  • Internet-versus group-administered cognitive behaviour therapy for panic disorder in a psychiatric setting : a randomised trial

    • Background: Internet administered cognitive behaviour therapy (CBT) is a promising new way to deliver psychological treatment, but its effectiveness in regular care settings and in relation to more traditional CBT group treatment has not yet been determined. The primary aim of this study was to compare the effectiveness of Internetand group administered CBT for panic disorder (with or without agoraphobia) in a randomised trial within a regular psychiatric care setting. The second aim of the study was to establish the cost-effectiveness of these interventions. Methods: Patients referred for treatment by their physician, or self-referred, were telephone-screened by a psychiatric nurse. Patients fulfilling screening criteria underwent an in-person structured clinical interview carried out by a psychiatrist. A total of 113 consecutive patients were then randomly assigned to 10 weeks of either guided Internet delivered CBT (n = 53) or group CBT (n = 60). After treatment, and at a 6-month follow-up, patients were again assessed by the psychiatrist, blind to treatment condition. Results: Immediately after randomization 9 patients dropped out, leaving 104 patients who started treatment. Patients in both treatment conditions showed  significant improvement on the main outcome measure, the Panic Disorder Severity Scale (PDSS) after treatment. For the Internet treatment the within-group effect size (pre-post) on the PDSS was Cohen's d = 1.73, and for the group treatment it was d = 1.63. Between group effect sizes were low and treatment effects were maintained at 6-months follow-up. We found no statistically significant differences between the two treatment conditions using a mixed models approach to account for missing data. Group CBT utilised considerably more therapist time than did Internet CBT. Defining effect as proportion of PDSS responders, the cost-effectiveness analysis concerning therapist time showed that Internet treatment had superior cost-effectiveness ratios in relation to group treatment both at post-treatment and follow-up. Conclusions: This study provides support for the effectiveness of Internet CBT in a psychiatric setting for patients with panic disorder, and suggests that it is equally effective as the more widely used group administered CBT in reducing panic-and agoraphobic symptoms, as well as being more cost effective with respect to therapist time.

  • Higher- and lower-order personality traits and cluster subtypes in social anxiety disorder

    • Social anxiety disorder (SAD) can come in different forms, presenting problems for diagnostic classification. Here, we examined personality traits in a large sample of patients (N = 265) diagnosed with SAD in comparison to healthy controls (N = 164) by use of the Revised NEO Personality Inventory (NEO-PI-R) and Karolinska Scales of Personality (KSP). In addition, we identified subtypes of SAD based on cluster analysis of the NEO-PI-R Big Five personality dimensions. Significant group differences in personality traits between patients and controls were noted on all Big Five dimensions except agreeableness. Group differences were further noted on most lower-order facets of NEO-PI-R, and nearly all KSP variables. A logistic regression analysis showed, however, that only neuroticism and extraversion remained significant independent predictors of patient/control group when controlling for the effects of the other Big Five dimensions. Also, only neuroticism and extraversion yielded large effect sizes when SAD patients were compared to Swedish normative data for the NEO-PI-R. A two-step cluster analysis resulted in three separate clusters labelled Prototypical (33%), Introvert-Conscientious (29%), and Instable-Open (38%) SAD. Individuals in the Prototypical cluster deviated most on the Big Five dimensions and they were at the most severe end in profile analyses of social anxiety, self-rated fear during public speaking, trait anxiety, and anxiety-related KSP variables. While additional studies are needed to determine if personality subtypes in SAD differ in etiological and treatment-related factors, the present results demonstrate considerable personality heterogeneity in socially anxious individuals, further underscoring that SAD is a multidimensional disorder.

  • Are athletes burning out with passion?

    • Passion is a strong motivational force towards an activity considered very important, possibly to the extent that the activity forms a part of an individual’s identity. Two forms of passion, harmonious and obsessive, are thought to lead to different cognitive and affective responses. Although being passionate about sport appears to be important from a motivational perspective, it possibly also increases the risk for burnout, which is a negative consequence blamed partly on too much training and inadequate recovery. The question voiced in this study is whether harmonious passion and obsessive passion pose equal risks for burnout. Participants were 94 female and 164 male competitive athletes from 21 sports. The results,  analysed using partial correlation and multivariate analyses of variance, showed that athletes with an obsessive passion scored higher on a burnout inventory than did harmoniously passionate athletes. Obsessively passionate athletes also scored higher on perceived stress and negative affect, and lower on positive affect. These findings support the assumption that even though the two forms of passion may be an integral part of elite sports, athletes scoring high on obsessive passion may be at greater risk of developing burnout than more harmoniously passionate athletes. Burning bright and burning out can thus be seen as two potential consequences for athletes driven by passion. Athletes and coaches who are aware of this may be better equipped to avoid the potential negative consequences associated with too much stress and too little recovery.

    • Biased processing of threatening information may play a casual role in the development of anxiety disorders. Even though empirical evidence points to the fact that preattentive bias can predict subjectively experienced distress in response to a stressor, it is still unknown whether it could be useful in predicting the physiological reactivity in response to a stressor. In the present study, the emotional Stroop task was used to measure preattentive bias. Whereas Stroop interference for masked threat words (i.e., preattentive bias) was found to be positively associated with emotional distress (self-reported) in response to a laboratory stressor, this association was reversed when the autonomic reactivity (electrodermal activity) was used as a measure of emotional response to the very same stressor. Also, neither of these effects were a function of pre-existing anxiety levels. The negative association between preattentive bias and autonomic reactivity corresponds to the autonomic inflexibility seen in clinical anxiety (or very high scores of trait anxiety) when exposed to stressful events. Results were discussed in terms of an inability to automatically inhibit the processing of threatening cues that seems to be a vulnerability marker for anxiety.

  • Adults with Autism and Mental Retardation. A Life-Span Perspective

    • A review of the literature with a life-span perspective on autism gave rise to the formulation of a general research problem: Can demographic factors, individual factors, and social factors (i.e., education, residential facilities, treatment and other services) explain some of the variance in autistic behaviour and social adaptation in adult life? Historic influences, such as the Acts on services for people with mental retardation, reflected in social factors were emphasised. In a retrospective design two groups of adults with autism (DSM-III-R criteria) and mental retardation were studied, the RFA group, sampled through Riksföreningen Autism (n = 48, mean age 35 years) and the County group, a treated population group (n =39, mean age 37 years). The results showed that the RFA group functioned on a higher intellectual level and had better adult social adaptation (measured by the Vineland Adaptive Behavior Scales) than the County group. There were no differences in autistic behaviour (measured by the Childhood Autism Rating Scale) either in childhood or in adulthood. Concerning social factors, the Acts for mentally retarded had had major practical consequences. From the common situation with confinement in large institutions, better opportunities for education, more normal residence (group homes), and for occupation (day-centres) had emerged. Regarding treatment, the most persistent trend was the high use of psychoactive medication. After merging the two groups, analyses showed that the major predictive factors of adult autistic behaviour and social adaptation were intellectual level, speech ability and, with regard to social adaptation, epilepsy. The main conclusion is that intellectual level and speech ability are relatively more important than other factors for functioning of adults with autism and mental retardation. The results are discussed with reference to the adequacy of the measures used to capture effects of the social factors and the importance of also investigating samples with higher intellectual levels.

  • Validation of the new brief 6-item version of the Shirom-Melamed Burnout Measure

    • The Shirom-Melamed Burnout Questionnaire/Measure (SMBQ/M) is one of the most commonly used measures of burnout. Using confirmatory factor analyses, the present study aimed to evaluate the model fit, composite reliability, and factorial (i.e. convergent and discriminant) validity of the new brief Swedish version of the scale-labeled SMBM-6. In addition, we used Cronbach’s α as an indicator of the internal consistency of the total scale. The SMBM-6 consists of two subscales: the emotional and physiological exhaustion subscale (three items) and the cognitive weariness subscale (three items). A total of 1251 teachers in Sweden were included in the study. The analyses showed that the Swedish version of the SMBM-6 has an excellent model fit and good convergent validity. The discriminant validity for the cognitive weariness subscale was good, but slightly inadequate for the physiological exhaustion subscale. Composite reliability and Cronbach’s α indicated high internal consistency for the subscales and the total scale, respectively. Multi-group invariance tests for age indicated no violation of invariance. These results are consistent with those of the study by Almén and Jansson (2021), in which the SMBM-6 was developed, and a subsequent psychometric study by Sundström et al. (2022). In conclusion, there is strong support for the Swedish version of the SMBM-6 as a reliable and valid scale for measuring burnout. Testing the scale in languages other than Swedish is warranted.

    • Panic Disorder with or without or without agoraphobia (PD/A) is common and can be treated effectively with SSRI medication or cognitive behavior therapy (CBT). There is however a great lack of access to CBT services, which has motivated the development of self-help approaches requiring less therapist contact. A novel treatment modality in this field, showing efficacy in several randomised trials but until now not evaluated within the context of regular psychiatric care, is Internet-based treatment. The present study evaluated the effectiveness of Internet-based CBT for 20 consecutively referred PD patients in a psychiatric setting. At post-treatment, 94% of patients no longer met DSM-IV criteria for PD (82% at 6-month follow up). The within-group effect sizes (for the main outcome PDSS; Panic Disorder Severity Scale) were Cohens d=2.5 (pre- to posttreatment) and 2.8 (pre-treatment to follow up) respectively. The proportion of responders on the PDSS was 75% at posttreatment and 70% at 6-month follow up. These results indicate that Internet-based CBT can be both an effective, feasible and potentially cost-effective alternative within regular psychiatric care for patients with PD.

  • Trainee psychotherapists’ emotion recognition accuracy improves after training : emotion recognition training as a tool for psychotherapy education

    • Introduction: Psychotherapists’ emotional and empathic competencies have a positive influence on psychotherapy outcome and alliance. However, it is doubtful whether psychotherapy education in itself leads to improvements in trainee psychotherapists’ emotion recognition accuracy (ERA), which is an essential part of these competencies.Methods: In a randomized, controlled, double-blind study (N = 68), we trained trainee psychotherapists (57% psychodynamic therapy and 43% cognitive behavioral therapy) to detect non-verbal emotional expressions in others using standardized computerized trainings – one for multimodal emotion recognition accuracy and one for micro expression recognition accuracy – and compared their results to an active control group one week after the training (n = 60) and at the one-year follow up (n = 55). The participants trained once weekly during a three-week period. As outcome measures, we used a multimodal emotion recognition accuracy task, a micro expression recognition accuracy task and an emotion recognition accuracy task for verbal and non-verbal (combined) emotional expressions in medical settings.Results: The results of mixed multilevel analyses suggest that the multimodal emotion recognition accuracy training led to significantly steeper increases than the other two conditions from pretest to the posttest one week after the last training session. When comparing the pretest to follow-up differences in slopes, the superiority of the multimodal training group was still detectable in the unimodal audio modality and the unimodal video modality (in comparison to the control training group), but not when considering the multimodal audio-video modality or the total score of the multimodal emotion recognition accuracy measure. The micro expression training group showed a significantly steeper change trajectory from pretest to posttest compared to the control training group, but not compared to the multimodal training group. However, the effect vanished again until the one-year follow-up. There were no differences in change trajectories for the outcome measure about emotion recognition accuracy in medical settings.Discussion: We conclude that trainee psychotherapists’ emotion recognition accuracy can be effectively trained, especially multimodal emotion recognition accuracy, and suggest that the changes in unimodal emotion recognition accuracy (audio-only and video-only) are long-lasting. Implications of these findings for the psychotherapy education are discussed.

  • Attachment and socialized religion within the Læstadian revival movement

    • The present questionnaire study examined attachment-religion connections among 175 believers within the Læstadian revival movement (Lutheran) in Sweden and Finland. Corroborating previous research findings from other religious populations, our findings strongly supported the idea that attachment security (i.e. positive, non-defensive representations of self and others) facilitates the intergenerational transmission of religion from parents to offspring (the socialized correspondence hypothesis) in families within the Læstadian revival movement. Extending previous research, attachment security was also linked to the adoption and transmission of the congregation’s religious standards and to perceived religious support from the congregation. Results pertaining to religion-as-compensation for attachment-related insecurity (the compensation hypothesis) were generally mixed, perhaps due to particularities of Læstadian faith.
