  • An international, interprofessional investigation of the self-reported podcast listening habits of emergency clinicians: A METRIQ Study

    • ObjectivesPodcasts are increasingly being used for medical education. A deeper understanding of usage patterns would inform both producers and researchers of medical podcasts. We aimed to determine how and why podcasts are used by emergency medicine and critical care clinicians.MethodsAn international interprofessional sample (medical students, residents, physicians, nurses, physician assistants, and paramedics) was recruited through direct contact and a multimodal social media (Twitter and Facebook) campaign. Each participant completed a survey outlining how and why they utilize medical podcasts. Recruitment materials included an infographic and study website.Results390 participants from 33 countries and 4 professions (medicine, nursing, paramedicine, physician assistant) completed the survey. Participants most frequently listened to medical podcasts to review new literature (75.8%), learn core material (75.1%), and refresh memory (71.8%). The majority (62.6%) were aware of the ability to listen at increased speeds, but most (76.9%) listened at 1.0 x (normal) speed. All but 25 (6.4%) participants concurrently performed other tasks while listening. Driving (72.3%), exercising (39.7%), and completing chores (39.2%) were the most common. A minority of participants used active learning techniques such as pausing, rewinding, and replaying segments of the podcast. Very few listened to podcasts multiple times.ConclusionsAn international cohort of emergency clinicians use medical podcasts predominantly for learning. Their listening habits (rarely employing active learning strategies and frequently performing concurrent tasks) may not support this goal. Further exploration of the impact of these activities on learning from podcasts is warranted.

  • Data Descriptor: A global multiproxy database for temperature reconstructions of the Common Era

    • Reproducible climate reconstructions of the Common Era (1 CE to present) are key to placing industrial-era warming into the context of natural climatic variability. Here we present a community-sourced database of temperature-sensitive proxy records from the PAGES2k initiative. The database gathers 692 records from 648 locations, including all continental regions and major ocean basins. The records are from trees, ice, sediment, corals, speleothems, documentary evidence, and other archives. They range in length from 50 to 2000 years, with a median of 547 years, while temporal resolution ranges from biweekly to centennial. Nearly half of the proxy time series are significantly correlated with HadCRUT4.2 surface temperature over the period 1850-2014. Global temperature composites show a remarkable degree of coherence between high-and low-resolution archives, with broadly similar patterns across archive types, terrestrial versus marine locations, and screening criteria. The database is suited to investigations of global and regional temperature variability over the Common Era, and is shared in the Linked Paleo Data (LiPD) format, including serializations in Matlab, R and Python. Since the pioneering work of D'Arrigo and Jacoby1-3, as well as Mann et al. 4,5, temperature reconstructions of the Common Era have become a key component of climate assessments6-9. Such reconstructions depend strongly on the composition of the underlying network of climate proxies10, and it is therefore critical for the climate community to have access to a community-vetted, quality-controlled database of temperature-sensitive records stored in a self-describing format. The Past Global Changes (PAGES) 2k consortium, a self-organized, international group of experts, recently assembled such a database, and used it to reconstruct surface temperature over continental-scale regions11 (hereafter, ` PAGES2k-2013'). This data descriptor presents version 2.0.0 of the PAGES2k proxy temperature database (Data Citation 1). It augments the PAGES2k-2013 collection of terrestrial records with marine records assembled by the Ocean2k working group at centennial12 and annual13 time scales. In addition to these previously published data compilations, this version includes substantially more records, extensive new metadata, and validation. Furthermore, the selection criteria for records included in this version are applied more uniformly and transparently across regions, resulting in a more cohesive data product. This data descriptor describes the contents of the database, the criteria for inclusion, and quantifies the relation of each record with instrumental temperature. In addition, the paleotemperature time series are summarized as composites to highlight the most salient decadal-to centennial-scale behaviour of the dataset and check mutual consistency between paleoclimate archives. We provide extensive Matlab code to probe the database-processing, filtering and aggregating it in various ways to investigate temperature variability over the Common Era. The unique approach to data stewardship and code-sharing employed here is designed to enable an unprecedented scale of investigation of the temperature history of the Common Era, by the scientific community and citizen-scientists alike.

    • Background: Increasing age is the biggest risk factor for dementia, of which Alzheimer's disease is the commonest cause. The pathological changes underpinning Alzheimer's disease are thought to develop at least a decade prior to the onset of symptoms. Molecular positron emission tomography and multi-modal magnetic resonance imaging allow key pathological processes underpinning cognitive impairment -including a-amyloid depostion, vascular disease, network breakdown and atrophy -to be assessed repeatedly and non-invasively. This enables potential determinants of dementia to be delineated earlier, and therefore opens a pre-symptomatic window where intervention may prevent the onset of cognitive symptoms. Methods/design: This paper outlines the clinical, cognitive and imaging protocol of "Insight 46", a neuroscience sub-study of the MRC National Survey of Health and Development. This is one of the oldest British birth cohort studies and has followed 5362 individuals since their birth in England, Scotland and Wales during one week in March 1946. These individuals have been tracked in 24 waves of data collection incorporating a wide range of health and functional measures, including repeat measures of cognitive function. Now aged 71 years, a small fraction have overt dementia, but estimates suggest that similar to 1/3 of individuals in this age group may be in the preclinical stages of Alzheimer's disease. Insight 46 is recruiting 500 study members selected at random from those who attended a clinical visit at 60-64 years and on whom relevant lifecourse data are available. We describe the sub-study design and protocol which involves a prospective two time-point (0, 24 month) data collection covering clinical, neuropsychological, beta-amyloid positron emission tomography and magnetic resonance imaging, biomarker and genetic information. Data collection started in 2015 (age 69) and aims to be completed in 2019 (age 73). Discussion: Through the integration of data on the socioeconomic environment and on physical, psychological and cognitive function from 0 to 69 years, coupled with genetics, structural and molecular imaging, and intensive cognitive and neurological phenotyping, Insight 46 aims to identify lifetime factors which influence brain health and cognitive ageing, with particular focus on Alzheimer's disease and cerebrovascular disease. This will provide an evidence base for the rational design of disease-modifying trials.

  • Spatial and temporal agreement in climate model simulations of the Interdecadal Pacific Oscillation

    • Accelerated warming and hiatus periods in the long-term rise of Global Mean Surface Temperature (GMST) have, in recent decades, been associated with the Interdecadal Pacific Oscillation (IPO). Critically, decadal climate prediction relies on the skill of state-of-the-art climate models to reliably represent these low-frequency climate variations. We undertake a systematic evaluation of the simulation of the IPO in the suite of Coupled Model Intercomparison Project 5 (CMIP5) models. We track the IPO in pre-industrial (control) and all-forcings (historical) experiments using the IPO tripole index (TPI). The TPI is explicitly aligned with the observed spatial pattern of the IPO, and circumvents assumptions about the nature of global warming. We find that many models underestimate the ratio of decadal-to-total variance in sea surface temperatures (SSTs). However, the basin-wide spatial pattern of positive and negative phases of the IPO are simulated reasonably well, with spatial pattern correlation coefficients between observations and models spanning the range 0.4-0.8. Deficiencies are mainly in the extratropical Pacific. Models that better capture the spatial pattern of the IPO also tend to more realistically simulate the ratio of decadal to total variance. Of the 13% of model centuries that have a fractional bias in the decadal-to-total TPI variance of 0.2 or less, 84% also have a spatial pattern correlation coefficient with the observed pattern exceeding 0.5. This result is highly consistent across both IPO positive and negative phases. This is evidence that the IPO is related to one or more inherent dynamical mechanisms of the climate system.

  • A Tripole Index for the Interdecadal Pacific Oscillation

    • A new index is developed for the Interdecadal Pacific Oscillation, termed the IPO Tripole Index (TPI). The IPO is associated with a distinct ‘tripole’ pattern of sea surface temperature anomalies (SSTA), with three large centres of action and variations on decadal timescales, evident in the second principal component (PC) of low-pass filtered global SST. The new index is based on the difference between the SSTA averaged over the central equatorial Pacific and the average of the SSTA in the Northwest and Southwest Pacific. The TPI is an easily calculated, non-PC-based index for tracking decadal SST variability associated with the IPO. The TPI time series bears a close resemblance to previously published PC-based indices and has the advantages of being simpler to compute and more consistent with indices used to track the El Niño–Southern Oscillation (ENSO), such as Niño 3.4. The TPI also provides a simple metric in physical units of °C for evaluating decadal and interdecadal variability of SST fields in a straightforward manner, and can be used to evaluate the skill of dynamical decadal prediction systems. Composites of SST and mean sea level pressure anomalies reveal that the IPO has maintained a broadly stable structure across the seven most recent positive and negative epochs that occurred during 1870–2013. The TPI is shown to be a robust and stable representation of the IPO phenomenon in instrumental records, with relatively more variance in decadal than shorter timescales compared to Niño 3.4, due to the explicit inclusion of off-equatorial SST variability associated with the IPO. © 2015, Springer-Verlag Berlin Heidelberg.
