Fear of hypoglycemia : relationship to hypoglycemic risk and psychological factors

- OBJECTIVE: The major aims of this study were to examine (1) the association between fear of hypoglycemia (FOH) in adults with type 1 diabetes with demographic, psychological (anxiety and depression), and disease-specific clinical factors (hypoglycemia history and unawareness, A1c), including severe hypoglycemia (SH), and (2) differences in patient subgroups categorized by level of FOH and risk of SH.RESEARCH DESIGN AND METHODS: Questionnaires were mailed to 764 patients with type 1 diabetes including the Swedish translation of the Hypoglycemia Fear Survey (HFS) and other psychological measures including the Perceived Stress Scale, Hospital Anxiety and Depression Scale, Anxiety Sensitivity Index, Social Phobia Scale, and Fear of Complications Scale. A questionnaire to assess hypoglycemia history was also included and A1c measures were obtained from medical records. Statistical analyses included univariate approaches, multiple stepwise linear regressions, Chi-square t tests, and ANOVAs.RESULTS: Regressions showed that several clinical factors (SH history, frequency of nocturnal hypoglycemia, self-monitoring) were significantly associated with FOH but R (2) increased from 16.25 to 39.2 % when anxiety measures were added to the model. When patients were categorized by level of FOH (low, high) and SH risk (low, high), subgroups showed significant differences in non-diabetes-related anxiety, hypoglycemia history, self-monitoring, and glycemic control.CONCLUSION: There is a strong link between FOH and non-diabetes-related anxiety, as well as hypoglycemia history. Comparison of patient subgroups categorized according to level of FOH and SH risk demonstrated the complexity of FOH and identified important differences in psychological and clinical variables, which have implications for clinical interventions.

Measurement of the Power Distribution in a Single Disc Refiner

- A vital parameter in the production of mechanical pulp through refining is the energy consumption. Until recently, the pulping industry has focused on the total energy consumed, without a full understanding of how this energy is distributed in the refiners. In this paper two methods are suggested by which it is possible to estimate the energy supplied at an arbitrary point along the disc radius on refiner segments. The methods are based on instrumented bar with strain gauges. In the first method, a bar is slotted so that the sensor can be viewed as consisting of a number of cantilever beams separated by slots. In the second method, the sensor consists of one continuous bar along the disc radius. The sensors are calibrated such that influence coefficients are obtained for the slotted case and an influence function is obtained for the continuous one. The tangential forces are obtained directly in the first method, while in the second they are given by an integral equation which can be solved numerically. From the knowledge of the tangential force the power distribution can be calculated. Experimental results from the use of the slotted sensor are also given.

- The impact of many unfavorable childhood traits or diseases, such as low birth weight and mental disorders, is not limited to childhood and adolescence, as they are also associated with poor outcomes in adulthood, such as cardiovascular disease. Insight into the genetic etiology of childhood and adolescent traits and disorders may therefore provide new perspectives, not only on how to improve wellbeing during childhood, but also how to prevent later adverse outcomes. To achieve the sample sizes required for genetic research, the Early Growth Genetics (EGG) and EArly Genetics and Lifecourse Epidemiology (EAGLE) consortia were established. The majority of the participating cohorts are longitudinal population-based samples, but other cohorts with data on early childhood phenotypes are also involved. Cohorts often have a broad focus and collect(ed) data on various somatic and psychiatric traits as well as environmental factors. Genetic variants have been successfully identified for multiple traits, for example, birth weight, atopic dermatitis, childhood BMI, allergic sensitization, and pubertal growth. Furthermore, the results have shown that genetic factors also partly underlie the association with adult traits. As sample sizes are still increasing, it is expected that future analyses will identify additional variants. This, in combination with the development of innovative statistical methods, will provide detailed insight on the mechanisms underlying the transition from childhood to adult disorders. - En myndighetsgemensam observatörsresa genomfördes våren 2012 för att lära av erfarenheterna av den omfattande naturkatastrofen 2011 i Queensland, Australien. Observatörsresan organiserades av avdelningen för utvärdering och lärande vid Myndigheten för samhällsskydd och beredskap, MSB. Planerade möten hos myndigheter och organisationer ägde rum dels på federal nivå i Canberra, dels i delstaten Queensland med studiebesök i Brisbane och andra orter.Syftet med denna observatörsinsats var sålunda att:–– lära från de erfarenheter Australien gjort med anledning avde stora översvämningar, orsakade av extrema regn och cyklonenYasi, som drabbade delstaten Queensland under periodendecember 2010 - januari 2011.–– studera det australiensiska civila samhällets samlade förmågaatt hantera och återhämta sig efter dessa extrema händelseroch påfrestningar.Observatörsinsatsen initierades via MSB:s samverkansområden. En begäran och önskan riktades till MSB att organisera en observatörsinsats, i enlighet med MSB:s uppdrag att samordna och samverka inom området observatörsverksamhet

- Objectives: To translate and evaluate the psychometric properties of the Swedish version of the Fear of Complications Questionnaire. Design: Cross-sectional study design and scale development. Settings: Totally, 469 adults (response rate 63.5%) with Type 1 diabetes completed the questionnaires. Participants were recruited from two university hospitals in Sweden. Participants: Eligible patients were those who met the following inclusion criteria: diagnosed with Type 1 diabetes, diabetes duration of at least 1 year and aged at least 18 years. Methods: The Fear of Complications Questionnaire was translated using the forward-backward translation method. Factor analyses of the questionnaire were performed in two steps using both exploratory and confirmatory factor analysis. Convergent validity was examined using the Hospital Anxiety and Depression Scale and the Fear of Hypoglycaemia Fear Survey. Internal consistency was estimated using Cronbach’s alpha.Results: Exploratory factor analysis supported a two-factor solution. One factor contained three items having to do with fear of kidney-related complications and one factor included the rest of items concerning fear of other diabetes-related complications, as well as fear of complications in general. Internal consistency was high Cronbach’s alpha 0.96. The findings also gave support for convergent validity, with significant positive correlations between measures (r = 0.51 to 0.54). Conclusion: The clinical relevance of the identified two-factor model with a structure of one dominant subdomain may be considered. We suggest, however a one-factor model covering all the items as a relevant basis to assess fear of complications among people with Type 1 diabetes.
