  • Mechanical work as predictor of force enhancement and force depression

    • The steady-state force following active muscle shortening or stretch differs from the maximum isometric force associated with the final length. This phenomenon proves that the isometric force production is not only dependent on current muscle length and length time derivative, but depends on the preceding contraction history. Isolated extensor digitorum longus and soleus muscles from mice (NMRI strain) were used to investigate the force produced by a muscle, and some parameters hypothetically influencing this history-dependent force modification. The muscles were pre-stimulated at a fixed length, then different stretch/shortening episodes were introduced, whereafter changes of the active force were recorded while the muscles were held isometrically to approach a steady-state force before de-stimulation. The mechanical work during active stretch and shortening was evaluated by integrating the product of force and ramp velocity over the length-varying period. The results show a negative linear correlation between the force modification and the mechanical work produced on or by the muscle, continuous between shortening and stretch. A corresponding modification of the passive force component following each stimulation was also observed. The conclusion is that the isometric force attained after stretch or shortening is well described by an asymptotic force which is determined by the mechanical work.

  • Muscular force production after concentric contraction

    • The steady-state force following active shortening does not reach the maximum isometric force associated with the final length. Isolated extensor digitorum longus and soleus muscles from mice (NMRI strain) were used to investigate the force produced by a muscle, and some parameters hypothetically influencing this shortening-induced force depression. The muscles were pre-stimulated at fixed length, shortened and then held isometrically to give maximum post-shortening forces, before de-stimulation. The shortening magnitude was 0.18. 0.36 or 0.72 mm (about 2-7% of optimal length), time of shortening was chosen as 0.03, 0.06 and 0.12 s, and final length as +0.72. 0 and -0.72mm, related to optimal length. The mechanical work during active shortening was evaluated by integrating the product of force and shortening velocity over the shortening period. The results show a positive correlation between the force depression and the mechanical work. whereas the force depression was not correlated to the velocity of shortening. Depression of the passive force component was also observed following all stimulations. Experiments show that the fully stimulated redevelopment of isometric force following concentric contraction follows a time function similar to the creation of force when isometric muscle is initially stimulated. The conclusion is that the isometric force development after active shortening can be well described by an asymptotic force which is decided by the produced work, and the initial isometric time constant.

  • Influence of electrical and thermal properties on RF ablation of breast cancer: is the tumour preferentially heated?

    • Background: Techniques based on radio frequency (RF) energy have many applications in medicine, in particular tumour ablation. Today, mammography screening detects many breast cancers at an early stage, facilitating treatment by minimally invasive techniques such as radio frequency ablation (RFA). The breast cancer is mostly surrounded by fat, which during RFA-treatment could result in preferential heating of the tumour due to the substantial differences in electrical parameters. The object of this study was to investigate if this preferential heating existed during experimental in vitro protocols and during computer simulations. Methods: Excised breast material from four patients with morphologically diagnosed breast cancers were treated with our newly developed RFA equipment. Subsequently, two finite element method (FEM) models were developed; one with only fat and one with fat and an incorporated breast cancer of varying size. The FEM models were solved using temperature dependent electrical conductivity versus constant conductivity, and transient versus steady-state analyses. Results: Our experimental study performed on excised breast tissue showed a preferential heating of the tumour, even if associated with long tumour strands. The fat between these tumour strands was surprisingly unaffected. Furthermore, the computer simulations demonstrated that the difference in electrical and thermal parameters between fat and tumour tissue can cause preferential heating of the tumour. The specific absorption rate (SAR) distribution changed significantly when a tumour was present in fatty tissue. The degree of preferential heating depended on tissue properties, tumour shape, and placement relative to the electrode. Temperature dependent electrical conductivity increased the thermal lesion volume, but did not change the preferential heating. Transient solutions decreased the thermal lesion volume but increased the preferential heating of the tumour. Conclusions: Both the computer model and the in vitro study confirmed that preferential heating of the tumour during RFA exists in breast tissue. However, the observed preferential heating in the in vitro studies were more pronounced, indicating that additional effects other than the difference in tissue parameters might be involved. The existing septa layers between the cancer tissue and the fatty tissue could have an additional electrical or thermal insulating effect, explaining the discrepancy between the in vitro study and the computer model.

  • History effect and timing of force production introduced in a skeletal muscle model

    • Skeletal muscle modelling requires a detailed description of muscular force production. We have performed a series of experiments on mouse skeletal muscles to give a basis for an improved description of the muscular force production. Our previous work introduced a force modification in isometric phases, which was based on the work performed by or on the muscle during transient-length-varying contractions. Here, state-space diagrams were used to investigate the timing aspects of the force production. These show a dominant exponential nature of the force development in isometric phases of the contractions, reached after a non-exponential phase, assumed as an activation or deactivation stage and not further analysed here. The time constants of the exponential functions describing isometric force redevelopment after length variations appear to be related to the one for an initial isometric contraction, but depending on the previous history. The timing of force production calculated from the state-space diagrams was in agreement with the generally accepted muscle properties, thereby demonstrating the reliability of the method. A macroscopic muscular model consisting of a contractile element, parallel and series elastic elements was developed. The parameters from the experiment analysis, particularly the force modification after non-isometric contractions and the time constants, were reproduced by the simulations. The relationship between time constants introduced in a mechanistic model and the measured macroscale timings is discussed.

  • Finite element simulations of a focal knee resurfacing implant applied to localized cartilage defects in a sheep model

    • Articular resurfacing metal implants have recently been tested in animal models to treat full thickness localized articular cartilage defects, showing promising results. However, the mechanical behavior of cartilage surrounding the metal implant has not been studied yet as it is technically challenging to measure in vivo contact areas, pressures, stresses and deformations from the metal implant. Therefore, we implemented a detailed numerical finite element model by approximating one of the condyles of the sheep tibiofemoral joint and created a defect of specific size to accommodate the implant. Using this model, the mechanical behavior of the surrounding of metal implant was studied. The model showed that the metal implant plays a significant role in the force transmission. Two types of profiles were investigated for metal implant. An implant with a double-curved profile, i.e., a profile fully congruent with the articular surfaces in the knee, gives lower contact pressures and stresses at the rim of the defect than the implant with unicurved spherical profile. The implant should be placed at a certain distance into the cartilage to avoid damage to opposing biological surface. Too deep positions, however, lead to high shear stresses in the cartilage edges around the implant. Mechanical sealing was achieved with a wedge shape of the implant, also useful for biochemical sealing of cartilage edges at the defect.

    • Individual differences in cognitive performance increase with advancing age, reflecting marked cognitive changes in some individuals along with little or no change in others. Genetic and lifestyle factors are assumed to influence cognitive performance in aging by affecting the magnitude and extent of age-related brain changes (i.e., brain maintenance or atrophy), as well as the ability to recruit compensatory processes. The purpose of this review is to present findings from the Betula study and other longitudinal studies, with a focus on clarifying the role of key biological and environmental factors assumed to underlie individual differences in brain and cognitive aging. We discuss the vital importance of sampling, analytic methods, consideration of non-ignorable dropout, and related issues for valid conclusions on factors that influence healthy neurocognitive aging.

  • A national approach for automated collection of standardized and population-based radiation therapy data in Sweden

    • Purpose: To develop an infrastructure for structured and automated collection of interoperable radiation therapy (RT) data into a national clinical quality registry. Materials and methods: The present study was initiated in 2012 with the participation of seven of the 15 hospital departments delivering RT in Sweden. A national RT nomenclature and a database for structured unified storage of RT data at each site (Medical Information Quality Archive, MIQA) have been developed. Aggregated data from the MIQA databases are sent to a national RT registry located on the same IT platform (INCA) as the national clinical cancer registries. Results: The suggested naming convention has to date been integrated into the clinical workflow at 12 of 15 sites, and MIQA is installed at six of these. Involvement of the remaining 3/15 RT departments is ongoing, and they are expected to be part of the infrastructure by 2016. RT data collection from ARIA (R), Mosaiq (R), Eclipse (TM), and Oncentra (R) is supported. Manual curation of RT-structure information is needed for approximately 10% of target volumes, but rarely for normal tissue structures, demonstrating a good compliance to the RT nomenclature. Aggregated dose/volume descriptors are calculated based on the information in MIQA and sent to INCA using a dedicated service (MIQA2INCA). Correct linkage of data for each patient to the clinical cancer registries on the INCA platform is assured by the unique Swedish personal identity number. Conclusions: An infrastructure for structured and automated prospective collection of syntactically inter operable RT data into a national clinical quality registry for RT data is under implementation. Future developments include adapting MIQA to other treatment modalities (e.g. proton therapy and brachytherapy) and finding strategies to harmonize structure delineations. How the RT registry should comply with domain-specific ontologies such as the Radiation Oncology Ontology (ROO) is under discussion.
