Living with heart failure : Depression and quality of life in patients and spouses

- Background: Although spouses are a key support for patients with heart failure, and help them remain in the community, no one has studied patient–spouse pairs to determine the nature of their experience. Therefore, we conducted a study of patients and spouses to compare their levels of depression and health-related quality of life (HRQOL), and to identify factors that contribute to depression and HRQOL in patient–spouse pairs. Methods: Forty-eight couples, in which all patients were men with heart failure, were recruited from a university-affiliated, outpatient heart failure clinic. Data were collected using the Beck Depression Inventory, the 12-item Short Form (that measures physical and mental components of QOL), and the 6-minute walk test. Results: Patients with heart failure were significantly more depressed and had poorer physical quality of life compared with spouses. Patients’ depression was correlated with their own functional status and mental quality of life, with the combination of 6-minute walk distance and mental QOL contributing 51% of the variance in patient depression. Spouse depression and HRQOL did not significantly influence patient depression. In contrast, spouses’ depression was related to their husbands’ functional status and employment, as well as their own mental QOL. The mental component of spouse QOL and the age of the patient accounted for 33% of the adjusted variance in spousal depression. Conclusions: Patients with heart failure and their spouses experience significantly different levels of depression and physical QOL. In developing interventions, it may be important to take these differences into account and focus on their unique needs as well as those issues that affect the couple together. Interventions that improve patient functional status may result in decreased depression and improved HRQOL on the part of both patients and spouses.

- In the last decade a new variety of nature inspired optimization algorithms has been appeared. After the swarm based models, researchers turned their inspiration in nature phenomena and laws of science. In this way a new category of algorithms was born, equally effective or even sometimes superior to known algorithms for optimization problems, like genetic algorithms and swarm intelligence schemes. The present survey depicts the evolution of research on nature inspired optimization algorithms related to physical phenomena and laws of science and discusses the possibilities of using the presented approaches in a number of different applications. An attempt has been made to draw conclusions on what algorithm could be used in which different problem areas, for those approaches that this information could be extracted from the related papers studied. The paper also underlines the usage of this kind of nature inspired algorithms in industrial research problems, due to their better confrontation with optimization problems represented with nodes and edges.

- Digital Twin (DT) is a concept that has become increasingly relevant in recent years. The basic idea is to connect (physical) facilities or devices with digital representations to monitor, manipulate and predict their behavior in real-time. This work presents a modeling approach supporting DT engineering for air conditioning facilities. We describe the architecture of our system for developing digital representations of physically existing facilities in an industrial use case. The paper focuses on (a) the developed modeling tool, (b) the cloud service configuration, and (c) data analysis that enables energy optimizations.

- Ti-Si-C-Ag-Pd and Ti-Si-C-Ag nanocomposite coatings were deposited by direct-current magnetron sputtering on Cu substrates with an electroplated Ni layer. Analytical electron microscopy, x-ray diffraction, and x-ray photoelectron spectroscopy show that the nanocomposites consist of TiC, Ag:Pd, and amorphous SiC. The contact resistance of these coatings against a spherical Au-Co surface was measured for applied contact forces up to 5 N. Ti-Si-C-Ag-Pd coatings with Ag:Pd top coating had similar to 10 times lower contact resistance at contact forces below 1 N (similar to 10 m Omega at similar to 0.1 N), and 2 to 3 times lower for contact forces around 5 N (< 1 m Omega at 5 N), compared with the Ti-Si-C-Ag coating.

- Autoimmune polyendocrine syndrome type 1 (APS1), a monogenic disorder caused by AIRE gene mutations, features multiple autoimmune disease components. Infertility is common in both males and females with APS1. Although female infertility can be explained by autoimmune ovarian failure, the mechanisms underlying male infertility have remained poorly understood. We performed a proteome-wide autoantibody screen in APS1 patient sera to assess the autoimmune response against the male reproductive organs. By screening human protein arrays with male and female patient sera and by selecting for gender-imbalanced autoantibody signals, we identified transglutaminase 4 (TGM4) as a male-specific autoantigen. Notably, TGM4 is a prostatic secretory molecule with critical role in male reproduction. TGM4 autoantibodies were detected in most of the adult male APS1 patients but were absent in all the young males. Consecutive serum samples further revealed that TGM4 autoantibodies first presented during pubertal age and subsequent to prostate maturation. We assessed the animal model for APS1, the Aire-deficient mouse, and found spontaneous development of TGM4 autoantibodies specifically in males. Aire-deficient mice failed to present TGM4 in the thymus, consistent with a defect in central tolerance for TGM4. In the mouse, we further link TGM4 immunity with a destructive prostatitis and compromised secretion of TGM4. Collectively, our findings in APS1 patients and Aire-deficient mice reveal prostate autoimmunity as a major manifestation of APS1 with potential role in male subfertility.

- The magnetic and transport properties of films based on discontinuous layers of Ni81Fe19 (Py) embedded in Al2O3 were investigated. In films with nominal Py thicknesses 6 and 8 angstrom superparamagnetic particles with median diameters D-med = 2.8 and 3.1 nm and distribution widths sigma(D)= 1.2 and 1.3 nm were formed. - Experimental data of bicrystal grain boundary magnetoresistance hysteresis shows that the resistance at remanence after saturation depends on whether the magnetic field was applied parallel or perpendicular to the grain boundary. In the latter case the behaviour can be described by the Stoner-Wohlfarth magnetization reversal model for coherent rotation of the magnetization, which leads to an alignment along the easy magnetization directions of the bicrystal in zero field. In the parallel configuration themagnetization directions of the two grains remain parallel in zero field. We have used this information to deduce an effective spin-polarization of P ~ 40 % of the grain boundary in a bicrystal of La0.7Sr0.3MnO3.

- The bicrystal magnetoresistance hysteresis is studied within the model of coherent rotation of magnetization. The magnetoresistance hysteresis is calculated numerically in the limit of fully spin polarized current for symmetric bicrystal junctions with biaxial magnetic anisotropy and of varying misorientation angles. The shape of the curves obtained from the model displays different characteristic features, depending on the angular relationships, which are consistent with experimental data from the literature. The results show a magnetoresistance increasing with increasingmisorientation angle. The influence of biaxial magnetocrystalline anisotropy is reflected in a hump in the maximum magnetoresistance curve at around a misorientation angle of 25 degrees. This structure is absent in the case of uniaxial anisotropy.

- We calculate numerically the magnetization direction as function of magnetic field in the StonerWohlfarth theory and are able to reproduce the shape of the low-field magnetoresistance hysteresis observed in manganite grain boundary junctions. Moreover, we show that it is necessary to include biaxial magnetocrystalline anisotropy to fully describe the grain boundary magnetoresistance in La0.7Sr0.3MnO3/SrTiO3 bicrystal tunnel junctions.
