    • To find out whether the incidence of infantile hypertrophic pyloric stenosis (IHPS) has changed over the past decade, and if so, to investigate possible contributory factors.All infants undergoing pyloromyotomy for IHPS in Sweden between 1987 and 1996 were studied. Using the national patient registers the yearly incidence was determined and evaluated in relation to sex, latitude, urbanisation, and type of surroundings by use of a Poisson model.There was a substantial decline from 2.7/1000 to 0.85/1000 over the time period. The incidence in the south was almost three times greater than in the north.The declining incidence and geographical difference suggest that environmental factors are of importance in this disorder.

    • The injection process plays an important role in Diesel engines in terms of future emission legislations. Higher injection pressures and multiple injection events every cycle are a reality. To be able to understand how the fuel injection process can be further improved studies are needed on how higher pressure, multiple injections and multi orifice nozzles affect the overall process. The objective of this study was to further develop a measurement technique to determine injection rates and discharge coefficient for multi orifice nozzles. The technique used is based on measuring the instantaneous force of a fuel jet for a non-stationary injection process. The technique is applicable for multi orifice nozzles at high injection pressures. Both single and multiple injections can be resolved. The accuracy of the technique depends on the distance between the sensor and orifice, and the sensor should be mounted as close as possible to the orifice, which can be a problem for multi orifice nozzles. This is less difficult for higher injection pressures. A comparison between the flow of an injector used in an engine that was measured with the impingement technique showed that the difference in total flow was less than 3 %. The impingement also showed great differences between the holes.

    • The feasibility of extracorporeal adsorption of 1.5-3 L plasma on protein A-Sepharose was investigated in six patients with advanced cancer. Anticoagulation with heparin was associated with respiratory distress syndrome in two patients, most likely caused by complement activation as indicated by a transient leukopenia during plasma reinfusion and appearance of C3 degradation products in the extracorporeal circulation. Addition of citrate abolished the respiratory symptoms, C3 degradation, and leukopenia, and no adverse reactions were observed. No objective tumor regression was observed in any of the patients. Three patients progressed during therapy. In one of these, multifocal central tumor necrosis was observed as a possible, although unproven, therapeutic effect. Increased natural killer and/or killer cell activities were recorded in three patients and increased complement-dependent serum cytotoxicity in one patient. The level of circulating immune complexes decreased significantly (18-28%) in three patients studied. It is concluded that extracorporeal plasma adsorption on protein A-Sepharose is feasible when citrate is added to the extracorporeal system, but its therapeutic efficacy is uncertain.
