    • Efforts are made in radiographic examinations to obtain the best image quality with the lowest possible absorbed dose to the patient. In dental radiography, the absorbed dose to patients is very low, but exposures are relatively frequent. It has been suggested that frequent low-dose exposures can pose a risk for development of future cancer. It has previously been reported that there was no significant difference in the diagnostic accuracy of approximal carious lesions in radiographs obtained using tube voltages of 60 and 70 kV. The aim of this study was, therefore, to evaluate the patient dose resulting from exposures at these tube voltages to obtain intraoral bitewing radiographs.

    • Texterna i denna digitala antologi är författade av studenterna och lärarna på kursen ”Deltagarkultur”, som är en del av Interreg-projektet Öresundsregionen som kreativ metapol. Texterna är författade av tjänstemän på kulturförvaltningar i Skåne och Danmark, samt av lärare från Malmö högskola. En gemensam nämnare är att de alla ringar in olika både möjligheter och problem med deltagarkultur i praktiken. Vad händer när idealen möter verkligheten? Hur uppfattar deltagarna själva sin roll i de olika kulturella eller pedagogiska projekt de mer eller mindre frivilligt söker sig till eller blir indragna i? Texterna spänner över ett brett spektrum av praktiker, erfarenheter och problemställningar. De använder olika begrepp och angreppsätt för att närma sig alltifrån deltagarstyrda musikfestivaler och konstprojekt till litteraturläsning och projektarbeten på lärarutbildningen. Nya former för kulturstöd som utmanar traditionella bidragssystem undersöks i en av texterna. Läsaren introduceras för fenomen som crowd funding, crowd sourcing, mikrofinansiering och viral spridning. Filosofins hantering av lek och begär konfronteras med kulturpolitiska visioner och policydokument i en annan text. Sist men inte minst ägnar sig texterna åt intensiv självreflexion – vilket inte skall förväxlas med självupptagenhet.

  • Self-reported post-operative recovery in children : development of an instrument

    • Rationale, aims and objectives: According to the United Nations (1989) , hildren have the right to be heard and to have their opinions respected. Since post-operative recovery is an individual and subjective experience and patient-reported outcome measures are considered important, our aim was to develop and test an instrument to measure self-reported quality of recovery in children after surgical procedures.Methods: Development of the instrument Postoperative Recovery in Children (PRiC) was influenced by the Quality of Recovery-24, for use in adults. Eighteen children and nine professionals validated the items with respect to content and language. A photo question- naire was developed to determine whether the children’s participation would increase compared with the text questionnaire. The final instrument was distributed consecutively to 390 children, ages 4–12 years, who underwent tonsil surgery at four hospitals in Sweden.Results: A total o f238 children with a mean age of 6.5 years participated. According to the parents, 23% circled the answers themselves and 59% participated to a significant degree. However, there was no significant difference in participation between those who received a photo versus a text questionnaire. Psychometric tests of the instrument showed that Cronbach’s alpha for the total instrument was 0.83 and the item-total correlations for 22 of the items were ≥0.20.Conclusion: Our results support use of the PRiC instrument to assess and follow-up on children’s self-reported post-operative recovery after tonsil operation, both in clinical praxis as well in research.

    • Objectives: We aimed to map the correlation between thermoluminescent dosimeters (TLDs) and Gafchromic film for measuring absorbed doses and to compare minimum, maximum, and mean absorbed doses over larger regions of interest and at various craniofacial organs and tissues during cone beam computed tomography (CBCT) exposure of the mandibular third molar region. Study Design: We positioned TLDs at 75 measurement points in a head phantom. Gafchromic film was cut to the same shape as the 5 levels of the phantom and was placed on top of the TLDs. Both dosimetry methods thus included the surface of each level simultaneously. CBCT scans were made using a 5 × 5 cm field of view and a rotation angle of 200°. Measurements included absorbed dose distributions, doses at all 75 points, and minimum, maximum, and mean doses within organs and tissues. Results: The correlation of point-dose measurements at all TLD sites with doses measured on film was strong (R2 = 0.9687), with greatest correlation at lower doses (<2 mGy). Large deviations between TLD and film measurements of minimum and maximum doses and absorbed doses to the organs occurred at all 5 levels. TLD positioning failed to cover several organ sites; for these, only absorbed dose measurements from the film were available. Conclusions: TLDs were unable to sample dose distributions and gradients accurately. The characteristics of Gafchromic LD-V1 film make it a favorable alternative in dental CBCT dosimetry.

  • Lifespan and mortality of old oaks - combining empirical and modelling approaches to support their management in Southern Sweden

    • Old oaks (Quercus robur L.) play an important role in the southern Scandinavian landscape by providing habitat for a wide range of species, a large proportion of them being currently on the National Redlists. To provide support for the management of these trees, we review data on oak mortality and formulate a mortality-driven stochastic model analysing interactions between mortality rate, oak recruitment rate into 100-150 age class, and amount of oaks older than 200 years. Empirical annual mortality rates varied between 0 and 13% with average 1.68%. Trees older 200 years had an average mortality rate of 1.1%. Oaks in the high density forests showed higher mortality (3.2%) as compared to the trees growing in the low density forests (1.2%). A 400-year long modelling exercises indicated that under current mortality rates (regular mortality being centred around 1% annually; and irregular mortality 7% with average return time of 13 years) the long-term maintenance of 20 trees older than 200 years per ha would require an input rate of 1 to 5 trees x year(-1) x ha(-1) into the 100-150 years old class. The modelling highlighted the importance of initial oak abundance affecting amount of old trees at the end of shorter (100 years) simulation period.

  • Comparison of Swedish and Norwegian Use of Cone-Beam Computed Tomography : a Questionnaire Study

    • Objectives: Cone-beam computed tomography in dentistry can be used in some countries by other dentists than specialists in radiology. The frequency of buying cone-beam computed tomography to examine patients is rapidly growing, thus knowledge of how to use it is very important. The aim was to compare the outcome of an investigation on the use of cone-beam computed tomography in Sweden with a previous Norwegian study, regarding specifically technical aspects. Material and Methods: The questionnaire contained 45 questions, including 35 comparable questions to Norwegian clinics one year previous. Results were based on inter-comparison of the outcome from each of the two questionnaire studies. Results: Responses rate was 71% in Sweden. There, most of cone-beam computed tomography (CBCT) examinations performed by dental nurses, while in Norway by specialists. More than two-thirds of the CBCT units had a scout image function, regularly used in both Sweden (79%) and Norway (75%). In Sweden 4% and in Norway 41% of the respondents did not wait for the report from the radiographic specialist before initiating treatment. Conclusions: The bilateral comparison showed an overall similarity between the two countries. The survey gave explicit and important knowledge of the need for education and training of the whole team, since radiation dose to the patient could vary a lot for the same kind of radiographic examination. It is essential to establish quality assurance protocols with defined responsibilities in the team in order to maintain high diagnostic accuracy for all examinations when using cone-beam computed tomography for patient examinations.

  • Self-reported postoperative recovery in children after tonsillectomy compared to tonsillotomy

    • Objectives: Tonsil surgery is associated with significant morbidity during recovery. Patient-reported outcome measures (PROM) are the golden standard for the planning and follow-up of delivered care, which should also be an axiom for children. The current aims were to describe self-reported postoperative recovery in children after tonsil surgery, and to compare tonsillotomy and tonsillectomy in this respect.Methods: In total, 238 children (4–12 years old) with a history of obstructive problems and/or recurrent tonsillitis, and undergoing tonsil surgery were included. Forty-eight per cent were operated with partial tonsil resection/tonsillotomy (TT) and 52% with total tonsillectomy (TE), all in day surgery.Postoperative recovery was assessed on days 1, 4 and 10 using the validated self-rating instrument PRiC, Postoperative Recovery in Children. This includes 23 items covering different aspects of recovery after tonsil surgery. A higher score indicates worse status in the respective items.Results: Daily life activities (sleeping, eating and playing), physical symptoms (e.g., headache, stomach ache, sore throat, otalgia, dizziness, nausea, defecation, urination), and emotional aspects (sadness, frightening dreams) were affected during the recovery period.The TE-girls showed higher scores than the boys regarding stomach ache, defecation and dizziness.Children above 6 years of age reported higher values for the physical comfort variables, while the younger group showed worse emotional states.Postoperative recovery improved from day 1–10 in all surgical groups. The TE-group showed lower recovery compared to the TT-group (p < 0.01–0.001) in most items.Conclusion: The goal of postoperative management is to minimize or eliminate discomfort, facilitating the recovery process and avoiding complications. Children are able to describe their recovery, and thus, PRiC seems to be able to serve as a PROM to obtain patient-centered data after tonsil surgery. The recovery process after TT causes less postoperative morbidity and a quicker return to normal activity compared to TE.

  • Detectability of normal anatomy in digital panoramic radiographs

    • The aim of this study was to evaluate the image quality of digital panoramic radiographs and its correlation with the detectability of normal anatomical structures. The effects of image enhancement on the detectability were also studied. A total of 500 panoramic images (DICOM format) obtained with a storage phosphor-based digital system were evaluated. The image quality and the detectability of selected normal anatomical structures were evaluated in all images. Images with inadequate image quality were subjected to enhancement after which the detectability of the structures was re-evaluated. Only 9% of the images were classified as having adequate technical quality. The main sources of poor image quality were that the patient's tongue was not held against the palate and incorrect positioning of the patient. Not holding the tongue against the palate was found to have a negative impact on the detectability of maxillary structures. Of the images with horizontal positioning errors the patient's head was rotated to the left in 81% (70 images). The most effective form of enhancement was a combination of increased contrast and decreased brightness. Images in which the tongue was not held against the palate were partially improved, whereas images with positioning errors remained unaffected by this enhancement. In conclusion, most of the panoramic images showed some technical flaws. The marginal bone level and the maxillary area were the most difficult areas to reproduce. Retakes could be avoided in some cases by using image enhancement. However, this should not be regarded as an option to avoid poor image quality.

    • Boverket ser ett behov av en kunskapssammanställning kring nuläget om fuktsäkra byggnader i Sverige för att ha en så bred och korrekt förståelse som möjligt för vem branschen är, vilka regler som tillämpas, var det finns kunskapscentra och vilka utvecklingsbehov som behöver stödjas för att branschen ska kunna ta större ansvar. De utmaningar som noteras är: Byggsektorn beredskap och förmåga att möta de nya förändrade byggregler som planeras av Boverket.Den stora andelen fuktrelaterade fel och skador som rapporteras i olika utredningar. Kraven kopplat till fuktrelaterade innemiljöproblem är otydliga och ofta svåra att följa upp. Detta kan leda till fuktrelaterade innemiljöproblem som orsakar lidande för de individer som drabbas.Miljö och klimatfrågan ställer krav både på resurseffektivitet och minskad klimatpåverkan samt resiliens och klimatanpassning. Förändrade boendevanor och brukarvanor som ändrar på förutsättningarna i en byggnad. Fragmentering och specialisering med många aktörer med ansvar för sin del endast. Några utvecklingsområden har noterats. Områden där det behövs både forskning och utveckling av teknik, processer och samarbeten: Kvalitetssäkring av nya material, konstruktioner och produktionsmetoderKompetensutveckling och kompetensspridning.Framtagning och tillgängliggörande av kunskap, teknikutveckling. Samsyn behöver utvecklas när det gäller bl.a. uttorkning i betong och i vilken omfattning byggnader behöver skyddas mot fukt under byggproduktionen. Undersökning av utvecklingsmöjligheter behövs för att ta vara på digitaliseringens möjligheter med sensorer, digitala tvillingar, AI m.m. i samband med fuktsäkerhet. Kartläggningen skulle förstärkas med en bredare internationell utblick. Det tycks finnas en del skillnader både i arbetssätt och erfarenheter kring fuktsäkra byggnader.Förhoppningen är att kartläggning av fuktsäkra byggnader ska bidra till kunskapsspridning om bransch, aktörer, teknik, system, konstruktion, kontroller, drift och underhåll samt forskning och fortsatt utvecklingsarbete.
