  • Is the efficacy of the Cloward procedure overestimated? Technique of evaluation affects the outcome

    • The purpose of the present study was to investigate the influence of the evaluation technique on the outcome of the Cloward procedure in cervical radiculopathy. The retrospective study included 94 consecutive patients operated on with anterior decompression and fusion with heterologous bone (Surgibone, Unilab). There were 56 men and 38 women, with a mean age of 48 years (range 27-78 years). Sixty-six patients had a single-level fusion, 26 a two-level fusion and one patient had a three-level fusion. The follow-up rate was 91/94 (97%) and evaluation was performed by an independent observer. Pain was quantified by visual analogue scale (VAS, range 0-100), functional disability by the new functional index Cervical Spine Functional Score (CSFS, range 0-100) and by the Neck Pain Disability Index (NPDI, range 0-100). The overall clinical outcome was assessed as excellent, good, fair or poor by both the patient and by the independent observer using Odom's criteria. At a mean follow-up of 26 months (range 12-56 months) the mean pain index was 39 (range 0-98), the mean CSFS 39 (range 0-85) and the mean NPDI 32 (range 0-76). The classification of the observer was 37% excellent, 40% good, 17% fair and 6% poor, and that of the patient was 53% excellent, 23% good, 20% fair and 4% poor. In the group classified as good by the observer, all scores were above 40, suggesting considerable remaining symptoms, and only 50% had returned to work. The results suggest that previous reports on the Cloward procedure using categorizations into excellent, good, fair or poor have overestimated the efficacy of the procedure. Only an excellent, but not a good, result as classified by the patient or an independent observer reflects a successful outcome. Neither of the variables studied seems independently sufficient for a balanced reflection of the outcome. The results suggest pain (VAS) as the primary outcome measurement, which, combined with the overall evaluation by the independent observer and work status, gives a multidimensional expression of the outcome.

  • Low Latency Communication in Virtualized and Multipath Networks

    • The demand from customers for high-quality and customized network services has increased. Telecom service providers have adopted a Network Functions Virtualization (NFV) based service delivery model, in response to the unprecedented traffic growth and increasing customer demand. However, in virtualized systems, achieving carrier-grade network performance such as low latency to guarantee the quality of experience (QoE) of customers is challenging. Moreover, queuing delays that may occur both in the datacenter networks and the IP network infrastructure inhibit the deployment of emerging low-latency services.In this thesis, we focus on addressing the problem of network latency. We study the delay overhead of virtualization by comprehensive network performance measurements and analysis, in a controlled virtualized environment. The study of virtualization delay provides a break-down of the latency imposed by the virtualization and the impact of the consolidation of virtualized applications of different workloads on the end-to-end latency. On the basis of our study, we developed an optimization model for the placement and provisioning of virtualized telecom applications subject to both the latency and cost-efficiency requirements.To mitigate network latency that results from queuing delays as well as to improve multipath network capacity utilization of a datacenter network, we propose a multipath congestion control, Multipath Datacenter TCP (MDTCP), that leverages Explicit Congestion Notification (ECN) to detect and react to queuing delays caused by incipient congestion. Furthermore, we extend MDTCP with the Low Latency Low Loss and Scalable Throughput (L4S) Internet service architecture support so that it can also be used in the Internet. This ensures the low-latency demand of delay-sensitive applications and improves QoE of Internet users by exploiting the multi-access or multi-connectivity technologies of user devices.

    • Dependence of computing resources on each other in cloud computing systems (CCS) makes them prone to fail in correlated manner which significantly impacts their service reliability and energy efficiency. Focusing on these two metrics of CCS while considering correlated failures remained an open question, which is the focus of this work. This paper proposes mechanisms for improving reliability and energy efficiency jointly under correlated failures in CCS. In order to model failure correlation, statistical cluster analysis techniques are applied to real failure traces. Then, mathematical models are built to calculate reliability and energy consumption of failure prone CCS. These mathematical models are used to design fault-tolerant and energy-aware resource provisioning mechanisms/policies. In order to further reduce the energy consumption, a correlated failure-aware VM consolidation policy is also proposed in this paper. A simulation based study of the proposed resource management policies and fault tolerance mechanisms is conducted by using real failure traces and Bag-of-Tasks workload. The results demonstrate that by exploiting failure correlation with the proposed resource management policies, we reduce the occurrence of failures on tasks by 34% and increase the energy efficiency of the system by 20%, approximately in comparison to the environments where failures are handled independently.
