  • Synthetic mRNA Encoding VEGF-A in Patients Undergoing Coronary Artery Bypass Grafting: Design of a Phase 2a Clinical Trial

    • Therapeutic angiogenesis may improve outcomes in patients with coronary artery disease undergoing surgical revascularization. Angiogenic factors may promote blood vessel growth and regenerate regions of ischemic but viable myocardium. Previous clinical trials of vascular endothelial growth factor A (VEGF-A) gene therapy with DNA or viral vectors demonstrated safety but not efficacy. AZD8601 is VEGF-A(165) mRNA formulated in biocompatible citrate-buffered saline and optimized for high-efficiency VEGF-A expression with minimal innate immune response. EPICCURE is an ongoing randomized, double-blind, placebo-controlled study of the safety of AZD8601 in patients with moderately decreased left ventricular function (ejection fraction 30% 50%) undergoing elective coronary artery bypass surgery. AZD8601 3 mg, 30 mg, or placebo is administered as 30 epicardial injections in a 10-min extension of cardioplegia. Injections are targeted to ischemic but viable myocardial regions in each patient using quantitative O-15-water positron emission tomography (PET) imaging (stress myocardial blood flow < 2.3 mL/g/min; resting myocardial blood flow > 0.6 mL/g/min). Improvement in regional and global myocardial blood flow quantified with O-15-water PET is an exploratory efficacy outcome, together with echocardiographic, clinical, functional, and biomarker measures. EPICCURE combines high-efficiency delivery with quantitative targeting and follow-up for robust assessment of the safety and exploratory efficacy of VEGF-A mRNA angiogenesis (ClinicalTrials.gov: NCT03370887).

  • Sclerosing Aortic and Coronary Arteritis Due to IgG4-Related Disease

    • We present the case of a 55-year-old woman admitted for a coronary artery bypass operation because of three-vessel coronary artery disease based on angiographic findings and clinical symptoms. Unexpected intraoperative findings with diffuse tissue thickening of the ascending aorta and coronary arteries indicated an alternate pathogenesis rather than coronary artery atherosclerosis. Histopathologic findings and clinical evaluation could confirm IgG4-related disease (IgG4-RD). IgG4-RD is a newly recognized fibroinflammatory condition that can present in a variety of organs and is characterized by common histopathologic features. Low disease awareness among clinicians makes this condition underdiagnosed. © 2017 The Society of Thoracic Surgeons

  • Wenlock metabentonites from Gotland, Sweden: geochemistry, sources and potential as chemostratigraphic markers

    • Two metabentonite suites occur within the Wenlock limestones and marls of Gotland, one within the Slite Formation (M. belophorus to the C. ellesae biozones) and the other in the Mulde Formation (G. nassaM. dubius Biozone). Their geochemical characteristics based on rare-earth element (REE) distributions in apatite crystals separated from the metabentonites suggest origins from three separate volcanic sources. One of these sources has an alkaline affinity, reflected in relatively high levels of Th, Nb and Zr, which suggests that it may have lain over thickening continental crust during the waning stages of the closure of the Tornquist Sea, while the other two represent calc-alkaline magmas. The source of the volcanic ash is placed some 400 km to the southwest of Gotland, on the TornquistTeysseyre Zone, which is the northernmost expression of the Trans-European Suture Zone. Distinctive differences in REE distribution in apatite from these metabentonites should assist in future correlation studies in Wenlock stratigraphy, both within the Baltic region and further afield.

  • Screening of some European medicinal plants for spasmolytic activity on isolated guinea-pig trachea

    • Eight European plants, used traditionally against diseases of the respiratory system, have been assayed for in vitro spasmolytic activity on guinea-pig trachea. The plants were collected or cultivated in the south of Sweden, and both water and 67% methanol extracts of the parts reported to be used in the traditional medicine were prepared and assayed. Five of the species did not show any significant activity at concentrations up to 0.6 mg extract per ml organ bath, the methanol extracts of 2 species (Glechoma hederacea and Hyssopus officinale) showed weak activity, while 0.6 mg/ml of the methanol extract of the aerial parts of Artemisia abrotanum relaxed the tone induced by 0.1 μmol/l carbachol almost (80%) as effectively as 2.2 mmol/l terbutaline.

    • Aim: The aim of this study was to evaluate the patterns of compliance and the frequency of adenomas and neoplasms in a Swedish population. Methods: In 1996, 2000 men and women born in 1935 or 1936 were selected at random from the population registers of Uppsala and Malmo/Lund. All subjects were invited by mail to participate. In a randomised study design, subjects were either called up by a nurse to schedule the appointment for sigmoidoscopy or instructed to call themselves. At sigmoidoscopy subjects with a cancer, an adenoma (neoplastic polyp) or more than three hyperplastic polyps were scheduled for a complete colonoscopy. Results: Thirty-nine percent (770/1988) of all the invited subjects had a sigmoidoscopy. The participation differed between the two centres, 47% at the Uppsala centre and 30% at the Malmo/Lund centre (P < 0.01). There was no statistically significant difference between the two different invitation groups. In all, 98 subjects (13%) were planned for colonoscopy. Thirty-one (35%) of the subjects having a colonoscopy were women and 57 (65%) were men. Fifty-five true adenomas were found in 46 subjects. All together, six subjects had proximal adenomas. Five adenocarcinomas were diagnosed, all within the reach of the sigmoidoscope. Conclusions: The compliance was lower and the adenomas were fewer than expected. To increase compliance it is necessary with rigorously controlled invitation routines. (C) 2002 Elsevier Science Ltd. All rights reserved.
