    • The aim of this epidemiological survey was to analyze the periodontal conditions of 19-year old individuals in two rural county areas, i.e. Fyrbodal and Skaraborg, Västra Götaland, Sweden, with special reference to gender and socioeconomic grouping. A randomized sample of 506 individuals (Fyrbodal 250 and Skaraborg 256 individuals, respectively) was clinically examined with regard to oral hygiene, gingivitis, periodontal pockets and gingival recession. Bitewing radiographs were used for assessment of alveolar bone level (ABL) and dental calculus. A questionnaire-based interview regarding oral hygiene habits was included. A majority of the subjects (76%) claimed to brush their teeth at least twice a day, while interdental hygiene means were used daily by 4%. The subjects showed a mean plaque score of 47% and a gingivitis score of 56%. Forty-six % of the adolescents had a plaque score of > or = 50%, whereas the corresponding figure for gingivitis was 62%. The subjects had on average 5.5 teeth with facial gingival recession. The mean prevalence of sites with probing depth (PPD) of > or = 4 mm was 8, out of which 99% were located at proximal sites. A radiographic bone level of > 2 mm was observed at on average 0.4 teeth per subject. Logistic regression analyses revealed that gender (males) and county area (Fyrbodal) were significant factors for a high plaque and gingivitis score. There was no significant difference in periodontal conditions in relation to socio-economic grouping. In conclusion, the survey revealed higher prevalence of plaque and gingivitis among male than female adolescents, but no differences between socioeconomic groups.

    • Objective: The aim of the study was to evaluate the effects of minocycline microspheres on periodontal probing depth reduction when used in combination with surgery in adults with moderate to severe, chronic periodontitis. Material and Methods: Sixty patients with a minimum of one non-molar periodontal site >/=6 mm in two oral quadrants received either local minocycline microspheres at baseline, immediately following each of two surgical therapies (Weeks 2 and 3), and at Week 5 or surgery alone. Results: The mean probing depth reduction at Week 25 at sites >/=5 mm at baseline was 2.51 mm in the test group and 2.18 mm in the control group. Smokers in the test group had a significantly greater probing depth reduction (2.30 mm) than smokers in the control group (2.05 mm). The number of sites with probing depth reductions of >/=2 and >/=3 mm were significantly higher in the test group than in the control group. Conclusion: Applications of local minocycline as an adjunct to surgery in adults with moderate to severe, chronic periodontitis were associated with statistically significant greater reductions in probing depth than surgery alone.

  • On antimicrobial approaches to arrest and control chronic periodontitis

    • The main objectives were to study the effect of frequently repeated supragingival plaque removal on the subgingival microbiota and periodontal pocket reduction and to analyze the adjunctive effect of different antimicrobial agents on the treatment of periodontal disease. Subjects with gingivitis or chronic periodontitis were included. The patients in Study I underwent professional supragingival removal of plaque and calculus 2-3 times/week for 30 weeks. At sites with suprabony and infrabony pockets and furcation sites, repeated supragingival plaque removal reduced the total number of microorganisms, as well as the percentage of sites with P. gingivalis. In Study II, six months of unsupervised use of a dentifrice containing 0.3% magnolia extract produced significantly less gingival inflammation than a corresponding control dentifrice. Furthermore, at sites with similar amounts of plaque, fewer signs of gingival inflammation were observed in the magnolia group than the control group. In Study III, the effect of topically applied PVP iodine used as an adjunct both during basic SRP and at re-treatment during long term-maintenance was studied. PVP iodine, applied topically during subgingival instrumentation, may improve the outcome of SRP therapy. In Study IV, the short- and long-term effects of the systemic administration of tetracycline in conjunction with SRP were studied. One year after active therapy, the probing attachment level in the test group was almost 3 times higher than in the control group. Re-examinations after 3, 5 and 13 years of SPT disclosed that this short-term benefit was not maintained in the longer perspective. In Study V, the effects of topically applied minocycline in combination with surgery on PPD reduction were analyzed. Local minocycline as an adjunct to surgery produced a statistically significantly larger reduction in probing depth (0.3mm) compared with surgery alone.

  • The effect of a dentifrice containing Magnolia extract on established plaque and gingivitis in man: a six-month clinical study

    • Aim The aim was to evaluate the clinical effect of a dentifrice containing 0.3% Magnolia extract on dental plaque and gingivitis. The trial was a 6-month double-blind, stratified, randomized and 2-armed parallel group study. Forty-six subjects in the test group brushed their teeth with a dentifrice containing 0.3% Magnolia extract and 48 subjects in the control group brushed with a placebo dentifrice. Plaque and gingivitis were assessed at baseline, 3 and 6months. There was a significantly larger gingivitis reduction in the Magnolia group than in the control group (0.26 +/- 0.11 versus 0.11 +/- 0.12) (P<0.001). There was a greater increase in the total number of healthy gingival units Gingival Index (GI score 0) in the Magnolia group than in the control group (149% versus 31%) and a larger reduction in inflamed gingival units (GI score 2/3) (60% versus 30%). Furthermore, at sites with similar amounts of plaque, less clinical signs of gingival inflammation were observed in the Magnolia group than in the control group. Six months' unsupervised use of a dentifrice containing 0.3% Magnolia extract resulted in significantly greater gingivitis reduction than a corresponding control dentifrice.
