  • Fear of Movement Is Related to Low Back Disability During a Two-Year Period in Patients Who Have Undergone Elective Lumbar Spine Surgery

    • Objective: To evaluate change in fear of movement and the relationship of fear of movement and pain intensity to low back disability and general health-related quality of life over a 2-year period. Methods: Consecutive patients scheduled for lumbar spine surgery were included. In addition to clinical background variables, back pain intensity, fear of movement, low back disability, and general health-related quality of life were assessed at baseline, 1 year, and 2 years after surgery. Linear mixed-effects models were used to analyze data. Results: In total, 348 patients were included in the final analyses. There was a significant reduction in fear of movement and a significant interaction between fear of movement and low back disability across assessments, showing that greater levels of fear of movement were related to greater levels of disability over the 2-year period. Similarly, greater levels of back pain intensity were related to lower levels of general health-related quality of life during this period. Conclusions: We found that greater levels of fear of movement were related to greater levels of low back disability, following lumbar spine surgery, in a longitudinal study. This shows the need to address fear of movement in prehabilitation/rehabilitation pre- or postsurgically to improve health outcomes for patients who undergo lumbar spine surgery. © 2020 Elsevier Inc.

  • Is low molecular hyaluronan an early indicator of disease in avian systemic sclerosis?

    • AIM OF THE STUDY: To further elucidate the pathogenesis of systemic sclerosis (SSc) an experimental avian model was used. University of California at Davies line 200-chicken (UCD-200) spontaneously develops a SSc like disease that has most features of human SSc with vascular effects, inflammation, autoimmunity and fibrosis. The first signs of disease in UCD-200 are swelling and ischemic lesions of the comb, a tissue containing high amounts of the glycosaminoglycan hyaluronan. The aim was to evaluate inflammatory and fibrotic processes of the disease with regard to the molecular weight of hyaluronan.MATERIAL AND METHODS: Comb biopsies from UCD-200 and healthy White Leghorn (WL) chickens as controls at different ages were studied with histochemical localization of hyaluronan, hyaluronidase 1, CD3, IgY and collagen I and III. Hyaluronan molecular weight distribution was estimated with gas phase electrophoretic analysis.RESULTS: At 2 days of age hyaluronan was visualized in UCD-200 at the dermal part of the comb with no simultaneous staining of Hyal-1. In adult UCD-200 the comb skin was almost totally devoid of hyaluronan compared to WL of the same age. An increase of low molecular weight (LMW) hyaluronan was detected in comb tissue from UCD-200 at 1 day, 1, 2, 4 weeks in contrast to adult animals.CONCLUSIONS: An early inflammatory process involving LMW hyaluronan was confirmed as a possible profibrotic process. This indicates that hyaluronan might be an important participant in early inflammatory events of SSc in UCD-200 chicken and that disappearance of hyaluronan in skin predisposes to fibrosis.

  • Contamination of polyethylene cups with polymethyl methacrylate particles: an experimental study

    • The articulating surfaces of 6 ultra-high molecular weight polyethylene cups were exposed to curing polymethyl methacrylate (PMMA) bone-cement and examined with scanning electron microscopy and laser ablation inductively coupled plasma mass spectrometry (LA-ICPMS). Three of the cups were exposed to blood and bone-cement, and the rest were exposed to bone-cement only. After removal of the bone-cement bulk, PMMA particles were found and identified in all 6 cups. The particles were verified by identifying zirconium with energy-dispersive x-ray fluorescence spectroscopy in 5 cups and with LA-ICPMS in 1 cup. The degree of surface contamination was estimated with LA-ICPMS. The number of zirconium-containing particles detected was on average 10 to 20/mm2. PMMA bone-cement left in polyethylene cups during polymerization can contaminate the articulating surface with adherent PMMA particles.

  • Cumulative revision rate with the Scan Hip Classic I total hip prosthesis. 1,660 cases followed for 2-12 years

    • We analyzed the cumulative revision rate in 1,474 patients (1,660 hips) operated on with a cemented Scan Hip Classic I prosthesis from November 1983 to January 1994 at Lund University Hospital. The revision rate was analyzed for 3 diagnoses--arthrosis, rheumatoid arthritis and complication after a hip fracture--and for 2 head diameters--22 and 32 mm. Until January 1996, 36 hips were revised: 31 because of aseptic loosening, 3 because of dislocation and 2 because of infection. The overall revision rate was 5.6% after 10 years and was similar in arthrosis, rheumatoid arthritis and fracture cases. Due to revisions because of dislocation in the 22 mm group, the total revision rate was lower in the 32 mm group (p = 0.03).

  • Polyethylene wear and synovitis in total hip arthroplasty: a sonographic study of 48 hips

    • Forty-six patients (48 hips), operated on with cemented total hip arthroplasty (THA) because of arthrosis, were examined radiographically and sonographically at 10-year follow-up. Polyethylene wear of acetabular cups was measured on conventional non-weight-bearing pelvic radiographs, and the volume of polyethylene debris was calculated. Radiographic signs of loosening were identified. The capsular distance (ie, thickness of the synovium or synovial contents) was measured sonographically. We found a significant correlation between increased volumetric wear and increased capsular distance. Hips with radiographically loose acetabular components had significantly greater volumetric wear and capsular distance than those without signs of acetabular loosening. This relationship was not observed in hips with radiographically loose femoral components. In cemented THA, the volume of polyethylene wear debris and the thickness of the synovium and the synovial contents are related. In the event of radiographic loosening of the acetabular component, they are both increased.

    • During revision total hip replacement using morcellised compacted bone allograft, 16 patients were randomised to receive a graft which had been rinsed in either an ibandronate solution or in saline. Patients were assessed by dual energy x-ray absorptiometry after operation and at 3, 6, 12 and 24 months. A region of interest between the tip of the femoral stem and the distal plastic plug was chosen to measure the changes in bone density over time. The study was double-blinded. In all the control patients the bone density decreased during the first three months and then remained constant at this lower level. A large proportion of the mass of the bone graft was lost. In contrast, all patients with grafts treated with bisphosphonate showed a slight increase in bone density. The difference between the groups was highly significant at all points in time. We conclude that rinsing the graft in a bisphosphonate solution prevents its resorption and may therefore reduce the risk of mechanical failure. The treatment is simple, inexpensive, and appears virtually free of risk. ©2006 British Editorial Society of Bone and Joint Surgery.

  • Wear and Loosening in Cemented Hip Arthroplasty

    • The mechanisms of loosening in total hip arthroplasty (THA) and the role of polyethylene (PE) wear debris in this process is unclear. Radiographic measurements showed that THA with 32 mm heads have about 3 times greater volumetric wear than those with 22 mm heads. If PE debris were the main cause of loosening, then there should be an obvious difference in revision rates between the prostheses with these two head sizes. However, the analysis of revision rates, due to aseptic loosening, showed no significant difference. These findings do not support the hypothesis that wear debris is the main cause of loosening. The intracapsular pressure and ultrasonographic synovitis in THA with loose components was studied. The pressure was high and with cyclic alterations dependent on hip joint movements. The synovitis in loose THA was more pronounced than in stable ones. There was a significant correlation between the intracapsular pressure with the hip in extension with inward rotation and synovitis. These results suggest that increased pressure and synovitis is important in loosening of THA. The correlation between synovitis, wear and radiographic loosening was studied. There was a significant correlation between volumetric wear and synovitis. Hips with cup loosening had significantly greater wear and synovitis than the others. In THA with loose cups the PE wear was higher than in those with loose stems. This indicates that PE debris is of certain importance in loosening of acetabular components, but of less significance in loosening of stems. The experimental study showed that bone cement, if left inside PE cups during curing, contaminates the articulating surface of the cups with cement debris. In clinical situations such contamination may predispose to increased wear.
