  • Beta-blocker Use and 30-day All-cause Readmission in Medicare Beneficiaries with Systolic Heart Failure

    • BACKGROUND: Beta-blockers improve outcomes in patients with systolic heart failure. However, it is unknown whether their initial negative inotropic effect may increase 30-day all-cause readmission, a target outcome for Medicare cost reduction and financial penalty for hospitals under the Affordable Care Act. METHODS: Of the 3067 Medicare beneficiaries discharged alive from 106 Alabama hospitals (1998-2001) with a primary discharge diagnosis of heart failure and ejection fraction <45%, 2202 were not previously on beta-blocker therapy, of which 383 received new discharge prescriptions for beta-blockers. Propensity scores for beta-blocker use, estimated for each of the 2202 patients, were used to assemble a matched cohort of 380 pairs of patients receiving and not receiving beta-blockers who were balanced on 36 baseline characteristics (mean age 73 years, mean ejection fraction 27%, 45% women, 33% African American). RESULTS: Beta-blocker use was not associated with 30-day all-cause readmission (hazard ratio [HR] 0.87; 95% confidence interval [CI], 0.64-1.18) or heart failure readmission (HR 0.95; 95% CI, 0.57-1.58), but was significantly associated with lower 30-day all-cause mortality (HR 0.29; 95% CI, 0.12-0.73). During 4-year postdischarge, those in the beta-blocker group had lower mortality (HR 0.81; 95% CI, 0.67-0.98) and combined outcome of all-cause mortality or all-cause readmission (HR 0.87; 95% CI, 0.74-0.97), but not with all-cause readmission (HR 0.89; 95% CI, 0.76-1.04). CONCLUSIONS: Among hospitalized older patients with systolic heart failure, discharge prescription of beta-blockers was associated with lower 30-day all-cause mortality and 4-year combined death or readmission outcomes without higher 30-day readmission.   • Biosorption optimization of nickel removal from water using Punica granatum peel waste

    • The present Study was undertaken to evaluate the feasibility of Punica granatum (pomegranate) peel waste for the removal of nickel from water. Batch experiments were performed to study the biosorption of nickel on prepared pomegranate peel adsorbent. The sorption process was well explained with pseudo-second-order kinetic model. The maximum sorption capacity of pomegranate peel adsorbent for nickel removal was ca. 52 mg g(-1). The sorption has been found to be endothermic and data conform to the Langmuir model. The Gibbs free energy was determined to be negative, indicating the spontaneous nature of the sorption process. The results of the present study suggest that pomegranate peel waste can be used beneficially for nickel removal from aqueous solution.

    • In the present work, various forms of algae Pelvetia canaliculata were prepared by different chemical modifications, in order to get the best form of algae for the maximum uptake of nickel. Potentiometric titration revealed that the carboxyl groups were more abundant (3.9 mmol/g) as compared to hydroxyl groups (2.0 mmol/g) on the biosorbent surface. Fourier transform infrared (FUR) analysis of algae was done to identify the role of different functional groups present on algae surface during nickel biosorption. The protonated algae showed least sorption of nickel suggesting that after acid treatment, some of the binding sites were destroyed. Among the various forms of prepared algae. Na-algae prepared directly from raw algae (without protonation) showed highest uptake of nickel. The release of sodium ions during the uptake of nickel ions has shown that the current biosorption mechanism involves ion-exchange being a stoichiometrical ratio of 2:1 between sodium and nickel ions.

    • Cells that grow together respond heterogeneously to stress even when they are genetically similar. Metabolism, a key determinant of cellular stress tolerance, may be one source of this phenotypic heterogeneity, however, this relationship is largely unclear. We used self-establishing metabolically cooperating (SeMeCo) yeast communities, in which metabolic cooperation can be followed on the basis of genotype, as a model to dissect the role of metabolic cooperation in single-cell heterogeneity. Cells within SeMeCo communities showed to be highly heterogeneous in their stress tolerance, while the survival of each cell under heat or oxidative stress, was strongly determined by its metabolic specialization. This heterogeneity emerged for all metabolite exchange interactions studied (histidine, leucine, uracil, and methionine) as well as oxidant (H2O2, diamide) and heat stress treatments. In contrast, the SeMeCo community collectively showed to be similarly tolerant to stress as wild-type populations. Moreover, stress heterogeneity did not establish as sole consequence of metabolic genotype (auxotrophic background) of the single cell, but was observed only for cells that cooperated according to their metabolic capacity. We therefore conclude that phenotypic heterogeneity and cell to cell differences in stress tolerance are emergent properties when cells cooperate in metabolism.

    • PURPOSE: This study evaluated and compared the clinical performance of laser-welded titanium fixed partial implant-supported prostheses with that of conventional cast frameworks in the partially edentulous jaw. MATERIALS AND METHODS: Forty-two patients provided with Brånemark system implants were arranged into two groups. Twenty-one patients (group A) were provided with a conventional cast ceramometal framework in one side of the jaw and a laser-welded titanium framework with low-fusing porcelain in the other side. In group B, 21 patients received a titanium framework prosthesis to replace a conventional implant prosthesis. Clinical and radiographic data were collected for 5 years. RESULTS: Four implants and one titanium framework were lost during the follow-up period. The two framework designs did not show any significant differences with regard to implant or prosthesis survival (P > .05). Few clinical complications were observed. In group A, marginal bone loss after 5 years was similar for both prosthesis designs, with an average of 0.1 mm and 0.3 mm in the maxilla and mandible, respectively. Basically, no bone loss was observed on average in group B. Furthermore, no significant relationship was observed between marginal bone loss and placement of prosthesis margin or prosthesis design. CONCLUSION: Except for an insignificant tendency toward a slightly higher incidence of small chips of porcelain veneers, laser-welded titanium frameworks presented an overall similar clinical performance as conventional cast frameworks in partial implant situations after 5 years.

    • PURPOSE: This study evaluated the 5-year clinical and radiographic performance of fixed implant-supported maxillary prostheses with either welded titanium or conventional cast-gold alloy frameworks. MATERIALS AND METHODS: Fifty-eight consecutive patients were provided with 349 osseointegrated Brånemark system implants in the edentulous maxilla at six different implant centers. Twenty-eight of the patients received, at random, prostheses with laser-welded titanium frameworks, and the remaining 30 patients had prostheses with conventional cast-gold alloy frameworks. Clinical and radiographic data were collected for 5 years after prosthesis placement. RESULTS: The titanium and cast-gold framework groups exhibited similar cumulative survival and success rates (CSR). The 5-year implant CSR from time of placement was 91.4% and 94.0%, respectively, and from prosthesis delivery the rate was 94.9% and 95.6%, respectively. The corresponding 5-year prosthesis CSRs were 96.4% and 93.3%. One patient from each group lost all the implants and turned to complete dentures within the first year of function. Another patient with a cast-gold framework had the prosthesis replaced after 4 years, basically because of problems with the veneering material. No fractures of implant components were observed during the follow-up period. Bone loss was on average 0.59 mm (SD 0.97 mm) during 5 years, with no statistical difference between the two groups. CONCLUSION: Welded titanium frameworks presented a similar favorable clinical performance as conventional cast-gold alloy frameworks in fixed implant-supported prostheses in the edentulous maxilla after 5 years in function. Implant failures were concentrated in only a few patients in each study group.
