    • The aim of this article is to contribute with knowledge about how teachers in school-age educaredescribe their teaching. Teachers in school-age educare move between two different value systemsas they are in a field of tension between tradition and new educational policy intentions. Writtenreflections from 48 teachers at 31 schools in four municipalities have been analyzed based on whathas been described as necessary teaching elements; intentionality, interactivity and intersubjectivity.The deductive analysis resulted in four themes; Play-oriented teaching, Subject-related teaching,Situation-based teaching and Value-based teaching. The results, based on the study's analyzingconcepts, show that the teacher's control is central to whether the activities that the teachers carryout can be called teaching or not. The results also show that teaching in school-age educare stillcenters around a historical core of play, child focus and adaptation to the situation, even though thishas been supplemented with new educational policy intentions that emphasize teaching andevaluation of childrens’ learning. However, the analyzes also show that teachers in school-ageeducare can move flexibly between the two value systems.

    • I skolans styrdokument betonas vikten av att elever ges möjlighet att utveckla sitt kritiska tänkande. Detta är inte minst angeläget i dessa tider av tillgång till sociala medier och spridande av så kallade alternativa fakta. Trots sin positiva klang finns det dock ingen etablerad konsensus kring vad kritiskt tänkande egentligen är. Inom ramen för ett skolutvecklingsprojekt i samverkan mellan Uppsala universitet och Tiundaskolan, en 4-9-skola i Uppsala, utforskar vi hur kritiskt tänkande kan uttryckas i undervisningspraktiken i ämnena svenska, historia, matematik och fysik. Som exempel har vi i fysikämnet designat, genomfört och analyserat en undervisningssekvens utifrån kärnkraft som tema, där elever i årskurs 9 gavs möjlighet att anamma olika åsikter och argument i frågan genom rollspel. Eleverna genomförde en debatt, där de representerade olika parter: boende nära Forsmark, miljöorganisationen Grön Fred, och företag som utvecklar kärnkraft, respektive vindkraft. Före och efter debatten skrev eleverna individuella texter där de argumenterade för sin personliga åsikt i frågan: Ska kärnkraften bevaras som den är, läggas ner, eller utvecklas? Vi fann att de genom debatten fick goda möjligheter att utveckla och visa kunskaper motsvarande flera kunskapskrav i kursplanen i fysik som annars sällan berörs i fysikklassrummet, såsom, för betyg A: ”Eleven kan samtala om och diskutera frågor som rör energi, teknik, miljö och samhälle och skiljer då fakta från värderingar och formulerar ställningstaganden med välutvecklade motiveringar samt beskriver några tänkbara konsekvenser.” Som exempel på naturvetenskapligt förankrade argument utnyttjade eleverna genererad energi per utsläppt mängd koldioxid som ett mått vid jämförelser mellan kärnkraft och andra energikällor. I de individuella texterna höll de flesta eleverna fast vid sina åsikter från innan de arbetade med temat även efteråt, men nu med fler och mer nyanserade argument.

  • Participation in occupational therapy in psychiatric care

    • One of the most important challenges of health and medical care is to strength the role of the patient in the treatment. In psychiatric care the patient must be seen as a resource and should be given the opportunity to participate in his treatment. The overall aim of the thesis was to investigate and describe how patients in psychiatric care perceive participation, and how existing assessments support participation.Study I describes how patients in psychiatric institutional care perceived their opportunities to be active and to participate in their own treatment. The ICF (International Classification of Functioning Disability and Health) inspired the study. By means of a questionnaire, 61 patients reported their opinions of the value of received care, highlighting concepts such as activity and participation. Ten of those patients were then selected for a semi-structured interview. The study showed that patients who were treated according to compulsory care (LPT) were generally more dissatisfied with their opportunities to be active and participate in their own care than patients treated according to the law of health and medical care (HSL). Younger patients in particular were more dissatisfied. Some important factors in the environment were continuity and reception from the staff. Facilitating factors for activity and participation were agreement between patient and staff about the treatment plan, discussions about expectations, and creating conditions for engagement in activities and or responsibility.Study II investigates if there is harmony between the CPRS-S-A (Comprehensive Psychopathological Rating Scale-Self-Assessment), the OCAIRS–S (Occupational Circumstances Interview and Rating Scale) and the OSA (Occupational Self Assessment), and if they can replace each other when the occupational therapist collects information about the patient. Another aim was to investigate how occupational therapist uses the collected information in the treatment plan process. Fourteen patients with depression disorders took part in the study. The study showed that even if the symptoms of the disease were improved at the end of the treatment period the patients still had problems with occupational performance, reduced self-confidence and the structure of their day. Consequently the assessments cannot replace each other. The study emphasized the importance of using both interview and self-assessments when collecting information about the patient, since these methods complement each other in identifying the needs and goals of the treatment. Many problems were related to the patient’s social environment but this was not reflected in the treatment plan; few goals were identified in this area.In conclusion, occupational therapists should use self-assessments and interviews in order to support the patient’s participation in psychiatric care. Furthermore, it is important to use assessments for both occupational performance and medical symptoms when identifying the patient’s needs and goals of treatment since there is a discrepancy between the two areas; symptoms are reduced earlier than perceived problems in occupational performance. Regardless of what kind of law the patient is treated under, all patients have the right to participate in their own treatment. This thesis also shows that the social environment is important in enabling the participation of patients in their psychiatric care.

    • PURPOSE: The aim of this study was to investigate the therapeutic efficacy of the alpha-radioimmunotherapy of ovarian cancer in mice, using different specific activities. This study was performed by using the monoclonal antibody, MX35 F(ab')(2), labeled with the alpha-particle-emitter, 211At. METHODS: Animals were intraperitoneally inoculated with approximately 1 x 10(7) cells of the cell line, NIH:OVCAR-3. Four (4) weeks later, five groups of animals were given 400 kBq of 211At-MX35 F(ab')(2) with specific activities equal to 130, 65, 32, 16, or 4 kBq/microg, respectively (n = 18 in each group). As controls, animals were given unlabeled MX35 F(ab')(2) (n = 12). Eight (8) weeks after treatment, the animals were sacrificed and the presence of macro- and microscopic tumors and ascites was determined. RESULTS: The tumor-free fractions (TFFs) of the animals, defined as the fraction of animals with no macro- and microtumors and no ascites, were 0.67, 0.73, 0.50, 0.50, and 0.17 when treated as above. Only the TFF of 0.17, for the specific activity of 4 kBq/microg, was significantly less, compared to that of the specific activity of 130 kBq/microg. The TFF for the specific activity of 4 kBq/microg showed a significant lowering, compared to the specific activity of 130 kBq/microg (p < 0.05). Treatment with unlabeled MX35 F(ab')(2) resulted in a TFF of zero. CONCLUSIONS: A specific activity-dependent therapeutic outcome could not be shown in the interval of 130- to 16 kBq/mug. For lower specific activities (i.e., 4 kBq/microg), the therapeutic efficacy was significantly lowered.

    • This study investigated the kinetic strategy and compensatory mechanisms during self-ambulatory gait in children with lumbo-sacral myelomeningocele. Thirty-one children with mid-lumbar to low-sacral myelomeningocele who walked without aids and 21 control children were evaluated by three-dimensional gait analysis. Joint moments in all planes at the hip and knee and sagittal moments at the ankle, as well as joint power and work done at all three joints, were analyzed. Joint moment capacity lost due to plantarflexor and dorsiflexor weakness was provided instead by orthotic support, but other joints were loaded more to compensate for the weakness at the ankles and restricted ankle motion. Subjects with total plantarflexor and dorsiflexor paresis and strength in the hip abductors had more knee extensor loading due to plantarflexor weakness and dorsiflexion angle of the orthotic, ankle joint. The subjects with orthoses also generated more power at the hip to supplement the power generation lost to plantarflexor weakness and fixed ankles. The most determinant muscle whose paresis changes gait kinetics was the hip abductor. Hip abductor weakness resulted in a characteristic pattern where the hips displayed an eccentric adduction moment, mediating energy transfer into the lower limbs, and the hips replaced the knees as power absorbers in early stance. Joint moment, power and work analyses complement a kinematic analysis to provide a complete picture of how children who have muscle paresis recruit stronger muscle groups to compensate for weaker ones.

  • A Novel Miniaturised Dynamic Hollow-Fibre Liquid-Phase Micro-Extraction Method for Xenobiotics in Human Plasma Samples

    • Bioanalytical chemistry is a challenging field, often involving complex samples, such as blood, plasma, serum or urine. In many applications, sample cleanup is the most demanding and time-consuming step. In the work underlying this thesis a novel dynamic miniature extractor, known as a hollow-fibre liquid-phase microextractor (HF-LPME), was designed, evaluated and studied closely when used to clean plasma samples. Aqueous-organic-aqueous liquid extraction, in which the organic liquid is immobilised in a porous polypropylene membrane, was the principle upon which the extractor was based, and this is discussed in all the papers associated with this thesis. This type of extraction is known as supported-liquid membrane extraction (SLM). The aim of this work was the development of a dynamic system for SLM. It was essential that the system could handle small sample volumes and had the potential for hyphenations and on-line connections to, for instance, LC/electrospray-MS. The design of a miniaturised HF-LPME device is presented in Paper I. The extraction method was developed for some weakly acidic pesticides and these were also used for evaluation. In the work described in Paper II, the method was optimised on the basis of an experimental design using spiked human plasma samples. Paper III presents a detailed study of the mass-transfer over the liquid membrane. The diffusion through the membrane pores was illustrated by a computer-simulation. Not surprisingly, the more lipophilic, the greater the retention of the compounds, as a result of dispersive forces. The main focus of the work described in Paper IV was to make the HF/LPME system more versatile and user-friendly; therefore, the extractor was automated by hyphenation to a SIA system.
