    • OBJECTIVE: To evaluate a management system designed to improve staff adherence to a federal regulation that stated restrained residents should be released, exercised, and repositioned every 2 hours. DESIGN: A delayed intervention, controlled, cross-over design with three phases. During phase one, baseline, the length of intervals that residents remained in restraints was monitored. The intervention was implemented at site A in Phase two while site B remained in baseline. During Phase three, the intervention was replicated at site B. SETTING: Two long-term care proprietary nursing facilities. PATIENTS: Sixty-three physically restrained residents in the two facilities. INTERVENTION: The intervention was a system of restraint release using colored pads corresponding to specific hours. The management rule was that the resident should be on a different colored pad every 2 hours. Staff had to lift residents to place the pad, and the colors made the system easy for supervisors to check. MAIN OUTCOME MEASURES: Checks by research personnel by black light and invisible ink, to detect movement of the knot tying the restraints. RESULTS: During the baseline phase, the majority of residents at both sites were inappropriately restrained longer than 2 hours (site A: 54.1%; site B: 60.1%). The percentage of residents restrained over 2 hours was significantly reduced during the intervention phase to 13.9% (site A) and 19.4% (site B). Three weeks after the end of the intervention, inappropriate use of restraints remained low, 14.2%, but rose to 47.7% after another 3 weeks. CONCLUSION: The management system is an effective way to increase the consistency with which nursing-home staff release and reposition restrained residents.

    • Oncogenesis of tumours related to multiple endocrine neoplasia type 1 (MEN1) is associated with somatic deletions involving the MEN1 locus, suggesting inactivation of a tumour suppressor gene in this region. Identification of meiotic cross-overs in MEN1 families has placed the MEN1 locus centromeric of D11S807. An extended deletion mapping was performed in 27 primary parathyroid tumours, and identified D11S427 as the closest centromeric flanking marker. Through physical mapping using newly isolated cDNA clones, we estimated the distance between the flanking markers D11S807 and D11S427 to be less than 900 kb. One of these cDNA clones showed expression of a 4.4 kb message in multiple tissues, including those affected in MEN1, while in five endocrine tumours no transcript was detected. Sequence characterization showed that this gene encodes for the phospholipase C beta 3, a key enzyme in signal transduction.

  • INVESTIGATIONS OF PLATINIZED AND RHODINIZED CARBON ELECTRODES FOR USE IN GLUCOSE SENSORS

    • Graphite electrodes were modified by the electrodeposition of either platinum or rhodium, using cyclic voltammetry. Following only brief periods nf treatment, plantinized carbon electrodes were able to directly oxidize glucose at a potential of +350 mV (vs. Ag/AgCl). By narrowing both the potential range of deposition (-450 to -500 mV) and cycling period (50 seconds at a scan rate of 10 mV s(-1)), the effects of direct glucose oxidation could be significantly: reduced. Detection of H2O2 at +400 mV was possible; glucose enzyme electrodes were constructed, with glucose oxidase immobilized in hydroxyethylcellulose and operated at this potential. These sensors had a linear range of 0.1 to 25 mM glucose and a slope of 8 nA/mM. Other electrodes were prepared by the electrodeposition of rhodium. These base electrodes proved to be insensitive to glucose and produced higher currents (compared to low platinum electrodes) to H2O2. Glucose sensors were constructed using rhodinized electrodes. Again operating at +400 mV, these sensors had a linear range from 0.1 to 20 mM and a slope of 21 nA/mM glucose.

    • Amperometric enzyme sensors for lactate, glutamate and glutamine were constructed using rhodinised carbon electrodes. All three sensors operated at a potential of +400 mV (Ag/AgCl). Lactate sensors were constructed using lactate oxidase immobilized in hydroxyethylcellulose, overlaid with a cellulose acetate membrane. Using a flow-injection system, the sensors had a linear range of 0.1 to 1.5 mM. Measurements of lactate concentrations from a mammalian cell culture were compared with results obtained from a commercial instrument. A correlation coefficient of r=0.982 (n = 15) was obtained. Glutamate and glutamine sensors were fabricated based on glutaraldehyde immobilization; the former by incorporating glutamate oxidase and the latter based on glutamate oxidase and glutaminase (both had a linear range of 0.1 to 1.5 mM). The feasibility of using these sensors, in conjunction with a previously described glucose sensor, for mammalian cell culture monitoring is discussed
