliver results in a short period of time (<15 min at GP; <2h in hospital), be portable or at least small in scale, and require no or extremely little sample preparation. State-of-the-art devices deliver information of several markers in the same measurement.

    • We demonstrate a new technique for using geodetic data to update a priori predictionsfor Glacial Isostatic Adjustment (GIA) in the Fennoscandia region. Global PositioningSystem (GPS), tide gauge, and Gravity Recovery and Climate Experiment (GRACE)gravity rates are assimilated into our model. The technique allows us to investigate theindividual contributions from these data sets to the output GIA model in a self‐consistentmanner. Another benefit of the technique is that we are able to estimate uncertaintiesfor the output model. These are reduced with each data set assimilated. Any uncertaintiesin the GPS reference frame are absorbed by reference frame adjustments that are estimatedas part of the assimilation. Our updated model shows a spatial pattern and magnitudeof peak uplift that is consistent with previous models, but our location of peak upliftis slightly to the east of many of these. We also simultaneously estimate a spatiallyaveraged rate of local sea level rise. This regional rate (∼1.5 mm/yr) is consistent forall solutions, regardless of which data sets are assimilated or the magnitude of a priori GPSreference frame constraints. However, this is only the case if a uniform regional gravityrate, probably representing errors in, or unmodeled contributions to, the low‐degreeharmonic terms from GRACE, is also estimated for the assimilated GRACE data. Ourestimated sea level rate is consistent with estimates obtained using a more traditionalapproach of direct “correction” using collocated GPS and tide gauge sites.

  • Hearing outcome measures for conductive and mixed hearing loss treatment in adults: a scoping review

    • Objective Rehabilitation options for conductive and mixed hearing loss are continually expanding, but without standard outcome measures comparison between different treatments is difficult. To meaningfully inform clinicians and patients core outcome sets (COS), determined via a recognised methodology, are needed. Following our previous work that identified hearing, physical, economic and psychosocial as core areas of a future COS, the AURONET group reviewed hearing outcome measures used in existing literature and assigned them into different domains within the hearing core area. Design Scoping review. Study Sample Literature including hearing outcome measurements for the treatment of conductive and/or mixed hearing loss. Results The literature search identified 1434 studies, with 278 subsequently selected for inclusion. A total of 837 hearing outcome measures were reported and grouped into nine domains. The largest domain constituted pure-tone threshold measurements accounting for 65% of the total outcome measures extracted, followed by the domains of speech testing (20%) and questionnaires (9%). Studies of hearing implants more commonly included speech tests or hearing questionnaires compared with studies of middle ear surgery. Conclusions A wide range of outcome measures are currently used, highlighting the importance of developing a COS to inform individual practice and reporting in trials/research.

  • Unprecedented reorganization of holocentric chromosomes provides insights into the enigma of lepidopteran chromosome evolution

    • Chromosome evolution presents an enigma in the mega-diverse Lepidoptera. Most species exhibit constrained chromosome evolution with nearly identical haploid chromosome counts and chromosome-level gene collinearity among species more than 140 million years divergent. However, a few species possess radically inflated chromosomal counts due to extensive fission and fusion events. To address this enigma of constraint in the face of an exceptional ability to change, we investigated an unprecedented reorganization of the standard lepidopteran chromosome structure in the green-veined white butterfly (Pieris napi). We find that gene content in P. napi has been extensively rearranged in large collinear blocks, which until now have been masked by a haploid chromosome number close to the lepidopteran average. We observe that ancient chromosome ends have been maintained and collinear blocks are enriched for functionally related genes suggesting both a mechanism and a possible role for selection in determining the boundaries of these genome-wide rearrangements.
