    • The objective is to describe how the uptake of online video consultations was affected by the Covid-19 pandemic, using data from a Swedish primary care setting. There seem to be a relationship between the use of video consultations and spread of Covid-19, especially in 2020 when the use varied more with the contagion waves. In 2021 the use was more consistent over time. In summary, the Covid-19 pandemic had a direct impact on the adoption of video consultations in our primary care setting, yet after the initial high use, it appears to have stabilized on a lower level.

    • Sweden has had significant progress with the the introduction of electronic health records. A pilot county deployed in an eHealth service in 2012, giving access to health records for all of its patients. This eHealth service is, however, a controversial issue. Two surveys were conducted to discover whether healthcare professionals' opinions differ between professionals, and between staff who have had experience with patients using eHealth records and those, to date, who have had none. Experienced nurses found this eHealth service more important for the patients compared to unexperienced nurses outside the pilot county, as well as both semi-experienced physicians.

    • BackgroundThe use of clinical practice guidelines in clinical and organizational decision-making improves the care of patients and patient safety. Guidelines make healthcare consistent and efficient. In many low- and middle income countries healthcare workers depend on guidelines developed in higher income countries. For these to be useful and accepted they need to be adapted to the local setting. The aim of the study was to implement pre-hospital treatment guidelines into an organization that was not currently using guidelines. The study was partnership between a Swedish pre-hospital training organization and local ambulance service organizations in Kosovo.MethodsAn iterative process of implementing the guidelines was used:Identify guidelines appropriate for the local organization.In sets of five, have the guidelines translated into Albanian.Adapt the guidelines to the local conditions and context.Approval of guidelines by an expert group.Begin using the guidelines within the target organization.ResultsThe first set of five guidelines was translated, approved, and implemented into the organizations. To improve acceptance, both practitioners and decision-makers were involved in the process. Seven workshops were held, with a total of 104 participants. In follow-up discussions participants said they accepted the guidelines and that they would be useful in their daily work.ConclusionsIt is important to evaluate to what extent the guidelines have been accepted, understood, and used. The success and acceptance is due to the flexible procedure managing the full implementation process. Adapting guidelines to fit with local needs and requirements using local experts made the guidelines accessible and useful. The workshop discussions established justification and approval.Key messagesInvolve local experts in contextualising guidelines to increase acceptance from the start.Support building a robust local implementation organization to assist and administer change.

    • There is a broad consensus surrounding the acutely vulnerable plight of asylum-seeking unaccompanied children, transcending international policy, academic literature, and expert opinion in both medicine and jurisprudence. Children migrating alone are at extreme risk of exposure to violence, exploitation, and poor health, and thus require extensive social, medical, and legal support, both during migration, and post migration whilst waiting for their asylum claims to be processed. The Swedish government has voiced intent to protect the human right to asylum in Europe, accenting that processes must be solidarity-based, legally certain, sustainable, efficient, and humane in facilitating reliable assessments protecting the rights of asylum-seekers. The present study explores current debate surrounding the deficient scientific reliability of recently introduced two step forensic age assessment procedures in Swedish asylum processes involving unaccompanied minors, and discusses the stakes involved from a medical, ethical, and legal perspective. The analysis included current policy documents, health professions unions' statements and reports, expert and media debate, and United Nations committee reports. Although the vast majority of forensic age assessment conducted thus far in Sweden have assessed biological age as 18 or above, expert consensus has decried methods applied as being unable to withstand the test of scientific scrutiny. Concerns surrounding the procedures’ medically unjustifiable and highly unethical nature, as well as the misuse of the medical profession' authority, expertise, and skill in forwarding political agendas have been noted. Furthermore, in disregarding the precautionary principle by default, current reductionist and pseudoscientific forensic methods risk facilitating the bypassing of laws intended to protect the human rights of children at their most vulnerable. These highly consequential forensic age assessments must yield results greater than chance, as the aftermath of failures are neither humane, sustainable, or legally certain. Flawed results can impede children from accessing foundational human rights, such as the right to health, to thrive, and to meet their full potential, raising resounding medical, ethical, and legal concerns. The social sciences must contribute with expertise and actively engage in this debate, and work toward developing and establishing a thorough and holistic age assessment process, including biomedical aspects such as growth curves, x-rays, and adding psychosocial components such as recollection of childhood memories, psychosocial maturity, along with reports from adults in the children’s social networks (such as teachers, social workers, and others) on how the assessed child handled everyday life.

  • Does the death postponement phenomenon really exist?

    • It is a common belief that people close to death from natural causes can postpone their imminent death if they see a strong reason to survive a bit longer. This is known as the Postponement hypothesis: that a meaningful occasion can act as a motivator to prolong life for a short amount of time. A few studies have already addressed this hypothesis but their conclusions are contradictory. To check the postponement hypothesis, we analysed almost 249 thousand cases in the dataset for South African people who died in the year 2015. We took a person’s birthday as the meaningful occasion and analyse the death rate around this date using statistical models offered by survival analysis. If the hypothesis is true, it can be expected that the mortality rate should be lower a period just before the birthday and, perhaps, higher shortly afterwards. The results of our analysis show that no postponement of death can be seen for the examined dataset. In fact, to the contrary, the data suggest that the mortality rate is higher both before and after the birthday. Speculations as to why this is the case might be a higher risk associated with the stress of expectations for the birthday as well as an earlier start of celebrations with associated departure from the recommended regime.

    • Rationale: Adolescents today use social media applications extensively and research demonstrates that peers in social media settings can influence adolescents regarding their food intake. These newly emerged channels also offer unique possibilities to observe adolescents’ dietary communication. Objective: This study aimed to explore how adolescents communicate food images in a widely used social media image-sharing application, Instagram. Methods: To find adolescent Instagram users we searched for images appended with the hashtag #14år (Swedish for “14 years”). The hashtag had been applied to 3479 images as of March 2014. However, as users change their privacy settings, delete their accounts, or change their user names, 1358 images were not retrievable. Users sometimes also applied the hashtag to several images, and we excluded accounts that we judged did not belong to adolescents (based on written and visual profile information); 1001 unique Instagram users’ photo streams were thus eligible for analysis. Content analysis was used to identify food items and categorize these based on types of food and how the food items were presented. Results: Most of the adolescent users (85 %) shared images containing food items. A majority of the images (67.7%) depicted foods high in calories but low in nutrients. Almost half of these images were arranged as a still life with food brand names clearly exposed. Many of these images were influenced by major food marketing campaigns. Fruits and vegetables only occurred in 21.8% of all images. This food group was frequently portrayed zoomed in with focus solely on the food, with a hashtag or caption expressing palatability. These images were often presented in the style of a cook book. Conclusions: Food was presented in varied ways. Adolescents themselves produced images copying food advertisements. This has clear health promotion implications since it becomes more challenging to monitor and tackle exposure to marketing of unhealthy foods to young people in these popular online networks because images are part of a lifestyle that the young people want to promote. Shared images contain personal recommendations, which mean that they may have a more powerful effect than commercial food advertising.
