  • High resolution deletion analysis of constitutional DNA from neurofibromatosis type 2 (NF2) patients using microarray-CGH

    • Neurofibromatosis type 2 (NF2) is an autosomal dominant disorder whose hallmark is bilateral vestibular schwannoma. It displays a pronounced clinical heterogeneity with mild to severe forms. The NF2 tumor suppressor (merlin/schwannomin) has been cloned and extensively analyzed for mutations in patients with different clinical variants of the disease. Correlation between the type of the NF2 gene mutation and the patient phenotype has been suggested to exist. However, several independent studies have shown that a fraction of NF2 patients with various phenotypes have constitutional deletions that partly or entirely remove one copy of the NF2 gene. The purpose of this study was to examine a 7 Mb interval in the vicinity of the NF2 gene in a large series of NF2 patients in order to determine the frequency and extent of deletions. A total of 116 NF2 patients were analyzed using high-resolution array-comparative genomic hybridization (CGH) on an array covering at least 90% of this region of 22q around the NF2 locus. Deletions, which remove one copy of the entire gene or are predicted to truncate the schwannomin protein, were detected in 8 severe, 10 moderate and 6 mild patients. This result does not support the correlation between the type of mutation affecting the NF2 gene and the disease phenotype. This work also demonstrates the general usefulness of the array-CON methodology for rapid and comprehensive detection of small (down to 40 kb) heterozygous and/or homozygous deletions occurring in constitutional or tumor-derived DNA.

  • Don't buy a pig in a poke: Considering challenges of and problems with performance analysis technologies in Swedish men's elite football

    • During the last decades, technologies to monitor, test and analyze athletes’ performance and health have rapidly developed. At present, global positioning systems (GPS), stadium camcorders, heart rate monitors and mobile applications are prominent performance analysis technologies (PATs) used in most elite sport environments. While PATs is understood as an aid, there is a growing body of literature that points to negative consequences. These negative consequences are concerning and call for research and measures to develop strategies for effective and productive implementation. To achieve this, this article first outlines key challenges and problems of PATs, using sport sociological research on coaching and athletes, historical knowledge of the scientization of training and the changing role of the coach, as well as scientific and experiential knowledge of performance analysis. Our findings show that key challenges and problems occur in a chain of six steps that concern the implementing of PATs: 1. Investment in PATs; 2. Production of performance data; 3. Interpretation of performance data; 4. Communication of performance data; 5. Decision-making based on performance data; and 6. Influence of PATs on coaches and athletes. The article then answers these challenges and problems by outlining recommendations for how sport managers and administrators can prevent buying “a pig in a poke” by acquiring competence about performance analysis and PATs, investing time, and developing effective communication between those working with PATs. © 2021 The Author(s)

    • In a recent paper by dos Santos Dias et al. [Phys. Rev. B 103, L140408 (2021)], a critique of earlier works analyzing low-energy spin Hamiltonians is put forth. To be precise, it is the large noncollinear contributions to the Dzyaloshinskii-Moriya interaction (DMI) that is the main concern of dos Santos Dias et al. In this Comment, we clarify the microscopic mechanisms for the large DMI that can be found in noncollinear magnets. Furthermore, we outline the complementary nature of the different parametrizations of a spin Hamiltonian, with strengths and weaknesses of both approaches. Specifically, we stress the physical insight in the interpretation of the DMI, when decomposed in microscopic electron and spin densities and currents.

  • Evaluating patient reported outcomes and experiences in a novel proton beam clinic : challenges, activities, and outcomes of the ProtonCare project

    • BACKGROUND: The ProtonCare Study Group (PCSG) was formed with the purpose to develop and implement a framework for evaluation of proton beam therapy (PBT) and the related care at a novel clinic (Skandionkliniken), based on patient reported data.METHOD: A logic model framework was used to describe the process of development and implementation of a structured plan for evaluation of PBT for all diagnoses based on patient reported data. After the mission for the project was determined, meetings with networks and stakeholders were facilitated by PCSG to identify assumptions, resources, challenges, activities, outputs, outcomes, and outcome indicators. RESULT: This paper presents the challenges and accomplishments PCSG made so far. We describe required resources, activities, and accomplished results. The long-term outcomes that were outlined as a result of the process are two; 1) Improved knowledge about health outcomes of patients that are considered for PBT and 2) The findings will serve as a base for clinical decisions when patients are referred for PBT. CONCLUSION: Using the logical model framework proved useful in planning and managing the ProtonCare project. As a result, the work of PCSG has so far resulted in long-lasting outcomes that creates a base for future evaluation of patients' perspective in radiotherapy treatment in general and in PBT especially. Our experiences can be useful for other research groups facing similar challenges. Continuing research on patients´ perspective is a central part in ongoing and future research. Collaboration, cooperation, and coordination between research groups/networks from different disciplines are a significant part of the work aiming to determine the more precise role of PBT in future treatment options.

    • PURPOSE: The aim of this study was to explore patients' experience of participation in the treatment decision of proton beam therapy versus conventional radiotherapy. BACKGROUND: Proton beam therapy (PBT) has become a treatment option for some cancer patients receiving radiotherapy. The decision to give PBT instead of conventional radiotherapy (CRT) needs to be carefully planned together with the patient to ensure that the degree of participation is based on individuals' preferences. There is a knowledge gap of successful approaches to support patients' participation in the decision-making process, which is particularly important when it comes to the situation of having to choose between two treatment options such as PBT and CRT, with similar expected outcomes.METHOD: We conducted a secondary analysis of qualitative data collected from interviews with patients who received PBT for their brain tumor. Transcribed verbatims from interviews with 22 patients were analyzed regarding experiences of participation in the decision-making process leading to PBT.FINDINGS: Participants experienced their participation in the decision-making process to a varying degree, and with individual preferences. Four themes emerged from data: to be a voice that matters, to get control over what will happen, being in the hand of doctors' choice, and feeling selected for treatment.CONCLUSION: A decision for treatment with PBT can be experienced as a privilege but can also cause stress as it might entail practical issues affecting everyday life in a considerable way. For the patient to have confidence in the decision-making process, patients' preferences, expectations, and experiences must be included by the healthcare team. Including the patient in the healthcare team as an equal partner by confirming the person enables and facilitates for patients' voice to be heard and reckoned with. Person-centered care building on a partnership between patients and healthcare professionals should provide the right basis for the decision-making process.

    • De senaste 100 åren har teknikens och vetenskapens betydelse för träning och tränarskap ökat lavinartat. Från att ha spelat en undanskymd roll under 1900-talets första hälft, har ny teknologi och nya forskningsrön, i syfte att förbättra prestationerna, blivit en central del av idrotten. Utvecklingen har kallats för en tekno-vetenskaplig revolution (Fouché, 2017). Det senaste decenniet har utvecklingen fortsatt och accelererat. Allt mer avancerad teknik används för att övervaka, utvärdera och optimera träningen i en rad sporter. Denna utveckling väcker frågor. Vad betyder den ökade användningen av avancerad teknik för coaching i praktiken? Vilka fördelar respektive risker kan uppstå och hur kan de hanteras? Dessa frågor är i fokus i ett pågående forskningsprojekt, som tittar närmare på användandet av teknik för att analysera idrottsliga prestationer inom svensk herrelitfotboll. Specifikt studerar vi teknikens roll för träningen i föreningarna i Allsvenskan.Genom att utrusta blivande tränare med verktyg för att själva hantera och värdera teknik i coachingen, kan många av de historiska misstagen (övertro på tekniska lösningar, oförmåga att kommunicera testresultat och andra värden på ett meningsfullt vis, etcetera) undvikas. Tekniken ger ökade möjligheter för tränare att mäta och i praktiken övervaka sina spelare. Detta ställer nya krav på etiska överväganden från både tränare och klubbar.
