  • Omics and Multi-Omics Analysis for the Early Identification and Improved Outcome of Patients with Psoriatic Arthritis

    • The definitive diagnosis and early treatment of many immune-mediated inflammatory diseases (IMIDs) is hindered by variable and overlapping clinical manifestations. Psoriatic arthritis (PsA), which develops in similar to 30% of people with psoriasis, is a key example. This mixed-pattern IMID is apparent in entheseal and synovial musculoskeletal structures, but a definitive diagnosis often can only be made by clinical experts or when an extensive progressive disease state is apparent. As with other IMIDs, the detection of multimodal molecular biomarkers offers some hope for the early diagnosis of PsA and the initiation of effective management and treatment strategies. However, specific biomarkers are not yet available for PsA. The assessment of new markers by genomic and epigenomic profiling, or the analysis of blood and synovial fluid/tissue samples using proteomics, metabolomics and lipidomics, provides hope that complex molecular biomarker profiles could be developed to diagnose PsA. Importantly, the integration of these markers with high-throughput histology, imaging and standardized clinical assessment data provides an important opportunity to develop molecular profiles that could improve the diagnosis of PsA, predict its occurrence in cohorts of individuals with psoriasis, differentiate PsA from other IMIDs, and improve therapeutic responses. In this review, we consider the technologies that are currently deployed in the EU IMI2 project HIPPOCRATES to define biomarker profiles specific for PsA and discuss the advantages of combining multi-omics data to improve the outcome of PsA patients.

    • Several methods have been proposed to model communication in systems with coherent caches, e.g. multi-cores, however they usually incur a large overhead on the application being analyzed. In this work we describe a low-overhead statistical communication model that is driven by a sparse sample of the memory accesses in the target application. Our model allows detection of hot-spots where coherence communication occurs between different threads in an application. Preliminary results suggest that we are able to detect most of the communication hot-spots in real applications with lower overhead than previously proposed models.

    • An important aspect of Indian women's political participation in the nationalist struggle against colonial rule was their imprisonment and confinement within the walls of the prison. To counter the difficulty and monotony of their prison existence, women developed strong solidarity networks which not only helped them to adjust to the temporary upheaval in their lives but also resulted in their becoming strong and determined individuals with a nationalist consciousness. These women resisted colonial rule through imprisonment and activities in the jail (such as writing poetry) just as they did through nationalist activities within the domestic sphere (such as spinning and weaving). The jail became a site where identities were continuously shaped and restructured. Feelings of pride, resentment, honour and humiliation were all experienced by women prisoners and were continuously sharpened. Women's entry into male dominated spaces dispelled the British stereotypes about Indian women as subordinate, weak and docile. Women were also aware that by endangering their womanhood on the streets and putting their bodies under risk of attack, they proved that they could share common experiences with their fellow men in the public sphere.

    • Diabet. Med. 29, 10551059 (2012) Aim To examine the availability of insulin pump therapy in patients with Type 1 diabetes. Methods Patients using insulin pumps among a cohort of 7224 patients with Type 1 diabetes were studied. Results In logistic regression, used to evaluate variables not changing over time among the total cohort, use of insulin pumps varied by outpatient clinic (P < 0.001) and sex (P < 0.001). Cox regression analysis in 5854 patients with detailed patient data prior to use of an insulin pump showed higher HbA1c (P < 0.0001), lower creatinine (P = 0.002), high and low insulin doses (P < 0.0001), younger age (P < 0.0001) and female sex (P < 0.0001) to be associated with use of an insulin pump. Women were 1.5-fold more likely to start using an insulin pump (hazard ratio 1.52, 95% confidence interval 1.291.79) and patients in the 20- to 30-years age range were more than twice as likely to begin use of an insulin pump than patients aged 4050 years (hazard ratio 8.63, 95% confidence interval 5.9112.59 and hazard ratio 3.98, 95% confidence interval 2.805.64, respectively). A 10-mu mol/l higher level of creatinine was associated with a hazard ratio of 0.56 (95% confidence interval 0.390.81) of starting use of an insulin pump. Conclusions At 10 hospital outpatient clinics in Sweden, use of insulin pumps therapy varied by clinic. A higher proportion of women began using insulin pumps. Younger patients and patients with fewer complications were also more likely to start using an insulin pump. Further research is needed to confirm these findings in other geographical regions and to understand whether the availability of insulin pumps today is optimized.

    • This brief derives efficient two-stage-based polyphase structures for arbitrary-integer sampling rate conversion. For even-integer conversions, the overall structures correspond to parallelized conventional two-stage structures, but the derivations in this brief offer further insights when comparing the two cases of odd-and even-integer conversions. For the class of linear-phase finite-length impulse response Mth-band filters, it is then demonstrated through design examples that conversions by odd factors are in fact more efficient than by even factors.

    • The effect of different concentrations of cysteine (0.125, 0.25, 0.5 and 1 mM) on the net efflux of endogenous amino acids was studied by the incubation of rat hippocampal slices. Addition of cysteine (1 mM) in bicarbonate containing low K+ medium (5 min) selectively increased the basal net efflux of glutamate and aspartate by 370% and 396%, respectively. High K+ media (50 mM) containing cysteine (1 mM) evoked the net efflux of glutamate and aspartate by 1 454% and 1 019%, respectively. The corresponding effects in control slices without cysteine were 669% and 404%, respectively. No changes were observed on the concentrations of GABA, glutamine and taurine. The cysteine oxidation products, cysteine sulfinate (0.5 μM) and cystine (0.25 mM) were without effects. The effect of cysteine (0.5 mM) was dramatically reduced in media with no added bicarbonate/CO2. Thus, cysteine in a bicarbonate-sensitive manner selectively increases the extracellular concentration of excitotoxic amino acids in adult rat brain in vitro, possibly by interfering with the carrier-mediated glutamate uptake/ release
