  • Older assistant nurses’ motivation for a full or extended working life

    • The aim of this study was to explore older workers’ motivation for a full or extended working life. With particular focus on assistant nurses aged 55–64 years, working in the elderly care sector. Focus group interviews were conducted with five different groups of assistant nurses. Inductive thematic analysis was used to analyse the interviews and five main themes were developed from the data: ‘Organisational issues’, ‘Health-related problems’, ‘Private issues’, ‘Meaningfulness and appreciation’ and ‘Social support’. Several of the main themes concerned problems with too high work demands of the assistant nurses. These findings suggest that it is important to improve the working conditions of assistant nurses in order to create a more sustainable working life. Increasing the number of staff and improving recovery opportunities and work–life balance could be important steps to improving the working conditions for this group. Finally, upgrading the competency and professionalism of assistant nurses could help to increase the motivation for a full or extended working life.

    • Social participation is important for health, and it is well known that high strain jobs impact negatively on mental and physical health. However, knowledge about the impact of psychosocial working conditions on social participation from a long-term perspective is lacking. The purpose of this study was to investigate the associations between different job types and social participation from a long-term perspective. A comprehensive public health questionnaire “The Scania Public Health Survey”, was used, and psychosocial working conditions were measured with a Swedish translation of the Job Content Questionnaire. Based on data from 1098 working respondents aged 55 at baseline and a 10-year follow-up when the respondents were not working, the analyses revealed that social participation varied by job type. Jobs with high decision latitude, as in active and relaxed jobs, seem to predict high social participation, even after cessation of employment. Besides that, the result suggests that high social participation during working life is a predictor of high social participation from a long-term perspective which promotes healthy aging. Incentives for working longer are strongly related to good working conditions. A supportive work environment with possibilities for employees to participate in decision making, i.e., high control, is vital for a sustainable working life. This may contribute to an extended working life and may also support social participation prior to retirement as well as after retirement and thus to healthy aging.

  • To work or not to work in an extended working life? Factors in working and retirement decisions

    • In most of the industrialised world, the proportion of older and retired people in the population is continuously increasing. This will have budgetary implications for maintaining the welfare state, because the active working section of the population must fund the non-active and old population. Aim: The overall aim of this thesis was to obtain knowledge about older workers’ work and life situation in association with their planning and decision to retire from working life. Method: The thesis includes one qualitative and three quantitative studies conducted in Sweden. Result: Self-rated health was found to be a better measure than diagnosed disease of whether older workers believed they could work until 65 years or beyond. Health seems not to be a general impediment to working in old age if older workers are satisfied with their work situation and have enough time and opportunities to recover from fatigue. In one of Sweden’s most hazardous work environments, older workers were not injured significantly more often than younger workers. Good mental and physical work environment, moderate working pace and working time, and the right competence and possibility for skills development were factors determining whether older workers believed they can extend their working life. Attitude to older workers in the organisation, motivation and work satisfaction were factors determining whether older workers want to extend working life. Health, personal economic incentives, family/leisure pursuits and attitude to pension in society affected both whether people believed they can and wanted to extend their working life. In their final retirement decision, older workers considered: i) their possibility to balance and adapt functional ageing and health to a sustainable work situation; ii) their economic situation; iii) possibilities for social inclusion and coherence; and iv) possibilities for meaningful activities. Whether these requirements were best fulfilled in or outside working life determined the decision to continue working or to retire. Conclusion: If it is desirable for society that people will to extend their working life, both the “can work” and the “want to work” factors need to be met. It is important to provide a good fit inside working life. This requires a focus not only on older workers, but also on organisations and managers in order to provide incentives that keep older workers in the work force.

  • Occupational risk of COVID-19 across pandemic waves: a two-year national follow-up study of hospital admissions

    • Objective Assuming that preventive measures to mitigate viral transmission of SARS-CoV-2 at the workplace may have been improved in the course of the COVID-19 pandemic, we examined the occupational risk of COVID-19 related hospital admission across the four pandemic waves in Denmark between week 8, 2020, and week 50, 2021. Methods The study included 4416 cases of COVID-19 related hospital admissions among 2.4 million Danish employees aged 20-69 with follow-up in 2020 through 2021. At-risk industrial sectors and a reference population were defined a priory by a job-exposure matrix on occupational risk for COVID-19. Incidence rate ratios (IRR) and potential effect modification by pandemic wave were computed with Poisson regression adjusted for demographic, social and health factors including completed COVID-19 vaccination. Results We observed an overall elevated relative risk in four of six at-risk industrial sectors, but the pandemic wave only modified the risk among healthcare employees, where the excess risk from a high initial level declined to background levels during the latest waves in models not adjusting for COVID-19 vaccination. In social care, education and transport, the elevated risk was not modified by pandemic wave. Conclusion Danish healthcare employees were to some extent protected against occupational transmission of SARS-CoV-2 during the two last pandemic waves even though the absolute risk conferred by occupation may not have been eliminated. Early vaccination of this group seems not to be the only explanation. The risk in other sectors remained elevated indicating a need to revisit preventive measures.

  • Healthy workplaces for women and men of all ages

    • The aim of this knowledge compilation is to contribute with knowledge about the work environment in relation to the ever-older workforce. How do employee needs and possibilities change from a course of life perspective? What should the employer and other work environment actors think about in order for the workforce to be able to and want to work to a high age?The Swedish Work Environment Authority wishes to give an overarching knowledge profile of different aspects of the work environment and the ageing workforce, and we therefore asked seven researchers to summarise the research-based knowledge within each of their areas, from a course of life and gender perspective. An eighth researcher acted as an editor for the anthology, and has also written the preface.In summary, the report shows that we are becoming even healthier, living ever longer and working to an ever higher age. Older people in the workforce are positive for the economy because productivity increases, and the business sector can make use of competent and experienced staff for a longer time. But for the older labour force to be healthy and want to work at higher ages, one needs to take into consideration how ageing influences health and the capacity to work. With age, all people are affected to different degrees by reduced vision, hearing and physical capacity, as well as longer reaction times. Even their cognitive capacity changes. Certain cognitive abilities are strengthened with rising age, while others deteriorate. With an ageing workforce, more employees have chronic illnesses, which, however, seldom affect the actual working ability. Changes in working life also affect health and wellbeing, for example deregulated work and the technical development. Age and previous experiences impact upon our ability to adapt to these changes. One factor that promotes adaptation is partly resilience (that is to say, resistance and the ability to adapt to the new), partly compensation strategies when the mental and physical resources change. There are no great differences between gender when it comes to the consequences of ageing on health and wellbeing in the work. On the other hand, the public health trend shows increasing differences in health between the lower educated and the higher educated - a difference increasing more quickly among women than among men. The gender-segregated labour market also means that more women than men work in physical and mentally burdensome work. Attitudes at the workplace also affect wellbeing and the will to continue working at higher ages. Men tend to be more sensitive to age discrimination while women run the risk of double discrimination, that is to say based upon both gender and age. Work environment and the attitude to an older workforce are central to the considerations that an employee makes in the choice between continuing to work and retiring. Other prerequisites that influence the decision are one’s own health, private finances and self-fulfilling activities.The employer can do a great deal to lengthen and improve their employees’ working life. Systematic work environment management benefits everyone, and it can contribute to everyone keeping their working ability and to older people wanting to and being able to work for longer. Occupational health services of good quality also play an important role. Technical aids and adaptation of the working pace and working tasks are other measures that improve the work environment for the older workforce. The employer can also contribute to stimulating work arrangements and organisational support for the employees in order to strengthen their resilience and promote the development of compensation strategies.

    • The frequent mounting and dismounting the tractor required by many farming operations increases the risk of falls, particularly for older farmers. The present study explored the risk factors related to tractor ingress and egress in older farmers from two countries with a different tradition in terms of safety culture: Sweden and Italy. Eighteen male farmers aged 65 + (8 from Skåne region, southern Sweden, and 10 from Piedmont region, northwestern Italy) were observed while mounting and dismounting their most used tractor, to investigate the routine behaviors adopted and to identify possible sources of risk of fall. The presence of three critical behaviors was recorded: the maintenance of three-point contact with the machine when entering and exiting the cab; facing the cab and the use of the last step when exiting. Farmers were also interviewed about their health status, attitudes toward safety, and perceived risks while performing the task. The results showed that similar unsafe behaviors were adopted by most of both Swedish and Italian participants; in particular, none of the farmers got off the tractor by facing the cab. Older farmers from both countries referred to age and previous experience as the major protective factors against falls, without acknowledging that new risks can rise from the age-related changes in their motor skills. The results raised some considerations about the need to develop targeted elderly-centered solutions to support the correct mounting/dismounting behaviors, both in the design of the machines and in information campaigns and training courses, which may have a cross-cultural validity.

  • Parents' Attitudes to Risk and Injury to Children and Young People on Farms

    • ObjectivesChildren and young people growing up in a farm environment run a greater risk of being injured or dying in an accident than their non-farming counterparts. This study examines farming parents' attitudes and experiences of having their children grow up on farms, one of the most dangerous work environments as their home, everyday environment and playground.MethodData were collected using two ethnological methods, a question list and interviews, with a study population of 20 parents. The data were analysed phenomenologically.ResultsThe analysis pursued four themes: i) the most dangerous places and situations on the farm; ii) children's tasks on the farm; iii) children as a safety risk on the farm; and iv) farm risk education for children.ConclusionsMost parents know the risks on their farm, but are sometimes careless when working under stress or exhaustion. Some parents wanted more information and some wanted compulsory preventative or safety measures by manufacturers, e.g. a safety belt as standard on the extra seat in tractors. Children's friends were described as one of the greatest risks for injury due to peer pressure. Some parents mentioned that people who grow up on farms are sometimes 'blind' to the dangers. Other parents seemed to overlook the risks and had their children carrying out tasks for which they were not mentally or physically equipped. Some of the tasks the children reportedly carried out on farms contravened Swedish legislation. It is thus important for farming parents to be repeatedly reminded of the risks to their children and to increase their awareness of how to prevent and eliminate risks in order to avoid accidents on the farm. The situation for farm children is highlighted in a critical discussion.

  • Åtgärder för att minska risken för arbetskador bland äldre lantbrukare

    • På grund av den förändrade demografin blir det allt fler äldre inom arbetskraften i västvälden. Detta inte minst inom lantbruket där antalet äldre som arbetar kvar till en hög ålder är betydande. De generella fysiologiska förändringarna med ökande ålder, som exempelvis sämre syn, hörsel och reaktionsförmåga, är riskfaktorer för att drabbas av arbetsskador. Lantbruket är samtidigt en av Sveriges farligaste arbetsplatser. Med anledning av detta är det viktigt att åldersanpassa arbetslivet utifrån dessa åldersförändringar. Det övergripande syftet med projektet var att studera genomförda åtgärder för att minska risken för arbetsskador bland äldre inom lantbruket. Projektet bestod av två delar: i) Att göra en kunskapsöversikt av internationella interventioner för äldre inom lantbrukets arbetsmiljö där åtgärderna ska ha utvärderats. Kunskapsöversikten avser därigenom att kunna visar på vilka insatser som verkligen har visat sig minska risker för olyckor och ohälsa i lantbruksarbetet bland äldre (55 år och äldre). ii) Att intervjua äldre lantbrukare om vilka åtgärder de hade vidtagit i sitt dagliga arbete vid ökande ålder för att minska risken för arbetsskador. Den systematiska litteraturgenomgången av internationella studier visade på bristen av utvärderade åtgärder och interventioner angående att minska risken för arbetsskador bland äldre på lantbruk. Trots åtskilliga sökningar återfanns enbart tre utvärderade interventioner varav enbart två hade utvärderat effekten av åtgärden och en hade utvärderat antalet deltagare i interventionen. Resultatet från intervjustudien med de äldre lantbrukarna visade på att de åtgärder som de hade vidtagit för att klara av sitt dagliga arbetsliv var: 1) Anpassa arbetet till sitt funktionella åldrande genom att lägga om produktionsinriktningen, köpa in tjänster eller låta någon annan göra alltför fysiskt tungt och stressande arbete; 2) Lyssna på sin kropp och ta pauser, vila och återhämta sig så man inte blev för sliten och för att minska risken för olyckor och skador; 3) Kontinuerligt underhåll av maskiner, byggnader och utemiljöer; 4) Använda ny teknik för att minska arbetsbelastningen och göra arbetet säkrare; 5) Fysiskträning för att stärka alla kroppens muskler och bevara/förbättra styrka och uthålligheten. 6) Personlig skyddsutrustning och funktionella arbetsskor och arbetskläder.
