  • Cross-Resistance to Antibiotics After Exposure to Qaternary Ammonium Compounds

    • Quaternary ammonium compounds (QACs) are common antimicrobials that are used in a variety of consumer products, such as lotions, sunscreen, hair conditioners and hand sanitizers, to inhibit bacterial growth. However, it has been noted that bacteria exposed to QACs can develop resistance, and additionally, resistance to QACs has been observed to provide cross-resistance to antibiotics. In order to identify genetic adaptations for this resistance pattern, we exposed E. coli to three different QACs at sub-MIC and above-MIC concentrations, and identified genetic changes by whole genome sequencing. We found that initial adaptation, at sub-MIC levels, happened through efflux mechanisms, and that subsequent genetic changes, during above-MIC exposure, involved genes associated with the cell membranes and with transcription/translation. We also found that these genetic changes provided cross-resistance to other QACs as well as to several antibiotics.

  • Mutation in the Copper-Induced sil Operon Enables High-Level Silver Resistance and Silver Facilitated Co-Selection of Multidrug Resistance Plasmid

    • Human activities are responsible for an accumulation of metals in health care and agricultural environments, and plasmid-encoded metal tolerance operons enable bacteria to rapidly adapt to metal exposure under such conditions. While the mechanisms of action of many metal resistance systems have been described, there is still limited understanding of their role in co-selection of antibiotic resistance in metal-containing environments. Whether plasmid-encoded metal resistance genes confer significant selective advantages is of interest as it has implications for plasmid enrichment and the spread of plasmid-borne antibiotic resistance genes. To increase our understanding of plasmid-mediated metal resistance, we studied the sil operon and its phenotypes in E. coli during growth in the absence and presence of silver and copper. We found that the sil operon provides resistance to both silver and copper. However, it is induced by copper only, and constitutive expression due to point mutations in the two-component silS gene provides high-level silver resistance. Furthermore, we showed that a high-level silver resistant mutant could be enriched in the presence of silver. This enrichment entailed co-selection of the multidrug resistance plasmid pUUH239.2. Our results show that a copper resistance operon can provide high-level silver resistance following a single point mutation, and that the silver resistance phenotype subsequently can co-select for antibiotic resistance in the presence of silver.

  • Potentiation of the Selective Effect of Antibiotics by Metal Ions

    • Complex mixtures of antibiotics and metals are present in many environments ranging from municipal sewage to irrigation water and manure used as agricultural fertilizer. Such mixtures of drugs and metals exert unique selection pressures on local bacterial communities and could function as hotspots for enrichment of antibiotic resistance genes. The presence of metals in the environment has previously been linked to increases in tolerance to antibiotics. In this study, we investigated metal-potentiated selection of antibiotic resistant Salmonella enterica strains. Six environmentally relevant metals were examined in combinations with three different antibiotics. By performing competitions between an antibiotic resistant mutant and the isogenic wild type in each metal-antibiotic combination, we assessed the minimal selective concentration (MSC) of the antibiotic for the resistant strain. The metals silver, cadmium and mercury all exhibited potentiating effects, reducing the MSC of the antibiotic up to 5-fold as compared to in the absence of the metal. We further show that the potentiating metals increased permeability of the cellular outer membrane. These results demonstrate that the presence of a metal can decrease the concentration of an antibiotic required to select for an antibiotic resistant strain, and they indicate that this process involves metal-facilitated uptake of the antibiotic following damage to the outer membrane.

    • TSCYC är ett frågeformulär som används för att mäta ett brett spektrum av akuta eller kroniska posttraumatiska symptom hos barn som har upplevt traumatiska händelser såsom övergrepp eller misshandel eller har bevittnat våld i hemmet eller i sin närmiljö. Den svenska versionen av TSCYC har översatts av docent Doris Nilsson och professor Carl-Göran Svedin som även har översatt, normerat och standardiserat den svenska versionen av traumainstrumentet TSCC för äldre barn och ungdomar.Den svenska standardiseringen och normeringen av TSCYC har gjorts i en grupp om 629 barn ur normalpopulation och en klinisk grupp om 59 barn i åldrarna 3–11 år. Normerna är uppdelade utifrån ålder och kön.TSCYC-formuläret innehåller 90 frågor och handlar om olika typer av traumasymptom som barnet kan uppvisa. Frågorna besvaras av barnets förälder eller vårdnadshavare. TSCYC ger en detaljerad utvärdering av posttraumatiska symptom på åtta kliniska skalor. En summaskala ger värdefull information om akuta och kroniska symptom samt andra vanligt förekommande symptom hos traumatiserade barn. PTSD-diagnosbladet, som ingår i svarsblanketten, kan användas som stöd i evalueringen av möjlig PTSD-diagnos.

  • On the physiological location of otoacoustic emissions

    • During recent years, much attention has been paid otoacoustic emissions in the clinical audiological practice. The received view locates their origin in the cochlea, more precisely in the outer hair cells. It is, however, still uncertain if there is an interaction between the ears regarding otoacoustic emissions. Earlier findings suggest an interaction at the level of the olivocochlear bundle. The aim of this pilot study was to find out if there is any interaction between the two cochleae in the case of otoacoustic emissions. Five subjects with normal hearing participated. Recordings were made of spontaneous otoacoustic emissions during the presentation of contralateral stimuli at three different frequencies (500, 1000 and 2000 Hz). In general, contralateral stimulation did not provoke otoacoustic emissions. It was concluded that otoacoustic emissions could be part of the fine-tuning mechanism in the cochlea. The frequency resolution, e.g. for speech, depends on very fast modulation of the incoming signal. Due to the neural distance, this modulation would lag behind, if otoacoustic emissions in one ear would effect the opposite one.

  • A decision tree multifactorial approach for predicting dementia in a 10 years’ time

    • Background: Dementia is a complex neurological disorder, to which little is known about its mechanisms and no therapeutic treatment was identified, to date, to revert or alleviate its symptoms. It affects the older adults population causing a progressive cognitive decline that can become severe enough to impair the individuals' independence and functioning. In this scenario, the prognosis research, directed to identify modifiable risk factors in order to delay or prevent its development, in a big enough time frame is substantially important.Objective: This study investigates a decision tree multifactorial approach for the prognosis of dementia of individuals, not diagnosed with this disorder at baseline, and their development (or not) of dementia in a time frame of 10 years. Methods: This study retrieved data from the Swedish National Study on Aging and Care, which consisted of 726 subjects (313 males and 413 females), of which 91 presented a diagnosis of dementia at the 10-year study mark. A K-nearest neighbors multiple imputation method was employed to handle the missing data. A wrapper feature selection was employed to select the best features in a set of 75 variables, which considered factors related to demographic, social, lifestyle, medical history, biochemical test, physical examination, psychological assessment and diverse health instruments relevant to dementia evaluation. Lastly, a cost-sensitive decision tree approach was employed in order to build predictive models in an stratified nested cross-validation experimental setup.Results: The proposed approach achieved an AUC of 0.745 and Recall of 0.722 for the 10-year prognosis of dementia. Our findings showed that most of the variables selected by the tree are related to modifiable risk factors, of which physical strength was an important factor across all ages of the sample. Also, there was a lack of variables related to the health instruments routinely used for the dementia diagnosis that might not be sensitive enough to predict dementia in a 10 years’ time.Conclusions: The proposed model identified diverse modifiable factors, in a 10 years’ time from diagnosis, that could be investigated for possible interventions in order to delay or prevent the dementia onset.

  • Patient experiences of developing Sick Building Syndrome (SBS) symptoms and related encounters – a qualitative study

    • Objective:This study aimed to describe the experiences and encounters of a group of people with Sick Building Syndrome (SBS) as their symptoms emerged.Design, methods and subjects:A semi-structured interview was performed with 10 informants with symptoms of SBS. Five of them were previously diagnosed and had participated in an earlier follow-up study. The remaining five had had SBS symptoms for a shorter period. The interviews were analysed using qualitative content analysis. To ensure trustworthiness triangulation was used. Results:The analysis resulted in a main theme: Conflicting Interpretations of SBS Symptoms, and three subthemes: Gradual Understanding of SBS Symptoms, Experience of Rejection and Experience of Confirmation. The main theme and the sub-themes together represent a process where the informants’ experiences of belief and support, lack of support and disbelief and limited understanding occurs when they are perceiving symptoms, looking for explanations, understanding  the connection between symptoms and being  in a poor indoor environment  and while managing their SBS symptoms.The symptoms, when they first appear, often appear similar to that of flu. When the informants realise the connection between the symptoms and the building some of them decide that “Okay, enough is enough, there’s nothing wrong with me, it’s the building”. Many of the informants act to  change the way they manage the situation as a result of increased symptoms. During this whole process the informants/patients perceived a lack of confirmation and support from e.g. employers, primary health care and occupational health care centres. The informants were confirmed by colleagues acting for a better indoor environment.Conclusion:The symptoms of SBS are diffuse and difficult to recognise for all involved. The experiences of those who have dealt with this issue are in line with knowledge relating to other diseases not recognised as legitimate. It is important for healthcare personnel to be aware of the possible connection between symptoms and the indoor environment. For employers and facility managers, knowledge of their obligations in terms of the working environment is important. The importance of confirmation in encounters has to be underlined.
