  • Topical nitroprusside may reduce histamine-induced plasma exudation in human nasal airways

    • Mucosal exudation of nonsieved bulk plasma is a key feature of airway defense and inflammation. We have previously observed in guinea pig tracheobronchial airways that endogenous nitric oxide (NO) of the mucosa may tonically suppress the permeability of the subepithelial microcirculation, and that topical administration of the NO donor nitroprusside may reduce plasma exudation responses. The present study examines whether nitroprusside affects histamine-induced mucosal exudation of plasma in the human nasal airway. In a dose-finding tolerability experiment, using changes in nasal patency as response, placebo and nitroprusside (1.2 and 3.6 mg per nasal cavity) were applied on the mucosal surface with a nasal-spray device. Nasal peak expiratory flow (PEF) rates were measured before the application and thereafter every third minute for 15 min. Nitroprusside produced a dose-dependent decrease in nasal PEF rates compared to placebo. Placebo or nitroprusside (7.2 mg) was then given to the right nasal cavity, followed 3 min later by challenge with saline or histamine (600 micrograms). The drug and the challenge were both applied with a nasal-spray device. With a nasal pool-device, the same large part of the nasal mucosal surface was lavaged before and after the treatment/challenge. The lavage fluid levels of alpha 2-macroglobulin were measured as an index of mucosal exudation of bulk plasma. The histamine-induced lavage fluid level of alpha 2-macroglobulin was significantly higher after treatment with placebo than with nitroprusside. The present data indicate that nitroprusside may have antiexudative effects in human airways. Hence, unlike other microvascular permeability active agents, this pharmacologic principle may be active in both guinea pig and human airways.

    • Human papillomavirus (HP) infections of the genital tract are sexually transmitted and prevalent worldwide. In this study, the role of HPV in 72 patients with anal squamous cell carcinoma was investigated. Patients and Methods: Polymerase chain reaction (PCR) in combination with in situ hybridization was used to identify HPV-DNA in the patients biopsies. The HPV typing was conducted by pyrosequencing. Cell cycle and DNA content were analysed by cytometry. Results: Ninety percent of the carcinoma biopsies carried high-risk oncogenic HPV in their malignant cells. Eighty-one percent of these demonstrated a single infection with HPV16, 18 or 33 and 19% were double infected with HPV16 and HPV18 Accumulations of viral genes were seen at the necrotic area of the tumours. The HPV genome in the tumour cell influenced significant the host cell cycle progression, but not DNA aberrations. Within these patients, HPV status in the malignant cells was not found to be associated with patient survival time. Conclusion: High-risk oncogenic HPV may play an important role in the initiation of host cell proliferation in anal squamous cell carcinoma. However, infection with HPV may not have any direct influence itself on the clinical outcome of these patients considering the treatments currently available.

    • This paper presents analytical expressions for the sensitivity of a low power envelope detector driven by a weak RF signal in the presence of a blocking signal. The envelope detector has been proposed for low power Wake-Up radios in applications such as RFID and wireless sensor systems. The theoretical results are verified with simulations of a modern short channel MOS transistor in a commonly used circuit topology. A discussion around a tutorial example of a radio frontend, consisting of an LNA and a detector, is presented. It is shown that the sensitivity of a low power envelope detector can reach -62 dBm with a low power LNA and in presence of a CW blocker. © 2011 IEEE.

    • With the advent of Internet of Things (IoT) it has become clear that radio-frequency (RF) designers have to be aware of power constraints, e.g., in the design of simplistic ultra-low power receivers often used as wake-up radios (WuRs). The objective of this work, one of the first systematic studies of power bounds for RF-systems, is to provide an overview and intuitive feel for how power consumption and sensitivity relates for low-power receivers. This was done by setting up basic circuit schematics for different radio receiver architectures to find analytical expressions for their output signal-to-noise ratio including power consumption, bandwidth, sensitivity, and carrier frequency. The analytical expressions and optimizations of the circuits give us relations between dc-energy-per-bit and receiver sensitivity, which can be compared to recent published low-power receivers. The parameter set used in the analysis is meant to reflect typical values for an integrated 90 nm complementary metal-oxide-semiconductor fabrication processes, and typical small sized RF lumped components.

    • An ultra-low power wake-up radio receiver using no oscillators is described. The radio utilizes an envelope detector followed by a baseband amplifier and is fabricated in a 130-nm complementary metal-oxide-semiconductor process. The receiver is preceded by a passive radio-frequency voltage transformer, also providing 50 Omega antenna matching, fabricated as transmission lines on the FR4 chip carrier. A sensitivity of -47 dBm with 200 kb/s on-off keying modulation is measured at a current consumption of 2.3 mu A from a 1 V supply. No trimming is used. The receiver accepts a dBm continuous wave blocking signal, or modulated blockers 6 dB below the sensitivity limit, with no loss of sensitivity.

  • Epidermoid anal cancer : a review of a population-based series of 308 consecutive patients treated according to prospective protocols

    • Purpose: The primary therapy in epidermoid anal cancer is radiotherapy, generally with chemotherapy. The use of neoadjuvant chemotherapy has been infrequently reported in the literature. This study presents results from a large population-based series and provides comparisons between different treatments.Methods and materials: Between 1985 and 2000, 308 patients with invasive epidermoid anal cancer were diagnosed in the Stockholm Health Care Region. Treatment was given according to defined protocols. External beam radiotherapy alone or with concomitant bleomycin and neoadjuvant chemotherapy followed by radiotherapy alone were the primary treatments. Radical surgery was reserved for poor responders or recurrences. Data were reviewed with regard to treatment, outcome, and prognostic factors.Results: Among the 276 patients (90%) treated with curative intent, 264 (96%) received treatment in accordance with the protocols. The overall 5-year survival rate was 68%. Among the 142 patients with locally advanced tumors (T > or =4 cm or N+), patients treated with neoadjuvant platinum-based chemotherapy (n = 91) had significantly better complete response rates compared with patients treated with radiotherapy with or without bleomycin (n = 51) (92% vs. 76%, p < 0.01). A significantly increased overall 5-year survival rate was also found among patients receiving neoadjuvant therapy (63% vs. 44%, p < 0.05).Conclusion: Structured treatment protocols result in favorable outcome on a population level. The results further suggest a significant therapeutic gain from including neoadjuvant chemotherapy in the treatment of locally advanced anal cancer.
