    • The impact of complement activation and its possible relation to cytokine responses during malaria pathology was investigated in plasma samples from patients with confirmed Plasmodium falciparum malaria and in human whole-blood specimens stimulated with malaria-relevant agents ex vivo. Complement was significantly activated in the malaria cohort, compared with healthy controls, and was positively correlated with disease severity and with certain cytokines, in particular interleukin 8 (IL-8)/CXCL8. This was confirmed in ex vivo-stimulated blood specimens, in which complement inhibition significantly reduced IL-8/CXCL8 release. P. falciparum malaria is associated with systemic complement activation and complement-dependent release of inflammatory cytokines, of which IL-8/CXCL8 is particularly prominent.

  • Tailored fluorouracil, epirubicin, and cyclophosphamide compared with marrow-supported high-dose chemotherapy as adjuvant treatment for high-risk breast cancer : A randomised trial

    • Background: Chemotherapy drug distribution varies greatly among individual patients. Therefore, we developed an individualised fluorouracil, epirubicin, cyclophosphamide (FEC) regimen to improve outcomes in patients with high-risk early breast cancer. We then did a randomised trial to compare this individually tailored FEC regimen with conventional adjuvant chemotherapy followed by consolidation with high-dose chemotherapy with stem-cell support. Methods: 525 women younger than 60 years of age with high-risk primary breast cancer were randomised after surgery to receive nine cycles of tailored FEC to haematological equitoxicity with granulocyte colony-stimulating factor (G-CSF) support (n=251), or three cycles of FEC at standard doses followed by high-dose chemotherapy with cyclophosphamide, thiotepa, and carboplatin (CTCb), and peripheral-blood stem-cell or bone-marrow support (n=274). Both groups received locoregional radiation therapy and tamoxifen for 5 years. The primary outcome measure was relapse-free survival, and analysis was by intention to treat. Findings: At a median follow-up of 34.3 months, there were 81 breast-cancer relapses in the tailored FEC group versus 113 in the CTCb group (double triangular method p=0.04). 60 deaths occurred in the tailored FEC group and 82 in the CTCb group (log-rank p=0.12). Patients in the CTCb group experienced more grade 3 or 4 acute toxicity compared with the tailored FEC group (p<0.0001). Two treatment-related deaths (0.7%) occurred in the CTCb group. Six patients in the tailored FEC group developed acute myeloid leukaemia and three developed myelodysplastic syndrome. Interpretation: Tailored FEC with G-CSF support resulted in a significantly improved relapse-free survival and fewer grade 3 and 4 toxicities compared with marrow-supported high-dose chemotherapy with CTCb as adjuvant therapy of women with high-risk primary breast cancer.

From an instructional design point of view aligned with the goals in governing curricula documents and real classroom constraints, we argue for the integration of the models and modelling perspective on teaching and learning mathematics with a data-modelling approach to facilitate students’ learning statistics and mathematical modelling. An application of the framework is given and future research discussed.

    • RATIONALE: Chronic obstructive pulmonary disease (COPD) includes respiratory symptoms and chronic airflow limitation (CAL). In some cases, emphysema and impaired diffusing capacity for carbon monoxide (DLCO) are present, but characteristics and symptoms vary with smoking exposure.OBJECTIVES: To study the prevalence of CAL, emphysema and impaired DLCO in relation to smoking and respiratory symptoms in a middle-aged population.METHODS: We investigated 28,746 randomly invited individuals (52% women) aged 50-64 years across six Swedish sites. We performed spirometry, DLCO, high-resolution computed tomography (HRCT) and asked for smoking habits and respiratory symptoms. CAL was defined as post-bronchodilator forced expiratory volume in 1 second divided by forced expiratory volume (FEV1/FVC)<0.7.RESULTS: The overall prevalence was for CAL 8.8%, for impaired DLCO (DLCOCONCLUSIONS: In this large population-based study of middle-aged people, CAL and impaired DLCO were associated with common respiratory symptoms. Self-reported asthma was not associated with CAL in never-smokers. Our findings suggest that CAL in never-smokers signifies a separate clinical phenotype that may be monitored and, possibly, treated differently from smoking-related COPD.   • Informell Statistisk Inferens i modelleringssituationer : En studie om utveckling av ett ramverk för att analysera hur elever uttrycker inferenser

    • Syftet med denna studie är att bidra med ökad kunskap om lärande och undervisning i informell statistisk inferens. I studien användes en kvalitativ forskningsstrategi inriktad mot prövning och generering av teorier med inspiration av grounded theory. Studiens kunskapsfokus är riktad mot karakterisering av statistiska processer och begrepp där system av begreppsramverk om informell statistisk inferens och modellering utgör en central del av forskningen. För att erhålla adekvat empiri utformades en undervisningssituation där elever engagerades med att planera och genomföra en undersökning. Studien genomfördes i en normal klassrumssituation där undervisningen inriktades mot ett område inom sannolikhet och statistisk där bland annat lådagram och normalfördelning med tillhörande begrepp introduceras. Det empiriska materialet samlades in genom videoinspelning och skriftliga redovisningar. Materialet analyserades genom ett sammansatt ramverk om informell statistisk inferens och modellering. Resultatet av analysen visar exempel på hur elever kan förväntas uttrycka aspekter av informella statistisk inferens då de genomför statistiska undersökningar. Vidare utvecklades ett ramverk som teoretiskt beskriver informell statistisk inferens i modelleringssituationer. Studien pekar på att ISI-modellering har potential att användas för att analysera hur informell statistisk inferens kan komma till uttryck och att identifiera potentiella inlärningsmöjligheter för studenter att utveckla sin förmåga att uttrycka informella statistisk slutledning och att identifiera potentiella inlärningsmöjligheter för elever att utveckla sin förmåga att uttrycka informella inferenser.
