  • Gene expression profiling for the identification of G-protein coupled receptors in human platelets

    • INTRODUCTION AND MATERIALS AND METHODS: G-protein coupled receptors (GPCRs) play an important role in platelet aggregation. To identify new platelet GPCRs, a platelet gene expression profile was generated and validated using quantitative real-time PCR. RESULTS: In total, mRNA of 28 GPCR genes was detected in human platelets. The 12 most abundant platelet GPCR transcripts were: thrombin receptor PAR1 (1865+/-178%), ADP receptor P2Y(12) (459+/-88%), succinate receptor 1 (257+/-48%), ADP receptor P2Y(1) (100%), orphan P2RY(10) (68.2+/-3.3%), lysophosphatidic acid receptors GPR23 (48.2+/-11%) and GPR92 (26.1+/-3.3%), adrenergic receptor alpha(2A) (18.4+/-4.4%), orphan EBI2 (6.31+/-0.42), adenosine receptors A(2A) (2.94+/-0.24%) and A(2B) (2.88+/-0.16%) and lysophosphatidic acid receptor LPA(1) (2.59+/-0.39%) (% relative to the chosen calibrator P2Y(1)). A surprising G-protein coupled receptor redundancy was found: two ADP receptors (P2Y(1) and P2Y(12)), three adenosine receptors (A(2A), A(2B), and A(1)), four lysophosphatidic acid receptors (LPA(1), LPA(3), GPR23 and GPR92), two l-glutamate receptors (mGlu(3) and mGlu(4)) and two serotonin receptors (5-HT(1F) and 5-HT(4)). The adenosine receptor A(2B) gene expression was validated with protein expression and functional studies. Western blot confirmed A(2B) receptor protein expression and platelet flow cytometry demonstrated inhibition of the effect of NECA by the adenosine A(2B)-antagonist MRS1754. CONCLUSIONS: We have detected several GPCRs not previously known to be expressed in platelets, including a functional adenosine A(2B) receptor. The findings could improve our understanding of platelet aggregation and provide new targets for drug development.

    • A fundamental functionality of a Programmable Logic Controller (PLC) is to control and execute a set of operations. But a large part of the program code is more involved in supporting the user with concerns like alarm, HMI, communication, safety and manual control. Code related to these supporting concerns is often tangled with operation execution code, the core concerns, which makes it hard to reuse.This paper describes a method to reuse code and functionality when developing PLC programs and code libraries. The method proposes that core concerns are planned with a software tool called Sequence Planner, and the supporting concerns are integrated into the core concerns with a tool based on aspect-oriented programming.

  • Maximal strength in one leg squat correlates with acceleration capacity and agility

    • INTRODUCTION: In many intermittent team sports capacities such as speed, agility and explosiveness are important for performance and are evaluated by sprint-, agility- and strength tests. Earlier studies have shown strong correlations between strength in the two leg squat exercise and sprint performance in various distances but not in sprint performance in agility. Studies evaluating squat strength predominantly perform tests on two legs even when they test athletes involved in intermittent sports where sprinting and agility are common features. Thus, the purpose of this study was to investigate the correlation between maximal strength in a one leg squat (Bulgarian split squat) and acceleration capacity in various sprint distances and agility.METHODS: The test group consisted of 19 men (mean age 24 ± 2 years ) with experience in intermittent team sports. Acceleration capacity was assessed by sprint tests at 5, 10 and 20 meters and agility was evaluated using the zigzag agility test. The timing was made using photocells (Muscle lab,Ergotest Technology,Norway). The Bulgarian split squat was performed in a smith machine with the barbell on the shoulders to a depth of 110 degrees between tibia and femur.RESULTS: The results show significant correlation between maximal strength in the Bulgarian split squat and sprint capacity in the 5 and 10 meter sprint test (Rp= -0,56; p<0.01) as well as the agility test. Maximal strength relative to bodyweight showed significant correlation with the 5 and 20 meter sprint (Rp=-0,62; p< 0,01) as well as the agility test. The zigzag agility test also showed significant correlation between all distances in the sprint tests (p<0.01).CONCLUSION: The results from this study show that there maximal strength in one leg correlate significantly with both acceleration capacity and agility. Implementing one leg exercises in the strength and conditioning routine can be useful for athletes in intermittent sports wanting to improve agility and short sprinting capacity. Further implications is that the Bulgarian split squat could be a more functional test for agility performance than the squat on two legs which  predominantly is being used today.

  • Operationalising resilience for disaster medicine practitioners : capability development through training, simulation and reflection

    • Resilience has in recent decades been introduced as a term describing a new perspective within the domains of disaster management and safety management. Several theoretical interpretations and definitions of the essence of resilience have been proposed, but less work has described how to operationalise resilience and implement the concept within organisations. This case study describes the implementation of a set of general resilience management guidelines for critical infrastructure within a Swedish Regional Medical Command and Control Team. The case study demonstrates how domain-independent guidelines can be contextualised and introduced at an operational level, through a comprehensive capability development programme. It also demonstrates how a set of conceptual and reflective tools consisting of educational, training and exercise sessions of increasing complexity and realism can be used to move from high-level guidelines to practice. The experience from the case study demonstrates the value of combining (1) developmental learning of practitioners’ cognitive skills through resilience-oriented reflection and interaction with dynamic complex open-ended problems; (2) contextualisation of generic guidelines as a basis for operational methodological support in the operational environment; and (3) the use of simulation-based training as part of a capability development programme with increasing complexity and realism across mixed educational, training and exercise sessions. As an actual example of a resilience implementation effort in a disaster medicine management organisation, the study contributes to the body of knowledge regarding how to implement the concept of resilience in operational practice.

  • Simulation-assisted burn disaster planning

    • The aim of the study was to evaluate the Swedish medical systems response to a mass casualty burn incident in a rural area with a focus on national coordination of burn care. Data were collected from two simulations of a mass casualty incident with burns in a rural area in the mid portion of Sweden close to the Norwegian border, based on a large inventory of emergency resources available in this area as well as regional hospitals, university hospitals and burn centres in Sweden and abroad. The simulation system Emergo Train System (R) (ETS) was used and risk for preventable death and complications were used as outcome measures: simulation I, 18.5% (n = 13) preventable deaths and 15.5% (n = 11) preventable complications; simulation II, 11.4% (n = 8) preventable deaths and 11.4% (n = 8) preventable complications. The last T1 patient was evacuated after 7 h in simulation I, compared with 5 h in simulation II. Better national coordination of burn care and more timely distribution based on the experience from the first simulation, and possibly a learning effect, led to a better patient outcome in simulation II. The experience using a system that combines both process and outcome indicators can create important results that may support disaster planning.

    • A direct digital-to-RF converter (DRFC) is presented in this work. Due to its digital-in-nature design, the DRFC benefits from technology scaling and can be monolithically integrated into advance digital VLSI systems. A fourth-order single-bit quantizer bandpass digital EA modulator is used preceding the DRFC, resulting in a high in-band signal-to-noise ratio (SNR). The out-of-band spectrally-shaped quantization noise is attenuated by an embedded semi-digital FIR filter (SDFIR). The RF output frequencies are synthesized by a novel configurable voltage-mode RF DAC solution with a high linearity performance. The configurable RF DAC is directly synthesizing RF signals up to 10 GHz in first or second Nyquist zone. The proposed DRFC is designed in 22 nm FDSOI CMOS process and with the aid of Monte-Carlo simulation, shows 78.6 dBc and 63.2 dBc worse case third intermodulation distortion (IM3) under process mismatch in 2.5 GHz and 7.5 GHz output frequency respectively.

    • Emerging evidence shows that the human microbiota plays a larger role in disease progression and health than previously anticipated. Helicobacter pylori, the causative agent of gastric cancer and duodenal and gastric ulcers, was early associated with gastric disease, but it has also been proposed that the accompanying microbiota in Helicobacter pylori-infected individuals might affect disease progression and gastric cancer development. In this study, the composition of the transcriptionally active microbial community and H. pylori gene expression were determined using metatranscriptomic RNA sequencing of stomach biopsy specimens from individuals with different H. pylori infection statuses and premalignant tissue changes. The results show that H. pylori completely dominates the microbiota not only in infected individuals but also in most individuals classified as H. pylori uninfected using conventional methods. Furthermore, H. pylori abundance is positively correlated with the presence of Campylobacter, Deinococcus, and Sulfurospirillum. Finally, we quantified the expression of a large number of Helicobacter pylori genes and found high expression of genes involved in pH regulation and nickel transport. Our study is the first to dissect the viable microbiota of the human stomach by metatranscriptomic analysis, and it shows that metatranscriptomic analysis of the gastric microbiota is feasible and can provide new insights into how bacteria respond in vivo to variations in the stomach microenvironment and at different stages of disease progression.
