  • Stress among medical students during clinical courses : a longitudinal study using contextual activity sampling system

    • Objectives: To investigate medical students’ experiences of stress and other emotions related to their professional roles, as defined by the CanMEDS framework, by using the Contextual Activity Sampling System (CASS).Methods: Ninety-eight medical students agreed to participate of whom 74 completed this longitudinal cohort study. Data was collected between 6th and 8th term via CASS methodology: A questionnaire was e-mailed to the participants every 3rd week (21questionnaires/measurements) during clinical rotations and scientific project work term. Emotions were measured by a 7-point Likert scale (e.g., maximum stress = 7). Answers were registered through mobile technology. We used a linear mixed-model regression approach to study the association between stress over time in relation to socio-demographic and learning activities related to CanMEDS roles.Results: Participants completed 1390 questionnaires. Mean stress level over all time points was 3.6. Stress was reported as highest during the scientific project term. Learning activities related to ‘Communicator,’ ‘Collaborator,’ ‘Scholar,’ ‘Manager’ and ‘Professional’ were associated with increased stress, e.g. ’Scholar’ increased stress with 0.5 points (t=3.91, p<0.001). A reduced level of stress was associated with ’Health Advocate’ of 0.39 points (t=-2.15, p=0.03). No association between perceived stress and demographic factors, such as gender or age was found.Conclusions: An association between different learning activities related to CanMEDS Roles and feelings of stress were noted. The CASS methodology was found to be useful when observing learning experiences and might support educational development by identifying course activities linked to stress.

  • Circulation and modification of warm deep water on the central Amundsen Shelf

    • The circulation pathways and subsurface cooling and freshening of warm deep water on the central Amundsen Sea shelf are deduced from hydrographic transects and four subsurface moorings. The Amundsen Sea continental shelf is intersected by Dotson Trough (DT) leading from the outer shelf to the deep basins on the inner shelf. During the measurement period, warm deep water was observed to flow southward on the eastern side of DT in approximate geostrophic balance. A northward outflow from the shelf was also observed along the bottom in the western side of DT. Estimates of the flow rate suggest that up to one third of the inflowing warm deep water leaves the shelf area below the thermocline in this deep outflow. The deep current was 1.2°C colder and 0.3 psu fresher than the inflow, but still warm, salty and dense compared to the overlying water mass. The temperature and salinity properties suggest that the cooling and freshening process is induced by subsurface melting of glacial ice, likely from basal melting of Dotson and Getz ice shelves. New heat budgets are presented, with a southward oceanic heat transport of 3.3 TW on the eastern side of the DT, a northward oceanic heat transport of 0.5-1.6 TW on the western side, and an ocean-to-glacier heat flux of 0.9-2.53 TW, equivalent to melting glacial ice at the rate of 83- 237 km3yr-1 . Recent satellite-based estimates of basal melt rates for the glaciers suggest comparable values, 145 km3yr-1 (Getz) and 45 km3yr-1 35 (Dotson) (Rignot et al. 2013).

    • The old city of Fez, Morocco, classified as Heritage of Humankind by the UNESCO, is subject to an integrated rehabilitation plan running over 15 years. The plan aims at restoring and preserving the economy, cultural value, and social heritage of the old city. The study follows a system dynamics approach to describe and analyze the rehabilitation plan in order to identify the strengths and defects of the plan. This study shows that while the plan is giving relatively satisfying results over the short and medium terms, it is unlikely to attain long term sustainability.
