  • Three-years clinical outcome of immediate provisionalization of single Osseospeed™ implants in extraction sockets and healed ridges

    • Aim The purpose of this prospective multicenter clinical study was to compare 3 years implant survival, bone and soft tissue changes following immediate loading using single Osseospeed™ implants (Astra Tech AB, Mölndal, Sweden) installed in healed ridges or extraction sockets. Material and methods Implants were provisionally restored the day of surgery using cemented acrylic crowns out of full occlusion. The provisional crowns were replaced after 12 weeks by full-ceramic crowns. Implant survival, bone levels, soft tissue levels and peri-implant health were monitored up to 3 years. Results 55 patients (22 men, 33 women; mean age 45) had the implant installed in extraction sockets and 58 patients (25 men, 33 women; mean age 42) received conventional implant treatment in healed ridges. Three implants failed in the extraction group and one implant failed in the healed group, all failures occurred before the final crown placement. No further losses occurred during 3 years, giving a statistically comparable survival rate of 94.6% and 98.3%, respectively. The total bone loss after 3 years, compared to implant placement was 0.4 mm (SD 1.5) in healed sites, whilst the immediate sites showed a bone gain of 1.6 mm (SD 2.4) due to bone fill in the alveolus. Papillae retracted with 0.3 and 0.0 mm in the period of provisionalization, but grew back after final crown was placed with 0.3 and 0.5 mm in extraction and healed sites respectively after 3 years. Plaque and inflammation scores were very low throughout the study time, irrespective of treatment modality. Conclusions Immediate implants restored at the day of surgery show comparable risk for implant failure, bone loss and midfacial soft tissue recession compared to conventionally installed implants. The 3-years results suggest both hard and soft tissue stability.

  • Long-Term Follow-Up of Turned Single Implants Placed in Periodontally Healthy Patients After 16 to 22 Years : Microbiologic Outcome

    • Background: Survival rates in implant dentistry today are high, although late failures do occur for many reasons, including peri-implant infections. The primary objective of this study is to investigate microbiota around single turned implants after 16 to 22 years. Secondary objectives are to compare teeth and implants and to correlate microbiologic, radiographic, and clinical parameters. Methods: A total of 46 patients with single implants were invited for a clinical examination. Clinical datawere collected from implants and contralateral natural teeth. Radiographic bone level was measured around implants. Microbiologic samples were taken from implants, contralateral teeth, and the deepest pocket per quadrant. Samples were analyzed with DNA-DNA hybridization including 40 species. Statistical analysis was performed using Wilcoxon signed-rank tests, McNemar tests, and Spearman correlation coefficients with a 0.05 significance level. Results: Mean follow-up was 18.5 years (range 16 to 22 years). Tannerella forsythia (1.5 x 10(5)) and Veillonella parvula (1.02 x 10(5)) showed the highest concentrations around implants and teeth, respectively. Porphyromonas gingivalis, Prevotella intermedia, and T. forsythia were significantly more present around implants than teeth. Mean counts were significantly higher around implants than teeth for Parvimonas micra, P. gingivalis, P. intermedia, T. forsythia, and Treponema denticola. Total DNA count was correlated to interproximal bleeding index (r = 0.409) and interproximal probing depth (r = 0.307). No correlations were present with plaque index or radiographic bone level. Conclusions: In the present study, bacterial counts around single implants in periodontally healthy patients are rather low. Although pathogenic bacteria are present, some in higher numbers around implants than teeth (five of 40), the majority of implants present with healthy peri-implant tissues without progressive bone loss.

    • Detecting redshifted 21-cm emission from neutral hydrogen in the early Universe promises to give direct constraints on the epoch of reionization (EoR). It will, though, be very challenging to extract the cosmological signal (CS) from foregrounds and noise which are orders of magnitude larger. Fortunately, the signal has some characteristics which differentiate it from the foregrounds and noise, and we suggest that using the correct statistics may tease out signatures of reionization. We generate mock data cubes simulating the output of the Low Frequency Array (LOFAR) EoR experiment. These cubes combine realistic models for Galactic and extragalactic foregrounds and the noise with three different simulations of the CS. We fit out the foregrounds, which are smooth in the frequency direction, to produce residual images in each frequency band. We denoise these images and study the skewness of the one-point distribution in the images as a function of frequency. We find that, under sufficiently optimistic assumptions, we can recover the main features of the redshift evolution of the skewness in the 21-cm signal. We argue that some of these features - such as a dip at the onset of reionization, followed by a rise towards its later stages - may be generic, and give us a promising route to a statistical detection of reionization.

    • One of the problems facing experiments designed to detect redshifted 21-cm emission from the epoch of reionization (EoR) is the presence of foregrounds which exceed the cosmological signal in intensity by orders of magnitude. While fitting them so that they can be removed, we must be careful to minimize `overfitting', in which we fit away some of the cosmological signal, and `underfitting', in which real features of the foregrounds cannot be captured by the fit, polluting the signal reconstruction. We argue that in principle it would be better to fit the foregrounds non-parametrically - allowing the data to determine their shape - rather than selecting some functional form in advance and then fitting its parameters. Non-parametric fits often suffer from other problems, however. We discuss these before suggesting a non-parametric method, Wp smoothing, which seems to avoid some of them. After outlining the principles of Wp smoothing, we describe an algorithm used to implement it. Some useful results for implementing an alternative algorithm are given in an appendix. We apply Wp smoothing to a synthetic data cube for the Low Frequency Array (LOFAR) EoR experiment. This cube includes realistic models for the signal, foregrounds, instrumental response and noise. The performance of Wp smoothing, measured by the extent to which it is able to recover the variance of the cosmological signal and to which it avoids the fitting residuals being polluted by leakage of power from the foregrounds, is compared to that of a parametric fit, and to another non-parametric method (smoothing splines). We find that Wp smoothing is superior to smoothing splines for our application, and is competitive with parametric methods even though in the latter case we may choose the functional form of the fit with advance knowledge of the simulated foregrounds. Finally, we discuss how the quality of the fit is affected by the frequency resolution and range, by the characteristics of the cosmological signal and by edge effects.

    • One of the aims of the Low Frequency Array (LOFAR) Epoch of Reionization (EoR) project is to measure the power spectrum of variations in the intensity of redshifted 21-cm radiation from the EoR. The sensitivity with which this power spectrum can be estimated depends on the level of thermal noise and sample variance, and also on the systematic errors arising from the extraction process, in particular from the subtraction of foreground contamination. We model the extraction process using realistic simulations of the cosmological signal, the foregrounds and noise, and so estimate the sensitivity of the LOFAR EoR experiment to the redshifted 21-cm power spectrum. Detection of emission from the EoR should be possible within 360 h of observation with a single station beam. Integrating for longer, and synthesizing multiple station beams within the primary (tile) beam, then enables us to extract progressively more accurate estimates of the power at a greater range of scales and redshifts. We discuss different observational strategies which compromise between depth of observation, sky coverage and frequency coverage. A plan in which lower frequencies receive a larger fraction of the time appears to be promising. We also study the nature of the bias which foreground fitting errors induce on the inferred power spectrum and discuss how to reduce and correct for this bias. The angular and line-of-sight power spectra have different merits in this respect, and we suggest considering them separately in the analysis of LOFAR data.

    • Objectives: This publication reports the EAO Workshop group-2 and consensus plenary discussions and statements on a narrative review providing the background and possible facilities and importance of a dental implant register, to allow for a systematic follow-up of the clinical outcome of dental implant treatment in various clinical settings. It should be observed that the format of the review and the subsequent consensus report consciously departs from conventional consensus publications and reports. Material and methods: The publication was a narrative review on the presence and significance of quality registers regarding select medical conditions and procedures. The group discussed and evaluated the publication and made corrections and recommendations to the authors and agreed on the statements and recommendations described in this consensus report. Results: Possible registrations to be included in an implant register were discussed and agreed as a preliminary basis for further development, meaning that additional parameters be included or some be deleted. Conclusions: It was agreed to bring the idea of an implant quality register, including the presented results of discussions and proposals by the group- and plenary sessions, to the EAO Board for further discussion and decision.

  • Somatosensory Impairments in the Upper Limb Poststroke: Distribution and Association With Motor Function and Visuospatial Neglect

    • Background. A thorough understanding of the presence of different upper-limb somatosensory deficits poststroke and the relation with motor performance remains unclear. Additionally, knowledge about the relation between somatosensory deficits and visuospatial neglect is limited. Objective. To investigate the distribution of upper-limb somatosensory impairments and the association with unimanual and bimanual motor outcomes and visuospatial neglect. Methods. A cross-sectional observational study was conducted, including 122 patients within 6 months after stroke (median = 82 days; interquartile range = 57-133 days). Somatosensory measurement included the Erasmus MC modification of the (revised) Nottingham Sensory Assessment (Em-NSA), Perceptual Threshold of Touch (PTT), thumb finding test, 2-point discrimination, and stereognosis subscale of the NSA. Upper-limb motor assessment comprised the Fugl-Meyer assessment, motricity index, Action Research Arm Test, and Adult-Assisting Hand Assessment Stroke. Screening for visuospatial neglect was performed using the Star Cancellation Test. Results. Upper-limb somatosensory impairments were common, with prevalence rates ranging from 21% to 54%. Low to moderate Spearman ρ correlations were found between somatosensory and motor deficits ( r = 0.22-0.61), with the strongest associations for PTT ( r = 0.56-0.61) and stereognosis ( r = 0.51-0.60). Visuospatial neglect was present in 27 patients (22%). Between-group analysis revealed somatosensory deficits that occurred significantly more often and more severely in patients with visuospatial neglect ( P < .05). Results showed consistently stronger correlations between motor and somatosensory deficits in patients with visuospatial neglect ( r = 0.44-0.78) compared with patients without neglect ( r = 0.08-0.59). Conclusions. Somatosensory impairments are common in subacute patients poststroke and are related to motor outcome. Visuospatial neglect was associated with more severe upper-limb somatosensory impairments.

  • Constraining the epoch of reionization with the variance statistic : simulations of the LOFAR case

    • Several experiments are underway to detect the cosmic-redshifted 21-cm signal from neutral hydrogen from the Epoch of Reionization (EoR). Due to their very low signal-to-noise ratio, these observations aim for a statistical detection of the signal by measuring its power spectrum. We investigate the extraction of the variance of the signal as a first step towards detecting and constraining the global history of the EoR. Signal variance is the integral of the signal's power spectrum, and it is expected to be measured with a high significance. We demonstrate this through results from a simulation and parameter estimation pipeline developed for the Low-Frequency Array (LOFAR)-EoR experiment. We show that LOFAR should be able to detect the EoR in 600 h of integration using the variance statistic. Additionally, the redshift (z(r)) and duration (Delta z) of reionization can be constrained assuming a parametrization. We use an EoR simulation of z(r) = 7.68 and Delta(z) = 0.43 to test the pipeline. We are able to detect the simulated signal with a significance of four standard deviations and extract the EoR parameters as z(r) = 7.72(-0.18)(+0.37) and Delta z = 0.53(-0.23)(+0.12) in 600 h, assuming that systematic errors can be adequately controlled. We further show that the significance of detection and constraints on EoR parameters can be improved by measuring the cross-variance of the signal by cross-correlating consecutive redshift bins.
