    • Starting from interactions with SMEs from a number of different areas, this short paper addresses the challenges of turning hands-on practice into an online supports tool for innovation. The SMEs are found in the North of Finland and the North of Sweden, thus already facing challenges related to the sparsely populated areas. Yet, digitalization and online support may become one way to bridge distances as well as filling knowledge gaps between different domains. Nevertheless, the suggestion to transform practical know-how into an online tool that can be globally accessed and used is non-trivial. Thus, problematizing such ideas could progress these efforts.

    • Fiber composites are widely used for space applications such as antennas, solar panels and spacecraft support structures. This paper presents a combined electromagnetic and acoustic analysis of a triaxial carbon fiber weave structure, designed for ultra lightweight reflector antennas in satellite communication systems. The electromagnetic and acoustic performance of the structure are analyzed over a wide range of parametric studies, both at a microscopic and mesoscopic length scale. The electromagnetic study indicates that the main parameter governing the electromagnetic reflection performance of the weave is the electric conductivity of the carbon fibers, given that the weave structure is significantly smaller than the wavelength of the incident signals. The acoustic study identifies a critical threshold in the mesoscale geometry in order to avoid a critically high resistive behavior of the weave structure, driven by viscous effects. Design guidelines are drawn from these analyses in order to achieve a trade-off between the electromagnetic reflection properties and the resistance to acoustic loading of such composite materials. These combined analyses allow to deepen the understanding from both an electromagnetic and acoustic perspective in order to open for some new design possibilities.

    • The aim of this article is to suggest recommendations for the management of fatigue in fibromyalgia (FM) by reviewing and compiling findings of nonpharmacological treatment for fatigue in patients with FM. The management of fatigue in FM should incorporate regular physical exercise maintained for a longer period of time, which contributes to improved general health and facilitates coping with fatigue and other symptoms. The evidence is still low and the effect of exercise on fatigue in FM needs to be further studied. Patients with FM appear to benefit from general sleep hygiene advice and also need to develop individual strategies to manage the complex factors in everyday life that may be related to fatigue.

  • Cortical gyrification in women and men and the (missing) link to prenatal androgens

    • Previous studies have reported sex differences in cortical gyrification. Since most cortical folding is principally defined in utero, sex chromosomes as well as gonadal hormones are likely to influence sex-specific aspects of local gyrification. Classic congenital adrenal hyperplasia (CAH) causes high levels of androgens during gestation in females, whereas levels in males are largely within the typical male range. Therefore, CAH provides an opportunity to study the possible effects of prenatal androgens on cortical gyrification. Here, we examined the vertex-wise absolute mean curvature—a common estimate for cortical gyrification—in individuals with CAH (33 women and 20 men) and pair-wise matched controls (33 women and 20 men). There was no significant main effect of CAH and no significant CAH-by-sex interaction. However, there was a significant main effect of sex in five cortical regions, where gyrification was increased in women compared to men. These regions were located on the lateral surface of the brain, specifically left middle frontal (rostral and caudal), right inferior frontal, left inferior parietal, and right occipital. There was no cortical region where gyrification was increased in men compared to women. Our findings do not only confirm prior reports of increased cortical gyrification in female brains but also suggest that cortical gyrification is not significantly affected by prenatal androgen exposure. Instead, cortical gyrification might be determined by sex chromosomes either directly or indirectly—the latter potentially by affecting the underlying architecture of the cortex or the size of the intracranial cavity, which is smaller in women.

  • Clinicians' views of factors of importance for improving the rate of VBAC (vaginal birth after caesarean section) : a study from countries with low VBAC rates.

    • BACKGROUND: Caesarean section (CS) rates are increasing worldwide and the most common reason is repeat CS following previous CS. For most women a vaginal birth after a previous CS (VBAC) is a safe option. However, the rate of VBAC differs in an international perspective. Obtaining deeper knowledge of clinicians' views on VBAC can help in understanding the factors of importance for increasing VBAC rates. Focus group interviews with clinicians and women in three countries with high VBAC rates (Finland, Sweden and the Netherlands) and three countries with low VBAC rates (Ireland, Italy and Germany) are part of "OptiBIRTH", an ongoing research project. The study reported here aims to explore the views of clinicians from countries with low VBAC rates on factors of importance for improving VBAC rates.METHODS: Focus group interviews were held in Ireland, Italy and Germany. In total 71 clinicians participated in nine focus group interviews. Five central questions about VBAC were used and interviews were analysed using content analysis. The analysis was performed in each country in the native language and then translated into English. All data were then analysed together and final categories were validated in each country.RESULTS: The findings are presented in four main categories with several sub-categories: 1) "prameters for VBAC", including the importance of the obstetric history, present obstetric factors, a positive attitude among those who are centrally involved, early follow-up after CS and antenatal classes; 2) "organisational support and resources for women undergoing a VBAC", meaning a successful VBAC requires clinical expertise and resources during labour; 3) "fear as a key inhibitor of successful VBAC", including understanding women's fear of childbirth, clinicians' fear of VBAC and the ways that clinicians' fear can be transferred to women; and 4) "shared decision making - rapport, knowledge and confidence", meaning ensuring consistent, realistic and unbiased information and developing trust within the clinician-woman relationship.CONCLUSIONS: The findings indicate that increasing the VBAC rate depends on organisational factors, the care offered during pregnancy and childbirth, the decision-making process and the strategies employed to reduce fear in all involved.

    • Physical indicators have been the measure of the success of childbirth by professionals educated in a medical perspective, while the experiences of the childbearing families in different cultures have been considered of minor importance. The medicalization has the consequence of regarding every birth as potentially risky and abnormal, and requiring medical interventions. However, life changes for good, especially for women embodied through their childbirth, and the memories of the events and caring and uncaring encounters are well kept. The childbirth experiences also influence the relationships with their baby and their partner in the postpartum period, and the onset of postnatal depression, anxiety and PSTD. Several qualitative studies have described women’s experiences of childbirth, care and support in varied ways and a metasynthesis is warranted to integrate the findings. The aim is also to reflect on the findings and the theories and the methods that implicit or explicit have guided the research. The intention is to influence clinical practice, and also to push the theory and the research designs, and ask new meaningful research questions. The threefold interpretative metastudy developed by Paterson and colleagues was chosen for this collaborative metasynthesis research program in childbearing. Inclusion and exclusion criteria were developed, and seven health related databases were searched in different disciplines and findings from different cultures with the chosen keywords. Previous literature reviews were reviewed, and author and ancestry search was performed to access studies not identified through the database search. The presentation will focus on the preliminary findings from this study.
